Cancer screening rate in people with diabetes in the Korean population: results from the Korea National Health and Nutrition Examination Survey 2007-2009

  • OBJECTIVES: To investigate the screening rates for gastric, breast, and cervical cancer in people with diabetes compared with people without diabetes. METHODS: Data from the Korea National Health and Nutrition Examination Survey (2007-2009) were used. Cancer-free men who were 40 years old and over and cancer-free women who were 30 years old and over were included. The lifetime screening rate and regular screening rate were compared in people with and without diabetes. RESULTS: Fewer people with diabetes than people without diabetes had ever received cancer screening (53.5 vs. 59.5%, p<0.001 for gastric cancer; 60.5 vs. 71.5%, p<0.001 for breast cancer; and 49.1 vs. 59.6%, p<0.001 for cervical cancer). Fewer people with diabetes than people without diabetes received the recommended screenings for gastric cancer (38.9 vs. 42.9%, p<0.001), breast cancer (38.8 vs. 44.6%, p<0.001), and cervical cancer (35.1 vs. 51.2%, p<0.001). In subgroup analyses according to socioeconomic factors, the lifetime and recommended screening rates were lower in the diabetic population in most socioeconomic subgroups. In the multivariate analysis adjusted for socioeconomic factors, people with diabetes showed lower lifetime screening rates for gastric and cervical cancer (odds ratio [OR], 0.8; 95% confidence interval [CI], 0.7 to 0.9 and OR, 0.7; 95% CI, 0.6 to 0.9), and lower regular screening rates for breast and cervical cancer (OR, 0.7; 95% CI, 0.6 to 0.9 and OR, 0.7; 95% CI, 0.5 to 0.9). CONCLUSIONS: The cancer screening rate in people with diabetes was lower than in people without diabetes. Considering the higher cancer risk in people with diabetes, efforts to increase the screening rate in this high-risk population should be implemented.

Knowledge and Practices on Breast and Cervical Cancer Screening Methods among Female Health Care Workers: A Sri Lankan Experience

  • Breast and cervical cancer are the most common causes of cancer mortality among women worldwide, but they are largely preventable. There are limited data on knowledge and practices on screening methods of breast and cervical cancers among female health care workers in Sri Lanka, in spite of having an organized screening programme islandwide. A cross-sectional survey was conducted among 219 female health care workers including public health midwives (68.9%) selected from 6 districts in Sri Lanka using convenient sampling methods. A self-administered questionnaire was used as a pre-test in a capacity building training programme to collect the data. The mean (SD) duration of work experience of the respondents was 12 years and 52.5% were aged over 35 years. Most (76.7%) were married, and afamily history of cancer was reported by 24.2%. Over 98% knew about self breast examination. Even though 84.1% practiced it, only 47.9% practiced it on a monthly basis. Clinical breast examination and mammography were known by 94.1% and 64.3% respectively. Only 19.2% had undergone a clinical braest examination within one year and 3.6% had ever undergone a mamography. Only 76.3% knew that a Pap smear detects precancerous stage of cervical cancer. Among 169 married workers, 73.4% had never had a Pap smear and only 17.2% had got it done within the preceding 5 years. Among the reasons for not doing a pap smear within 5 years, 47.0% belived it as not nescessary, 17.3% due to fear/dislike, 23.2% as not having symptoms, 3% had not known about it and 3% not known about availability of services. The study findings suggest that the knowledge and practices on breast and cervical cancer screening methods among female health care workers need to be improved. Considering the role that health care workers play in communicating health behaviors to the general public, strengthening health education interventions for this group of females is essential.

