Prevalence of Human Papillomavirus Infection and Genotype Distribution Determined via Real-Time PCR in a Korean Medical Check-up Population

  • Human papillomavirus (HPV) plays a critical role in the development of cervical carcinoma. This study analyzed the efficiency of multiplex real-time PCR in detecting and identifying HPV genotypes in samples from women who visited a Korean hospital for checkups. Cervical swab specimens were obtained from women who attended a checkup at the Health Improvement Center of Hospital in Dankook University Cheonan, South Korea and were referred for an HPV genotyping test between January and September 2014. A total of 1703 cervical swab specimens were collected consecutively during this period. PCR results were compared with those of the traditional cytological assay for the same population. Among the 1,703 specimens, 19.91% were HPV positive, of which 14.50% indicated a single infection and 5.40% indicated multiple infections. However, cytology identified only 2.52% of positive cases, including 1.23% cases of atypical squamous cells of undetermined significance, 1% of low grade squamous intra-epithelial lesion, and 0.29% of high grade squamous intra-epithelial lesion. The rate of high-risk and low-risk HPV in the abnormal cytology group was 48 and 23, respectively, and 274 and 136 in the normal group, respectively. HPV types 56, 52, 43 were the most prevalent in that order. Our results confirm the efficiency of the HPV DNA assay for the detection of 28 different HPV genotypes with reasonable sensitivity. A screening strategy that comprises the HPV DNA assay and cytology would help overcome the low sensitivity of a cytological diagnosis.

Outdoor Workers' Use of Sun Protection at Work and Leisure

  • Background: Outdoor workers are at risk of high ultraviolet radiation exposure, and may have difficulty using sun protection. The objectives were to determine the prevalence of sun protection behaviors in a sample of outdoor construction workers, and to assess which factors predict better sun protection practices. Methods: Participants were recruited via construction unions. Workers answered a questionnaire on demographics, skin cancer risk, sun protection behaviors, and job. Sun protection behavior scores (from questions on sunscreen use, sleeved shirt, hat, shade seeking, sunglasses) were calculated by converting Likert-scale answers to scores from 0 to 4, and taking the mean (separately for work and leisure). Determinants of sun protection behavior scores were examined for work and leisure using generalized linear models. Results: Seventy-seven workers had complete questionnaire data (participation 98%). Sun protection behaviors used most often were hats (79% often/always) and sleeved shirts (82% often/always); least prevalent were shade-seeking (8% often/always) and sunscreen (29% often/always). For both work and leisure scores, the strongest predictor was skin type, with fairer-skinned individuals having higher sun protection behavior scores. Workers had higher scores at work than on weekends. Workplaces that required hats and sleeved shirts for safety purposes had higher protection behavior scores. Conclusion: This high-participation rate cohort helps characterize sun protection behaviors among outdoor workers. Workers practiced better sun protection at work than on weekends, suggesting that workplace policies supportive of sun protection could be useful for skin cancer prevention in the construction industry.

Cancer Registration in Basrah-Southern Iraq: Validation by Household Survey

  • On an international scale, the burden of cancer in absolute numbers continues to increase, mainly due to aging of population in many countries, the overall growth of the world population, changing lifestyle with increasing cancer-causing behavior, like cigarette smoking, changing dietary habits and sedentary life. Cancer is the second-leading cause of death and disability in the world, after only heart disease. Recently, increasing incidence and mortality of cancer have also become evident in the developing world. In Iraq and particularly in Basrah in the southern part of the country, the burden has definitely increased and deserves extensive research. The present paper is part of an extensive household survey carried out in Basrah in 2013. Among the objectives was to validate official cancer registration in the governorate. The cross-sectional survey had a retrospective component to inquire about the incidence of cancer and cancer-related deaths during the three years preceding the date of inquiry (2010-2012). A convenient sample of 6,999 households with 40,688 inhabitants using multistage cluster sampling was surveyed involving all urban and rural areas of Basrah. The official cancer registration activities in Basrah seemed to have attained a high level of registration coverage (70-80%) but the gap, represented by missed cases, is still high enough to criticize the system. Most of the missing cases were either not notified by treating facilities or they were diagnosed and treated outside Basrah. Using a set of parameters, the pattern of cancer was consistent based on data of the household survey and data of the cancer registry but a gap still existed in the coverage of incident cancer and mortality by cancer registration. Integrated serious steps are required to contain the risk of cancer and its burden on the patient through improving the registration process, improving early detection, diagnostic and management capabilities and encouraging scientific research to explore the hidden risk factors and possible causes of low registration coverage. Periodic household surveys seemed feasible and essential to support routine registration.

Clinical Validation of a Protein Biomarker Panel for Non-Small Cell Lung Cancer

  • We validated the diagnostic performance of a previously developed blood-based 7-protein biomarker panel, $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc., Pohang, Korea) using modified aptamer-based proteomic technology for lung cancer detection. Non-small cell lung cancer (NSCLC), 200 patients and benign nodule controls, 200 participants were enrolled. In a high-risk population corresponding to ${\geq}55years$ of age and ${\geq}30pack-years$, the diagnostic performance was improved, showing 73.3% sensitivity and 90.5% specificity with an area under the curve of 0.88. $AptoDetect^{TM}$-Lung (Aptamer Sciences Inc.) offers the best validated performance to discriminate NSCLC from benign nodule controls in a high-risk population and could play a complementary role in lung cancer screening.

