인공슬관절 전치환술을 받은 만성 퇴행성관절염 환자의 우울 영향요인 (Factors Influencing Depression Among Patients with Chronic Degenerative Arthritis after Total Knee Arthroplasty)

  • Purpose: This study was to identify the factors influencing depression among patients with degenerative arthritis after total knee arthroplasty. Methods: The subjects were 108 patients who admitted or visited K hospital in K city after total knee arthroplasty. Data were analyzed using SPSS WIN 18.0 program. Results: The level of depression was 2.72 with a possible range of 1 to 5. Social support was 3.71 out of a total score 5. Self-efficacy was 64.47 ranged from 10 to 100. Self-esteem was 2.59 ranged from 1 to 5. The associated factors with depression were marital status, length of illness, perceived health status, pain, social support, self-efficacy, and self-esteem. Marital status, length of illness, and perceived health status accounted for 5.8% of depression. Next, all variables including pain, social support, self-efficacy and self-esteem accounted for 66.4% of depression. Conclusion: The level of depression among the subjects significantly be related to marital status, length of illness, perceived health status, pain, social support, self-efficacy and self-esteem. It indicates a need to develop nursing interventions for them to decrease depression and develop quality of life during recovery.

방사선 형질전환 차조기와 백출 복합추출물이 퇴행성관절염 관련 매개체에 미치는 영향 (Effects of Radiation Mutant Perilla frutescens var. crispa and Atractylodes macrocephala Koidzumi Complex Extract on the Mediators Related to Degenerative Arthritis)

  • 본 연구는 방사선 형질전환 차조기와 백출복합물(차조기 복합물)이 퇴행성관절염 관련 매개체에 미치는 영향을 확인하고자 MIA(monosodium iodoacetate)로 퇴행성관절염을 유도한 랫드로 평가하였다. 차조기 복합추출물을 2주 동안 25, 50, 100 mg/kg/day의 용량으로 경구 투여하고 랫드의 우측관절 내 공간에 MIA를 주입한 후 동일 용량을 4주 동안 지속 투여하였다. 이후, 혈청 바이오마커와 무릎 관절 분석의 형태학 및 조직병리학적 분석에 기초한 치료 효과를 평가하였다. 대조군 랫드와 비교하였을 때 차조기 복합추출물은 혈청 내 염증 및 골 대사 마커(TNF-α, MMP-3, COX-2, PGE2, COMP, Aggrecan)의 생성량을 유의하게 감소시켰다. 이와는 반대로 연골 흡수 매개체인 CTX-2 생성을 증가시켰으며, 방사선 형질전환 차조기는 무릎 연골과 활막을 효과적으로 보존하였다. 그 결과, 차조기 복합물은 퇴행성관절염 증상을 개선하였다. 따라서, 차조기 복합물은 퇴행성관절염 관리를 위한 식품소재로 사용될 수 있다.

무릎관절 전치환술 환자에게 적용한 무릎 안정화 운동이 균형과 보행능력에 미치는 영향 (Effect of Knee Stabilization Exercise on Balance and Walking Ability in Patients with Total Knee Replacement)

  • Background: Degenerative arthritis accounts for a large portion of the elderly, causing a lot of inconvenience in daily life. Total knee replacement (TKR) are performed to relieve pain in the knee joint. The purpose of this study was to determine whether knee joint stabilization exercises are effective in improving balance and walking ability in degenerative arthritis patients who have undergone TKR. Methods: A total of 30 TKR patients participated in this study. They were assigned to two groups. The experimental group, the group that underwent knee stabilization exercises, joint mobilization and general physical therapy. On the other hand, and the controlled group, the group that underwent joint mobilization and general physical therapy. All exercises were conducted thrice a week, for four weeks. The main balance outcomes were evaluated using the posture balance training system, while walking ability was assessed using a wireless 3-axis accelerometer. Results: The experimental group had significantly higher scores in postural stability testing (PST), limits stability testing, and waling ability. The post-intervention evaluation, there were significant differences in all variables between the two groups. However, no significant difference was noted in the foam eyes closed test in PST. Conclusion: This study suggests that knee joint stabilization exercises effectively improve the balance and walking ability of TKR patients. Thus, it could be presented as useful in clinical practice.

