A Case of Acute Respiratory Failure After Trichloroethylene Inhalation (트리클로로에틸렌 흡입으로 인한 호흡부전 1례)

  • Trichloroethylene (TCE, $C_2HCl_3$), which was introduced as a gas for general anesthesia and analgesia in early 1900's has been widely used in industry as an organic solvent. Occupational exposure to TCE is an important medical problem. Manifestations of acute exposure to TCE include mucocutaneous irritation, hepatotoxicity, cognitive impairment, sleep, headache, respiratory insufficiency and death. We report a 38-year-old man who was admitted to a department of emergency medicine after occupational inhalation exposure to TCE. He rapidly developed semicoma and respiratory depression. After mechanical ventilation, hypercapnea and hypoxemia disappeared and his mental state again became alert. Careful evaluation and proper respiratory support are important for respiratory failure after occupational TCE inhalation.

  • The security control can be defined as all control actions and counter-measures to return the operating state of the system to a normal state. In an emergency state, fault clearing and/or overload suppression is enabled as a security control in order to prevent the extension of the fault. In the alert state, counter-measures should be set up in advance for the dangerous points of the system operation in drder to protect the system from expected accidents. In the normal state, the routine scenario is conducted to analyze system state. In the decision-making of the classification of system states, the heuristic and experienced knowledge can be well applied and thus application of expert system to this area attains considerable achievements. In this study, it is attempted to extract empirical rules through heuristic analysis and establish the knowledge base. Finally, the incorporation method with the conventional program packages in proposed. The expert system is designed to select an appropriate method and to perform the corresponding package. The input data can be automatically set up by using the data base. The computation results can be automatically added to the data base.

  • Heart related diseases mainly caused by heavy work load and increasing stress in human daily life. Therefore, researches on mobile healthcare monitoring for daily life has been carried out. Notably, wearable healthcare monitoring system which has least restriction has been tried to provide an emergency alert of abnormal heart rate. In this study, we developed chair type unconstrained BCG measurement system which able to perform continuous heart status monitoring at the office and daily life in the unconstrained way. Furthermore, adaptive threshold is used to detect the heart rate from BCG signals. The HRV(heart rate variability) is calculated from heart rate interval. ECG signal measured using conventional method and BCG signal measured using unconstraint system are carried out simultaneously for the purpose of performance evaluation. From the comparison result, BCG signal shows a similar heart beat characteristic as ECG signal. This proves the possibility of practical implementation of unconstraint healthcare monitoring system. In addition, medical examination like valsalva maneuver is performed to observe the changes in HRV due to stress. By performing valsalva maneuver, heart is said to be placed under an artificial physical stress condition. Under this artificial physical stress condition, the time and frequency domain of HRV parameters are evaluated.

  • Nowadays a disaster event such as a building on fire, an earthquake or typhoon could occur any time, and any where. In such event, a warning notification system is a vital tool to send warning notifications to at-risk people in advance and provide them useful information to escape the dangerous area. Though some systems have been proposed such as emergency alert system using android, SMS or P2P overlay network, these works mainly focus on a reliable message distribution methods. In this work, we introduce a full prototype implementation of a personalized warning notification system based on geosocial information, which generates a personalized warning message for each user and delivers the messages through email or an android application. The system consists of four main modules: a web interface, database, a knowledge-based message generator, and message distributor. An android application is also created for user to receive warning messages on their smart phone. The prototype has been demonstrated successfully with a building-on-fire scenario.

  • Recent studies on a fusion of health care system and the information and communication technology of wearable is being developed Anytime and anywhere without being constrained to measure the biological information of the user. However existing wearable monitors the measured biological information. If the user is hard to deal with for the event of dangerous situations. In this paper, it proposes a system that identifies the status of a user to correct the problem it utilizes sensors and algorithms to measure the biological information. This enables the user will be able to respond quickly to dangerous situations. In the event of a dangerous situation, such as falling or stumbling sends an emergency alert to a designated guardian.

