Experiences of Life-Sustaining Treatment Decisions among Patients with Terminal Cancer

  • Purpose: The purpose of this study was to examine the life-sustaining treatment decisions of terminal cancer patients. Methods: Data on 10 terminal cancer patients who decided to withhold or withdraw from treatment were collected using in-depth interviews conducted from February 8 to October 30, 2019. Data were collected until saturation was reached and then analyzed using Colaizzi's phenomenological method. Results: In this study, six thematic clusters were identified: "having complicated feelings", "making choices to protect everyone", "accepting and preparing for death", "feeling distress", "pursuing spiritual wellbeing", and "evaluating the new system". Conclusion: When facing death, terminal cancer patients often made choices to protect their family and their dignity with uneasiness of mind when deciding to withdraw from life-sustaining treatments. Though many patients had accepted and prepared for death, they experienced distress about leaving children behind after death. They also pursued spiritual well-being to find peace after deciding to withdraw from life-sustaining treatment. In addition, participants evaluated the new system of policies pertaining to decisions on life-sustaining treatment. Thus, various approaches regarding acceptance and preparation for death, communication with family, hope, and spiritual comfort should be taken in educational interventions to assist terminal cancer patients as they decide whether to withdraw from life-sustaining treatment.

대학생의 죽음에 대한 주관성 연구 (Subjectivity toward Death among College Students)

  • The purpose of this study was to classify the subjectivity type toward death among college students and to understand the characteristics of attitudes and orientations toward death of each type. Since attitudes and orientations toward death is very subjective and unique, Q-methodology was employed in this study. Q-methodology explains the respondent's subjectivity by objectifying his subjectivity for himself. The P-sample consisted of 63 students of K university in D city. Forty statements concerning attitudes and orientations toward death, which developed by Yeun(1999) were utilized for Q-sample. Forty Q-statements were sorted according to the level of agreement or disagreement by forced normal distribution. The Q-sorts by each subjects were coded and analysed with the PC-QUANL program. Five types of subjectivity toward death were identified and labeled. Type 1 'the death- preparation' think frequently about his own death and talk openly about the problem of death with a positive attitude. Type 2 'the life-esteemed' respect the dignity of life most of all. Type 3 'the realty-oriented' do not believe the afterlife and is very concerned about the present life itself instead of thinking about death. Type 4 'the ambivalent' think importantly the aspect of present life and orient the afterlife at the same time. This type reveals opposite attitude of preparing and scaring the death at the same time. Type 5 'the destiny-recipient' attribute death to the destiny and deny suicide. On the basis of this result, the studies about applying and developing educational program on death and hospice care for nurses who care terminal cancer patients and families are needed.

죽음불안도에 영향을 미치는 요인들에 관한 탐색적 연구 - 죽음불안도 4가지 영역에 따른 노년층과 비노년층의 차이를 중심으로 - (The Relationship between Selected Personal Demographic Variables and the Four Dimension of Death Anxiety - difference between elderly group and non-elderly group -)

  • How an elderly people meets death is the matter of how he has lived his life. It is very important for an elderly people at the last step of his life to re-light up life and to meet death with dignity. The purpose of this study is to investigate where fear or anxiety of death come from among the four dimensins of death anxiety and to compare the differenced between the elderly group and non-elderly group, For this research, the 473 of the subjects from 20 to 80 years old attending social welfare center and community areas in Seoul have been questionned. The summary for the study mentioned the following: First, the overall scores of death anxiety, in the non-aged group, gender and religiosity are important factors affecting the decrease of death anxiety, On the other hand, in the aged group, self-respect, death readiness and number of friends are significant factors. Secondly, for death anxiety of self, age and spouse are significant relationships among non-aged group and gender, death readiness and number of friends for aged-group. In the dying of self, the following each three significant variables: gender, self-respect and spouse among non-aged group and gender, self-respect and number of friends among aged group. In death anxiety of others, age, view on next world and spouse are best predictor for non-aged group. Finally, family-relationship, self-respect and spouse are significant factors for aged group. In dying of others, only one factors are influenced for non-aged group, on the other hand, gender, self-respect and death readiness are important factors for aged group. There remains the need for more detailed examination into the nature of this relationship and the extent to which core components strongly affecting the above subscales.