Efficacy and Safety of Cheong-A-Won Gagambang (JCE003) on Knee Osteoarthritis: Randomized Controlled Pilot Trial

  • Background: The aim of this study was to evaluate the effectiveness and safety of Cheong-A-Won Gagambang (JCE003) treatment for degenerative knee osteoarthritis. Methods: This was a single-center, randomized, double-blind, placebo-controlled pilot clinical trial. There were 36 adults with degenerative knee osteoarthritis who were randomly allocated into JCE003 1,000 mg, JCE003 2,000 mg, or the placebo group (in a 1:1:1 ratio). The participants received 12 weeks of treatment and had scheduled tests every 6 weeks. The primary outcomes were measured using the Korean Western Ontario and McMaster Universities scale, and the secondary outcomes were measured using the visual analog scale, European quality of life-5-dimensions, patient global impression of change, C-reactive protein, and erythrocyte sedimentation rate. Changes between baseline scores and scores following study completion were analyzed. Results: There were 29 participants whose data were analyzed in this study. The change of Korean Western Ontario and McMaster Universities, visual analog scale, European quality of life-5-dimensions scores showed significant improvement in the JCE003 1,000 mg group. The change of patient global impression of change was significantly improved in the placebo group. There were 14 adverse events, but there was no clinically significant relationship with the intake of JCE003 compared with the placebo. Conclusion: Taking JCE003 may be effective at improving knee pain in patients with degenerative knee osteoarthritis and appears to be safe. Based on this study, the concentration and feasibility of the test group may be used when conducting a large-scale clinical trial of degenerative knee osteoarthritis in the future.

슬관절 퇴행성 관절염에서 관절경적 내측 반월상 연골 절제술의 효과 (The Effect of Arthroscopic Medial Meniscectomy in Degenerative Arthritis of the Knee)

  • 목적: 퇴행성 관절염에서 발견되는 반월상 연골 병변에 대한 치료의 필요성에 대하여는 논란이 많다. 본 연구의 목적은 슬관절 퇴행성 관절염 환자에서 내측 반월상 연골 파열에 대한 치료로 관절경적 내측 반월상 연골 절제술을 시행하였을 경우의 치료 효과에 대하여 알아보고, 적절한 적응증과 위험 요소를 찾는 것이다. 대상 및 방법: 2006년에서 2008년 사이에 287명이 본원에서 내측 반월상 연골 파열에 대하여 관절경적 수술을 시행 받았다. 이 중 Kellgren-Lawrence grade II, III 슬관절 퇴행성 관절염이 확인되고, 나이가 50세 이상이며, 외래 추시가 1년 이상인 환자 103명을 대상으로 하였다. 수술 비디오, 방사선 사진, 의무기록, 설문지와 인터뷰를 통해 후향적으로 임상적 결과를 평가하였다. 시각 상사 척도(VAS) 점수와 Lysholm 점수를 통해 통증과 기능의 호전을 평가하였다. 결과: Lysholm 점수는 수술 전의 평균 69점에서 수술 후에 평균 85점으로 증가하였고, VAS 점수는 수술 전의 평균 7점에서 수술 후에 평균 3.1로 통증이 호전되었다. Kellgren-Lawrence grade II 환자군에서 III 환자군보다, Outerbridge grade I 환자군에서 III와 IV 환자군보다 의미있게 통증이 호전되었다. 외상력이 있는 환자군에서 없는 환자군보다 의미있게 통증이 호전되었고 기능의 회복이 관찰되었다. 다중 회귀 분석에서 확인된 Lysholm 점수의 호전에 영향을 미치는 인자로는 외상력과 Outerbridge grade였다. 결론: 1년 단기 추시에서 Kellgren-Lawrence grade II, III 슬관절 퇴행성 관절염 환자에서 내측 반월상 연골 병변에 대하여 시행한 관절경적 내측 반월상 연골 절제술은 환자의 통증 호전과 기능의 회복에 도움을 줄 수 있다. 특히 Kellgren-Lawrence grade가 낮은 경우와 외상력이 있는 환자, 관절 연골 손상이 심하지 않은 환자에서 좋은 결과를 기대할 수 있다.

