An Analysis on Treatment Schedule of Carbon Ion Therapy to Early Stage Lung Cancer

  • A total of 134 patients with stage 1 of non-small cell lung cancer treated by carbon ion beam of HIMAC NIRS were investigated for control rate and delivered dose. The delivered dose of every patient was converted to biological effective dose (BED) of LQ model using fraction number, dose per fraction and alpha beta ratio which shows the maximum correlation between BED and tumor control. The BED of every patient was classified to establish a BED response curve for control. Assuming fraction numbers, dose response curves were introduced from BED response curve. The total doses to realize several control rates were obtained for the treatment of small fraction number.

  • The existing six-bed patient rooms, general type of multiple-bed rooms in Korea causes many problems for the amenity of patients. We should reconsider their inconvenience carefully and try to provide the right to keep their privacy and enjoy amenity. The number of patients of multi-bed rooms is very critical point to improve the environmental condition of the patient rooms. So This paper aims to provide the possibilities of four-bed rooms through the comparison and analysis of six and four-bed room for planning and design.

Analysis of charges per case by hospital characteristics - In regard to acute appendicitis and NSVD - (병원 특성에 따른 건당 진료비 분석 - 급성충수염과 정상분만을 대상으로 -)

  • To identify the factors influencing the charges per case of acute appendicitis and normal spontaneous vaginal delivery (NSVD), the personal data-base files and hospital-characteristics-reporting data files of Korea Medical Insurance Corporation were analyzed. One hundred and twenty-nine institutions were selected. The results of this study were as follows 1. The differences of charges per case with respect to hospital ownership, location, and equipment levels were statistically significant. 2. The results of multiple regression analysis revealed that bed capacity was the most significant variable in both diseases. 3. Ownership was significant variable in acute appendicitis. In NSVD, ownership and hospital equipment level were statistically significant. In conclusion, bed capacity was statistically the most significant variable in the analysis of charages per case. And we thought that the results of this study would influence the policy of the hospital bed supply.

Effect of Bed Side Exercising on Back Pain and Bleeding Complications after Transcatheter Arterial Chemoembolization (간동맥 화학색전술 후 절대안정기간 동안의 침상운동요법이 요통과 출혈합병증에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of bed side exercising on back pain and bleeding during absolute bed rest in patients who had received transcatheter arterial chemoembolization (TACE). Methods: A nonequivalent control group pretest-posttest design was used. A total 46 patients were sampled from a gastrointestinal unit of a urban general hospital in Seoul. The control group received 8 hours of bed rest and conservative care. The experimental group received 8 hours of bed rest and bed side exercising every one hour from the time having absolute bed rest for 3 hours after TACE. Results: The experimental group with bed side exercising experienced significantly less back pain compared to the control group. There was no significant difference in the incidence of bleeding complications between two groups. Conclusion: The results indicate that a bed side exercising is associated with a reduction of back pain and with no increased risk of bleeding complications in patients after TACE.

The Patient Days and Medical Care Benefits of Finger-Amputated Patients due to Industrial Accident by Hospital and Patient Characteristics (병원 및 환자특성에 따른 수지절단 산업재해환자의 요양기간과 진료비)

  • This study was conducted to analyze patient days and medical care benefits of finger-amputated patients due to industrial accident. The 161 personal data on medical care for finger-amputated patients due to industrial accident(88 in 1994, 73 in 1995) of Regional Labor Office and hospital characteristics were analyzed. The major results of this study were as follows : According to stepwise multiple regression analysis of patient days, number of amputated finger, location of hospital, bed capacity of hospital, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, sick leave benefits per day were the major significant variables in order. In stepwise multiple regression analysis with medical care benefits as a dependent variable, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, age, bed capacity of hospital were the major significant variables in order. The minimum optimal size with the lowest medical care benefits was a hospital with 300 beds. This shows that the economy of scale is also applicable for hospital industry. In summary, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, bed capacity of hospital were the major significant variables in both patient days and medical care benefits.

Fingertip reconstruction with a subcutaneous flap and composite graft composed of nail bed and volar pulp skin

  • Background Fingertip injuries are very common; however, the reconstruction of volar pulp defects with nail bed defects is challenging in the absence of the amputated segment. We reconstructed fingertip amputations with nail bed defects using a new surgical approach: a subcutaneous flap and composite graft. Methods We treated 10 fingertip amputation patients without an amputated segment, with exposed distal phalangeal bone and full-thickness nail bed defects between February 2018 and December 2020. All patients underwent two-stage surgery: in the first stage, a subcutaneous flap was performed to cover the exposed distal phalanx, and in the second stage, a composite graft, consisting of nail bed, hyponychium, and volar pulp skin, was applied over the subcutaneous flap. Results All flaps survived and all composite grafts were successful. The wounds healed without any significant complications, including the donor site. The average follow-up duration was 11.2 months (range, 3-27 months). The new nail and the shape of the volar pulp were evaluated during follow-up. All patients were satisfied with their natural fingertip shapes and the new nails did not have any serious deformities. Conclusions A subcutaneous flap in combination with a composite graft fitting the shape of the defect could be another option for fingertip injuries without amputated segments.

