사회적 지지가 농촌 조손가족 조모의 심리적 복지에 미치는 영향 (The Effect of the Social Support on the Psychological Well-being of Custodial Grandmothers in Rural Korea)

  • Family dissolution and economic difficulties of the grandchild's parents are the most prevailing reason for an increase of grandparent-headed families in rural Korea. However, there is a lack of empirical research looking at the experiences of custodial grandmothers, social support, and their psychological well-being. In this regard, this study examined the effect of social support (formal & informal) on the psychological well-being of custodial grandmothers in rural Korea. For this purpose, quantitative and qualitative research methods were conducted. Quantitative data were gathered, using structured questionnaire, from 311 grandmothers who were raising their grandchildren in rural Korea as primary care-givers. Statistical methods used for data analysis were descriptive statistics and hierarchical regression analysis with SPSS WIN 19.0 programs. Qualitative data were collected by in-depth interviews to 10 custodial grandmothers in rural area. The results of this study are as follows. First, almost half of the parents of the grandchild didn't visit and didn't call anymore. Some relatives -aunt, uncle, etc.- support the grandparent-headed family economically and emotionally. 3/4 of the grandparent-headed families were supported by the government and community. This was not small numbers, but the amount of economic support was low. Second, grandmothers in the study showed quite a high level of depression while their happiness level was relatively low. There were differences in the factors influencing the level of happiness/depression of these rural grandmothers. Overall, the study's results showed the importance of formal & informal support for psychological well-being of the custodial grandmothers in rural Korea. Policy and theoretical implications are discussed.

가정환경 변인과 학업성취에 관한 어머니의 교육신념이 유아의 사교육 실태에 미치는 영향 (Contributors of Extra-curricular Activities of Preschool Children: Focused on Home Environment Factors and Maternal Belief about Factors on Academic Achievement)

  • This study aimed to identify the contributors of extra-curricular activities of preschool children focusing on home environmental factors(financial capital, human capital, and social capital) and maternal belief about factors on academic achievement(child's effort, child's ability, teacher's instruction, and maternal teaching). One hundred and seventy three mothers whose preschool children were attending child-care centers in Seoul and Kyunggi-Do participated in the survey. Kinlaw et al.(2001)'s Importance of Effort/Ability Scale was used to assess the mother's belief about the factors on academic achievement. Stipek et al.(1992)'s Activities Sub-scales was revised to measure a social capital, that is the mother's formal and informal activities providing to their preschool child. Educational level was utilized as human capital and monthly income was considered as financial capital. Also, using open ended questions, the subjects were asked to respond to the numbers of extra-curricula activities their child was presently attending, and when he or she began to attend the extra-curricular activity. The main results are as follows. In general, the mothers provided informal activities to their preschool child more than formal activities. Also, they believed the teacher's instruction is the most important and the child's ability the least important in their child's academic achievement. The number of extra-curricular activities was predicted by mothers' formal activities and their belief on their child's ability. The age of starting extra-curricular activities were explained by mothers' formal activities and their educational level.

근로자들의 근무유연성에 대한 요구 분석 - 한 가족친화인증기업 근로자들의 사례 - (A Case Study on the Family-Friendly Firm Workers' Flexibility Needs)

  • This qualitative case study focused on the workers' flexibility needs. We selected eight full-time workers from one 2009 family friendly certified firm for observation and in-depth interview. The workers are categorized into three groups; male employees with housewives, double income family workers depending on extended family, and double income family workers depending on paid babysitter. The spouse's work status and informal extended familial support to child care were crucial factors to influence their work family life styles and flexibility needs. The flex time is the most desirable everyday needs for the dual working child caring parents depending on the paid help. And the female workers' concerned about the stigma effect of the institutionalized flexibility use of the inferior labors.

지역사회 보건사업을 위한 보건교육 (Health Education for Improving the Effectiveness of Community Health Programmes)

  • Health education is the first of the nine essential services of primary health care which has been advocated as a key in achieving “Health for All” by World Health Organization and its Member States since 1978. The purpose of this paper is to assist community health workers to improve the effectiveness of community health programmes through understanding key issues related to health education, and applying the recommended skills to conducting their health education programmes in the community. Chapter 1 shows the relationship between health and behavior, and the importance of understanding several key factors for people's health behavior in designing and implementing health education programmes in the specific community, and discusses ways to facilitate people's health behavior changes. Chapter 2 deals with conducting face-to-face health education with emphasis on counselling skills, and chapter 3 touches with health education for informal group, in particular at the hospital setting. Chapter 4 introduces how to create a supportive verbal communication climate, and proposes applying these skills to health education so as to improve the effectiveness of health education.

