요실금(尿失禁) 환자의 삶의 질에 대한 전침치료 효과 (Effect of Electroacupuncture on Quality of Life of Patients with Urinary Incontinence)

  • Objectives : This study was designed to evaluated the effect of electroacupuncture on Quality of life of patients with urinary incontinence Methods : Subjects were voluntarily recruited by newspapers and internet. Electroacupuncture was performed three times a week for 3 weeks. Acupuncture point for EA group was B32, Electrical stimulation frequency was 2Hz, duration 20 minutes, and intensity was up to pain threshold according to patients. The patients's symptoms were assessed before, after 3 weeks of treatment by QOL item of International Prostate Symptom Score(IPSS), Medical Outcomes Study(MOS) 36-Item Short-Form Health Survey(SF-36). Results : QOL score of IPSS were significantly improved after 3 weeks(p<0.05) compared to the pre-treatment. There were significant changes in Social functioning(SF), role-physical(RP), role emotional(RE), mental health(MH), bodily pain(BP) score of SF-36 after 3 weeks(p<0.05), but there were no significant changes in physical functioning(PF), vitality(VT), general health(GH) score of SF-36. Conclusion : This study suggests that electroacupuncture treatments can be applicable to improve symptoms in patients with urinary incontinence.

카페인이 HRV에 미치는 영향과 내관 자침의 효과에 대한 예비연구 : 무작위 이중맹검시험 (Short Time Effect of Caffeine on Heart Rate Variability and the Effect of Acupuncture at Neiguan (PC6): A Randomized Double Blind Pilot Study)

  • Objectives : This study was to investigate the short time effect of caffeine on heart rate variability(HRV) and the effect of Neiguan(PC6) acupuncture stimulation on HRV. Methods : 27 healthy adult volunteers were randomly allocated to two groups: Neiguan group (N=13) or placebo group (N=14). The study was carried out under a randomized double-blinded placebo-controlled trial method. Each group orally received the same tablets with 200 mg caffeine. After 1 hour, acupuncture was applied to the Neiguan(PC6) points for the Neiguan group, and for the placebo group was applied to a non-acupuncture point. Both groups were estimated with HRV 3 times, before and after caffeine ingestion, 20 minutes after acupuncture stimulation. Results : After taking caffeine, pulse rate, mean-HRV, and pNN50(the proportion derived by dividing NN50 by the total number of NN intervals) decreased, SDNN(standard deviation of all normal-to-normal (NN) intervals), RMSSD (the root square of successive differences), TP log, HF(high frequency), and HRV index was increased. There were significant changes to the autonomic nervous system after taking caffeine. There were no significant differences between the two groups after acupuncture at Neiguan. Conclusion : Caffeine could induce general activation of the autonomic nervous system. Neiguan acupuncture stimulation may not have significant influence on the autonomic nervous system.

『황제내경(黃帝內經)』의 침구법(鍼灸法) 연구 - 시간(時間) 개념을 중심으로 - (A Study on the Acupuncture & Moxibustion of Huangdineijing - focused on Concept of Time -)

  • Subject : Treatment of acupuncture & moxibustion in Huangdineijing Objectives : Discover the principles and substances of acupuncture & moxibustion treatment of Huangdineijing Methods : The author reviewed the Huangdineijing: Suwen, Lingshu to gain a realistic sense of the facts and the medical book's contents that deal with the practices of acupuncture & moxibustion treatment for clinical praxis. Conclusions : 1. The acupuncture & moxibustion of Huangdineijing can be defined as follows. A treatment for correcting of meridian's drift by proper selection of surgical site, using suitable implementation and appropriate manipulation skill from a correct judgement of meridian's flowing through sensing the pulse for diagnosis when the meridian's flowing which come and go constantly driven by the tidal order of defensive Qi tactually appeared hard or soft caused by diseases, thus bring out the soft & gradual 50 laps circulation of nutrient Qi moderately driven by defensive Qi. 2. Today's acupuncture & moxibustion treatments greatly differ from those of Huangdineijing in that we do not know the substance of that treatment which is correcting of meridian's drift and do not obligatorily carry out feeling of the pulse for diagnosis which is a measure & judgement of meridian's flowing against a preliminary treatment in parallel with treatment so reduce use and power of that treatment. 3. I could form a list of general principle from the substances of acupuncture & moxibustion of Huangdineijing like as the flowing or fate of meridian Qi(tidally going in order of defensive Qi and nutrient Qi), deficiency or excess of Meridian flowing, comparing diagnosis before and after treatment, bring out the soft & gradual Qi. That was imposing "the concept of time" on recognition and treatment to human body.