웃음요법이 퇴행성 슬관절염 노인의 관절통증, 관절기능상태, 우울에 미치는 효과 (Effects of Laughter Therapy on Joint Pain, Functional Status of Knee Joint and Depression in Elderly with Degenerative Knee Arthritis)

  • 본 연구는 웃음요법이 퇴행성 관절염 노인 환자의 관절 통증, 관절기능 이상, 우울에 주는 효과를 확인하기 위해 시도되었다. 단일맹검, 무작위 배정을 통해 S시의 경로당 및 복지관을 이용중인 65세 이상 퇴행성 슬관절염 노인을 대상으로 실험군 26명, 대조군 26명을 선정하였다. 2019년 1월 8일부터 3월 11일까지 주 2회, 회당 60분, 8주, 총 16회의 웃음요법을 적용하고 그 효과를 VAS, 압통역치, WOMAC, 한국형 노인 우울 단축형 척도로 측정하였다. 연구결과 퇴행성 슬관절염 노인의 통증 역치(p=.012)와 통증, 관절경직, 신체기능을 측정하는 관절기능상태(p<.001), 우울(p<.001)은 대조군에 비해 실험 후 개선되어 웃음요법이 효과가 있음이 확인되었다. 웃음요법은 육체적 또는 지적 활동을 통해 웃음을 경험할 수 있는 치료적 개입으로 침습적인 시술이 이루어지지 않고 약간의 훈련으로 간호사들이 쉽게 적용할 수 있는 장점이 있으므로 웃음요법을 지역사회 및 임상 현장에서 퇴행성 슬관절염 노인의 통증을 완화시키고 우울을 개선하는 중재방법으로 활용할 수 있을 것으로 사료된다.

장애물 보행 훈련이 무릎관절 전치환술 환자의 균형 및 낙상효능감에 미치는 영향 (The Effects of Obstacles Gait on Balance and Falls-efficacy in Patients With Total Knee Arthroplasty)

  • Background: This study was conducted to investigate the effects of obstacles gait on balance and fall-efficacy in patients who underwent total knee arthroplasty. Methods: The 24 subjects of this study were recruited from individuals diagnoses with degenerative arthritis who had undergone total knee arthroplasty. The 24 patients were randomly divided into a control groups and experimental groups, obstacles gait exercise was conducted for 4 weeks three exercises. Balance and falls-efficacy were measured to compare the effects of the exercises. Results: The results of the balance ability and falls-efficacy showed that the experimental group showed significantly enhanced results than the control group(p<.05). Conclusions: Based on these results, Obstacles gait exercise effectively improves the recovery of patients with total knee arthroplasty.

In Vivo 실험모델에서 오디추출복합물의 퇴행성관절염 개선 효능 연구 (Effect of Mulberry Extract Complex on Degenerative Arthritis In Vivo Models)

  • 본 연구에서는 오디추출복합물(mulberry extract complex, MEC)의 퇴행성관절염 증상 완화 및 개선 효과 가능성을 탐색하기 위하여 monosodium iodoacetate(MIA)로 유도한 퇴행성관절염 in vivo 실험모델을 이용하였다. 연골의 주요 구성성분인 glycosaminoglycan(GAG) 및 collagen의 농도를 실험동물의 연골에서 측정한 결과 MIA로 인해 감소하였던 GAG 및 collagen의 농도가 MEC를 경구 투여한 실험군에서 농도 의존적으로 증가하였다. 또한 교원질 합성을 억제하고 분해를 촉진하는 matrix metalloproteinase-2, 9, 13의 농도를 측정한 결과는 MEC의 농도에 따라 감소하는 결과를 보여주었다. 연골 손상 지표인 cartilage oligomeric matrix protein과 C-terminal telopeptide 2의 측정결과에서는 대조군보다 유의성 있는 감소를 나타내어 MEC가 퇴행성관절염의 진행 억제에 도움을 줄 수 있을 것으로 생각된다. 관절염 지수 평가에서도 MEC는 모든 농도에서 대조군보다 유의성 있게 개선되는 결과를 나타내었다. 이상의 실험 결과를 통하여 MEC가 퇴행성관절염에서 나타나는 연골 구성성분의 분해를 억제하고, 여러 중요한 퇴행성관절염 진행 인자를 효과적으로 억제하여 결국 연골파괴 감소와 더불어 통증을 줄여줌으로써 퇴행성관절염에 대한 증상 완화 및 개선 효과가 있을 수 있는 건강기능식품의 원료로 활용될 수 있을 것으로 생각된다.