Improving the Workplace Experience of Caregiver-Employees: A Time-Series Analysis of a Workplace Intervention

  • Background: Rapid population aging in developed countries has resulted in the working-age population increasingly being tasked with the provision of informal care. Methods: An educational intervention was delivered to 21 carer-employees employed at a Canadian University. Work role function, job security, schedule control, work-family conflict, familywork conflict, and supervisor and coworker support were measured as part of an aggregated workplace experience score. This score was used to measure changes pre/post intervention and at a follow-up period approximately 12 months post intervention. Three random intercept models were created via linear mixed modeling to illustrate changes in participants' workplace experience across time. Results: All three models reported statistically significant random and fixed effects intercepts, with a positive coefficient of change. Conclusion: This suggests that the intervention demonstrated an improvement of the workplace experience score for participants over time, with the association particularly strong immediately after intervention.

  • I. Background The problem of discharging patients from hospital have been well documented in the literature over the last 20 years. They included poor communication between hospital and community, inadequate notice of discharge, over-reliance on informal support and lack of statutory support, inattention to patients needs before leaving hospital, and wasted or duplicated visits by community nurses. Most patients discharged from hospital are able to return home with little or no support, while others will require a 'package of care' to support them back to good health. Patient with complex care needs, including the frail elderly and those with mental health problems, may require continuing care in special housing, residential, or nursing homes. With this population,effective discharge arrangement is needed and the study on this problem is urgent in Korea because the Medical Reform Project is on suspension of success. II. Results of the Study: 1. Discharge service needs assessed on 360 elderly patients who were hospitalized during the survey period at four university hospitals. Patients want to know the information on disease management after discharge. Follow-up telephone service is the most frequently checked service. 2. Multidisciplinary Discharge Planning is recommended at the hospital level to reduce the readmission and decrease the length of stay. 3. Further research is needed to validate and test the assumption of the solution which is developed in this research.

남녀 고등학생들의 우정에 대한 개념도 연구 (A Concept Mapping Study of Korean High School Students' Conceptions of Friendship)

  • 본 연구의 목적은 우리나라 남녀고등학생의 우정에 대한 경험과 인식을 탐색적으로 알아보고, 귀납적으로 우정을 개념화하는 것이었다. 이를 위해 질적 자료를 양적으로 분석하는 개념도 방법을 사용하여 우정에 대한 참여자들의 경험과 인식을 확인하고 그 내용을 시각화하였다. 브레인스토밍 과정을 통해 남학생그룹에서 93개, 여학생그룹에서 100개의 진술문을 산출하였으며, 이를 분류하고 범주화하는 작업을 통해 최종 개념도를 완성하였다. 분석결과, 남녀 집단의 최종 개념도 모두 '실제적-관념적' 측면과 '행동적-정서적' 측면의 2개 차원을 가지는 것으로 나타났다. 범주의 수는 4개로 동일했으나, 범주에 속한 진술문의 구체적 내용에는 남녀 집단에 있어 다소간 차이가 있었다. 남학생들은 우정을 행복의 원천이자, 일상을 공유하는 격 없는 사이, 그리고 감정을 공유하고 정서적인 유대를 나누며 서로 돕고 돌보는 관계로 개념화한 반면, 여학생들은 일상을 공유하는 격 없는 사이이자, 서로 돕고 돌보는 관계, 그리고 행복의 원천이자 정서적 연결과 헌신을 주고받는 관계로 인식하는 것으로 나타났다. 범주의 중요도를 살펴본 결과, 남학생은 '공감과 유대', 그리고 '행복의 원천'을 더 중요시하고, 여학생은 '돌봄과 지원'과 '격 없는 사이'를 더 중요시하는 것으로 나타나 차이를 보였다.

요보호 노인 부양가족의 재가복지 및 재가보건 서비스 유형별 이용의향에 관한 연구 - 2002년도 서울시 노인복지 기초수요조사 자료 분석 - (A Study on Family Caregivers' Preferences for Utilization of Community-based Welfare and Health-care Services)

  • 본 연구에서는 노인요양보험제도의 도입을 앞두고 재가 요보호 노인 가족의 재가복지 및 재가보건서비스의 이용의향을 유형별로 알아보고, 이에 관련된 요인을 행동주의모델을 이용하여 검토함과 동시에 서비스를 이용하지 않는 이유를 분석하는 것을 목표로 하였다. 각 서비스 이용의향에 관련된 요인을 로지스틱 회귀분석을 통해 분석한 결과, 선행요인으로는 재가복지와 재가보건 서비스 모두 연령이 낮은 사람이 이용의향이 높았다. 서비스 이용촉진 및 저해요인 중에는 경제적인 여유가 있거나 부부양자가 있는 사람, 혹은 부양과 관련된 정보를 제공해 줄 사람이 있어, 물적 자원이 있거나 비공식적 지원을 받고 있는 사람이 재가복지와 재가보건 서비스에 대한 이용의향이 모두 높게 나타났다. 욕구요인에서는 예상과는 달리, 노인의 증상이 심각하거나 거동불편 정도가 심한 사람, 혹은 요보호 노인 외에도 요보호 가족이 있어 서비스에 대한 이용 욕구가 높은 집단이 오히려 이용의향이 낮게 나타났는데, 서비스의 유료화를 위해서는 경제적 자원과 같은 촉진요인을 가지고 있지 않으면 서비스에 대한 욕구(필요도)가 높아도 서비스 이용으로 연결되지 못할 위험이 있는 것으로 해석된다. 한편 서비스를 이용하지 않는 이유로는 경제적인 부담 이외에 '가족이 돌봐야 한다'. '노인이 싫어한다'와 같은 전통적인 부양의식이 차지하는 비율이 높았다. 요보호 노인의 증가로 서비스에 대한 이용 의향이 이전에 비해 높아지고 있지만, 서비스에 대한 욕구가 높음에도 불구하고 서비스 이용의향이 없는 사람이 적지 않았다. 본 연구를 통해 재가복지 서비스에 대한 이용의향과 재가보건 서비스에 대한 이용의향을 나타낸 집단의 특성이 유사한 것이 확인되었고, 향후 클라이언트 위주의 서비스 제공을 위해서는 보건복지 서비스의 연계를 통한 서비스 제공의 필요성이 재확인되었다.

