Relationship of MicroRNA according to Immune Components of Breast Milk in Korean Lactating Mothers

  • Purpose: Human breast milk (HBM) contains immune components that produced and delivered from the mother along with nutrients necessary for the baby. MicroRNA (miRNA) is a small noncoding RNA molecule, that is used as an ideal biomarker for diagnosis and prognosis of various diseases and are more abundant in HBM. We analyzed and compared the immune components and miRNAs of HBM. Methods: HBM were collected from 20 healthy breastfeeding mothers. We measured the amount of lactoferrin, lysozyme, and immunoglobulin A (IgA) and extracted the miRNAs from each breast milk samples. Next, the top 5 and bottom 5 expressed miRNAs were compared and analyzed based on the amounts of the 3 immune components. Results: The mean levels and ranges of lactoferrin, lysozyme, and IgA were 6.33 (2.24-14.77)×106 ng/mL, 9.90 (1.42-17.59)×107 pg/mL, and 6.64 (0.48-20.01)×105 ng/mL, respectively. The miRNAs concentration per 1 mL of skim milk was 40.54 (14.95-110.01) ng/μL. Comparing the bottom 5 and top 5 groups of each immune component, 19 miRNAs were significantly upregulated (6, 9, and 4 targeting lactoferrin, lysozyme, and IgA, respectively) and 21 were significantly downregulated (4, 9, and 8 targeting lactoferrin, lysozyme, and IgA, respectively). There were no miRNAs that were expressed significantly higher or lower in common to all 3 components. However, 2 and 3 miRNAs were commonly overexpressed and underexpressed, in the top 5 groups of lysozyme and IgA concentrations. Conclusion: We identified the immune components and miRNAs in breast milk and found that each individual has different ingredients.

남녀 대학생의 일반적 특성에 따른 모유수유 관련지식에 관한 연구 (A Study on Knowledge on Breast Feeding of College Students by Their General Characteristics)

  • The breast feeding habits of 507 college students were evaluated between March 27 to April 26 (2006). Data were collected from self-administered questionnaires and analyzed using SPSS for Windows V.12.0. The study population consisted of 245 (48.3%) male and 262 (51.7%) female college students that answered ‘yes’ when asked whether they had been breast feed. A total of 52.7% of the study population were fed with breast milk (most frequent answer), as opposed to 11.0% that were fed with infant formula (least frequent answer) during their infancy. When asked whether they ever got educated on breast feeding, 78.1% of the college student subjects answered ‘no’. A total of 95.1% of the female college students replied ‘yes’ when asked if they intended to breast feed their child. Moreover, 62.2% of the college student subjects indicated that their parents had an influence on their decision to breast feed. A comparison between male and female college students indicated that female college students had a superior knowledge level of the general characteristics of breast feeding over the male subjects (p<0.05) . Moreover, a comparison of the different levels of college attained suggest that freshman college students had the highest knowledge level, followed by senior, sophomore and junior college students in terms of advantage, BF Tabu (p<0.05). Also, students provided with education on breast feeding had a higher knowledge level than college students with no formal education. In summary, the results suggest that the knowledge on breast feeding in college students were different by general characteristics such as gender, major and school year, and education on breast feeding in advance was appeared to be an important factor, therefore nutritional education course on breast feeding is recommended for the college students.

수유기간별 영아의 모유섭취량 및 Na, K의 섭취량 (Milk, Sodium and Potassium Intaken of Breastfed Infants During Lactation)