Notification of Terminal Status and Advance Care Planning in Patients with Cancer

  • As population aging increases the burden of cancer, the quality of death of patients with cancer is emerging as an important issue alongside their quality of life. To improve the quality of death, it is necessary to prepare for death, allowing patients to die comfortably and with dignity at the end. Considering these issues, I aim to discuss the practical aspects of notifying the patient of the terminal phase of cancer and planning for end-of-life care (i.e., advance care planning). When cancer treatment that can extend the patent's lifespan becomes difficult, the patient enters a treatment transition period. Treatment is shifted from life-prolonging care to life-enhancing care, and end-of-life care must be well planned. Medical providers often worry too much about whether the patient will be disappointed or psychologically traumatized when notified of the terminal phase of their cancer, thus delaying plans for end-of-life care. In fact, patients can accept their condition and prepare for end-of-life care better than we expect. During the treatment transition period, notification of terminal status should be given, and a well-prepared advance care plan should be established early when the patient has decision-making ability. In addition to conveying information, it is always necessary to be sensitive to whether the patient and caregiver understand the information and respond to their emotions.

노인이 인식하는 좋은 죽음 (Good death recognized by the elderly)

  • 본 연구는 좋은 죽음의 의미가 무엇인가에 대한 질문을 가지고 지역사회에 거주하는 65세 이상의 노인을 대상으로 하여 '좋은 죽음이 무엇이라고 생각 하십니까?' 라는 연구 질문으로 시작 하였다. 지역에 거주하는 65세 이상 노인 350명을 대상으로, 2010년 9월부터 2011년 2월 까지 5개월에 걸쳐 '좋은 죽음이 무엇이라고 생각하십니까?'라는 단일질문에 대해서 인터뷰를 하였다. 전사된 인터뷰 자료를 내용 분석한 결과는 다음과 같다. 노인들이 인식하는 좋은 죽음의 하위범주는 '배우자와 비슷한 시기에 함께 하는 죽음', '어느 누구에게 폐치지 않는 죽음', '다른 사람에게 좋은 사람으로 기억되는 죽음', '적당한 수명을 누리는 죽음', '명을 다하는 죽음', '내 집에서 맞이하는 죽음', '편안한 죽음', '잠자는 듯한 죽음', '아프지 않는 죽음', '임종과정이 길지않는 죽음', '준비된 죽음', '삶의 최선을 다하고 맞이하는 죽음', '삶을 즐기고 맞이하는 죽음', '베푸는 삶을 살고 가는 죽음', '신앙 속에서의 죽음', '자손이 잘되는 것 보고 가는 죽음'등 16개의 하위 범주가 도출 되었으며, 이는 다시 '주변사람을 배려하는 죽음', '천수를 누리는 죽음', '내 집에서 맞이하는 죽음', '편안한 모습으로의 죽음', '준비된 죽음', '원하는 삶을 누리다 가는 죽음' 등 6개의 범주로 묶여졌다.

