A Study of community diagnosis activity by Community Health Nurse Working in Health Centers (보건소 보건간호사의 지역사회 진단활동에 관한 조사연구)

  • An important role of community health nurses in health centers is to solve community health problems found through data collection methodology which has been used to identify the health needs of the community, diagnose the health problems and to plan health programs suitable for the health problems. Also community health nurses must be prepared to know the community health needs and to participate in the planning process. Since 1956 when the health center law was established, community health nurses have really implemented only the services which the government has asked them to do. This has kept them busy enough. But these days as society is in rapid change, community health nurses should have the flexibility to deal with the social change and demands that are unique to their community each which has different health needs and demands. So community health nurses need to identify what community health problems exist in their particular communities. The purposes of this study were as follows. 1) To explore the suitability of the health programs which the government has asked the community health nurses to do for their own communities and if these programs are not suitable, to explore the reasons why. 2) To explore the degree to which the community health nurses have the ability to identify health problems in their own communities and activate the community diagnostic process. 3) To identify the degree that the community health nurses have the ability to implement plans related to community diagnosis. 4) To find out how much data related to community health problems, the community health nurses have and how they are utilizing it. 5) To measure the community health nurses self-confidence concerning diagnostic activities for community health. The study subjects were 454 Community Health Nurses working in Health Centers in Seoul, Korea. The period of data collection was 6 days(Nov. 9th 1991-Nov. 15th 1991). A questionnaire used for data collection was composed of three different items; general characteristics, community health diagnostic activities and self-confidence in performing diagnostic activities. The results of the study are as follows. First, over one third of the respondents replied that the government required activities for their communities are not appropriate. Of these activities the most frequent reply $(51.2\%)$ indicated that many of the activities in the community were inappropriate to the actual situation. Further, $25\%$ of the replies indicated that many activities were only administratively oriented and as such not appropriate. Second, $49.8\%$ of the respondents replied that they had done general assessments and had a general idea of the health problems of their community. Effective solutions to health problems could be found with an increase in health personnel and management ability according to $41.5\%$ of the respondents. Third, to the question as to whether they had ever independently implemented a plan towards solving community diagnosed problems, $52\%$ of nurses replied 'never', $40\%$ 'occasionally' but only $7.5\%$ replied that they did it frequently. Actually there was very little done even in the basic work of collecting the necessary data. Fourth, when asked how much of basic information they had collected that might be used in community diagnosis activity, of 26 items in 5 areas, there was hardly one for which complete data had been collected. Fifteen percent did have data on the geographical aspects of their area, housing distribution and types of housing, while $17.8\%$ knew the frequency with which the health center was used. Concerning community resources, even with a list of community resources, only $12.3\%$ had data on any of these resources, and this data was incomplete. Further, information about social work institutions, and facilities was also incomplete, only $14.2\%$ of the respondents had any data and even it was incomplete; that is, in general, the nurses did not have this information. Fifth, concerning the confidence of the community health nurse in their ability to carry out community diagnoses activities, $60\%$ replied that they were very or at least nominally confident, indicating that although they were not doing community diagnostic activities they felt they could do so, as they were carrying out home visits and program planning as part of their official duties. The following recommendations are made based on the results of this study. First; since the community health nurses have a high perception of the need for community diagnostic activities and. high confidence in their ability to carry out this activity and high percentage of respondents replied that with a little training they could do this even better it is recommended that community diagnostic activity training be included in the continuing education program for community health nurses. Second, in order for the Community Health Nurses to successfully solve the health problems of their respective community they reported to a need to increase the number of health personnel, improve the facilities and the system of managing their work. Considering this, it is recommended that ways be sought to remedy these deficits.

Customer' Perceptions of Motivators, Barriers, and Expansion of Menu Labeling in Restaurants (외식 영양표시 제도에 대한 소비자의 사용동기, 장애요인과 확대 실시에 대한 인식)

  • Restaurants implement menu labeling to provide nutritional information to customers in an attempt to help customers select healthy menu items. Considering the increase in food-away-from-home consumption, the purpose of this study was to identify motivators and barriers in restaurant customers regarding use of menu labeling. Data were collected from a survey on restaurant customers in Seoul, Korea. The findings of this study indicate that customers used menu labeling for health reasons. However, barriers to using menu labeling were identified as small font size, difficulty in locating nutritional information display, and difficulty in interpreting nutritional information. In addition, they also suggested expanding the scope of menu labeling for restaurants by including chain restaurants with less than 100 units. The findings of this study offer strategies for the government to improve menu labeling practices for customers.

