Arthroscopic Treatment for Pyogenic Arthritis of the Shoulder in an Infant - A Case Report - (영아 화농성 견관절염의 관절경적 치료 - 증례 보고 -)

  • Pyogenic arthritis of the shoulder is very rare disease in infant. Early diagnosis and proper treatment are very important prognostic factors. We treated a 10-month-old infant who was suffering pyogenic arthritis of the shoulder by arthroscopic irrigation and synovectomy with direct inspection. Finally, satisfactory result was achieved without any major scar or complications.

Sleep Characteristics in Infants (영아의 수면 특성)

  • An infant's sleep varies considerably from that of adults in terms of structure, amount, and breathing pattern. After birth, sleep becomes evenly distributed throughout the day and night. Nighttime sleep gradually increases with the maturation of circadian rhythm, and sleep is gradually consolidated. Electroencephalography characteristics change with age, from early and dominant active (REM) sleep in newborns to increasing NREM sleep. Similar to other elements of growth, the upper respiratory tract and ribcage gradually increase in size with age, and respiratory control also improves. With these changes, sleep patterns also change. At this time that various sleep disorders may appear. Improved understanding of age-dependent changes in infant sleep can help determine the etiology and facilitate diagnosis of infant sleep diseases.

The Effect of Parental Socioeconomic Position on the Association between Birth Outcomes and Infant Mortality in Korea: Focusing on Early and Late 2000's (부모의 사회경제적 지위가 출산결과 및 영아사망에 미치는 영향: 2000년대 초반과 후반을 중심으로)

  • This study examined the effect of parental socioeconomic position on the association between birth outcomes and infant mortality in early and late 2000's. Linked Birth and Infant Death data sets from 2001 to 2003 (T1) and from 2006 to 2008 (T2), provided by the Korea National Statistical Office, were used for analysis. Birth outcomes were categorized into four groups: normal term, small-for-gestational age (SGA), large-for-gestational age (LGA) and intrauterine growth retardation (IUGR). Infant death was defined as the death of a live-born child under one year of age; indicators of parental socioeconomic position were limited to parental education and occupation. The results showed that T2 infant mortality hazard ratios of SGA and IUGR have increased compared to T1. Particularly, preterm and low birth weight babies with shorter gestational age and lighter birth weight than T1 have elevated in T2, possibly indicating that population quality might be continuously aggravated. Moreover, the effect of maternal age on infant mortality has disappeared, rather that of parents' socioeconomic position has increased during the periods, which entailed growing disparities in infant mortality by their social class. Further studies should be therefore done to estimate the effect of parental socioeconomic position on the relationship between birth outcomes and infant mortality in the near future.

An Analysis of Structural Relationships of the Cognitive and Communication Ability Related Variables on Infants (영아기 인지·의사소통 발달 관련요인 간의 구조적 관계 분석)

  • This study investigated the structural relationships among infant's cognitive and communication ability, temperament, teacher-infants interaction, and mothers' parenting stress. Subjects used in this study were 249 infnats whose mean age of 30.21 months, and who were attended in preschool in S city. Research instruments used in collecting data were 'K-ASQ(Korean Ages and Stages Questionnaires)', 'EAS(Emotionality, Activity, and Sociability-Temperament Survey for Children-Parental Ratings)', 'PSI/SF(Parenting Stress Index/ Short Form)', 'ECOI'. The collected data were analyzed through SPSS 18.0 and AMOS 18.0 program. Pearson's correlation and Structural equation model analysis were used to analyze the data. Findings are as follows. First, there were positive correlations between infant's cognitive and communication ability with social-ability temperament, teacher-infants interaction. There were negative correlations between infant's cognitive and communication ability with infant's emotionality temperament, mothers' parenting stress. Second, social-ability temperament directly affected disposition toward cognitive ability, but in-direct effect was not significant. Emotionality temperament partially mediated the effects of parenting stress toward cognitive ability. Third, social ability directly affected disposition toward language ability, but in-direct effect was not significant. There has no significant path of temperament to teacher-child interaction, but teacher-child interaction has direct effect to infant's cognitive ability. Finally, all path about infant's communication ability were not significant.

Development and Validation of 25~36 Months Old Infant's Happiness Scale for Teachers (교사용 25~36개월 영아 행복감 척도 개발 및 타당화 연구)

  • The purpose of this study is to identify factors of 25~36 months old infant's happiness and to develop a scale to measure infant happiness. The subjects were 250 infants aged 25~36 months. The results of this study are as follows: First, the components and items of infant's happiness scale selected through the literature, the previous measurement tool, and the focus group interview were 8 components and 29 items. The items were composed of 4 items of problem behavior, 4 items of achievement through immersion, 4 items of relationship with friends, 4 items of relationship with teacher, 3 items of physical environment, 4 items of positive emotion, 3 items of negative emotion, and 3 items of health. Second, as a result of validation of validity for infant's happiness scale, the item fit was confirmed with CFI=0.89, TLI=0.88, and RMR=0.05. Also, the construct validity was confirmed with convergent validity and discriminant validity. The concurrent validity was significantly correlated with the components of the previously developed measurement tools, and the reliability was appeared with 0.71-0.89. The infant's happiness scale developed in this study ensured reliability and validity as a measurement to measure infant happiness. Therefore, this scale is expected to be used as a major test tool to develope and verify a program for promoting infant happiness.