보편적 건강보장을 향한 노정 : 베트남 보건의료 부문의 역사·문화적 맥락을 중심으로 (The way to achieve Universal Health Coverage: Focusing on the Historical and Cultural Context of Health Care Sector in Vietnam)

  • 본 연구는 지속가능개발목표(Sustainable Development Goals) 하에 보편적 건강보장(Universal Health Coverage) 달성을 목표로 전 국민의 건강보험 가입을 추진하고 있는 베트남의 보건의료 부문에 주목하여 보건의료 체계와 건강보험법의 개혁 과정에서 나타난 특징을 역사 문화적 맥락을 통해 파악해보고 개발의 관점에서 그 함의를 살펴보고자 한다. UHC의 세 가지 차원, 즉 인구 집단에 대한 보장성 확대, 다양한 의료서비스 제공, 그리고 재정적 보호를 기준으로 베트남 보건의료 부문의 현황을 요약하면 다음과 같이 정리할 수 있다. 첫째, 2015년 발효된 개정 건강보험법에서 가구 단위의 의무 가입과 그에 따른 건강보험료 정산 방식이 새로운 제도로 시행되고 있다는 점이다. 둘째, 1차 의료시설, 즉 사($X{\tilde{a}}$, Commune)급 단위의 보건소를 중심으로 예방 및 건강관리 서비스를 제공할 수 있는 의료 네트워크가 구축되어 있다는 점이다. 셋째, 의료보험법 및 다양한 제도 시행 이후 공공 지출이 증가하고 민간 지출이 감소하고 있지만 여전히 본인부담 의료비 지출(Out-of-Pocket Expenditure)이 많은 비중을 차지하고 있다는 점이다. 베트남의 사회건강보험 개혁은 현재 과도기이다. 따라서 베트남은 보건의료 체계와 건강보험 제도를 어떻게 구축해 나갈 것인가에 대한 문제가 더욱 중요할 수밖에 없는 시기적 상황에 당면해 있다. 제도와 체계에 대한 개발은 효율성보다는 그것을 고스란히 감당해내야 하는 주체, 즉 해당 사회의 구성원들에게 적절하고 정당한 방식으로 설계되어야 한다. 본 연구는 제도와 문화, 즉 제도를 공유하는 사회적 가치, 가족 문화 그리고 비공식적인 제도 등과의 상호작용 등으로부터 그 함의를 이끌어내고자 하였다.

병원약국학(病院藥局學) 대학원전공개설(大學院專攻開設)을 위(爲)한 교육내용(敎育內容)에 관(關)한 연구(硏究) (Establishment of Graduate Education in Hospital Pharmacy)

  • Since the national health insurance was introduced in 1978, the increased utilization of hospitals and the growing importance of pharmaceutical services to hospital patients have made the administration of these services a very complex and specialized responsibility. The pharmaceutical services has always been an essential component of comtemporary hospital care. In the hospital, the pharmaceutical services is the professional department which concerns itself with the evaluation, selection, control and utilization of drugs. The director of this service must be a versatile professional person who can work effectively in a heterogenous society of educated persons. However, graduate education in hospital pharmacy has not been introduced yet in Korea. The necessity of graduate education hospital pharmacy has been discussed in this research. Graduate education in hospital pharmacy emphasizes preparation for assumption of responsibility as the senior hospital pharmacist or the director of pharmaceutical services. Graduates should also be prepared as administrators of a department that must operate with great efficiency. They should be prepared serve as a consultant on drugs for the medical and allied health professional staff, organizing and disseminating a large and dynamic body of information in their interest and to establish professional roles that emphasize procurement, storage, manufacturing, packaging, distribution, control and evaluation of drugs. Senior hospital pharmacist is a teacher charged with responsibility fer formal and informal instruction of other hospital personnel in pharmaceutical sciences. In addition, the graduates have the opportunity to be a researcher dealing with aspect of hospital care and are intensively educated in the professional aspects of hospital pharmacy practices. The curriculum of graduate education in hospital pharmacy should be established detailly and carefully to fit the educational objective.
