Objectives: Malaria remains a public health concern worldwide, including Indonesia. Purworejo is a district in which endemic of malaria, they have re-setup to entering malaria elimination in 2021. Accordingly, actions must be taken to accelerate and guaranty that the goal will reach based on an understanding of the risk factors for malaria. Thus, we analysed malaria risk factors based on human and housing conditions in Kaligesing, Purworejo, Indonesia. Methods: A case-control study was carried out in Kaligesing subdistrict, Purworejo, Indonesia in July to August 2017. A structured questionnaire and checklist were used to collect data from 96 participants, who consisted of 48 controls and 48 cases. Univariate, bivariate, and multivariate analyses were performed. Results: Bivariate analysis found that education level, the presence of a cattle cage within 100 m of the house, not sleeping under a bednet the previous night, and not closing the doors and windows from 6 p.m. to 5 a.m. were significantly ($p{\leq}0.25$) associated with malaria. Of these factors, only not sleeping under a bednet the previous night and not closing the doors and windows from 6 p.m. to 5 a.m. were significantly associated with malaria. Conclusions: The findings of this study demonstrate that potential risk factor for Malaria should be paid of attention all the time, particularly for an area which is targeting Malaria elimination.
To evaluate the factors that determine the transmission level of vivax malaria using vectorial capacity, entomological surveys were conducted from .lune to August, 2000. From 6 nights of human-bait collection in Paju, the human biting rate (ma) was counted as 87.5 bites/man/night. The parity of Anopheles sinensis from human baiting collections fluctuated from 41% to 71% (average 48.8%) of which the rate gradually increased as time passed on: 35.2% in Jun. ; 55.0% in July; 66.2% in Aug. From this proportion of parous, we could estimate the probability of daily survival rate of An. sinensis to be 0.79 assumed with 3 days gonotrophic cycle and the expectancy of infective life through 11 days could be defined as 0.073. Blood meal analysis was performed using ELISA to determine the blood meal source. Only 0.8% of blood meals were from human hosts. We could conclude that An. sinensis is highly zoophilic (cow 61.8%) Malaria is highly unstable (stability index < 0.5) in this area. From these data, vectorial capacity VC) was determined to be 0.081. In spite of a high human biting rate (ma), malaria transmission potential is very low due to a low human blood index. Therefore, we could conclude that malaria transmission by An. sinensis is resulted by high population density, not by high transmission potential. For this reason, we need more effort to decrease vector population and vector-human contact to eradicate malaria in Korea.
우리나라의 말라리아는 1990년대 초에 재 출현한 이후로 그 발병사례가 지난 10년 동안 증감을 거듭해오고 있다. 말라리아의 주된 발병 지역이 비무장지대 인근의 남북한 경계를 따라 분포하고 있는 점을 고려할 때, 우리나라 말라리아 발생은 이 지역의 자연과 거주 환경에 의해 영향을 받고 있는 듯하다. 말라리아는 말라리아 원충을 옮기는 모기에 의해 감염되는 전염병이므로 이 질병의 발병율은 특정 기후 및 사회인구학적 인자들과 밀접한 연관성을 가지고 있다. 본 연구에서는 말라리아 발생의 공간적 특성이 모기 서식환경의 비율, 모기서식지와 주거지역 간의 거리, 도시화에 따른 자연환경 및 사회인구적 환경, 그리고 해당 지역의 국지적 지형과 관련이 있음을 논의하였다.
1980년대에 근절된 것으로 여겨졌던 말라리아는 재출현 이후 지난 10년간 증감을 거듭하고 있다. 우리나라의 경우에는, 휴전선에 인접한 지역이 말라리아 발생의 핫스팟으로 분류될 만큼 지역적 특징이 뚜렷하게 나타나고 있다. 특히, 휴전선 부근 시 군지역이 말라리아 발생의 위험지역으로 분류될 뿐만 아니라 지역 간 말라리아 발생률에서 차이가 있으며, 질병 발생의 중심이 변화하는 경향을 보이고 있다. 이는, 말라리아 발생이 지역별로 상이하게 펼쳐져 있는 자연 및 거주지 환경에 영향 받고 있음을 말해준다. 말라리아는 매개모기에 의해 병원체가 전달되는 것으로 이 과정은 특정 기후 및 인문환경 시스템 속에서 발현된다. 기후 요소는 매개모기와 병원체에 매우 다양하게 영향을 미치며 기온은 가장 대표적인 환경요인이다. 또한 매개모기와 인간과의 접촉 빈도와 접촉 거리는 말라리아 발생률에 가장 직접적인 영향을 미치는 요인이다. 본 연구에서는 지역별로 다른 국지적인 기후조건, 매개모기의 산란 및 서식환경의 비중, 매개모기와 거주지 간의 거리에 따라 말라리아 발생이 지역별로 차이가 나타난다는 점을 기상자료, 인공위성 기반의 토지피복 자료를 활용하여 확인하였다.
In this paper, we have studied dynamics of fractional order mathematical model of malaria transmission for two groups of human population say semi-immune and non-immune along with growing stages of mosquito vector. The present fractional order mathematical model is the extension of integer order mathematical model proposed by Ousmane Koutou et al. For this study, Atangana-Baleanu fractional order derivative in Caputo sense has been implemented. In the view of memory effect of fractional derivative, this model has been found more realistic than integer order model of malaria and helps to understand dynamical behaviour of malaria epidemic in depth. We have analysed the proposed model for two precisely defined set of parameters and initial value conditions. The uniqueness and existence of present model has been proved by Lipschitz conditions and fixed point theorem. Generalised Euler method is used to analyse numerical results. It is observed that this model is more dynamic as we have considered all classes of human population and mosquito vector to analyse the dynamics of malaria.
