The effect of radiographic imaging modalities and the observer's experience on postoperative maxillary cyst assessment

  • Purpose: The purpose of this study was to compare the accuracy of postoperative maxillary cyst (POMC) diagnosis by panoramic radiographs versus computed tomography (CT) and by oral and maxillofacial radiologists versus non-specialists. Materials and Methods: Sixty-five maxillary sinuses with POMCs and 63 without any lesion were assessed using panoramic radiographs and CT images by five oral and maxillofacial radiologists and five non-specialists on a five-point scale. The areas under receiver operating characteristic (ROC) curves were analyzed to determine the differences in diagnostic accuracy between the two imaging modalities and between the two groups of observers. The intra-observer agreement was determined, too. Results: The diagnostic accuracy of CT images was higher than that of panoramic radiographs in both groups of observers (p<0.05). The diagnostic accuracy of oral and maxillofacial radiologists for each method was higher than that of non-specialists (p<0.05). Conclusion: The use of CT improves the diagnosis of POMC, and radiological training and experience leads to more accurate evaluation.

페녹시기와 카르복시레이토페녹시기가 함께 치환된 포스파젠 고분자를 이용한 당뇨병 진단막의 제조 (Preparation of Diabetic Diagnostic Membranes with Phenoxy-Carboxylatophenoxy Cosubstituted Polyphosphazenes)

  • 새로운 형태의 페녹시-카르복시레이토페녹시기가 함께 치환된 포스파젠 고분자를 여러 치환율을 가지고 합성하였다. 이러한 형태의 포스파젠 고분자를 이용해 당뇨병 진단막을 제조하여 혈액 속의 글루코우즈의 농도를 변화시켜가며 680 nm에서의 흡광도를 측정하였다. 기본적으로 흡광도가 시간에 따라 변화하는 양(K/S)과 글루코우즈의 농도가 직선적인 관계를 가지는 것으로 나타났다. 소수성기인 페녹시기와 친수성기인 카르복시레이토페녹시기의 치환율이 글루코우즈 농도 측정에 미치는 영향을 조사하였다. 친수성기의 치환율이 10% 이하인 경우에는 글루코우즈의 침투속도가 너무 느려 글루코우즈의 농도 측정이 어려웠다. 그러나 친수성기의 치환율이 10% 이상으로 증가함에 따라 글루코우즈의 농도와 K/S의 기울기 값(Dose-Response Slope : DRS)이 점차 증가하였다. 하지만 친수성기의 치환율이 30% 이상일 경우에는 글루코우즈가 너무 빨리 침투하여 DRS가 급격히 증가하였다.

양도락의 진단적 활용에 대한 문헌 고찰 - 국내문헌을 중심으로 - (Ryodoraku Application for Diagnosis : a Review of Korean Literature)

  • Objectives : This study is aimed to evaluate the diagnostic value of Ryodoraku for traditional Korean medicine practice reported in the Korean literature. Methods : A search of seven Korean databases was performed up to May 2011 using the search term 'Ryodoraku(in Korean)'. All clinical studies on diagnostic Ryodoraku characteristics were included. Results : The search yielded 59 studies, of which 5 were considered suitable for this analysis. Four were observational studies. Only one was a controlled trial. Average Ryodoraku score(RS), mean values for each measure point, physiologic limit, number of Pyesaek and Gyeokcha, and pattern classification were used as references for the analysis. Correlations were observed between average RS and idiopathic Parkinson's disease, some sub-groups of respiratory disease, chronic low back pain, and menopause. Some mean values for measure points showed particularly high or low tendencies in patients with idiopathic Parkinson's disease, chronic low back pain, and menopause. Positive correlations were observed between the number of Pyesaek and H-Y stage in idiopathic Parkinson's disease and F2(肝) and the modified Kupperman's index with menopausal women. The four classes, organized based on Ryodoraku pattern, showed differences in various characteristics. Conclusions : Since only five studies met the experimental conditions, this evaluation of the diagnostic value of Ryodoraku in various situations is limited. Future studies should be conducted using various physiologic and pathologic situations.

페녹시-메틸아미노 포스파젠 당뇨병 진단막의 글루코우즈 농도 측정에 미치는 온도와 습도의 영향 (Effects of Temperature and Humidity on the Glucose Measurements with Phenoxy-Methylamino Phosphazene Diagnostic Membranes)

  • 당뇨병 환자의 혈당을 측정하기 위해 새로운 형태의 페녹시-메틸아미노 포스파젠 진단막을 제조하였다. 먼저 측정 환경이 혈당 측정에 미치는 영향을 조사하였다. 활성화된 진단막을 가지고 5, 15, 25, 35, $45^{\circ}C$ 그리고 상대습도 20%에서 80%까지 변화시키면서 시간에 따른 680 nm에서의 흡광도 변화량(K/S)를 측정하였다. 높은 측정온도와 습도에서도 K/S와 글루코우즈 농도와의 기울기 값(Dose-Response Slope : DRS)은 크게 영향을 받지 않았다. 두 번째로, 제조된 진단막을 여러 환경에서 보관한 후, 보관 환경이 글루코우즈의 농도 측정에 미치는 영향을 조사하였다. 새로운 포스파젠 진단막을 $50^{\circ}C$에서 8주 보관한 후 측정한 결과, 보관시간과 온도가 크게 영향을 주지 않았다. 또 상대습도 80%에서도 새로운 포스파젠 진단막의 안정성이 확인되었다.