  • This study was to investigate the intake of human milk, sodium and potassium of breast-fed infants at 7, 15, 30, 60 and 90 days postpartum. Milk samples were obtained from 20 healthy lactating women living in In-Cheon area. Milk intakes of 20 breastfed infants were determined by the test-weighing method and soldium and potassium contents of human milk were analyzed by Atomic Absorptin Spectrophotometer after Wet-Digestion. Infant milk intakes per day tended to increase during lactation. The mean($\pm$SD) intakes at 7, 15, 30, 60 and 90 day were 460(164), 547(202), 626(199), 718(139) and 688(162)g/day, respectively. The sodium contents decreased significantly from 9.7(1.1)mEq/kg at 7 day to 6.3(1.8)mEq/kg at 90 day(p<0.05). The sodium intakes of infants were 100.8(39.6), 103.3(46.7), 107.2(47.9), 115.5(41.5) and 105.2(41.2)mg/day, respectively and average intake was 107.2(43.9)mg/day. The potassium contents were ranged from 10.5(1.3) to 13.3(2.3) mEq/kg and the potassium intakes of infants were 241.8(11.2), 267.8(98.8), 314.1(98.6), 318.0(66.1) and 276.1(62.6)mg/day, respectively and average intake was 288.3(91.8)mg/day. The mean sodium and potassium intakes of infants between boys and girls were not significantly different at 5% level.

한국 수유부 유즙의 콜린과 베타인 농도 및 영아의 콜린과 베타인 섭취량 (Choline and Betaine Concentrations in Breast Milk of Korean Lactating Women and the Choline and Betaine Intakes of Their Infants)

  • 모유 수유를 받는 영아는 생후 6개월까지 모유에 거의 모든 영양소 섭취를 의존하기 때문에 유즙의 콜린 농도가 영아의 콜린섭취에 영향을 미칠 수 있다. 식품에는 콜린과 베타인이 함께 들어 있고 체내에서 콜린은 베타인으로 산화되어 메칠기 운반에 관여하므로 식품속의 총 콜린 함량만이 아니라 베타인 함량도 함께 고려하여야 한다. 따라서 본 연구에서는 수유기간별 유즙의 총 콜린, 유리 콜린, 베 타인의 변화를 조사하고자 하였다. LC-MS방법을 사용하여 수유부 유즙의 콜린과 베타인 농도를 분석하여 영아의 콜린과 베타인의 섭취상태를 알아보고자 하였다. 1) 모유의 총 콜린 농도는 수유 첫 1개월째 145.03 mg/L (1.39 mmol/L), 2개월째 157.83 mg/L (1.52 mmol/L), 3개월째 165.99 mg/L (1.60 mmol/L), 4개월째 153.67 mg/L (1.48 mmol/L), 5개월째 145.05 mg/L (1.39 mmol/ L)이었다. 모유의 유리 콜린 농도는 수유 1개월 24.96 mg/L (0.239 mmol/L), 2개월 29.56 mg/L (0.284 mmol/L), 3개월 29.40 mg/L (0.283 mmol/L), 4개월 23.41 mg/L (0.225 mmol/L), 5개월 24.03 mg/L (0.231 mmol/L)로 수유월령에 따라 큰 차이는 없었다. 모유의 베타인 농도는 수유 1개월에 4.424 mg/L (0.038 mmol/L), 2개월 3.442 mg/L (0.033 mmol/L), 3개월 3.200 mg/L (0.031 mmol/L), 4개월 2.257 mg/L (0.022 mmol/L), 5개월 2.433 mg/L ($0.020\;{\mu}mol/L$)이었다. 2) 수유기간별 영아의 유즙 섭취량은 2개월에 771.1 mL/d, 3개월에 792.0 mL/d, 4개월에 796.5 mL/d, 5개월에 711.0 mL/d이었다. 3) 영아의 총 콜린 섭취량은 2개월 124.61 mg/d, 3개월 120.90 mg/d, 4개월 126.50 mg/d, 5개월 104.09 mg/d로 4개월까지는 큰 변화가 없었으나 생후 5개월에 크게 감소하는 경향이었다. 영아의 체중 kg당 총 콜린 섭취량은 2개월 19.76 mg/d, 3개월 16.67 mg/d, 4개월 16.14 mg/d, 5개월 12.80 mg/d이었다. 4) 영아의 유리콜린 섭취량은 2개월 24.15 mg/d, 3개월 21.57 mg/d, 4개월 19.35 mg/d, 5개월 17.10 mg/d이었고, 수유기간별로 유의적인 차이는 없었으나 점차 감소하는 경향이었다. 영아의 체중 kg당 유리콜린 섭취량은 2개월 3.85 mg/d, 3개월 2.97 mg/d, 4개월 2.47 mg/d, 5개월 2.08 mg/d이었다. 5) 영아의 베타인 섭취량은 2개월 3.12 mg/d, 3개월 2.82 mg/d, 4개월 2.10 mg/d, 5개월 1.83 mg/d로 유리콜린과 유사한 경향을 보였다. 영아의 체중 kg당 베타인 섭취량은 2개월 0.50 mg/ d, 3개월 0.39 mg/d, 4개월 0.27 mg/d, 5 개월 0.22 mg/d이었다. 6) 영아의 콜린 섭취량은 모유의 콜린농도와는 r = 0.982 (p = 0.000), 모유섭취량과는 r = 0.414 (p = 0.028)로 유의적인 양의 상관관계를 보였다. 이상의 연구결과로부터 한국인 수유부 유즙의 총 콜린농도와 유리콜린농도는 외국인 수유부의 유즙에 비해 결코 낮지 않은 수준이며, 이로 인해 영아의 콜린 섭취상태는 부족하지 않을 것으로 사료된다. 더욱이 영아의 총콜린 섭취량은 모유 섭취량보다 모유의 총콜린 농도와 더 강한 양의 상관관계를 나타냄으로써 모유의 콜린농도가 영아의 콜린 섭취량에 영향을 미칠 수 있을 가능성을 보였다.