Poor subsistence farmers who live in a semi-arid area of northern Ethiopia build irrigation systems to overcome water shortages. However, there is a high risk of malaria transmission when increased standing water provides more favorable habitats for mosquito breeding. This is a serious problem because there are many barriers to malaria control measures and health care systems in the area. Using a causal loop diagram and computer simulations, the author attempted to visually illustrate positive and negative feedbacks between mosquito and human populations in the context of Simret, which is a small village located in northern Ethiopia and is generally considered a malaria-free area. The simulation results show that the number of infectious mosquitos increases to 17,215 at its peak, accounting for 3.5% of potentially dangerous mosquitos. At the same time, the number of sick people increases to 574 at its peak, accounting for 15% of local population. The malaria outbreak is controlled largely because of a fixed number of vulnerable people or local population that acts as an intermediate host.
지구온난화로 대표되는 전반적인 기후변화 속에서도 지역적인 기상조건에 따라 말라리아 감염 사례의 증감이 비교적 민감하게 영향 받는 것으로 사료된다. 말라리아를 매개하는 모기의 서식환경에 직접적인 영향을 주는 기후환경적인 인자, 즉 강수, 기온, 지표수 분포, 토양수분, 토지이용에 대한 광범위한 관측과 추정에 원격탐사 자료의 적용이 매우 중요한 수단이 되었다. 다국적 원격탐사 센서의 개발이 이어지고 있고, 공간 및 분광해상도 면에서 기술적인 진화를 보이고 있으며, 자료 획득에 필요한 비용도 크게 줄어드는 등 말라리아를 비롯한 모기매개 감염병 연구에 원격탐사 기법의 적용이 크게 각광받을 전망이다. 우리나라의 경우, 1980년대에 퇴치되었던 것으로 보고된 말라리아는 1990년대 중반부터 크게 증가하여 2000년 이후 증감을 거듭하고 있어 보건관리의 주요 대상으로 떠올랐다. 감염자수로 볼 때, 휴전선 인근 지역에서 큰 비중을 차지하고 있기 때문에 말라리아 보균 모기의 북방 유입설 등 지리적인 특성에 주목할 필요가 있다. 말라리아 매개모기의 환경적인 서식조건은 모기 개체수 규모를 추정하는 데에 중요한 역할을 하지만, 말라리아 감염과 전파는 환경조건 외에도 인간의 활동, 인구구성, 경관의 구조, 거주지와 매개모기 서식처간의 공간적 관계 등 매우 복합적인 보건지리학적 메카니즘의 산물이라는 점을 이해해야 한다.

Objectives This study was conducted to describe the regional malaria incidence in relation to the geographic and climatic conditions and describe the effect of altitude on the expansion of malaria over the last decade in Papua New Guinea. Methods Malaria incidence was estimated in five provinces from 1996 to 2008 using national health surveillance data. Time trend of malaria incidence was compared with rainfall and minimum/maximum temperature. In the Eastern Highland Province, time trend of malaria incidence over the study period was stratified by altitude. Spatio-temporal pattern of malaria was analyzed. Results Nationwide, malaria incidence was stationary. Regionally, the incidence increased markedly in the highland region (292.0/100000/yr, p =0.021), and remained stationary in the other regions. Seasonality of the malaria incidence was related with rainfall. Decreasing incidence of malaria was associated with decreasing rainfall in the southern coastal region, whereas it was not evident in the northern coastal region. In the Eastern Highland Province, malaria incidence increased in areas below 1700 m, with the rate of increase being steeper at higher altitudes. Conclusions Increasing trend of malaria incidence was prominent in the highland region of Papua New Guinea, while long-term trend was dependent upon baseline level of rainfall in coastal regions.
Astrocytes are the most abundant cells in the central nervous system that play roles in maintaining the blood-brain-barrier and in neural injury, including cerebral malaria, a severe complication of Plasmodium falciparum infection. Prostaglandin (PG) $D_2$ is abundantly produced in the brain and regulates the sleep response. Moreover, $PGD_2$ is a potential factor derived from P. falciparum within erythrocytes. Heme oxygenase-1 (HO-1) is catalyzing enzyme in heme breakdown process to release iron, carbon monoxide, and biliverdin/bilirubin, and may influence iron supply to the P. falciparum parasites. Here, we showed that treatment of a human astrocyte cell line, CCF-STTG1, with $PGD_2$ significantly increased the expression levels of HO-1 mRNA by RT-PCR. Western blot analysis showed that $PGD_2$ treatment increased the level of HO-1 protein, in a dose- and time-dependent manner. Thus, $PGD_2$ may be involved in the pathogenesis of cerebral malaria by inducing HO-1 expression in malaria patients.
Advancements in the field of proteomics have provided great opportunities for the development of diagnostic and therapeutic tools against human diseases. In this study, we analyzed haptoglobin and amyloid A protein levels of vivax malaria patients with combinations of depletion of the abundant plasma proteins, 2-dimensional gel electrophoresis (2-DE), image analysis, and mass spectrometry in the plasma between normal healthy donors and vivax malaria patients. The results showed that the expression level of haptoglobin had become significantly lower or undetectable in the plasma of vivax malaria patients due to proteolytic cleavage when compared to healthy donors on 2-DE gels. Meanwhile, serum amyloid A protein was significantly increased in vivax malaria patient's plasma with high statistical values. These 2 proteins are common acute phase reactants and further large scale evaluation with a larger number of patient's will be necessary to establish the possible clinical meaning of the existential changes of these proteins in vivax malaria patients. However, our proteomic analysis suggests the feasible values of some plasma proteins, such as haptoglobin and serum amyloid A, as associating factor candidates for vivax malaria.
