인천지역 초등학교 영양사의 영양교육 실태 및 인식에 관한 연구 (A Study on Elementary School Dietitian's Status and Recognition of Nutrition Education in Incheon)

  • The purpose of this study was to investigate the elementary school dietitian's status and recognition of nutrition education (NE) in Incheon. A cross-sectional study was carried out using a self-administered questionnaire and subjects were 100 elementary school dietitians. The results are as follows. As for training in NE, $61.2\%$ of the dietitians attended training in NE. After training in NE, $86.5\%$ of the dietitians who attended training in NE were more concerned about NE. Also $59.5\%$ of the dietitians gave students NE and most of them did as a weekly printout 2-4 times per month. There was a significant difference in experience of NE for teachers between subgroups by experience of training in NE; while $48.1\%$ of the dietitians with training in NE gave teachers NE, $20.0\%$ of the dietitians without training in NE gave teachers NE. The main reason for not giving NE was too much work load and low concern of school administration. Also $96.4\%$ of the dietitians answered that NE is necessary in elementary school and the main reason for being necessary was correction of unbalanced diet and good table manner, As for proper time to start NE for children, $51.8\%$ of dietitians answered 'kindergarten' and $45.8\%$ of them answered 'lower grade of elementary school'. As for effective type for NE, $59.5\%$ of the dietitians answered 'NE as a part of other subject' and $23.8\%$ of them answered 'NE as a separate subject'. Also $79.5\%$ of the dietitians answered 'teacher' as the suitable person for NE. Most of the dietitians recognized menu formation as the ideal major work load and office work as the most time-consuming work load. As to job satisfaction, most of them were dissatisfied with office work and NE. Therefore, it is nationally supported for elementary school students' health and well-being that school dietitians as NE specialists give NE with minimizing their office work and developing a standardized NE program.

농촌지역 노인을 위한 중풍예방 프로그램의 효과 (Effect of Stroke Prevention Program for The Elderly in The Rural Area)

  • 본 연구는 농촌지역 노인의 중풍을 예방하기 위해 건강관리교육, 영양지도, 운동으로 구성된 중풍예방 프로그램이 체질량지수, 혈압, 혈당, 혈청지질에 미치는 효과를 검증하기 위해 2014년 5월부터 8월까지 1주 3회 1회 50분 총 12주 동안 실험군 28명, 대조군 27명을 대상으로 실시하였다. 중풍예방 프로그램 전과 후의 차이를 비교한 결과, 실험군은 대조군보다 실험 전보다 실험 후에 체질량지수(p<.001), 총콜레스테롤(p<.001), 중성지방(p=.047), 저밀도인지질단백질(p=.010)이 유의하게 감소하였다. 또한 실험군은 실험 전보다 후에 체질량지수, 공복시 혈당, 총콜레스테롤, 중성지방, 고밀도인지질단백질이 감소하였으며 저밀도인지질단백질이 증가하였다. 이는 중풍예방 프로그램이 중풍의 위험요인을 감소시켜 중풍을 예방 할 수 있음을 의미한다.

제주 지역주민기반 대사성질환 개선 프로그램 중재 효과 (The Effect of Community-based Health Intervention Program to Improve Metabolic Disease in Jeju Island)

  • 본 연구는 대사성질환 관련 위험 인자를 가지고 있는 제주 지역주민을 대상으로 마을 기반 건강프로그램의 효과성을 입증하기 위해 수행되었다. 50명의 실험군은 BMI가 25 이상인 지역주민으로 3주 동안 고유수용성신경근촉진(PNF) 트레이닝과 스트레칭을 포함한 운동요법 7회, 영양관리 식습관 개선을 위한 쿠킹테라피 프로그램 4회 그리고 대체의학활용 프로그램인 힐링터치 마사지 프로그램 3회의 건강중재프로그램에 참여하였다. 건강프로그램 중재의 효과성 평가를 위해 프로그램 중재 전과 후에 체 성분, 혈액의 지질 프로파일, 혈당 및 허리둘레를 측정하여 비교하였다. 건강프로그램 중재 후에는 중재 전에 비해 TC, HbA1C, 이완기혈압, 체지방 그리고 허리둘레가 크게 감소하여 거의 정상수치가 되었고 특히 HbA1C, 체지방 그리고 허리둘레는 유의한 수준으로 감소 (P<0.001)되었다. 그러나 반대로 건강프로그램 중재를 받지 않은 대상군의 경우 HbA1C, 체지방 및 허리둘레가 유의한 수준으로 증가 (P<0.001)하는 부정적 결과를 보였다. 이상의 결과로부터 대사성질환 관련 인자들은 건강프로그램의 중재로 개선되어 호전될 수 있음을 알 수 있었다. 본 연구에서는 건강중재프로그램인 운동과 쿠킹테라피 효과를 구분하여 조사하지는 않았지만 3주간의 단기간에 보여진 효과가 두 가지를 병행해서 수행함으로써 나타난 결과임을 암시하며 식사조절과 운동을 병행하는 것이 대사성질환 개선에 보다 효과적임을 제시하고자 한다.