Development and Clinical Evaluation of a Rapid Diagnostic Test for Yellow Fever Non-Structural Protein 1

  • A rapid diagnostic test (RDT) kit was developed to detect non-structural protein 1 (NS1) of yellow fever virus (YFV) using monoclonal antibody. NS1 protein was purified from the cultured YFV and used to immunize mice. Monoclonal antibody to NS1 was selected and conjugated with colloidal gold to produce the YFV NS1 RDT kit. The YFV RDTs were evaluated for sensitivity and specificity using positive and negative samples of monkeys from Brazil and negative human blood samples from Korea. Among monoclonal antibodies, clones 3A11 and 3B7 proved most sensitive, and used for YFV RDT kit. Diagnostic accuracy of YFV RDT was fairly high; Sensitivity was 0.0% and specificity was 100% against Dengue viruses type 2 and 3, Zika, Chikungunya and Mayaro viruses. This YFV RDT kit could be employed as a test of choice for point-of-care diagnosis and large scale surveys of YFV infection under clinical or field conditions in endemic areas and on the globe.

CRISPR/Cas 진단의 원리와 현황 (The Principle and Trends of CRISPR/Cas Diagnosis)

  • The POCT (point-of-care test) sensing that has been a fast-developing field is expected to be a next generation technology in health care. The POCT sensors for the detection of proteins, small molecules and especially nucleic acids have lately attracted considerable attention. According to the World Health Organization (WHO), the POCT methods are required to follow the ASSURED guidelines (Affordable, Sensitive, Specific, User- friendly, Robust and rapid, Equipment-free, Deliverable to all people who need the test). Recently, several CRISPR (Clustered Regularly Interspaced Short Palindromic Repeats) based diagnostic techniques using the sensitive gene recognition function of CRISPR have been reported. CRISPR/Cas (Cas, CRISPR associated protein) systems based detection technology is the most innovative gene analysis technology that is following the ASSURED guidelines. It is being re-emerged as a powerful diagnostic tool that can detect nucleic acids due to its characteristics that enable rapid, sensitive and specific analyses of nucleic acid. The first CRISPR-based diagnosis begins with the discovery of the additional function of Cas13a. The enzymatic cleavage occurs when the conjugate of Cas protein and CRISPR RNA (crRNA) detect a specific complementary sequence of the target sequence. Enzymatic cleavage occurs on not only the target sequence, but also all surrounding non-target single-stranded RNAs. This discovery was immediately utilized as a biosensor, and numerous sensor studies using CRISPR have been reported since then. In this review, the concept of CRISPR, the characteristics of the Cas protein required for CRISPR diagnosis, the current research trends of CRISPR diagnostic technology, and some aspects to be improved in the future are covered.

호흡기 바이러스 감염의 진단법과 감염관리 (Diagnostic Methods of Respiratory Virus Infections and Infection Control)

  • 호흡기 바이러스는 감염된 방문자와의 직접적인 접촉을 통해 병원 환경에서 감염된다. 감염관리 분야에서 호흡기 바이러스에 의한 병원내 획득 감염의 주요한 문제를 유발한다. 임상 검사실에서 파생된 감시 데이터는 또한 의료 서비스를 제공하는 기관과 공중 보건 분야에서 치료, 소모품 및 진단 제품 구매를 위해 병원과 지역 사회에 의료자원을 적절하게 배분하기 위해 종종 사용된다. 호흡기바이러스의 감염에서 조기 진단은 필수적이며 호흡기 검체를 사용하는 진단법에 활용될 수 있는 방법에는 바이러스 배양, 분자 진단 및 분석 등이 포함된다. 랩온어칩(LoC)/마이크로칩은 보다 다양하고 강력한 기술인 차세대 현장검사 시험법을 개발할 수 있는 새로운 전략으로 제공한다. 호흡기계의 중요성은 의료관련 종사자의 직업적 건강과 안전을 보장하기 위해 감염 관리 지침에 엄격히 적용되어야 한다. 이 연구를 포함하여 점점 더 많은 임상적 효능 증거가 감염 전파에 대한 오랜 패러다임에 도전하고 있다. 바이러스의 감염 가능성을 의심하는 새로운 호흡기 증상이 시작된 입원 환자로부터 호흡기 바이러스를 탐지하기 위한 빈번한 검사에 대해 추가 지원이 요구되고 감염통제의 노력에 집중적으로 도움이 이루어져야 할 것이다.