Prospective Study on Factors Related to Development in Premature Infants at Six-Months (미숙아로 출생한 영아의 6개월 발달 관련 요인에 대한 전향적 연구)

  • Purpose: This study was conducted to monitor the development of Korean premature infant at six-month age and to explore factors related to developmental status of the premature infants. Methods: Participants were 58 premature infants whose corrected age was six-months old and their mothers. The developmental states of infants were followed-up with the Korean Prescreening Developmental Questionnaire (KPDQ-II). Clinical characteristics of the infants were identified from the medical records. Other characteristics including Edinburgh Postnatal Depression Scale, husband's support, social support, and mother-infant attachment were assessed using self-report questionnaires from the mothers. Results: Forty three percent of the infants were in the group of questionable status of development on the KPDQ-II. There were significant differences between the premature infants with normal developmental status and those with questionable developmental status depending on gender ($x^2$=5.03, p=.034), gestational age (t=2.59, p=.012), hospital stay (p=.013), revised Neurobiologic Risk Score (p=.005) and mother-infant attachment score (t=2.12, p=.040). Conclusion: Mother-infant attachment, as well as physiological state of premature infants, is an important variable in early development. Therefore, early monitoring for the development has to be done for physiologically vulnerable premature groups. Also, providing proper nursing support to improve maternal attachment needs to be considered.

Speech Perception and Language Outcome in Congenitally Deaf Children Receiving Cochlear Implants in the First Year of Life (생후 12개월 이전에 인공와우이식을 받은 선천성 농 아동의 말지각과 언어능력)

  • Background and Objectives The objective of this study was to examine the effect of early cochlear implantation (CI) performed in infants less than 12 months of age. Subjects and Method Twenty-five children who received their first CI before 12 months of age were included in this study (infant group). The speech perception and language outcomes of these children were compared with those of 14 children who received their first CI between 13 and 24 months of age (older group). All children received sequential bilateral CI with the inter-stage interval of less than 2 years. Speech perception was measured using Categories of Auditory Performance, monosyllabic word test and sentence test, and language ability was measured using Sequenced Language Scale for Infants, Preschool Receptive-Expressive Language Scale, or Receptive & Expressive Vocavulary Test, depending on the age at the time of testing. Results There were no significant differences in speech perception abilities between the infant group and the older group. The mean expressive language score of infant group was higher than that of the older group, but the difference was not statistically significant. However, the receptive language score of infant group was significantly higher than that of the older group. Conclusion Children who received CI before 12 months of age achieved better receptive language ability than those who received it after 12 months of age. Thus CI should be performed as early as before 12 months of age to achieve better language ability.

The difficulties of Teachers During the Process of Early Adaptation in Day Care Center (어린이집 초기적응과정에서 나타난 영아반 보육교사의 어려움)

  • This study attempts to reveal the difficulties of teachers during the process of infant's early adaptation in the day care center and realize the thought and demand of teachers. And the purpose of this study is to make a desirable development in infant's early adaptation. The participants of the study were 9 infants all 1 year of age, their 7 parents, and 8 teachers of S public child day care center in P city. The 8 teachers were interviewed several times. The results of the study can be summarized as follows. First, the teachers' difficulties in their relationships with the parents are 1) egotism focusing on only their own child, 2) distrust between the teachers and parents, 3) a lack of understanding in the role as parents at early adaption programs, 4) and a lack of understanding in the management of the day care center. Second, in their relationships with the infants, the teachers'difficulties are 1) the uncontrollable behaviors of infants, 2) the arbitration of struggle between children, 3) and the inequality between enrolled and fresh infants. Third, in their relationships with the teachers, the difficulties are 1) co-work with other teachers 2) and conversation with the director of the day care center. This study is expected to give actual help to understand the difficulties of teachers who experience difficulties in the early infant adaptation process and to provide important preliminary data to plan specific management which assist with the difficulties of teachers.

  • Recently, various health care methods for infants have been suggested in the impending era of low birth rate society. We propose, in this context, an early diagnosis method for common infant respiratory diseases. Particularly, the method is regarding infant cold and infant pneumonia. Firstly, sounds of infant crying, only expressing means of infants, among the infant cold group and the infant pneumonia group are compared and examined to find the differences from those among the healthy infant group. For this, the link between infected organs and articulatory organs is investigated. Also, resulting wave forms and frequency bandwidths among each group are compared and analyzed, by using the spectrum for a component voice, to diagnose the infant cold and pneumonia. Finally, the effectiveness of this method is verified through the experiments.

The Effects of the Child Care Center-Family Linked Sensory Activity on Infant's Physical Development and Infant-Parent Interaction (보육시설과 가정의 연계를 통한 감각활동이 영아의 신체발달과 영아-부모 상호작용에 미치는 효과)

  • The purpose of this study is to investigate effects of 1 or 2 years old infant to target infant care facilities and the sense of family activities conducted in conjunction with the infant's physical development and infant-parent interaction. The purpose of this study, research questions are as follows. First, child care center-family linked sensory activity is effective in the infant's physical development? Second, child care center-family linked sensory activity is effective in the infant-parent interaction? The subjects of this study are 56 infants of 1-2 year old and their mother or father 56 people in S and D infant care facilities in K city. S infant care center, experimental group, carry out child care center-family linked sensory activity. D infant care center is control group to apply an integrated program. The child care center-family linked sensory activity are conduct on 24 times, 3 times a week, totally 8 weeks. The results were as follows. First, child care center-family linked sensory activities are partially effective on the infant's physical development than the control group. Therefore, in order to promote the development of the infant's physical development, home and child care center will be done in conjunction with sensory activities are needed. Second, child care center-family linked sensory activities had positive effect on infant-parent interaction than the control group. Therefore, to improve infant-parent interaction, it is necessary to carry out child care center-family linked sensory activities. In conclusion, child care center-family linked sensory activities are correlated in positive effects on the physical development of infants and the infant-parent interaction.