퇴행성 슬관절염에서 Ketorolac의 관절강내 주사 효과 (Effects of Intra-articular Injection of Ketorolac in Degenerative Arthritis of Knee)

  • Background: Intra-articular injection is a commonly performed procedure in patients with degenerative osteoarthritis of the knee. Several drugs are used for relief of pain in such cases. Local anesthetics, clonidine and steroids have been confirmed to be effective when used in an intra-articular injection. Ketorolac has recently become one of the most commonly used and potent NSAIDs. There have been many studies about the effect of ketorolac. Methods: Sixty-four patients were divided into 2 groups. In Group I (n = 31), 0.5% bupivacaine 3 ml and sodium hyaluronate 20 mg were used and in Group II (n = 33), 0.5% bupivacaine 3 ml, sodium hyaluronate 20 mg and ketorolac 5 mg were used. We observed the pain relief scale (PRS) at 15 minutes, 1 week and 1-3 months after injection. Results: After 15 minutes, PRS scores were $6.6{\pm}2.7$ (Group I) and $5.1{\pm}3.1$ (Group II), so there was a statistical difference between the two groups. After 1 week, the PRS scores were $5.9{\pm}2.0$ (Group I) and $5.8{\pm}2.4$ (Group II). At 1-3 months later, PRS scores were $5.6{\pm}3.0$ (Group I) and $5.1{\pm}2.7$ (Group II). Thus, there were no significant statistical differences between the two groups at 1 week or 1-3 months later, although some patients were more satisfied with pain relief in Group II at 1-3 months. Conclusions: Ketorolac provides more rapid pain relief of degenerative arthritis when used in intra-articular injection. And there was no statistical difference of effect after 1 week or 1-3 months later. Further studies are required on the effect of the intra-articular use of ketorolac.

보행 시 농작업 종사자들의 슬관절 퇴행성 등급에 따른 지면반력 특성 분석 (Analysis of the Characteristics of Ground Reaction Force According to the Level of Knee Osteoarthritis During Gait)

  • Objective : This study was conducted with an aim to use it as basic data for developing assistive devices, such as insoles that can suppress the progress of degenerative diseases and strategies, to improve early degenerative diseases by assessing walking characteristics of farm workers who were classified as KL-grade in the perspective of motor mechanics. Method : 38 male and female adults who complained of knee joint pain for more than six months were selected, and they were classified according to KL-grade. KL-grade was assessed by an orthopaedic specialist and an occupational environment health specialist. Filming equipment (FX-1, CASIO, Japan) and a ground reaction force system (AMTI OR6, AMTI, USA) were used to identify ground reaction force characteristics, and WOMAC was used for a pain rating scale. Results : There was a difference between the right and left side (axis-X) according to KL-grade, and when the grade was higher, the internal ground reaction force was also higher. Changes in COP were not affected by KL-grade of the knee joint, but it tended to increase as the grade increased. There were differences in the time required for limb support while walking according to the grades, and when the grade was higher, walking was more inefficient with long braking force and short propulsion forces. Also, pain rating scale, the right and left side, and COP changes while in support phase were related. Conclusion : There was a partial, statically significant difference in KL-grade and ground reaction force occurring during the support phase, and there were differences in ground reaction forces according to the grades of degenerative arthritis in the knee joint, indicating that this study is worthy as basic data for future studies.