Indonesian plastic surgeons' attitude during early period of the COVID-19 pandemic

  • Background: This study aims to report how the practice of plastic surgeons and their attitude was during the first measure period of coronavirus disease 2019 (COVID-19) pandemic. Methods: A survey study was held among members of the Indonesian Association of Plastic Reconstructive and Aesthetic Surgeons during week 5 after the first report of COVID-19. A 10 multiple-choice questions (MCQs) and 1 essay covered key questions on the area of surgery and operating room, clinics, internal meeting, and consultation. The only open-ended question relates to the last MCQ addresses a future "flipped" medical practice. Results: Response rate was 45.6% among 228 members, with 89.4% did no practice or limited their service to emergency and urgent cases only. Only 1.9% kept their official meeting as usual, while the majority modified it. The practice in the operating theatre and clinic were also altered to comply with the measures; with 21.2% from the total respondents only allowed patients with exposure to come for visit after taking 14 days of self-quarantine. Teleconsultation was practiced by 50% of the respondents, while 41.3% agreed and 10.6% disagreed upon the future "flipped" medical practice. Conclusion: In general plastic surgeons have made supportive actions during the pandemic. Surgery was performed with all precautions at the utmost as a reflection of high alert of viral infection. Teleconsultation has been embraced via existing social media. Agreement upon the future "flipped" medical practice is reasonable. All in all, the actions were considered as most relevant.

  • Cardiac arrest is a serious intensive emergency disease that causes death within less than several minutes by depriving the body and brain of blood supply. Survival rate of cardiac arrest patients outside of hospitals is especially low. This is because pedestrians usually do not perceive the patient as a sick person, also, even if they do so, they have no medical knowledge to properly react to such emergency. The purpose of this study is to propose a solution that uses widely spread smart phones to alert pedestrians of the cardiac arrest patient, prevents cardiac arrest, and provides first-aid measures. By applying the proposed solution, cardiac arrest can be prevented in advance, pedestrians can be alerted to keep the golden time(4 minutes), and first witness can quickly proceed with CPR, ultimately enhancing the survival rate of the cardiac arrest patient.

Establishing meteorological drought severity considering the level of emergency water supply (비상급수의 규모를 고려한 기상학적 가뭄 강도 수립)

  • Recent intensification of climate change has led to an increase in damages caused by droughts. Currently, in Korea, the Standardized Precipitation Index (SPI) is used as a criterion to classify the intensity of droughts. Based on the accumulated precipitation over the past six months (SPI-6), meteorological drought intensities are classified into four categories: concern, caution, alert, and severe. However, there is a limitation in classifying drought intensity solely based on precipitation. To overcome the limitations of the meteorological drought warning criteria based on SPI, this study collected emergency water supply damage data from the National Drought Information Portal (NDIP) to classify drought intensity. Factors of SPI, such as precipitation, and factors used to calculate evapotranspiration, such as temperature and humidity, were indexed using min-max normalization. Coefficients for each factor were determined based on the Genetic Algorithm (GA). The drought intensity based on emergency water supply was used as the dependent variable, and the coefficients of each meteorological factor determined by GA were used as coefficients to derive a new Drought Severity Classification Index (DSCI). After deriving the DSCI, cumulative distribution functions were used to present intensity stage classification boundaries. It is anticipated that using the proposed DSCI in this study will allow for more accurate drought intensity classification than the traditional SPI, supporting decision-making for disaster management personnel.

  • In ubiquitous computing, there is increasing demand for ubiquitous displays that react to a user's actions. We propose a method of navigating pictures on an ambient display using implicit interactions. The ambient display can identify the user and measure how far away they are using an RFID reader and ultrasonic sensors. When the user is a long way from the display, it acts as a digital picture and does not attract attention. When the user comes within an appropriate range for interaction, the display shows pictures that are related to the user and provides quasi-3D navigation using the TIP(tour into the picture) method. In addition, menus can be manipulated directly on a touch-screen or remotely using an air mouse. In an emergency, LEDs around the display flash to alert the user.

  • Current terrestrial Digital Multimedia Broadcasting (T-DMB) is conducting the emergency alert broadcast, or can view a variety of broadcasting. However, propagation shadow area is a situation where the service is limited due to limitations of facilities investment. In addition, there is the problem of T-DMB broadcasting is for viewing only a restricted area and a mobile device because the mobile is also T-DMB viewing device impossible. In this paper, it receives a T-DMB broadcasting as a way to solve the problems of the T-DMB system, which was studied the re transmission to the mobile device. Accordingly, by receiving the broadcast may be watched in the mobile device the T-DMB reception impossible. Also provides a one-way/two-way authentication mechanism using a conditional access function, and the system was configured so that the user can watch only the registered broadcasting.
