Cone-beam CT로부터 제작된 측모 두부계측방사선사진의 정확도 평가 (The reliability of the cephalogram generated from cone-beam CT)

  • 최근 computed tomography (CT) 데이터로부터 실제 두부계측방사선사진 촬영기에서 촬영한 사진과 동일한 확대율을 갖는 두부계측방사선사진을 형성할 수 있는 기능을 갖춘 소프트웨어(Accurex, CyberMed, Seoul, Korea)가 소개되었다. 이에 본 연구에서는 cone-beam CT (이하 CBCT) 데이터로부터 이 소프트웨어를 이용하여 제작된 측모 두부계측방사선사진과 기존의 두부계측방사선사진촬영기로 촬영된 측모 두부계측방사선사진에서 각각 두부계측방사선사진 분석을 시행하여 그 차이를 비교함으로써 소프트웨어를 통해 CBCT로부터 제작된 측모 두부계측방사선사진의 활용 가능성을 평가하고자 하였다. 20명의 교정 환자를 대상으로 CBCT와 측모 두부계측방사선사진을 동시에 촬영하였다. CBCT로부터 제작된 측모 두부계측방사선사진과 기존 방식의 측모 두부계측방사선사진에서 두부계측 방사선사진 분석을 시행한 후 두 사진의 계측치의 차이를 paired t-test로 검정하였다. 23개의 계측치들 중, 20개 항목에서는 통계학적으로 유의한 차이가 관찰되지 않았으나, U1-FH, Mx6 to PTV, Maxillomandibular difference의 항목에서는 유의한 차이가 나타났다 이러한 차이는 CBCT로부터 형성된 측모 두부계측방사선사진의 해상도가 낮아 일부 해부학적 지표를 확인하기 어렵기 때문이며, 임상에서 CBCT로부터 제작된 측모 두부계측방사선사진을 porion, PTV, condylion과 같은 일부 해부학적 지표를 이용하지 않는 SNA, SNB, U1 to SN, IMPA, interincisal angle 등 흔히 사용되는 간단한 측모 두부계측방사선사진 계측에 활용 가능할 것이다.선 병용요법은 절제 불가능한 원발성 간암의 증상호전 및 국소적 치료에 효과가 있는 것으로 생각되며 생존율 및 부작용 등에 관해서는 앞으로 연구가 더 진행되어야 할 것으로 생각된다.용 선형가속기의 전자선 방출구에 장착된 산란판과 조사면을 조정하는 cone에 의하여 발생되는 저 에너지 산란전자선이 alanine dosimeter에 측정된 것으로서 에너지가 증가될수록 오염 정도가 증가되었다. 본 실험을 통하여 지금까지 고에너지 전자선량계측에서 전리상에 의한 전기량 측정과 산란선이 없는 단일 에너지로만 간주하여 계산하였던 전자선 흡수선량 측정방법을 직접 흡수선량 측정이 가능한 Alanine/ESR dosimetry로서 교정하는 것이 바람직하다고 생각한다.$6\%$, 수술이 필요하였던 환자는 $1\%$로 합병증의 증가 없이 A점에 8900cGy, 직장에 7300cGy정도의 국소치료가 가능함을 보여 주었으나 생존율에 대한 추후 분석이 따라야 할 것으로 사료된다.보면 6,000rad이하에서 20/63$(34.9\%)$의 실패율에 비하여 6,000rad이상일 때는 10/49$(20.4\%)$의 실패율이었다. 연령 벨로는 $40\~49$세에서 실패율(14/41 $24.1\%$)이 많았다. 본 성적으로 보아서 생존율은 여러 저자들과 큰 차이가 없음을 알 수 있었고 A점 선량은 8,000rad 이상, B점은 6,000rad이상이 조사되어야 적정선량이 됨을 시사해 주고 있다.\%$가 대부분이 하루 세끼 식사를 규칙적으로 하고 있었으며 식사속도는 허겁지겁 빨리 섭취하는 경우가 남자는 $31.0\%$,

