소아 서혜부 탈장(脫腸)·탈항(脫肛) 치료에 대한 중의학 연구동향 - 2000년 이후 발표된 RCT 논문을 중심으로 - (The Trend of Clinical Research on Treatment for Pediatric Inguinal Hernia and Pediatric Rectal Prolapse - Focusing on Recent Studies in the China National Knowledge Infrastructure (CNKI) -)

  • Objectives The purpose of this study is to analyze recent clinical studies on traditional Chinese medicine (TCM) treatment for pediatric inguinal hernia and pediatric rectal prolapse in China, and to seek better methods to treat and to study for Inguinal hernia and Rectal prolapse in Korea. Methods We searched the clinical studies from the China National Knowledge Infrastructure (CNKI) that were published between January 2000 to February 2020 by key words '疝气', '直腸脫出', '直腸脫垂', '脫肛', '小腸疝', '腹股溝疝', '儿童', '小儿', '少儿', '幼年', '治療', '中医治療', '中藥', '中医藥', '顆粒', '膠囊', '自擬', '湯', '丸', '散', '方'. We analyzed the literatures in regards to the treatment methods and results. Results Among the 193 searched studies, 10 randomized controlled trials were selected and analyzed. In most of the studies, the effectiveness of the traditional Chinese medicine (TCM) treatment on inguinal hernia and rectal prolapse was significantly high. Conclusions Based on the results of the clinical studies from China, use of the TCM for the treatment of inguinal hernia and rectal prolapse has been shown to be effective in relieving symptoms. Also, based on the result of this study, it will be possible to widen the scope of the TCM treatment on inguinal hernia and rectal prolapse. Additional clinical studies and experimental studies are needed to be performed to solidify these findings. The TCM has been shown as an effective treatment for pediatrics as well. These research results can be utilized in other clinical studies and in treatment.

뇌졸중 후 우울증의 약침치료에 대한 중의학 임상논문 연구동향 (Trend Review of Traditional Chinese Medicine Studies on Pharmacopuncture Treatment for Post-Stroke Depression)

  • Objectives: To provide clinical evidence of pharmacopuncture treatment for post-stroke depression by reviewing randomized controlled trials on Traditional Chinese medicine pharmacopuncture for post-stroke depression. Methods: Randomized controlled trials on pharmacopuncture treatment for post-stroke depression were searched from the China National Knowledge Infrastructure (CNKI). Literature searches of English and Chinese databases were performed. Selected literature was assessed with the Jadad scale. Results: Twelve papers were selected from 20 studies. The following results were obtained: 1) Pharmacopuncture was used for post-stroke depression; 2) Dengzhan-Xixin was the most used injection medicine for post-stroke depression; 3) BaiHui (GV20) was the most used in pharmacopuncture treatment; 4) Based on the Hamilton Depression Rating Scale (HAMD), it was confirmed that all studies using pharmacopunture had significant efficacy. Conclusions: For the treatment of post-stroke depression, Dengzhans-Xixin was commonly used as an injectable medication. Additional research studies on pharmacopuncture including its practical use are needed.

구미진심과립(九味鎭心顆粒)의 불안장애 활용에 대한 중의학 연구 고찰 (A Study of Chinese Medicine on the Utilization of Jiuwei Zhenxin Granules for Anxiety Disorders)

  • Objectives: The purpose of this study is to investigate the composition, origins, and utilization of Jiuwei Zhenxin Granules for anxiety disorders in Chinese medicine. Methods: Studies found in six databases were chosen, and major indicators were analyzed. Results: Jiuwei Zhenxin Granules are modified from Pyeonbojinsimdan on Taepyeonghyeminhwajegukbang. In Jiuwei Zhenxin Granule studies, there were significant effects in nine outcomes in twenty-four studies of primary anxiety disorders and seventeen outcomes were similar in effect to that in the control. Three outcomes in ten studies of secondary anxiety disorders showed significant effects, and five outcomes were similar in effect to that in the control. Conclusions: Jiuwei Zhenxin Granules is a prescription that focuses on harmony of vital energy (qi) and blood, and stability of mind. The significance of the constitution of Jiuwei Zhenxin Granules is similar to that of domestic anxiety disorder studies, so it seems to be applicable to anxiety disorders. With regard to domestic use of Jiuwei Zhenxin Granules, more research is needed on the dosage according to the formulation.