Causal Effects on Attitude toward Retirement and the Level of Retirement Planning (은퇴태도와 은퇴계획수준의 인과관계에 관한 연구)

  • The purpose of this study was to analyze the affecting factors on male salary workers' level of retirement planning. The focus was on the causal effects on salary workers' attitude toward retirement and their level of retirement planning. The major findings were as follows.: First, the salary worker's attitude toward retirement was neutral, neither positive nor negative, and they estimated their level of retirement planning relatively high. Second, the major factors affecting on attitude toward retirement were salary workers' age, their current economic status, and future perspectives for the post-retirement life. Third, the major affecting factors on level of retirement planning were salary workers' age, health status, and level of self-esteem, household net asset, savings and investment for elderly life, and attitude toward retirement. forth, the causal effects on the level of retirement planning was verified, and attitude toward retirement had direct effect as well as indirect effect on the level of retirement planning. From the findings, it can be concluded that the salary workers' attitude toward retirement played a important role in the process of retirement planning.

A Survey on Pain and Self-Care Behavior of Patients with Chronic Arthritis (만성 관절염 환자의 통증과 자기간호행위 관련요인)

  • Purpose: This study was designed to explore pain and self-care behaviors and identify related variables in patients with chronic arthritis. Method: One hundred fifty patients with arthritis were recruited from two university based arthritis centers according to selection criteria. Collected data were analyzed using the SAS program to analyze the responses to the structured questionnaires of the study. Result: Most of the participants expressed pain and the intensity of the pain was moderate. There were significant differences according to age, educational level, diagnosis, duration of illness, number of affected joint, and use of complementary therapy. Self-care behavior scores were moderately high. The highest practice was for 'regular visits to the hospital', and the lowest for 'applying physical therapy at home'. The mean self-care behavior scores showed significant differences according to economic status and educational level. Pain scores showed no correlation with self-care behavior. Conclusion: Developing self-management programs for patients with chronic arthritis should focus on self-care skills which are applicable in the relief of pain and enhancement of knowledge. The skills are recommended not only for better health practices but also for enhancing the level of well-being and life satisfaction.

Aggregate Risk Assessment on Xylene and Ethylbenzene (자일렌과 에틸벤젠에 대한 매체통합위해성평가 연구)

  • The aggregate risk assessment on xylene and ethylbenzene was carried out according to the guidance established newly in 2010 with the purpose of providing information for risk management. In human exposure assessment, the results indicated that lower ages were exposed more and that, in the interior space at home, the highest level of human exposure occurred via inhalation. At outdoor spaces, exposures via inhalation and drinking were less than 1%. In human health risk characterization, xylene showed HI(Hazard Index) < 1 in all ages. When reasonable maximum exposure(RME) was applied, HI for young children was 0.64. The HI of ethylbenzene was also below 1(0.02~0.04) in all ages, indicating no potential risk. From this study, it is considered that xylene need to be continous monitoring with interest because this substance may be more sensitive on young age group. In additon, to reduce the uncertainty of the risk assessment, the korean exposure factors on young age group such as infant, children had to be established as soon as possible.

The Structural Relationship among Social Support, Parenting Stress, Self-perception and Parenting Behavior Perceived by Mother (어머니가 지각하는 사회적 지지와 양육스트레스, 자아인식 및 양육행동 간의 구조적 관계)

  • The purpose of this study was to explore structural relationships among social support, parenting stress, self-perception and parenting behavior perceived by mother and provide preliminary data useful for desirable parenting behavior. For this purpose, the data of fourth wave Panel Study on Korean Children(PSKC) including social support, parenting stress, self-perception and parenting behavior measured by 1,746 mothers with 3-year-old children were analyzed. We identified structural relationships among the variables using SPSS 18.0 and AMOS 18.0 applying structural equation modeling. Measurement model and structure model had favorable goodness of fit and the results of structure models on each path were as follows. First, parenting behavior had positive correlations with social support and self-perception but there was a negative correlation between parenting behavior and parenting stress. Second, the relationship between social support and self-perception was mediated by parenting stress and parenting stress and self-perception mediated the relationship between social support and parenting relationship. In conclusion it is required to raise awareness about the importance of development of various parent education programs and parenting behavior.