A Review of Organ Dose Calculation Tools for Patients Undergoing Computed Tomography Scans

  • Background: Computed tomography (CT) is one of the crucial diagnostic tools in modern medicine. However, careful monitoring of radiation dose for CT patients is essential since the procedure involves ionizing radiation, a known carcinogen. Materials and Methods: The most desirable CT dose descriptor for risk analysis is the organ absorbed dose. A variety of CT organ dose calculators currently available were reviewed in this article. Results and Discussion: Key common elements included in CT dose calculators were discussed and compared, such as computational human phantoms, CT scanner models, organ dose database, effective dose calculation methods, tube current modulation modeling, and user interface platforms. Conclusion: It is envisioned that more research needs to be conducted to more accurately map CT coverage on computational human phantoms, to automatically segment organs and tissues for patient-specific dose calculations, and to accurately estimate radiation dose in the cone beam computed tomography process during image-guided radiation therapy.

  • Purpose: To determine the physical properties of a newly developed cone beam computed tomography (CBCT). Materials and Methods: We measured and compared the imaging properties for the indirect-type flat panel detector (FPD) of a new CBCT and the single detector array (SDA) of conventional helical CT (CHCT). Results: First, the modulation transfer function (MTF) of the CBCT were superior to those of the CHCT. Second, the noise power spectrum (NPS) of the CBCT were worse than those of the CHCT. Third, detective quantum efficiency (DQE) of the indirect-type CBCT were worse than those of the CHCT at lower spatial frequencies, but were better at higher spatial frequencies. Although the comparison of contrast-to-noise ratio (CNR) was estimated in the limited range of tube current, CNR of CBCT were worse than those of CHCT. Conclusion: This study shows that the indirect-type FPD system may be useful as a CBCT detector because of high resolution.

Comparison of panoramic radiography with cone beam CT in predicting the relationship of the mandibular third molar roots to the alveolar canal

  • Purpose: Preoperative radiographic assessment of the mandibular third molars is essential to prevent inferior alveolar nerve damage during extraction. The purpose of this study was to assess the reliability of panoramic signs of association between the roots of teeth and the canal, and to compare the panoramic signs with cone beam computed tomography (CBCT) findings. Materials and Methods: CBCT images of 132 impacted mandibular third molars were evaluated to determine the association of the root to the canal. The CBCT findings were compared with the corresponding panoramic images. Logistic regression analysis was used to define the diagnostic criteria of the panoramic images. Results: Among the panoramic signs, loss of the cortical line was the most frequent radiographic sign predicting association (sensitivity: 79.31). Contact of the tooth with the canal was observed in all cases in which the loss of cortical line of the canal or darkening of the roots was found on the panoramic radiographs. Conclusion: Darkening of the roots and loss of the cortical line on panoramic radiographs might be highly suggestive of the risk of nerve injury.

Anomalies of the clivus of interest in dental practice: A systematic review

  • Purpose: The clivus is a region in the anterior section of the occipital bone that is commonly imaged on large-volume cone-beam computed tomography (CBCT). There have been several reports of incidental clivus variations and certain pathological entities that have been attributed to the variations. This study aimed to evaluate the effects of these variations within the scope of dentistry. Materials and Methods: Medical databases (PubMed, Scopus, and Web of Science) were searched using a controlled vocabulary (clival anomalies, cone-beam CT, canalis basilaris medianus, fossa navicularis magna, clival variation). The search was limited to English language, humans, and studies published in the last 25 years. The articles were exported into RefWorks® and duplicates were removed. The remaining articles were screened and reviewed for supporting information on variations of the clivus on CBCT imaging. Results: Canalis basilaris medianus and fossa navicularis magna were the most common anomalies noted. Many of these variations were asymptomatic, with most patients unaware of the anomaly. In certain cases, associated pathologies ranged from developmental (Tornwaldt cyst), to acquired (recurrent meningitis). While no distinct pathognomonic aspects were noted, there were unique patterns of radiographic diagnosis and treatment modalities. Most patients had a normal course of follow-up. Conclusion: Interpretation of CBCT volumes is a skill every dentist must possess. When reviewing large-volume CBCT scans, the clinician should be able to distinguish pathology from normal anatomic variations within the skull base. The majority of clivus variations are asymptomatic and will remain undetected unless incidentally noted on radiographic examinations.