교통사고 후 발생한 요통에 대한 봉약침 치료의 효과 (Effect of Bee-venom Acupuncture on Low Back Pain by Traffic Accidents)

  • Objectives : The purpose of this study was to investigate the effect of Bee-venom acupuncture on low back pain by traffic accidents. Methods : We included 34 patients with low back pain caused by TA for this study. The experimental group was treated by usual therapy plus Bee-venom acupuncture therapy, the control group was treated by usual therapy such as acupunture therapy and herbal medicine therapy. Oswestry Disability Index(ODI) and Visual Analog Scale(VAS) were evaluated at 4 week from baseline for the treatment effect in the both group. Results : After the subjects being treated by Bee-venom or general acupuncture therapy. It was detected that the scores of ODI and VAS in both groups decreased at the 4 week from baseline. There was significant difference between both groups in the VAS. However, there was no significant difference between both groups in the ODI. Conclusions : The results suggested that bee-venom acupuncture was effective on low back pain by traffic accidents.

<침금방(千金方)>의 침구처방용혈(鍼灸處方用穴)의 일반규율(一般規律) 연구(硏究) (Research for general regulation acupoints prescription's in )

  • Objective : <千金方(Qianjinfang)> which documents a great amount of prescription of Acupuncture and moxibustion takes an important role during the development of acupuncture and moxibustion. Thus, it have given rise us to the research-situation of Tang Dynasty China of previous period. This study was to outline the rules of choosing acupoints as well as the prescriptions of contemporary times in reference with <千金方(Qianjinfang)>, and to discuss the conception of acupuncture and moxibustion in <千金方(Qianjinfang)>. Method : Based on <千金方(Qianjinfang)>, the parameters were categorized to a certain scope, syndromes in different type were classified. Also numbers of acupoints, route of meridians locations, utilities of special acupoints even prescription methods were statistically analyzed in reference with <千金方(Qianjinfang)>. Results & Conclusion : Acupuncture prescription in <千金方(Qianjinfang)> was mostly presented by single acupoint and this was basic prescription of its rule of choosing acupoints. Choosing acupoint for the majority of various diseases started from choosing meridians with the disorder, but no rules for choosing acupoints was clear. There was basically various methods in choosing acupoints, but little were used by Biao-li meridian choosing method (表裏經配穴). In the high frequency of use, wu-shu acupoints was higher used than other specific acupoints (特定穴). While wu-shu acupoints treated the major parts of the diseases, shu-mo acupoints were strictly used on system. Consequently, The theoretical basis originating the choosing rules of the acupoints and prescriptions was shown in <手金方(Qianjinfang)> as representative clinical reference, through which it was valuable to analyze the prescription rule and specificity of the acupoints.

전신성 홍반성 낭창(SLE)환자 1예(例)에 대한 증례 보고 (One Case of Systemic Lupus Erythematosus treated with traditional Korean Medicine)

  • After observing a patient diagnosed with Systemic Lupus Erythematosus from the September 21, 2000 to the January 13, 2001, the following results are obtained. Method and Result: We treated one case of Systemic Lupus Erythematosus with the Korean Bee Venom Therapy and the other Oriental Medical treatments. In acupuncture therapy, Korean Bee-venom Therapy is believed to be outstanding way to treat Systemic Lupus Erythematosus. Korean Bee Venom Therapy is treated on the following acupuncture points: ST36(B23(Shinsu:腎兪), BL26(Guanyuanshu:關元兪), ST36(Chok-Samni:足三里), LI4(Hapkok:合谷), LV3(Taechung), SP10(Hyolhae:血海) SP6 (Samumgyo;三陰交). In addition, CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;) Herbal-Acupuncture was applied on the acupucture points of GB20(Pungji: 風池), GB21(Kyonjong:), as well as BUM(It was made of Boviscalculus(牛黃), Moschus and Fel ursi Herbal-Acupuncture was given on CV17(Chonjung). In herbal medication, based on the Sasang Constitution, Taeyumin Chungsimyunja-Tang was given to the patient. As the results of these treatments, general conditions of this patient improved drastically. Conclusion: Based on the clinical results, traditional Korean Medical treatment is believed to be effective for treating Systemic Lupus Erythematosus, and futher studies should be carried out to provide more valuable information.

한의사의 증례연구에 대한 인식 및 활용 (Traditional Korean Medicine Doctors' Awareness and Utilization of the Case Report)

  • Objectives : The purpose of this study is to assess Traditional Korean Medicine (TKM) doctors' awareness and utilization of the case report, based on the opinion that careful observation on the case sometimes provides us with the important information as clinical trial especially in the traditional medicine research field. Methods : A questionnaire study was conducted among TKM doctors who participated in the annual continuous maintenance education (CME) held at 5 regions of the Republic of Korea. Results : Almost 60% of the respondents had read case reports published in medical journals and 67% had openly shared their clinical cases with their colleagues. Of the respondents, 18.6% had been educated on reporting cases, and only 16% had the experience of reporting cases on one's own. However, 32.6% of the respondents had the intention to report cases in the future. These results show significant differences between general physicians who holds a license but no hospital training experience and board certified TKM doctors who have training experience. Conclusions : A majority of TKM doctors have read case reports but holds little experience of having been properly trained. Through this research, it has been found that awareness of case reports is rising in hospital training. Thus, the objective of case report education for TKM doctors who have hospital training experience should be set on encouraging them to do more whereas for doctors without hospital training experience, the objective should be set on making them more exposed to case reports to heighten one's awareness.