미국 노스다코타주 농촌지도사업에 있어서 노인복지 프로그램의 현황과 전망 (The Situations and Its Challenge for Rural Elderly Welfare Program on Extension Education in North Dakota, USA)

  • The objective of this study is to explore the situations and extension roles for rural elderly welfare program in North Dakota, USA. With a growing older population, aging has become an important issue for extension. Services for rural elderly available in North Dakota were adult day care, home health care, senior insurance counseling, nutrition and medication assistance programs, support groups, legal assistance, meals on wheels, nursing homes and more. With a growing older population, aging has become an important issue for extension. Extension provides programs and services for rural elderly. This study was conducted by literature review. First, many rural elderly Americans are actively engaged in volunteer work and have made substantial contributions to their communities. Second, extension educators from interdisciplinary areas should work together to develop programs. Extension programs can include intergenerational programs to help younger generations learn about the issue. Third, extension can collaborate with other agencies and groups to offer support groups. Offering educational programs is a key to empowering older people. Fourth, elderly residents may be the only increasing natural resource for volunteering in general, and for participation in community improvement in particular. Fifth, extension educators should be proactive in working with agencies to provide social access and in helping older people be actively engaged in their lives, especially in rural areas.

심뇌혈관질환 고위험군을 위한 지역사회 단계별 교육프로그램 개발 및 효과 평가 (Development and Evaluation of a Community Staged Education Program for the Cardiocerebrovascular Disease High-risk Patients)

  • 본 연구는 대구광역시 심뇌혈관질환 고위험군 등록관리 시범사업단 고혈압 당뇨병 교육정보센터를 방문한 고위험군환자와 교육자의 교육요구도를 반영하여 질환, 영양, 운동 3영역의 기본, 심화, 개별 3단계로 단계별 교육프로그램을 개발하였다. 이 프로그램의 효과평가를 위하여 교육정보센터에 방문한 기본교육군 32명, 단계교육군 37명을 대상으로 교육 전후 임상적 지표, 행동변화단계, 자아효능감을 측정하였고 주요 연구결과는 다음과 같다. 교육요구도조사결과 고위험군과 교육자의 요구도가 높은 소그룹형태, 30분-1시간이내 수업 및 이론 50%와 실습 50%의 구성과 형태로 이루어지며 3개월의 교육과정인 질환, 영양, 운동영역의 단계 교육프로그램을 개발하였다. 교육요구도조사 결과 교육영역별 교육내용은 고위험군과 교육자의 필요도와 지식정도의 통계적으로 유의한 차이에 따라 5가지 범주로 분류하여 프로그램 구성에 적용할 수 있었다. 첫째, 고위험군과 교육자간의 요구도에 유의한 차이가 없는 항목은 기본과정내용으로 반복하게 하였고 둘째, 고위험군의 인지도가 평균점수 이하인 항목은 기본교육과정내용을 조정하여 효과적인 실습방법으로 모든 단계에서 반복할 수 있도록 하였다. 셋째, 고위험군의 지식정도 평균 편차가 큰 항목은 개별교육과정에 포함시켜 문제를 해결할 수 있는 기술습득을 목표로 이루어지도록 하였다. 넷째, 고위험군과 교육자간 지식정도의 유의한 차이가 있는 항목과 다섯째, 고위험군과 교육자간 요구도에서 유의한 차이가 있는 항목은 고위험군의 참여도가 높은 실습 중심의 심화교육으로 구성하며 교육자도 표준 매뉴얼에 따라 교육방법, 교육환경을 일관성 있게 유지하도록 교육훈련을 강화하여 교육자 간 격차를 줄이도록 하였다. 교육자와 고위험군과의 격차를 줄이기 위하여 자가관리 목표, 단기 계획 설정을 고위험군과 교육자 상호약속에 의해서 계획하여 실천동기와 문제해결능력을 향상시키도록 하는 과정을 단계마다 필수적으로 포함시켰다. 교육프로그램의 평가는 기본교육군, 단계교육군을 대상으로 임상적 지표, 행동변화단계 자기 효능감을 측정하여 비교하였다. 임상지표 중 허리둘레, 수축기혈압, 이완기혈압(p<0.05), 자기효능감 중 혈압/혈당조절 불량시 병원방문, 의사의 지시에 따라 약 복용, 식사량 일정유지(p<0.05), 정기적으로 합병증 검사, 정상적인 혈압/혈당을 유지, 허리둘레 유지, 체중 유지(p<0.01), 행동변화단계에서는 싱겁게 먹기(p<0.05)에서 유의적인 차이가 있었다. 환자와 교육자의 교육요구도를 반영하여 개발된 실습 중심의 단계별 교육프로그램은 이론중심의 기본교육프로그램과 비교한 결과 임상지표, 자기효능감, 행동변화단계에서 유의한 차이가 있었다. 대상자 수가 적고 추구관리기간이 짧았으며 임상지표측정이 부족했던 한계점은 있으나 환자가 노인이며 만성질환환자로서 유병기간이 긴 점을 감안하면 이 프로그램을 지속적으로 시행한다면 환자의 심뇌혈관질환 예방에 기여할 수 있을 것이다.