Molecular evolutionary analysis reveals Arctic-like rabies viruses evolved and dispersed independently in North and South Asia

  • Background: Arctic-like (AL) lineages of rabies viruses (RABVs) remains endemic in some Arctic and Asia countries. However, their evolutionary dynamics are largely unappreciated. Objectives: We attempted to estimate the evolutionary history, geographic origin and spread of the Arctic-related RABVs. Methods: Full length or partial sequences of the N and G genes were used to infer the evolutionary aspects of AL RABVs by Bayesian evolutionary analysis. Results: The most recent common ancestor (tMRCA) of the current Arctic and AL RABVs emerged in the 1830s and evolved independently after diversification. Population demographic analysis indicated that the viruses experienced gradual growth followed by a sudden decrease in its population size from the mid-1980s to approximately 2000. Genetic flow patterns among the regions reveal a high geographic correlation in AL RABVs transmission. Discrete phylogeography suggests that the geographic origin of the AL RABVs was in east Russia in approximately the 1830s. The ancestral AL RABV then diversified and immigrated to the countries in Northeast Asia, while the viruses in South Asia were dispersed to the neighboring regions from India. The N and G genes of RABVs in both clades sustained high levels of purifying selection, and the positive selection sites were mainly found on the C-terminus of the G gene. Conclusions: The current AL RABVs circulating in South and North Asia evolved and dispersed independently.

  • 감염병의 팬데믹 상황 속에서, 국가의 방역 대책은 안보로서의 속성을 지니며, 공중보건과 공공의 이익의 이름으로 개인의 자유에 대한 일정한 제한이 정당화되어왔다. 2020년 3월, 대한민국 국회는 「감염병의 예방 및 관리에 관한 법률」 개정안을 통과시켰으며, 이를 통해 '감염의심자'의 검사 및 격리거부에 대한 처벌의 법적 근거를 신설하고 격리위반과 치료거부의 벌칙을 상향하였다. 본 논문에서는 국가가 개인의 자유를 제한하는 행위의 정당성 판별기준에 대한 자유주의 법철학의 논변과 원리들을 검토하고, 피해자임과 동시에 매개체로서의 속성을 지니는 감염병 환자(patient as victim and vector)에 대한 자유제한원리의 적용은 파인버그(Joel Feinberg)가 제시한 '스스로에 대한 해악(harm to self)'과 '타인에 대한 해악(harm to others)'이 중첩되는 지점에 있음을 개념화하였다. 파인버그가 제기한 자유제한원리(liberty-limiting principle)를 불확실성(uncertainty)을 지니는 팬데믹 상황에 적용하기 위해서는, 해악에서 리스크(risk)로 해악의 원리를 확장시킬 것이 요구된다. 이러한 해악에서 리스크로의 전환은, 불확실한 위기상황 하에서 국가가 사전주의 원칙(precautionary principle)을 통해 개인의 자유를 사전적으로 제한하는 것을 정당화함과 동시에, 충분한 근거 없이 개인의 행위를 처벌의 대상으로 삼는 과잉범죄화(overcriminalization)의 우려를 낳는다. 본 글에서는 리스크를 지닌 개인에 대한 사전적 자유제한을 둘러싼 사전주의의 원칙과 과잉범죄화의 우려 사이에서 균형을 이룰 수 있는 원칙들을 검토한다. 이어서 '타인에 대한 해악' 원칙이 공익과 공중보건 상황에 적용되기 위한 두 번째 확장으로, 인구집단 개념으로의 전환을 다룬다. 팬데믹과 같은 공중보건 위기 상황에서는 '개인'이 아닌 '인구집단'을 하나의 단위로 고려하는 인구집단 접근법(population approach)이 필요하며, 나아가 앞선 두 논의를 결합한 '인구집단에 대한 리스크(risk to population)'가 팬데믹 상황에서 해악의 원리의 중요한 구성요소로 고려되어야 함을 제안한다. 논문의 마지막에서는, 앞서 개념화한 '확장된 해악의 원리' 하에서 개정 「감염병의 예방 및 관리에 관한 법률」의 자유제한이 정당화될 수 있는지 검토한다. 격리위반 처벌조항은 '인구집단에 대한 리스크'에 대한 자유제한에 해당하여, 강제검사 또한 무증상 감염자라는 감염병의 특성에 의거하여 '확장된 해악의 원리'의 차원에서는 정당성이 부정되지 않음을 보일 수 있었다. 그러나 치료거부 처벌조항은 전통적 해악의 원리뿐만 아니라 '인구집단에 대한 리스크'라는 팬데믹의 특성을 고려한 '확장된 해악의 원리' 하에서도 정당화되기 어려우며, 추가적 단서조항을 포함하여야만 정당화 근거를 획득할 수 있을 것임을 논증하였다.