The Development of a Life Coaching Educational Program at the Healthy Family Support Center (건강가정지원센터에서 활용할 수 있는 라이프코칭교육프로그램 개발에 관한 연구)

  • The essence of family health is to make the family stronger, which can be found in the philosophy of coaching that focuses on the individual's own change and development. In this context, this study aims to develop a coaching educational program at the Healthy Family Support Center. The educational program, 'The Happiness Balloon in My Mind' consists of 4 sessions, which are 'Perceiving the Balloon', 'Making the Balloon', 'Blowing up the Balloon', and 'Flying the Balloon'. Through the questionnaires that included open and closed questions, the effectiveness of the program was evaluated. The educators replied that their expectation to the program was met and they were highly satisfied with the program. They reported that the program brought the a change in life, elly, in their own consciousness. Even though the program would need to adjust its level of difficulty and scheduling, the results suggest that coaching educational program showsits own plausibility to the Healthy Family Support Center.

The Elderly Willingness to Improve Residential Environment (노인의 주거공간 개조의사에 관한 연구)

  • This study is focused on the residential spaces that the elderly wish to renovate and their willingness to improve residential environment as they grow old. The analysis shows, first, the most inconvenient places as they grow old are the stairs, following by the toilet and the bath. Second, the place that the elderly most wish to renovate is the floor, upgrading floors with non-slippery materials. And they wish to remove the height differences on the floor to protect them from tripping over, upgrade the floor plan for their convenience and renovate the storage and working spaces at the kitchen and dining areas. The more the government supports the costs of the residential renovation service, the more the elderly are willing to use the renovation service. The factors influencing their willingness to residential renovation are health, residential environment, experience to accidents and their allowances. The elderly who experienced accidents inside their residence are more willing to renovate their house. Even though the elderly wish to renovate their houses, it's not carried out due to the financial difficulties and the lack of information. Thus, the government should share the useful information about residential renovation for the safety of the elderly and find a way to raise money to cover the costs of the renovation.

Time Poverty and Quality of Life in Dual-Earner Families with Preschool Children: A Comparison between Time-Poor and Non-Time-Poor Groups (미취학 자녀를 둔 맞벌이 가정의 시간빈곤 수준과 삶의 질: 개인유지시간을 기준으로 한 시간빈곤 여부에 따른 집단 간 비교)

  • This study analyzes diverse factors in time poverty and quality of life in dual-earner families with preschool children that pertain to the individual, family, and occupation. Data were taken from the 17th edition of the Korean Labor and Income Panel Study developed by the Korea Labor Institute in 2014. The sample consists of 826 households who are dual-earner families with preschool children. The major findings are as follows. First, this study identified inadequacies in personal care time for dual-earner families with preschool children. Second, the results show that gender, recognition of gender role, and overall satisfaction of occupation are related to the time poverty of dual-earner families. Men are more likely to experience time poverty than women, and equal recognition of gender role and satisfaction of occupation indicate a negative relation on the time poverty of dual-earner families with preschool children. Last, quality of life in non-time-poor groups is higher than for groups who experience time poverty. Also, health state, earned income, work-family life conflict, and overall satisfaction of occupation are commonly related to quality of life in both groups. The results suggest implications for comprehensive policies to address family time issues.

  • The purpose of this study was to compare the factors affected on the leisure participation leisure satisfaction and leisure effect of the middle-aged with those of the aged who were paticipated in the leisure program of the public community facilities. the sample of he study consisted of 218 midle-aged and 163 the aged living in Seoul and Daegu. Frequencies means ,$x^2$,T-test and multiple regression were employed for the analysis. The results of this study were as follows; The middle-aged had more positive leisure attitude than the gaged less leisure participation and more leisure satisfaction. The factors affected one he leisure participation of the middle-aged were region and fee and those of the aged were region leisure attitude and fee. The factors affected on the leisure satisfaction of the middle-aged were number of the family region and the access of the transportation as compared with those of the aged were sex health status and the access of transportation. And the factors had effects on the leisure effect of the middle-aged were sex educational attainment number of the family the access of transportation and fee comparing with those of the aged were educational attainment and leisure attitude.