조선 침구(鍼灸)의 지향에 대한 소고(小考) (A Study on the Mind of Joseon's Acupuncture and Moxibustion Technology)

  • Objectives : This paper looks at the history of East Asian acupuncture and moxibustion in the context of the general orientations of craft and art. It is understood that acupuncture and moxibustion was the ultimate integration of reason and craft in which text and experience mutually influenced each other throughout its development. In Joseon, the pursuit of integration between the body and the order of the Heaven and Earth was particularly prominent. Methods : As in the words of Zhunagzi(莊子), the state in which something that deeply touches the mind manifests in the hands has been understood as the ultimate state in modern and ancient art. Starting here, I examined the possibility that Korean acupuncture and art has evolved from such foundations step by step. Major acupuncture and moxibustion texts were examined historically, starting with "Shenyingjing(神應經)", leading to "Junggan-Shenyingjing (重刊神應經)", "Chimgu-Gyeongheombang(鍼灸經驗方)", "Chijongjinam(治腫指南)", "Chijongjinam-Joseonchambon治腫指南朝鮮?本", "Chimgutaegilpyeonjip(鍼灸擇日編集)", "Chimgu-Myeonggam(鍼灸明鑑)". Introductions and main contents of the texts were analyzed and presented appropriately. Results : East Asian acupuncture and moxibustion has developed based on the study of pathways that correspond to Shen, of the accordance and harmony between body and the order of Heaven and Earth together with theoretical refinement, technical development, and acceptance of new discoveries. In the perspective of craft, this was motivated by artistic passions such as '得於心 應於手' and '醫者意也'. In the case of refined acupuncture methods, due to their difficulty in education and transmission there has been some fluctuation. Conclusions : Korea, China, and Japan share a largely similar philosophical and cultural basis. Reason and craft, text and experience mutually influenced each other while the core of refined acupuncture was widely shared. However, a certain bifurcation point could be detected.

Single-dose and 4-week repeated dose Toxicity of Aconitum Sinomontanum Nakai Pharmacopuncture: An Experimental Study

  • Background: This study aimed to assess the toxicity of Aconitum sinomontanum Nakai (ASN) pharmacopuncture. Methods: To investigate the toxicity of ASN pharmacopuncture, single and 4-week repeated dose toxicity experiments were conducted on BALB/c mice. In the single-dose toxicity experiment, mice were assigned 1 of 4 groups (5 males, 5 females per group). Then, 31.25, 62.5, and 125 mg/kg of ASN pharmacopuncture were administered to the mice in the experimental groups at acupoint ST36, while 0.2 mL of normal saline was administered to the control group at ST36. After a 4-week repeated dose regimen, the mice were assigned into 4 groups (5 males, 5 females per group). Then, 15.625, 31.25, and 62.5 mg/kg of ASN pharmacopuncture at ST36 were administered to the mice in the experimental groups, while 0.2 mL of normal saline was administered to the control group at ST36. Mortality, morbidity, general body and organ weight changes (after 4 weeks repeated dose), serum hematological and biochemical values, and histopathological changes in the liver and kidney were observed. Results: In both single and 4-week repeated dose toxicity experiments, no deaths or symptoms occurred in any of the groups. There were no significant differences between groups in terms of body and organ weights, serum hematological and biochemical values, and specific organ histopathological changes. Conclusion: ASN pharmacopuncture injection did not demonstrate significant toxicity in BALB/c mice compared with the control group, with a no-observed-adverse-effect level for a single dose of >125 mg/kg, and for 4 weeks repeated dose it was more than 62.5 mg/kg/day.

『의림촬요(醫林撮要)』의 침구법에 대한 연구 (A Study of the Acupuncture of Uirimchalyo)

  • Uirimchalyo (醫林撮要) was published in advance of the Donguibogam (東醫寶鑑) in the middle of the Joseon Dynasty. This book was a pioneering form of the Donguibogam. This paper examines the acupuncture and moxibustion methods used within Uirimchalyo. Because this book was produced for the general public as well as professional Korean medicine doctors, it did not present complex symptom classification and complicated acupuncture prescriptions. Instead, it summarized acupoints in a simple way and could be used as a standardized treatment guideline. In addition, in the acu-moxa methods of the Uirimchalyo, moxibustion was used more than acupuncture. This implies that the author's intention was to treat the disease gently, and furthermore, that the book inherits the acu-moxa method of Hyangyakjipseongbang (鄕藥集成方). There are many suggestions of experienced prescriptions, which can be seen as focusing on experience-based medicine. In addition, the characteristic of not using many acupuncture points served as the cause of the emergence of Saam acupuncture techniques.