일부 모유 영양아와 인공 영양아의 혈중 타우린 함량 비교 (Comparison of Plasma Taurine Levels in Some of Breast-fed Infants and Formula-fed Infants)

  • The propose of this study was to investigate taurine intake in formula-fed and breast-fed infants and to estimate the level of taurine of blood and urine in order to determine the requirement of taurine intake in infants. These results will be useful to suggest the guideline of requirement of taurine intake and may contribute toward the proper use of breast milk substitutes. Experimental groups were breast-fed infants (n=10) and formula-fed infants (n=10) of 20 normal delivery infants in general hospital. This study was longitudinal study from birth up to 16weeks (0 week, 4 weeks, 8 weeks, 12 weeks, 16 weeks). The items of test were anthropometry(weight, height, head circumference, chest circumference), intake of taurine, taurine level of blood and urine in breast-fed and formula-fed infants. There were no significant differences between breast-fed and formula-fed infants in weight, height, head and chest circumference. There is a need for future studies of exclusive infants with larger samples to determine which growth pattern should be considered as the norm. Taurine concentration of plasma and urine did not differ between breast-fed and formula-fed infants. Taurine intake recommendations for infants is about 30mg/day from this study. This data will be useful for production of human-like formula milk and suggestion of an index of selection of a consumer in taurine.

체지방 조직 및 초유중에 잔류되어 있는 다이옥신 함유도에 관한 연구 (The Concentrations and TEQ Levels of PCDFs and PCDDs in Human Adipose Tissue and First Breast Milk of Korean)

  • 우리 나라 사람들의 체지방 조직과 초유중에서, 다이옥신의 잔류 농도를 HRGC/HRMS를 이용하여 동위원소희석법으로 측정하였다. 체지방 조직 시료는 서울에 있는 모병원에서 채취하였으며, 체지방 조직에서의 PCDFs의 총농도와 TEQ 농도는 각각 5.812 pg/g, 1.485 pgTEQ/g 이었고, PCDDs의 총농도와 TEQ 농도는 각각 26.648 pg/g, 1.176 pgTEQ/g 이었다. 서울대학 병원으로부터 받은 산모 20명의 초유에 함유된 다이옥신의 잔류농도는 PCDFs의 총 농도가 7.019 pg/mL, TEQ 농도로 환산하면 0.177 pgTEQ/mL 이었고, PCDDs의 총 농도와 TEQ 농도는 각각 14.224 pg/mL, 0.693 pgTEQ/mL이었다. 체지방 조직과 모유에서의 다이옥신 이성질체들의 분포는 PCDDs가 PCDFs보다 상대적으로 높게 나타났으며, 그중 OCDD가 가장 높은 농도로 포함되어 있다.