소아와 청소년의 건강행태와 질병 이환 비교 (Health Behavior and Disease of Children and Adolescents)

  • Objectives: The purpose of this study was to examine health behavior and disease of children and adolescents. Method: Data was taken from the Korean National Health and Nutrition Examination Survey. The SPSS 12.0 program was used for statistical analysis of data collected. Results: For the past one year, 57% of study group had oral examinations, 43.9% of them were vaccinated against influenza, and 16.9% tried to control body weight. Many children and adolescents (37.7%) had diseases, and 16% were diagnosed chicken pox. Of the subjects who were over 10 years old 35.9% had low high density lipid. The difference of sickness status between children and adolescents was significant ($x^2$=8.376, P=.015). The differences between children and adolescents in subjective health status, oral examination, vaccination of influenza, subjective physical change, body weight change and weight control efforts in the past one year were significant. The differences between children and adolescents in diagnosis of sinusitis, pneumonia and low HDL were statistically significant. Conclusions: The children and adolescents have different characters in health perimeters and these factors should be considered in order to improve the health status of children and adolescents.

전신질환자 중 치주질환자 구강관리 효과 (Oral health care effects of periodontal disease patients with systemic diseases: case report)

  • Objectives: The purpose of the study was to investigate dental care effects of periodontal diseases patients with systemic diseases. Methods: The study subjects were ten patients. The study consisted of direct examination and interview survey. Direct examination comprised pocket depth, bleeding on the brush, O'Leary plaque record, salivary flow rate, and oral bacterial culture for three months. Results: The number of diabetic patients was eight. Four patients xerostomia and one of them had 0.7 mL per minute of salivary flow rate, decreased O'Leary plaque record, and bleeding in the brush. Those who received education were able to take control of plaque management. They recognized the need for oral care and had good self-management of oral care skills. Conclusions: The professional dental care and oral health education improved periodontal health and self-management skills of plaque in periodontal disease patients with systemic diseases.

Relationship between smoking history and periodontal disease among the elderly in Korea

  • Objectives: The objective of this study was to investigate the relationship between smoking history and periodontal disease among the elderly in South Korea. Methods: The study subjects comprised 2,703 elderly people who underwent oral health examination as part of the 7th South Korea National Health and Nutrition Examination Survey (KNHANES; 2016-2018). Data were analyzed using frequency analysis, Rao-Scott chi-square test, t-test, and binary logistic regression analysis. Results: A complex sample logistic regression analysis showed that the odds for periodontal disease development were higher in past smokers (odds ratio [OR]=1.461; 95% CI=1.070-1.994) and current smokers (OR=1.601; 95% CI=1.011-2.536) than in lifetime non-smokers. Conclusions: Smokers must actively participate in smoking cessation programs and interventions starting from middle age.

융합적 측면에서의 직장인 당뇨병 환자의 삶의 질 영향요인 및 비교 분석: 2007-2015년 국민건강영양조사를 바탕으로 (Quality of Life Factors and Comparative Analysis in Workers with Diabetes Mellitus in terms of Convergence : using the Korean National Health and Nutrition Examination Survey, 2007-2015)

  • 본 연구는 직장인 당뇨병 환자의 삶의 질 영향요인을 시기별로 파악하여 삶의 질 향상을 위한 중재방안을 모색하기 위하여 시행되었다. 연구 대상은 2007-2015(4기, 5기, 6기) 국민건강영양조사를 활용하였으며, 만 19세 이상 65세 이하의 성인 중 당뇨병을 진단받고, 직장을 가지고 있는 총 1141명을 대상으로 하였다. 분석은 SPSS 23.0 프로그램을 이용하여 기술통계, ANOVA, 다중회귀분석을 시행하였다. 본 연구결과, 기수별 삶의 질 정도는 4기보다 5기, 6기가 높았으며, 통계적으로 유의하였다. 다중회귀분석 결과 성별, 소득수준, 주관적 건강상태, 활동제한여부, 수면시간, 스트레스 인지율이 삶의 질에 영향을 주는 요인으로 나타났다. 기수별로는 주관적 건강상태, 활동제한여부, 스트레스 인지율이 공통적인 영향요인이었으며, 4기는 수면시간, 5기는 성별, 6기는 소득수준과 협심증진단여부가 삶의 질에 영향을 주는 요인으로 나타났다. 이상의 결과를 바탕으로, 직장인 당뇨병 환자들의 삶의 질 향상을 위한 관리 프로그램 및 사회적 역할에 대한 융합 연구에 활용할 수 있을 것이다.