우리나라 농촌지역의 메뉴패턴에 관한 연구 (III) -끼니별, 연령별 비교 분석 - (The Study of Menu Patterns in Korean Rural Areas (III) - Compared by the Heal and Age -)

  • The purpose of this study is to compare menu patterns by the meal and age (10- 19, 20- 49, 50 - 69, 70 - 84 years old) in Korea rural areas. Frequently consumed menu patterns were investigated using the 24-hour recall method with 1,185 subjects in 5 Korea rural areas for the spring, summer, fill and winter. Results were as follow : most frequently used basic menu pattern, excluding side dish, was rice + soup in breakfast, and was only rice in lunch and dinner. Most frequent menu pattern by the number of side dish was rice + soup + kimchi + 1 side dish in the breakfast and dinner. The Mean Adequacy Ratio (MAR) in the dinner is higher than that of the breakfast. For the 10 - 19 and 70 - 84 years old, frequently used menu pattern was rice + stew + kimchi + 1 side dish. Most frequently used menu patterns, was rice + soup + kimchi + 1 side dish for the 20 - 29 years old, was rice + stew + kimchi for the 50 - 69 years old. MAR with the same menu pattern in 10 - 19 years old is higher than that of 70 - 84 years old. Intake frequency of menu pattern including noodles was higher in 10 - 19 and 20 - 49 years old than that of the other age groups. With these results, for the nutrition program in the community menu patterns should be carefully examined by the meal and age. The result from this study can be used as basic data for nutrition education program in Korean rural areas.

우리나라 청소년의 우유.유제품 섭취와 영양소 섭취 수준과의 관계 (Survey on the Relationship between Milk and Milk Product Consumption and Dietary Nutrient Intake among Korean Adolescents)

  • This study was performed to investigate the relationship between milk and milk product consumption and dietary nutrient intake among Korean adolescents. Questionnaire survey and 3-day diet survey using the food record method were completed by a total of 664 subjects. Subjects were divided into three groups, Q1 (low group), Q2 (middle group), and Q3 (high group), according to dairy equivalent of calcium. Dairy equivalent of calcium was determined by the amount of calcium eaten from milk and milk products by individual subjects. As a result, the ratio of school milk service was higher in Q3 (P<0.001). The most frequent answer about the reason for consuming milk and milk products was 'to be taller' followed by 'good taste' and 'health promotion'. Preference for all types of milk such as white-, enriched-, and flavored-milk was higher in Q3 followed by Q2>Q1 (P<0.05). Ratio of mean daily dietary nutrient intakes of dietary fiber, vitamin C, folate, and calcium to RNI was lower than 2/3 for all of the groups. These mean daily dietary nutrient intakes were significantly higher in Q3 (P<0.05), and in particular, mean daily dietary calcium intake, which was the lowest nutrient consumed by Korean adolescents, was also the highest in Q3 followed by Q2>Q1 (P<0.05). The above results suggest that the school milk program is very helpful in encouraging adolescents to consume milk and milk products and consequently ensure their optimal nutrition. Therefore, we should try to encourage adolescents to participate in the school milk program more actively through nutritional education and government policy.