  • 인간의 존엄성과 환자의 자율성에 터잡아 연명의료의 중단이 제도화된 이래, 최근에는 스스로 존엄하게 죽음에 이르기 위해 환자가 자기 생명을 단축시키고자 의료적 도움을 받을 권리가 있는지, 그러한 행위가 어느 범위에서 정당화될 수 있는지의 문제가 활발하게 논의되고 있다. 우리나라에서는 2016년 연명의료결정법의 제정으로 연명의료중단이 제도화된 이래 여러 차례 개정을 거쳤는데, 최근에는 '조력존엄사'를 합법화하기 위한 내용의 연명의료결정법 개정안이 발의되었다. 이번 개정안에서는 조력존엄사를 '환자의 의사로 담당의사의 조력을 통해 스스로 삶을 종결하는 것'으로 정의하고 있는데, 이는 네덜란드, 벨기에 등의 유럽 국가와 미국 일부 주에서 시행되고 있는 조력사망(Aid in Dying)에 해당하는 내용으로 보인다. 조력사망이란 의사결정능력이 있는 환자가 치료가 불가능한 질병으로 고통을 받고 있을 때 환자가 사망을 앞당길 수 있는 약물을 의사로부터 처방받아 이를 이용하여 사망에 이르는 것을 의미한다. 존엄사에 대한 논의는 연명의료중단에서 조력사망의 순서로 진행되는 것이 세계적 추세인데, 2000년대에 이르러 일부 국가에서는 조력사망, 나아가 적극적 안락사까지 합법화하였다. 미국에서는 오리건 주를 필두로 여러 주에서 조력사망을 인정하는 법률을 두고 있지만 적극적 안락사에 대해서는 금지하고 있다. 이 논문에서는 일찌기 존엄한 죽음에 관한 논의를 시작하여 환자의 자기결정권을 제도화한 미국의 일부 주에서 조력사망의 입법화가 어떤 과정을 거쳐 이루어졌는지를 살펴보고 캘리포니아의 임종선택법의 주요 내용과 법시행 이후의 결과를 분석하였다.

무대 의상에 나타난 블랙의 미적 특성 - 뮤지컬 '시카고'를 중심으로 - (The Study on the Aesthetic Characteristics in Black Shown in Stage Costume - Focused on Musical 'Chicago' -)

  • This study examined that the black dress is the one of having aesthetic value as well as symbolism while delivering specific image, from the perspective of color, which is essential element in forming design of the Stage Costume. This study carried out the theoretical research of centering on the literature consideration and the empirical research of centering on a case side by side. It analyzed the works and design based on the clothing, which had been performed in Korea with a revival version of Musical 'Chicago'. Aesthetic characteristics in more than two are being shown in one dress. If so, the following are the results that considered on aesthetics characteristics in black, which were shown in Stage Costume of Musical 'Chicago', which is increasing the theme consciousness through Stage Costume, which is simple and sexy black. First, as emotion of coming to stay in agony and sadness on the basic fact called death, the Stage Costume in black led to being shown sorrow of implying death, fear and murder. Second, the dignity, which corresponds to noble beauty in the aesthetic category, expresses dignity and elegance with flat material in black, and is revealed the aspect of expressing fashion, which emphasized spirit rather than body. Third, as the aesthetic characteristic of causing sexual desire, the sensuality was shown, which has the aspect of being expressed through exposure, concealment and emphasis. Fourth, it was expressed with androgyny, which evokes androgynous image. Fifth, black in Musical 'Chicago' formed the whole mood with a sense of weight by arranging the whole feeling with color in similar tone, thereby having been indicated the harmony of being formed with theme, which is aimed to be indicated in drama.

종교유무에 따른 한국인의 '연명의료' 결정과 태도에 관한 융합연구 (A Convergence Study on the Decision and Attitude of Korean "life-prolonging medical care" according to whether or not religion)

  • 본 연구에서는 '호스피스 완화의료'의 개념 및 '연명의료' 결정 내용에 대해 일반인들이 어느 정도 인식하고 있는지 알아보고자 하는 목적으로 시행되었다. 2016년 9월 일개 지역에서 열린 행사에 참여한 참가자 및 관계자 중 자발적으로 동의한 346명을 대상으로 설문조사를 진행하였다. 연구결과를 보면, 종교를 가진 사람들이 종교가 없는 사람들에 비해 자연스런 죽음을 원하고 있었으며, 존엄사와 관련된 결정에서 적극적인 태도를 취하는 것으로 보인다. 종교가 있는 집단에서 연명치료 중 인공호흡시행에 동의한 대상이 많이 나왔다. 종교는 삶과 죽음에 대한 인식에 영향을 미칠 수 있는 중요 요소로서 죽음 후 내세가 있다고 믿기 때문에, 무의미한 삶의 연장에 대해서 보다 단호한 입장을 취할 수 있는 것으로 생각된다. 따라서 호스피스 완화의료와 사전연명의료결정법이 일반인들에게 안정적으로 정착하여 존엄한 삶의 마무리와 죽음의 질을 높이기 위해서는 연령, 교육정도, 종교의 유무 등을 고려하여 다양한 교육 프로그램을 통한 인식 확산을 하여야 할 것이다. 또한 호스피스 완화의료 및 연명의료 관련 지식을 일반인이 충분히 이해할 수 있도록 교육 및 홍보를 강화해야 할 것이며 호스피스에 대한 정부차원의 표준화와 정책적 뒷받침이 시급할 것으로 보인다.