Cost Behaviors and Cost Structure of Public Hospitals in India: Analysis from the Perspective of Congestion Costs

  • The goal of this study is to understand better the relationship between hospital bed occupancy rate and cost rigidity as a proxy for the degree of hospital bed congestion, as well as the relationship between the risk of changes in hospital bed occupancy rate and congestion cost, targeting public hospitals. As public hospitals for analysis, we selected hospital projects from the Public Enterprises Survey Reports published by the Department of Public Enterprises, Ministry of Finance, and obtained unbalanced panel data consisting of 1,505 hospitals and 15 years, totaling 12,595 hospitals and years. The analysis revealed that the risk of changes in the bed occupancy rate increases the degree of cost rigidity and leads to a decrease in the variable cost ratio; furthermore, an increase in the bed occupancy rate decreases the degree of cost rigidity and leads to an increase in the variable cost ratio. These findings suggest that although public hospitals are taking managerial actions to avoid congestion costs, congestion costs resulting from higher bed occupancy rates have not been eliminated. The regression analysis results show that even if congestion costs arise as the occupancy rate increases, they are covered by the increase in revenue associated with the increase in the occupancy rate.

A Study on the Planning of the Four-bed room Unit in Public Hospitals (공공병원 4인병실 유니트의 건축계획에 관한 연구)

  • Purpose: This paper tries to set up the design guidelines for the 4 bedroom in Public Hospital wards as the standard multi-bed room in order to upgrade the bedroom environment, increase inpatients' and nurses' satisfaction level, and decrease cross infection possibility. Methods: Literature survey containing legal standards related to hospital bedrooms in Korea, Japan and Germany have been conducted. Questionnaires have been distributed to inpatients and nurses in 39 Public Hospital, and face to face talk with ward nurses in 9 Public Hospitals have been conducted. Thereafter 10 architectural drawings, 28 questionnaires from nurses and 544 questionnaires from inpatients have been analysed. Results: This article suggests some design guidelines and example drawings of standard 4 bedroom unit in Public Hospital. Implications: The result of this research would be useful as a reference when the architect tries to design a Four-bed room unit in Hospitals.

The Fitness and Organizational performance Analysis of Hospital Specialization Strategy Types (병원전문화 전략유형의 적합성과 조직성과 분석)

  • This research classified hospital specialization strategy types through cluster analysis, analyzed fitness of hospital specialization strategy types for external environment or organizational structure, and examined relation between hospital specialization strategy types and organizational performance. This research surveyed 1,437 hospitals which have more than 30 patient's bed and practice national health service in Korea. Specifically, this research divided into two part : external fit - analysis of relation between external environment and specialization strategy, internal fit-analysis of relation between organizational structure factors and specialization strategy. also, as the organizational performance for achieving specialization strategic purpose, not only the productivity, efficiency, profit but also the medical quality was considered. In case of external fit, many hospitals chose integration type if there are a lot of competitive hospitals and regional population. Particularly, if there are many competitive hospitals, concentration type is chosen. In contrast, if there are many doctors in the region, differentiation type is chosen. In case of internal fit, according to organization type and patient's bed number, hospitals chose different types. If it is a general hospital and has a few bed number, generalization or concentration type is chosen. Tertiary hospital or the hospital with many patient's bed chose differentiation type. According to the number of specialists, if there are a few specialists, generalization or concentration type is chosen. If there are many specialists, differentiation type has high fitness for the hospital. In relation to strategy types and organizational performance, differentiation type has best result. Differentiation type has a good result in 7 items out of 11.

Evaluation on Safety of Two-bed Therapy Rooms (2인용 치료병실 안전성 평가)

  • Purpose: Europe and U.S use multi-bed therapy rooms. Hereupon, this study aims to examine the safety when current one-bed therapy rooms in Seoul National University Hospital is changed into two-bed ones. Materials and Methods: This study evaluated external exposure by gamma radiation emitted from other patients and internal and external exposure caused by pollutions from other patients in case that Seoul National University Hospital installs a shielding wall between beds in existing therapy rooms. Results: When internal and external exposure was evaluated to evaluate safety of two-bed hospital rooms, 'isolation amount of patients' 5mSv exposure or below is received according to the Atomic Energy Act. Conclusion: With the increasing number of patients with thyoid cancer, patients using therapy rooms are on the rise. Therefore, improving one-person therapy rooms to two-person ones in line with international trend would increase cost reduction and management efficiency, and patients' alienation and isolation can be reduced to increase healing effects.