한국.중국의 전통의약 교육제도 현황 비교를 통한 시사점 연구 (A Study on Implication by Comparing Current Status of Educational Systems between Korea and China in connection with Traditional Medicine of Each Country)

  • Arriving in the '90s, the worldwide trend of longing for naturalism and popularity of complementary and alternative medicine in America has caused traditional Oriental Medicine and medicinal plants markets to develop rapidly. And China has been pursuing the globalization policy of Chinese medicine by the initiation of the society of traditional Chinese medicine. Under this situation, it is a time for us to think about in a serious manner whether existing organization and system of Oriental medicine and the department of Oriental medicine at the schools in Korea reflects reality or whether we should turn it to some different direction. The purpose of this research is to compare the educational systems in relation to the traditional medicine between Korea and China, and to seek and look into its implication, and also to make a contribution to further developments and changes of direction for Oriental medicine education in Korea. 1. I investigated carefully the educational system of the colleges of traditional Chinese medicine, and results from this survey revealed that the academic institutions for the medicinal training in China consists of varied systems, such as 7-year program for medicinal training linking with master degree course, 6-year program, 5-year program (more than 90%), 4-year program, and so on, so then China has been raising the specialists in their traditional medicine arena through those varied academic programs. Such an educational system as the department of Chinese medicine in order to educate and produce specialists or pharmacists specializing in traditional Chinese medicine is operated only by Beijing University of Chinese Medicine in terms of 7-year academic program for medicinal major that linked with master degree course, and the rest of schools run 5-year program or 4-year program (more than 90%). And other human resources required for cultivation of medicinal plants and manufacturing herbal medicines are mostly trained at 3-year course colleges or 2-year course vocational schools. 2. In connection with traditional Chinese medicine, there are a variety of departments in the schools in China other than Chinese Medicine and Pharmacology: i.e. Acupuncture, Moxibustion and Tuina, Preclinical Medicine, Pharmaceuticals, Materials of Medicine, Phrenology and Law, Languages and Literature, etc. Therefore, these programs constitute multi academic system and also an appropriate educational base that fits in varied needs of market. Particularly, the university having 7-year program emphasize, English proficiency so that it can be considered that this academic program is a specialized course in order to achieve globalization of Chinese medicine. 3. In Korea, there are only 11 Oriental medicine schools with 6-year program which have been established by the private foundations and 3 departments of Oriental medicine at 4-year university. Therefore, we need to establish varied departments related to branches of our traditional medicine like China. 4. It is necessary to establish varied new departments related to Oriental Medicine that will be able to take a professional role in the course of pursuing the strategic goals such as scientification, globalization, standardization of Oriental Medicine, also that will meet needs of the world alternative and complementary medicine and herbal medicine markets. In order to achieve such strategic goals, we need to organize an academic system that will be different from existing systems and programs, also we are required to research further on the educational and training programs.

우울증 치료의 중의학 최신 임상연구 동향-무작위 대조군 임상연구와 체계적 문헌 고찰을 중심으로- (Review of Recent Clinical Trials for Depression in Traditional Chinese Medicine-Based on Randomized Controlled Trials and Systematic Reviews-)

  • The purpose of this study was to review the research on treating depression in Traditional Chinese Medicine(TCM) over the last 5 years to set the foundation for further studies. We searched for and analyzed articles about depression in CNKI(China National Knowledge Infrastructure) from January 2010 to December 2014. The results were as follows : The most frequently used diagnostic criteria was CCMD-3(The Chinese Classification of Mental Disorders-3), and the most used evaluation criteria was HAMD(Hamilton Depression Rating Scale). Prescription of decoction of medical herbs was most frequently used as a therapeutic method. Acupuncture, traditional Chinese psychotherapy, and music therapy were also used for some studies. The most frequent TCM Syndrome Differentiation Type was stagnation of liver-QI. For decoction of Chinese herbs, Soyo-san(Xiaoyao-san) and Sihosogan-san(Chaihushugan-san) were most often prescribed, and Bupleuri Radix, Paeoniae Radix Alba, Curcumae Radix, Poria cocos wolf, Angelicae Gigantis Radix, Atractylodis Rhizoma Alba were mainly used for medical herbs. BaekHoi(GV20) and Yindang(Ex-HN3) were often used as acupuncture sites. Post-Stroke Depression was the most common case of intercurrent or secondary depression. According to the Jadad Quality Assessment Scale, the quality of the reports was not high as most of the reports had a score of 3 or below. Most systematic reviews on depression were conducted by Chinese researchers. The problem with Clinical research on depression, according to those reviews, was that there were no standardized criteria for the diagnosis and treatment and the trials were usually not randomized nor controlled. We found out there are various clinical methods for treating depression in TCM, and hope that this research could provide the preliminary data for designing and conducting clinical trials for depression.