한국 성인의 수면시간과 맥압과의 관련성: 국민건강영양조사 제7기 자료를 중심으로 (Relationship between sleep duration and pulse pressure in Korean adults: Focusing on the data of the 7th National Health and Nutrition Survey)

  • 본 연구는 한국 성인의 수면시간과 맥압과의 관련성을 알아보고자 하며, 연구방법은 국민건강 영양조사 제7기(2016-2018) 자료를 이용하였다. 대상자는 20세 이상 5,707명을 대상으로 빈도와 백분율 연속형 자료는 평균과 표준편차, 교차분석과 independent t-test, 고 맥압에 대한 단계적인 로지스틱 회귀분석을 실시하였으며, SPSS WIN 18.0 version 통계프로그램을 이용하였다. 연구결과 전체 인구(OR=1.344, 95% CI=1.002-1.803)와 여성(OR=1.624, 95% CI=1.112-2.371)에서는 수면시간이 6시간 이하에 비하여 9시간 이상에서 고 맥압의 유병률이 높았지만, 남성(OR=1.012, 95% CI=0.631-1.550)에서는 그 유의성이 없었다. 따라서 적절하지 않은 수면시간이 건강에 미치는 영향을 고려할 때 이에 대한 보건학적인 교육과 관리가 필요할 것으로 생각하며 추후 수면시간과 맥압의 관계에 관해서는 계속된 연구가 필요할 것이다.

국민기초생활보장사업 수급 가정에서의 식품공급안정성과 아동의 영양상태 (Food Security and Children′s Nutritional Status of the Households Supported by the National Basic Livelihood Security System)

  • In order to provide basic data for the means to improve food situation and nutritional status of those supported by the National Basic Livelihood Security System (NBLSS), we examined household food insecurity and nutritional status of children under the support of NBLSS. This study included 209 children aged 3-12 years (99 boys and 110 girls) and their caretakers. We measured house food insecurity using Radimer/cornell Scale, children's body sizes and nutrient intake by semi-quantitative food frequency questionnaire, and caretakers' nutritional management skills. Only 9.6% of the households were in food secured (FS) while 8.1% were in household food insecured, but without hunger (HFI), 42.1% were in adult food insecured with hunger (AFI), and 40.2% were in child hungry (CH). Important predictors of food security were nutritional management skills of the caretakers as well as their education, but neither income nor food expenditure of the households. Mean energy intake of the children was 86.0% of the Korean Recommended Dietary Allowance (RDA). Intakes of protein, phosphorous, vitamin A and B$_1$ were relatively high ranging from 112.3% to 124.4% of the RDAs while those of calcium, iron, niacin, vitamin C were low showing 74.8-83.3% of the RDAs. Height, weight and weight/height ratio were close to the reference levels. Lower nutrient intakes of children were observed as the households were more food insecured. However, nutrient intakes and body sizes of children did not differ as a function of household socioeconomic status representing by income, food expenditure and caretakers' education. Results of this study suggest the importance of food security and nutritional management skills for the children's nutrient intakes. Concerning this matter, a need for nutrition education in the program for NBLSS was discussed.

A Study on the Discrimination and the Real State of High Calorie Foods with Low Nutrition Values in Children's Snacks sold within Green Food Zone

  • This study surveys the state of 'children's snacks' sold within the Green Food Zone and that of High Calorie Foods with Low Nutrition Value (HCFLNV). The main purposes are the analyses of foods in accordance to KFDA Program for HCFLNV and an analysis of differences in HCFLNV that are dependent on relevant factors such as food types, school types, origins, the scale of manufacturing company, area and price. Based on the analyses, educational and political implications have been sought that will form nutritious dietary habits, contribute to the prevention of obesity, and improve health in child consumers. The methodologies of this study are literature studies and surveys. The results of this study can be summarized as following. First, the number of children's snacks is 517 items of total 645 gathered within 150 Green Food Zones. Candies are the most popular item, next are cookies, chocolates, and breads. Second, in the real state survey there are 186 HCFLNV (36.0%) among children's snacks sold within Green Food Zone. Based on the survey results, the marking of HCFLNV on the package of children's snacks and the extension of the ban of HCFLNV sales to all stores within the Green Food Zone are strongly suggested. This provides preliminary data related to children's snacks and food safety. With enforcement of the Special Act on the Safety Management of Children's Dietary Life, the rate of HCFLNV has decreased and the child snack product environment in stores has improved. However, it is necessary to supervise low-priced snacks and promote an awareness of HCFLNV along with the child consumer education of food safety is needed.