한국 성인의 대사증후군이 치주질환에 미치는 영향 (Effects of Metabolic Syndrome on Periodontal diseases in Korean Adults)

  • 2009년 국민건강영양조사 자료를 이용하여 한국 성인의 대사증후군과 치주질환과의 관련성에 대해 평가하고자 하였다. 만 19세 이상의 성인에서 대사증후군으로 진단된 환자 중 지역사회치주지수(Community Periodontal Index: CPI)에 대한 정보가 모두 갖춰진 1,315명을 최종분석대상자로 하여 다음과 같은 결론을 얻을 수 있었다. 1. 치주질환 유병과 관련이 있는 인구 사회학적 특성은 연령과 교육수준으로 나타났다. 연령이 증가할수록, 교육수준이 낮을수록 치주질환의 비율이 높게 나타났다(p<0.001). 2. 치주질환 유병과 관련이 있는 일반건강행위는 흡연과 음주 및 체질량 지수로 나타났다. 현재 흡연을 하는 경우(p<0.05), 일주일에 음주횟수가 많을수록(p<0.001), 체질량 지수가 높을수록(p<0.005) 치주질환의 비율이 높게 나타났다. 3. 치주질환 유병과 관련이 있는 구강건강행위는 지난 1년간 구강검진 여부, 치간칫솔 및 치실 사용 여부, 주관적 구강건강으로 나타났다. 지난 1년간 구강검진을 받지않고(p<0.001) 치간 칫솔 및 치실을 사용하지 않는 경우(p<0.05), 주관적 구강건강이 건강 하지 않다고 생각하는 경우(p<0.001) 치주질환의 비율이 높게 나타났다.

1인가구의 건강관련 습관적 소비, 생활시간이 주관적 건강에 미치는 영향: 비1인가구와 세대별 비교를 중심으로 (The Effect of Health-Related Habitual Consumption and Lifetime on Subjective Health of One Person Households: Focusing on Comparison between Non-One Person Households and Generations)

  • Recently, one-person households have surpassed nuclear families. One-person households are expected to have many problems with health due to an irregular life style and a tendency to be more isolated from society. In addition, we need to divide the generations and survey each generation due to differences in one-person household factors and characteristics as well as the unique physical conditions of each generation. Therefore, based on the sixth Korea National Health and Nutrition Examination Survey, this survey examined health behavior differences between one-person households and non-one-person households according to generation (respectively) as well as studied how one-person households and health behavior influence subjective health conditions. The major result of this survey is as follows. First, one-person households reveal a higher rate in the negative health behavior than non-one-person households. Second, the physical activity of all households reveals a high rate of non-activity. Third, health scores of one-person households are lower than that of non-one-person households; consequently, the analysis of health scores for one-person households and non-one-person households indicated that the middle age health scores for one-person households have been lowest in generations. Fourth, the factors that influence subjective health conditions indicated that one-person household and health behavior has meaningful influence; in addition, the subjective health condition of one-person households are lower than non-one-person households under controlled health behavior.

설문과 비침습적 검사를 통하여 심혈관계 건강 정도를 알려주는 심혈관지수의 개발 (Development of a Cardiovascular Index That Estimates Cardiovascular Health Degree by Survey and Noninvasive Examination)

  • An index was developed that estimates cardiovascular health degree with easily available physiological information such as survey and noninvasive measurement. The survey score was calculated by utilizing questions related to personal disease history, self-feeling, and management state. The measurement score was calculated using physiological parameters such as blood pressure, accelerated plethysmograph(APG), and heart rate variability(HRV), and augmentation index(AI). In order to evaluate effectiveness of the cardiovascular index and modify weighting factors used in each item, a clinical trial was done in a general hospital. The cardiovascular index showed a clear correlation of 0.685 with the doctor's score on the cardiovascular health degree. The correlation between the self-estimated score and doctor's score was as low as 0.217. The large gap between these two scores demonstrated necessity of more objective tools like the cardiovascular index. The cardiovascular score showed a significant difference between normal persons and patients suffering hypertension or diabetes. (p=0.000).
