The Problem and Improvement Plan on Acupuncture Treatment in Clinical Trial Based on EBM (근거 중심 의학(EBM)에 바탕을 둔 임상시험(Clinical Trial)에서 침 치료 문제점과 개선 방안에 대하여)

  • Introduction : Even though Acupuncture has been know for its effect for a long time, recently it is required to verifiy its effect. To solve this, clinical trial, based on EBM, has been the way to explain acupuncture's treatment effect. Methods: Last year we had a clinical trial based on acupuncture. From this experience we came to a conclusion mentioned below. Results : 1. To find out acupuncture's effect more certainly, it is needed that Korean medical method or treatment should be connected to diagnosis. 2. We had found out that sham and minimal acupuncture are the most appropriate way in single blinding. 3. It is required for the acupuncturist to practice the right real acupuncture point and sham's point in clinical trial. 4. The most important thing to maintain the subjects from dropping out is to always be concerned to the subjects and management them. Conclusion : The best way to advance clinical trial on acupuncture is to use single blinding system with sham acupuncture together in controlled groups and most of all also needs a standardized acupuncture point and depth.

Mediating effect of self-Esteem in the Relationship between Dental Hygiene Students' Powerlessness and Career Identity in the IoT (사물인터넷에서 치위생과 학생의 임상실습관련 무력감과 진로정체감과의 관계에서 자아존중감의 매개효과)

  • This study aimed to examine the mediating effect of self-esteem on the relationship between clinical practice powerlessness and career identity among dental hygiene students with clinical practice experience in the IoT. A questionnaire was conducted on 172 dental hygiene students in a university located in Chungcheong area. For data analysis, Pearson's correlation analysis was conducted to exam the relationships between clinical practice powerlessness, self-esteem, and career identity. Baron & Kenny's three-step mediation analysis was used to investigate the mediating effect of self-esteem in the relationship between clinical practice powerlessness and career identity. The results of this study showed that the clinical practice powerlessness showed significant negative correlations with both self-esteem and career identity, and self-esteem showed a positive correlation with career identity. Partial mediating effects of self-esteem were found between clinical practice powerlessness and career identity. Thus, since self-esteem mediates the relationship between clinical practice powerlessness and career identity, it's essential to implement online-based programs aimed at increasing self-esteem among dental hygiene students before they participate in clinical practice.

Effects of Clinical Decision-making on Job Satisfaction among Pediatric Nurses: The Mediating Effect of the Nurse-Parent Partnership (아동병동 간호사의 임상의사결정 능력이 직무만족도에 미치는 효과: 아동병동 간호사-환아 부모 간 파트너십 매개효과를 중심으로)

  • Purpose: The purpose of this descriptive survey study was to characterize the relationship between clinical decision-making and job satisfaction among pediatric nurses and to elucidate the mediating effects of the nurse-parent partnership on that relationship. Methods: The subjects of the study were 174 nurses who had worked in a pediatric ward in a university hospital, general hospital, or children's hospital. Data were collected from June 20, 2016 to August 10, 2016 and analysed using descriptive statistics, the t-test, analysis of variance, the Pearson correlation coefficient, and three-step mediated regression analysis in SPSS version 22.0 for Windows. Results: The nurse-parent partnership had significant effects on clinical decision-making and job satisfaction, with an explanatory power of 19% and 26%, respectively. The nurse-parent partnership had a partial mediating effect on the relationship between clinical decision-making and job satisfaction (Sobel test: Z=4.31, p<.001). Conclusion: The nurse-parent partnership had a partial mediating effect on the relationship between clinical decision-making and job satisfaction among pediatric nurses. Therefore, in order to improve the job satisfaction of pediatric nurses, it is necessary to develop effective educational programs and strategies to address their clinical decision-making and their experiences of the nurse-parent partnership.