도시 저소득층 지역의 모자 영양 및 섭식에 관한 생태학적 연구 -II. 수유부의 인체계측, 식이 섭취, 모유 성분 함량 및 혈청 지질/지방산 조성- (Ecological Studies of Maternal-Infant Nutrition and Feeding in Urban Low Income Areas -II.   • This study was aimed at evaluating the nutritional status of Korean lactating women, who were attending peripheral community clinics in low-income areas, by anthropometric measurement, estimating dietary intakes and analyzing the contents of breast milk components, serum lipid content and fatty acid composition. Regarding anghropometric measurements, maternal circumferences decreased significantly at the trunk but not at the limbs. The skinfold thickness on all regions had the tendency to decrease during lactation. It was found that intakes of energy, protein, lipid and carbohydrates were desirable while calcium, zinc, copper and vitamin A were low and sodium was excessive. The energy $\%$of fat was 22.41$\%$of the total energy which is higher than the RDA for adults but it is proper for lactating women, but energy $\%$ from each fatty acid was usually low and an intake ratio of $\omega$6/$\omega$3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Concentrations of the nutrients in matured human milk showed the same range as that reported form different regions with the exception of calcium and magnesium which were relatively low. Serum lipid contents were decreased with the time postpartum and the average serum lipid content of lactating women were similar to the normal range of adults. Serum polyunsaturated fatty acids. especially $\omega$6 fatty acids have gradually increased with the time postpartum, but the composition were not greatly influenced by dietary intakes.

Breast Milk-Transmitted Cytomegalovirus Infection in Preterm Infants

  • Purpose: The purpose of this study is to describe the rate of cytomegalovirus (CMV) virolactia, and the prevalence of breast milk (BM)-transmitted postnatal CMV infection among premature infants after freeze-thawing (FT) and Holder pasteurization (HP) of breast milk. Methods: This is a single-center, retrospective study of 312 infants born at less than 32 weeks of gestation, or with a birth weight less than 1,500 g from January 2013 to June 2017. All infants were screened for CMV-specific immunoglobulin (Ig) G and IgM at birth. Initial CMV specific polymerase chain reaction (PCR) and CMV culture were performed on mothers' BM and babies' urine within the first 21 days of life. FT and HP of BM was used to prevent the transmission of CMV. For the surveillance of postnatal CMV infection, CMV culture and CMV specific PCR of urine from babies were repeated one to two months after the initial screening. Screening for viremia and viruria was performed if postnatal CMV infection was suspected. Results: Among 178 BM samples obtained from mothers of CMV-IgG-seropositive infants, 80 (44.9%) were CMV PCR positive. CMV deoxyribonucleic acid (DNA) was detected in five of the 22 BM samples (22.7%) obtained from the mothers of CMV-IgG seronegative infants. When CMV DNA load in BM was measured before and after HP, various results were shown. Sixty-three infants out of 232 (27.2%) were evaluated for postnatal CMV infection and four infants out of 63 (6.3%) were infected. Conclusion: Interventions to prevent BM-transmitted CMV infection can reduce the chance of postnatal CMV infection, but not completely eliminate it.

Anti-inflammatory effects of Lactobacillus reuteri LM1071 via MAP kinase pathway in IL-1β-induced HT-29 cells

  • Lactic acid bacteria are well-known probiotics, conferring several health benefits. In this study, we isolated lactobacilli from human breast milk and identified Lactobacillus reuteri LM1071 (RR-LM1071) using 16S rDNA sequencing. We tested the hemolytic activity, biogenic amine production, and antibiotic susceptibility of this strain to assess its safety. RR-LM1071 was found to be negative for hemolytic activity and biogenic amine production, as well as was measured in susceptible level for antibiotics in the minimal inhibitory concentration (MIC) test. The adhesive properties of RR-LM1071 were higher than those of LGG in HT-29 cells, and showed a greater hydrophobicity than LGG in hexadecane solvent. Under inflammatory conditions, RR-LM1071 suppressed the mRNA expression of IL-6, TNF-α, and IL-4 produced in IL-1β-induced HT-29 cells. Our results suggest that RR-LM1071 is a safe and valuable probiotic that can be used for the treatment of inflammatory bowel disease.