연명(延命)치료적 인공기계호흡요법의 보류(保留)/중지(中止)를 전후한, 법의학적 및 윤리적 문제들과 그 대처방안 (The Medico-Legal and Ethical Problems of Withholding / Withdrawing of Futile Life-Sustaining Mechanical Respirator treatment)

  • The first and the longest criminal indictment case of Korean medico-legal battle, so called BORAMAE Hospital Incident, was finally on its end by Korean Supreme Court's decision on June 24, 2004, after 7 years long legal dispute via Seoul District Court and Seoul Superior Appeal Court's decision. Boramae Hospital case was the first Korean legal case of Withdrawing Life-sustaining treatment of mechanical respirator on 58 years old Extradural Hematoma victim who was on Respirator under Coma after multi-organ failure postoperatively(APACHE II score: 34-39). Two physicians who have involved patient's care and had helped to make discharge the Near-death patient to home after repeated demand of patient's wife, due to economic reason, were sentenced as homicidal crime. This review article will discuss the following items with the review of US cases, Quinlan(1976), Nancy Cruzan(1990), Barber (1983), Helen Wanglie(1990), Baby K (1994) and Baby L cases, along with Official Statement of ATS and other Academic dignitaries of US and World.: [1] Details of Boramae Hospital incident, medical facts description and legal language of homicidal crime sentence. [2] The medical dispute about the legal misinterpretation of patient's clinical status, regarding the severity of the victim with multi-organs failure on Respirator under coma with least chance of recovery, less than 10% probability. [3] Case study of US, of similar situation. [4] Introduction of ATS official Statement on Withdrawing/ Withholding Life sustaining treatment. [5] Patient Autonomy as basic principle. [6] The procedural formality in Medical practise for keeping the legitimacy. [7] The definition of Medical Futility and its dispute. [8] Dying in Dignity and PAS(Physician Assisted Suicide)/and/or Euthanasia [9] The Korean version of "Dying in Dignity", based on the Supreme Court's decision of Boramae Hospital incident (2004.6.24.) [10] Summary and Author's Note for future prospects.

노인요양시설 봉사경험 간호대학생의 생명윤리 주관성 (Subjectivity about Bioethics among Nursing Students with Experience Volunteering in Elderly Care Facilities)

  • Purpose: This study aims to determine the bioethical subjectivity of nursing students with experience volunteering in elderly care facilities, and the characteristics that comprise the types thereof. Methods: Q methodology, which analyzes the subjectivity of each type, was used. The 37 selected Q statements from 32 participants were classified into the shape of a normal distribution using a 9-point scale. The collected data were analyzed using the PC-QUANL program. Results: The survey revealed that the bioethical subjectivity of nursing students with experience volunteering in elderly care facilities can be divided into three types: a rational dignity emphasis, an autonomous right to life belief, and conflict avoidance. All three attitudes regard human beings as possessing dignity, and life and death as elements to be experienced as aspects of human life. Bioethical values are critical to treatment and care; however, it is sometimes also held that humans have the right to commit suicide. Conclusion: This study enhanced our awareness of nursing students' bioethics. The findings can be used as a basis for the design of differentiated bioethics education according to each type of bioethical subjectivity. This calls for diverse research on bioethics and the implementation of effective bioethics education.