The Mediating Effect of Self-Efficacy in the Relationship between Emotional Intelligence and Clinical Performance among Nursing Students (간호대학생의 감성지능과 임상수행능력 간의 관계에서 자기효능감의 매개효과)

  • Purpose: The purpose of this study was to identify the mediating effect of self-efficacy in the relationship between emotional intelligence and clinical performance in nursing students. Methods: Data were collected through structured questionnaires between March 1 and March 25, 2016. Study participants were 223 fourth year nursing students in B and K metropolitan city. Data were analyzed using t-test, ANOVA, Scheffe' test, Pearson's correlation coefficients, and hierarchical multiple regression with SPSS WIN v 18.0. Results: The mean scores of emotional intelligence was $3.85{\pm}0.43$, self-efficacy was $3.25{\pm}0.40$, and clinical performance was $3.63{\pm}0.51$. There were significant positive correlations between emotional intelligence, self-efficacy and clinical performance. There were partial mediating effect of self-efficacy in the relationship between emotional intelligence and clinical performance. Conclusion: In order to enhance nursing students' clinical performance, strategies for intervention development such as stimulating emotional intelligence and motivation by self-efficacy are necessary.

The Mediating Effect of Resilience on the Relationship between Emotional Labor and Happiness of Clinical Nurses (임상간호사의 감정노동과 행복감의 관계에서 회복탄력성의 매개효과)

  • Purpose: The purpose of this study was to investigate the mediating effect of resilience on the relationship between emotional labor and happiness of clinical nurses. Methods: A descriptive, cross-sectional study was conducted with 224 clinical nurses who had one or more years of clinical experiences from four general hospitals in D city in Korea. Data collected from the survey using self-reported questionnaires were analyzed by t-test, one-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis using IBM SPSS statistics 21.0 program. Results: There were statistically significant correlations among variables; a negative correlation between happiness and emotional labor, a positive correlation between happiness and resilience, and a negative correlation between emotional labor and resilience. Meanwhile, resilience had a partial mediating effect on the relationship between emotional labor and happiness. That is, resilience can play an important role between emotional labor and happiness of clinical nurses as a mediator. Conclusion: Considering the findings of this study, developing programs that can improve resilience of clinical nurses who experience emotional labor are needed to increase their level of happiness.

The Effect of Intellectual Capital on the Clinical and Financial Performance of Hospitals (병원의 지적자본이 진료 및 재무 성과에 미치는 영향)

  • Objectives : This study applied the research model of Bontis (1998), which has been used to investigate everything from business corporations to hospitals in Korea, examined its generalizability and attempted to verify which factors of the intellectual capital affects organizational performance, represented by indicators for the clinical and financial performance of hospitals. Methods : Primary data for the study were collected from 91 general hospitals through self-administered questionnaires, and secondary data on clinical and financial performance of hospitals were collected form existing statistics. Collected data were analyzed with SPSS 11.5 (ver.) and AMOS 18.0 (ver.). Results : It was found that only human capital and customer capital had indirect effects on hospital performance. It was also revealed that structural capital had a direct effect on clinical performance, and an indirect effect on financial performance. Conclusions : This result means that there is a need to individually consider both the clinical performance and financial performance when measuring the organizational performance of a hospital, and it also shows that positive improvement in the clinical performance can contribute to the improvement of the financial performance through this process.

A study on the relationship between clinical practice stress and clinical practice satisfaction in students of department of dental technology (치기공과 학생의 임상실습 스트레스와 임상실습 만족도의 연관성)

  • Purpose: This study aimed to identify the effects of clinical practice stress on clinical practice satisfaction and to provide basic data for clinical practice improvement strategies. Methods: 150 students who had clinical experiences in Daegu Health College from June 12 to August 11, 2017 were selected by convenience sampling method and analyzed 130 questionnaires excluding 20 incomplete questionnaires. Statistical analysis was conducted using SPSS (Statistical Package for Social Science) 22.0 for windows Results: The satisfaction on major and the type of residentice showed statistically significant effect on the clinical practice satisfaction. The training environment in practice and the interpersonal relationship conflicts mostly effect on clinical practice satisfaction. Conclusion : In order to increase the clinical practice satisfaction, it is necessary to consider ways to improve the practical training environment and reduce interpersonal conflicts.