신생아 발열 환자에서 중증 감염의 예측 인자 (Predictors of Serious Bacterial and Viral Infections among Neonates with Fever)

  • 목 적 : 본 연구는 발열로 입원한 신생아 중 임상 경과가 불량했던 환자의 특징을 조사하고 입원 후 경과가 양호했던 환자와의 임상증상, 이학적 진찰 소견과 검사 결과를 비교하여 중증 감염의 가능성을 예측하는 척도로 이용할 수 있는지 평가하였다. 방 법 : 2000년부터 2006년까지 입원한 30일 이하의 만삭아 중 액와 체온으로 38$^{\circ}C$ 이상의 발열을 보인 123명을 대상으로 하였다. 산전 위험요인이 있었거나 선천성 기형아, 병원을 방문하기 전까지 항생제를 투여받거나 원내 감염이 의심되는 경우는 제외하였다. 입원기록을 후향적으로 검토하여 진단명과 임상경과를 기준으로 고위험군과 저위험군으로 분류하고 두 군 간에 증상, 진찰 소견 및 검사 결과를 비교하였다. 결 과 : 고위험군에는 30명(24.4%)이 포함되었고 진단명을 빈도순으로 나열하면 무균성 수막염, 요로감염, 세균성 수막염, 감염성 장염, 파종성 혈관내 응고장애를 동반한 패혈증, 균혈증, 폐렴, 연조직 감염, 제대 감염순이었다. 총 백혈구 수의 증가와 혈소판 감소증은 두군 간에 통계적으로 유의한 차이를 보였으나 C-반응 단백은 그렇지 않았다. 경련, 끙끙거림, 맥박수, 입원 전까지의 발열 일수, 총 발열 기간 및 입원시까지의 최고체온은 고위험군과 저위험군 사이에서 차이가 있었다. 결 론 : 본 저자들은 이전의 보고와 달리 배양검사 양성인 경우 외에도 임상적으로 명확한 패혈증이나 무균성 수막염 환자 등을 고위험군으로 포함시켰다. 그러나 이런 대상 선정의 차이에도 불구하고 두 군은 백혈구의 수의 증가, 혈소판 감소증 및 활력징후의 이상에서 유의한 차이를 보였다.

Genetic Variation in a DNA Double Strand Break Repair Gene in Saudi Population: A Comparative Study with Worldwide Ethnic Groups

  • DNA repair capacity is crucial in maintaining cellular functions and homeostasis. However, it can be altered based on DNA sequence variations in DNA repair genes and this may lead to the development of many diseases including malignancies. Identification of genetic polymorphisms responsible for reduced DNA repair capacity is necessary for better prevention. Homologous recombination (HR), a major double strand break repair pathway, plays a critical role in maintaining the genome stability. The present study was performed to determine the frequency of the HR gene XRCC3 Exon 7 (C18067T, rs861539) polymorphisms in Saudi Arabian population in comparison with epidemiological studies by "MEDLINE" search to equate with global populations. The variant allelic (T) frequency of XRCC3 (C>T) was found to be 39%. Our results suggest that frequency of XRCC3 (C>T) DNA repair gene exhibits distinctive patterns compared with the Saudi Arabian population and this might be attributed to ethnic variation. The present findings may help in high-risk screening of humans exposed to environmental carcinogens and cancer predisposition in different ethnic groups.

조선업 근로자의 작업능력지수에 영향을 미치는 요소에 관한 연구 (A Study on Factors Influencing the Work Ability Index of Shipbuilding Industry Workers)

  • The population over 65 years of age in Korea will be 10.3% of the entire population in 2008 indicating that Korea is already in an aging society. In addition, it is expected that Korea will be in an aged society in 2018 (population over 65 years of age : 14.3%) and that it will be in a post-aged society in 2026 (20.8%). Consequently, aged workers may also increase. It is well-known that the possibility to work longer and to live better is highly related to the work ability of individual. The objective of this study was to evaluate the work ability of each age group using the Work Ability Index(WAI) in the shipbuilding industry, and to assess the effects of social psychological stress, drinking characteristics, smoking characteristics, etc. on the WAI score. The results showed no significant difference on the effects of aging, drinking, and smoking characteristics. Interestingly, a statistical analysis on the effects of social psychological stress revealed a significant difference by stress levels. The difference of the average WAI score between the healthy group and the high risk group was about 10 points. Based on this result, it can be concluded that the stress level is an influential factor on WAI.

  • 우리 사회는 인공지능, 사물 인터넷, 자율주행, 공장 자동화 등 ICT 기술 기반의 4차 산업혁명과 함께 인구 고령화라는 큰 변화에 직면해있다. 더욱이 인구소멸위험 지수가 높은 지방자치단체에서는 고도화된 산업 자동화와 인구 고령화라는 변화에 대응하기 위해 지역 특성에 맞는 시장 확대 및 재교육 정책의 고려가 필수적이다. 고령자 재취업을 위해 노인 친화형 일자리를 발굴하고 고령친화 산업에 대한 투자 확대, 고령자를 많이 고용하는 기업의 작업환경 개선 정책에 대해 알아보고, 천안의 노인 노동시장의 실태와 개선방향에 중점을 두고 해결책을 모색하고자 한다.