Cone-beam CT영상으로부터 얻어진 정모두부방사선사진에서 수평기준선의 설정 (Validity of Horizontal Reference Planes on Cone-Beam Computed Tomography Generated Postero-Anterior Cephalogram)

  • Purpose: The purpose of this study was to measure the angular differences between the horizontal reference planes on the CBCT generated PA cephalogram and the modified interpupillary plane, which was usually used in the clinical examination, and to evaluate the validity of the horizontal reference planes. Methods: The CBCT generated PA cephalogram was used to measure the angles between the FH, Lo and IP planes. The subjects consisted of 42 patients with facial asymmetry (males: 21, females: 21, mean-age: 21.6 years). The control groups were also assessed (males: 10, females: 10, mean-age: 23.8 years). The distance of the interpupil was measured on the soft-tissue volume rendered image. The angular differences were statistically analyzed using the $Mann-Whitney$ $U$ $test$ for inter-group comparisons and the $Friedman$ $test$ for intra-group comparisions. Results: The angle between the FH plane and IP plane (the angle of the FH-IP line) showed a statistically significant difference between the two groups ($p$ <0.05). There was no statistical differences between each angle (angle of the FH-IP line, angle of the FH-Lo line, angle of the Lo-IP line) on the intra-group comparision ($p$ >0.05). Conclusion: The angle between the Lo line and IP line (angle of the Lo-IP line) showed no statistically significant difference in both the control and asymmetry groups. Therefore, the Lo line could be used as a horizontal reference plane in CBCT generated PA cephalograms.

안전한 임플란트 식립을 위한 하악골 내측면의 CBCT를 이용한 해부학적 연구 (An anatomical study on the mandibular medial surface by CBCT analysis for safer implant placement)

  • Introduction: This study examined the anatomical morphology of the medial surface of the posterior mandible using 3-dimensional cone-beam computed tomography (CT) images to reduce the number of complications related to dental implant placement. Materials and Methods: Fifty patients were enrolled in this study with an average age (${\pm}$standard deviation) of 44.28 (${\pm}13.05$). On the coronal views cone-beam CT of the first molars, the distance between the top of the canal and alveolar crest vertical distance (VD), the distance between the upper-most point of the canal and the point perpendicular to the lingual cortical margin of the mandible lingual distance (LD), the location of the starting point of VD for reducing from the vertical reference line (VD point), and the inclination of the mandibular medial surface (lingual inclination) were measured, and a statistical evaluation was performed using SPSS for Windows version 15.0. Results: The mean VD0 was $16.91{\pm}2.47\;mm$ and VDx decreased with increasing x value. The mean LD was $5.27{\pm}1.36\;mm$. The VD began to decrease at the mean location of $6.12{\pm}0.96\;mm$ from the vertical reference line. The mean lingual inclination was $1.52{\pm}0.72^{\circ}$. Conclusion: These results will assist in the accurate placement of dental implants and the reduction of complications, particularly in the case of preoperative implant planning using only 2-dimensional imaging methods. Conversion coefficients for the estimation of effective dose in cone-beam CT