모바일 물리치료 진단 어플리케이션 개발 및 사용성 평가 (Development and Usability Evaluation of a Mobile Physical Therapeutic Diagnosis Application)

  • Purpose: The physical therapy diagnosis process requires high-level background knowledge, the ability to obtain added information from patients, accurate examination skills, and a framework for transforming thoughts into a diagnostic decision. Thus, the physical therapy diagnostic process is highly complicated and difficult work. To function as autonomous professionals, physical therapists must develop effective clinical diagnosis skills. As such, mobile application aids can help with accurate and scientific diagnoses. Therefore, this study aims to develop and evaluate the usability of a mobile application for physical therapy diagnoses. Methods: In this study, a diagnostic application was developed using App Inventor, the development environment was the Chrome web browser for Windows 10, and the mobile application was run on a Google Pixel 5. The developed application was evaluated for usability by 20 physical therapists with more than 5 years of clinical experience in the musculoskeletal physical therapy field, and a usability evaluation was conducted using a 5-point Likert scale for accuracy, convenience, satisfaction, and usability. The collected Likert scores were converted into percentages and analyzed as descriptive statistics. Results: The graphical user interface consisted of an initial screen with program guidance, 18 screens presenting the algorithm, and 12 screens presenting the estimated diagnosis based on the algorithm. As such, the usability evaluation of the developed application was as follows: accuracy 100%, convenience 90%, satisfaction 91%, and usability 88%. Conclusion: The newly developed mobile application for physical therapeutic diagnoses has a high accuracy, and it will aid in building an easy and reliable physical therapy diagnosis system.

Potential impact of metal crowns at varying distances from a carious lesion on its detection on cone-beam computed tomography scans with several protocols

  • Purpose: This study evaluated the impact of artifacts generated by metal crowns on the detection of proximal caries lesions in teeth at various distances using cone-beam computed tomography (CBCT). Additionally, the diagnostic impacts of tube current and metal artifact reduction (MAR) were investigated. Materials and Methods: Thirty teeth were arranged within 10 phantoms, each containing 1 first premolar, 1 second premolar, and 1 second molar. A sound first molar (for the control group) or a tooth with a metal crown was placed. Of the 60 proximal surfaces evaluated, 15 were sound and 45 exhibited enamel caries. CBCT scans were acquired using an OP300 Maxio unit (Instrumentarium, Tuusula, Finland), while varying the tube current (4, 8, or 12.5 mA) and enabling or disabling MAR. Five observers assessed mesial and distal surfaces using a 5-point scale. Multi-way analysis of variance was employed for data comparison, with P<0.05 indicating statistical significance. Results: The area under the curve (AUC) varied from 0.40 to 0.60 (sensitivity: 0.28-0.45, specificity: 0.44-0.80). The diagnostic accuracy was not significantly affected by the presence of a metal crown, milliamperage, or MAR(P>0.05). However, the overall AUC and specificity were significantly lower for surfaces near a crown (P<0.05). Conclusion: CBCT-based caries detection was not influenced by the presence of a metal crown, variations in milliamperage, or MAR activation. However, the diagnostic accuracy was low and was further diminished for surfaces near a crown. Consequently, CBCT is not recommended for the detection of incipient caries lesions.

진단용 방사선 발생장치의 수시 정도관리 항목 및 기준에 관한 기초 연구 (A Base Study on the Constancy Quality Control Test and Clause of Diagnosis Radiation Equipment)

  • 국내 진단용 방사선 발생장치는 3년 주기로 시행되는 정기적인 정도관리 검사에 앞서 일별, 주별, 월별, 분기 반기별로 시행하는 수시 정도 관리에 대한 항목 및 규정이 명확하게 지정되어 있지 않다. 미국과 유럽 등의 선진국에서는 정도 관리 항목에 대해 검사를 수행하는 검사자를 구분하여 국가 기관에 보고하도록 되어 있으므로 체계적인 검사 수행과 더불어 검토가 가능하다. 그러므로 의료 장비의 현대화가 이루어지는 시점에서 국외의 주기별 정도관리 시스템을 국내 진단용 방사선 발생 장치에 적합하도록 도입하여 정도관리 항목과 기준의 재정립이 반드시 필요하다. 이에 본 논문에서는 진단용 방사선 발생장치 중 가장 빈번히 사용되고 있는 일반 X선 촬영 장치의 3년 정기점검 사이에 주기적인 수시 정도 관리 검사 항목과 방법 및 기준 수립을 위해 미국, 캐나다 등의 국외 문헌 조사 및 국제 전자 기술 위원회(International Electro-technical Commission, IEC)의 규정 항목과 비교하였다. 이를 바탕으로 국내 수시 정도관리 항목 신설 시 필요한 검사 항목 및 기준 설정을 위한 기초 자료로 제시하고자 하였다.