Factors Affecting Clinical Competence in Dental Hygiene Students

  • Background: This study aimed to examine the factors that influence clinical performance of dental hygiene students to provide useful data for developing strategies to improve clinical competence. Methods: The effects of variables on clinical competence by quantile level were analyzed using quantile regression analysis in 247 dental hygiene students. Quantile regression and multiple regression analyses were conducted using the Stata 11.0 program to analyze predictors of clinical competence. Results: The clinical competence score of dental hygiene students was 42.69±5.90, the satisfaction of clinical practice was 49.90±7.44, the clinical practice stress was 50.62±7.37, and the professional self-concept was 31.68±4.41. Empathy was the highest at 50.87±4.93. Multiple regression analysis showed that school year, stress from clinical training, satisfaction with clinical training, professional self-concept, and empathy had significant impact on clinical competence. Quantile regression analysis showed that the effects varied depending on the clinical competence level. School year and professional self-concept had a significant positive effect, regardless of the clinical competence level, while empathy had a significant positive effect at the top 10% (Q90) of the clinical competence level. Satisfaction with clinical practice affected clinical competence at Q25, Q50, and Q90. Stress from clinical practice had significant effects at Q25, Q50, and Q90 (p<0.05). Conclusion: According to the study results, different factors affected clinical competence according to the quantile of clinical competence. This study provides valuable implications for designing clinical competence enhancement programs and strategies. In addition, objective indicators for considering factors that may affect the clinical competence, such as academic competence and satisfaction of practice hospitals, are expected to require detailed analysis and measures.

Antifungal Effect of Chitosan against Malassezia pachydermatis Isolated from the Dog with Otitis Externa (개 외이도에서 분리된 Malassezia pachydermatis에 대한 키토산의 항진균 효과)

  • The effect of pH of chitosan solution (w/v) on antifungal activity against Malassezia pachydermatis isolated from a dog with otitis externa was evaluated. This study was investigated to find out the optimal pH of chitosan solution that could be able to eliminate yeast cells. For comparison of antifungal activity of chitosan solution 2% chlorhexidine that have been already proved the antifungal effect against M. pachydermatis was used as positive control. The chitosan solution at different pH (2.5, 3.0, 3.5, 4.0, 4.5, 5.0) were involved this experiment. The chitosan solutions at each pH could eliminate a number of yeast cells when they were compared to other antifungal agents. In chitosan solution's antifungal effect of varying degrees according to the pH M. pachydermatis was inhibited at pH of < 5.0. Especially it has been proved an effective antifungal effect at pH 3.5. Therefore, the pH of chitosan solution had a effect on antifungal activity and the optimal pH was 3.5 in vitro. The possibility of topical therapy with acidific solution of chitosan has a potential in skin infection against M. pachydermatis.

Effect Analysis of Electronic Clinical Trial Systems (효율성 측정지표를 활용한 전자적 임상시험프로세스 효과분석)

  • The purpose of this study is to empirically examine how much the electronic clinical trial data management system actually enhances its efficiency. While the development of clinical trial markets highlights the significance of data management with increasing rate of adoption of electronic systems, its effects have not been fully supported with rigorous evidences. Particularly, the adoption rate of electronic clinical trial systems is low in domestic clinical trials markets. This study attempts to analyze the effect of the systems for reminding the importance of e-data management in clinical trials. The measurement indicator is experimented with the time related data collected from a multi-center clinical trial case. The result showed that the speed of the electronic clinical trial processes can be improved. The implication of this study lies in its first attempt to empirically analyze the effect of electronic clinical data management systems. Furthermore, application of the indicator in conjunction with electronic clinical trial processes is expected to facilitate strategic data management.