  • Purpose: To determine the conversion coefficients (CCs) from the dose-area product (DAP) value to effective dose in cone-beam CT. Materials and Methods: A CBCT scanner with four fields of view (FOV) was used. Using two exposure settings of the adult standard and low dose exposure, DAP values were measured with a DAP meter in C mode ($200mm{\times}179mm$), P mode ($154mm{\times}154mm$), I mode ($102mm{\times}102mm$), and D mode ($51mm{\times}51mm$). The effective doses were also investigated at each mode using an adult male head and neck phantom and thermoluminescent chips. Linear regressive analysis of the DAP and effective dose values was used to calculate the CCs for each CBCT examination. Results: For the C mode, the P mode at the maxilla, and the P mode at the mandible, the CCs were 0.049 ${\mu}Sv/mGycm^2$, 0.067 ${\mu}Sv/mGycm^2$, and 0.064 ${\mu}Sv/mGycm^2$, respectively. For the I mode, the CCs at the maxilla and mandible were 0.076 ${\mu}Sv/mGycm^2$ and 0.095 ${\mu}Sv/mGycm^2$, respectively. For the D mode at the maxillary incisors, molars, and mandibular molars, the CCs were 0.038 ${\mu}Sv/mGycm^2$, 0.041 ${\mu}Sv/mGycm^2$, and 0.146 ${\mu}Sv/mGycm^2$, respectively. Conclusion: The CCs in one CBCT device with fixed 80 kV ranged from 0.038 ${\mu}Sv/mGycm^2$ to 0.146 ${\mu}Sv/mGycm^2$ according to the imaging modes and irradiated region and were highest for the D mode at the mandibular molar.

콘빔형 전산화단층영상을 이용한 치과임플란트 식립유도장치 개발 (Surgical stent for dental implant using cone beam CT images)

  • Purpose : The purpose of this study is to develop a surgical stent for dental implant procedure that can be easily applied and affordable by using cone beam computerized tomography (CBCT). Materials and Methods : Aluminum, Teflon-PFA (perfluoroalkoxy), and acetal (polyoxymethylene plastic) were selected as materials for the surgical stent. Among these three materials, the appropriate material was chosen using the CBCT images. The surgical stent, which could be easily placed into an oral cavity, was designed with chosen material. CBCT images of the new surgical stent on mandible were obtained using Alphard-3030 dental CT system (Asahi Roentgen Co., Ltd., Kyoto, Japan). The point of insertion was prescribed on the surgical stent with the multiplanar reconstruction software of OnDemand3D (CyberMed Inc., Seoul, Korea). Guide holes were made at the point of insertion on the surgical stent using newly designed guide jig. CBCT scans was taken for the second time to verify the accuracy of the newly designed surgical stent. Results : Teflon-PFA showed radiologically excellent image characteristics for the surgical stent. High accuracy and reproducibility of implantation were confirmed with the surgical stent. Conclusion : The newly designed surgical stent can lead to the accurate implantation and achieve the clinically predictable result.

콘빔형전산화단층장치를 이용한 함치성낭의 방사선학적 연구 (Radiographic evaluation of dentigerous cyst with cone beam CT)

  • Purpose : The purpose of this study was to accurately analyze the radiographic characteristics of dentigerous cyst (DC) with multiplanar images of cone beam computed tomography (CBCT). Materials and Methods : Thirty eight radiographically and histopathologically proven cases of DCs were analyzed with panoramic radiograph and CBCT, retrospectively. The radiographic CT pattern, symmetry of radiolucency around the unerupted tooth crown, ratio of long length to short length, degree of cortical bone alternation, effects on adjacent tooth, and cyst size were analyzed. Relative frequencies of these radiographic features were evaluated. In order to compare the CBCT features of DC with those of odontogenic keratocyst (OKC), 9 cases of OKCs were analyzed with the same method radiographically. Results : DCs consisted of thirty unilocular cases (79.0%), seven lobulated cases (18.4%) and one multilocular case (2.6%). Eight were asymmetric (21.0%) and thirty were symmetric (79.0%). Maxillary DC showed rounder shape than mandibular DC (L/S ratio; maxilla 1.32, mandible 1.67). Alternations of lingual cortical bone (14 cases, 48.2%) were more frequent than those of buccal side (7 cases, 24.1%). CBCT images of DC showed definite root resorption and bucco-lingual tooth displacement. These findings were hardly observed on panoramic radiographs of DCs. Comparison of CBCT features of DC with those of OKC showed several different features. Conclusions : CBCT images of DC showed various characteristic radiographic features. Therefore, CBCT can be helpful for the diagnosis of DC radiographically.