중의학 데이터베이스 (CAJ)를 이용한 소아 비만의 한약치료에 대한 임상 연구 동향 (Review of Clinical Studies for Herbal Medicine Treatment on Childhood Obesity - Focusing on Studies from the China Academic Journal (CAJ) -)

  • Objectives The purpose of this study was to analyze recent traditional Chinese medicine clinical studies for treatment of childhood obesity. Methods We analyzed 11 clinical studies from January, 2000 to March, 2018 about childhood obesity from the China Academic Journal (CAJ) and China National Knowledge Infrastructure (CNKI). All selected studies were Randomized Controlled Trials and analyzed in order of publication year, demographic information, treatment method, evaluation methods, results, adverse reactions and relapse after treatment discontinuation. Results The main Traditional Chinese Medicine (TCM) treatment was the herbal decoctions, granules and pills. In most studies, the total efficacy of the treatment group was reported to be higher than that of the control group. Adverse reactions were found in small number of studies. Frequently used medical herbs were Crataegi Fructus (山?), Atractylodis Rhizoma (蒼朮), Citri Pericarpium (陳皮), Poria (茯?), and Nelumbinis Folium (荷葉). Also, Water-draining and swelling-dispersing medicinal (利水退腫藥), Qi-regulating medicinal (理氣藥), Resolving dampness with aroma medicinal (芳香化濕藥), Blood-activating and stasis-dispelling medicinal (活血祛瘀藥), Qi-tonifying medicinal (補氣藥), and Digestant medicinal (消食藥) were commonly used. Conclusions This study showed that traditional Chinese medicine, especially herbal medicine treatment can be effective and safe option for treating childhood obesity. These research results can be utilized in other clinical studies as well as in treatment of childhood obesity. Additional well-designed randomized controlled trials and systematic reviews are need to confirm the conclusion.

  • This study analyzed the contents of the research papers concerning the common cold recorded in Chinese medical journal published over the period between 2000 and 2004. As a result, the following conclusion was drawn. 1. Among the clinical research papers, There are a lot of treatment papers of traditional chinese medicine in the method of treatment. There are a lot of papers about Feng-Re-Xing(風熱型) and Xu-Zheng-Xing(虛證型) in the Bian-Zheng-Lei-Xing(辨證類型) 2. "Zhong-Yi-Xu-Zheng-Bian-Zheng-Can-Kao-Biao-Zhun" ("中醫虛證辨證參考標準"), "Zhong-Yi-Nei-Ke-Wu-Ban-Jiao-Cai"("中醫內科五版敎材"), "Zhong-Hua-Ren-Min-Gong-He-Guo-Zhong-Yi-Yao-Hang-Ye-Biao-Zhun"("中華人民共和國醫藥行業標準"), "Gan-Mao-Zhen-Duan-Biao-Zhun"("感冒診斷漂準"), "Zhong-Yi-Bing-Zheng-Zhen-Duan-Liao-Xiao-Biao-Zhun"("中醫病證診斷療效標準"), "Quan-Guo-Gao-Deng-Yi-Xue-Yuan-Xiao-Zhong-Yi-Zhuan-Ye-Jiao-Cai"("全國高等醫藥院校中醫專業敎材") are used as the criterion for diagnosis in Chinese medicine. 3. It is mainly used "Zhong-Yi-Bing-Zheng-Zhen-Duan-Liao-Xiao-Biao-Zhun"("中醫病證診斷療效標準") as the criterion for treatment effect evaluation, and symptom of traditional chinese medicine, the frequency and the duration of common cold, measurement of immunologic function are used as assistant evaluation indicator. 4. The research papers reported that the use of prescriptions such as Chai-Qi-Fang-Jiao-Tang, Tui-Re-He-Ji(退熱合劑), Ti-Xu-Gan-Mao-He-Ji(體虛感冒合劑), Yu-Ping-Feng-San-He-Gui-Zhi-Tang(玉屛風散合桂枝湯), Chai-Guan-Jie-Re-Ke-Li(柴貫解熱顆粒), Hu-Qin-He-Ji(蒿芩合劑), Lian-Hua-Feng-Cha(蓮花峰茶), Kang-Gan-He-Ji(抗感合劑), Bing-Du-He-Ji(病毒合劑), Zhong-Gan-Ling-Pian(重感靈片) led to the high efficacy 5. The pharmacological research papers reported that Yu-Ping-Feng-San(玉屛風散) have influence on IgA, phagocytic function of macrophage, the total number of splenocyte and PEC.

소아 기능성 위장 장애의 한약 치료에 대한 임상연구 동향 - 중의학 무작위 대조군 임상시험을 중심으로 (Trends in Clinical Research of Herbal Medicine Treatment for Functional Gastrointestinal Disorders in Children - Focused on Randomized Controlled Trials in traditional Chinese medicine)

  • Objectives The purpose of this study is to analyze randomized clinical trials (RCTs) on traditional Chinese medicine to summarize its efficacy and safety for the treatment of functional gastrointestinal disorders (FGIDs) in children. Methods We searched literatures published up to March 19, 2021 using two Chinese electronic databases. Data regarding patients, interventions, results, and adverse events were extracted from RCTs of herbal medicine for children with FGIDs. Results A total of 34 RCTs were included: 16 trials on functional dyspepsia, 7 trials on functional constipation, 6 trials on functional abdominal pain, 4 trials on irritable bowel syndrome, and 1 trial on functional diarrhea. 26 of 29 trials that reported total effective rate, the treatment group showed a significant improvement compared to the control group. Most of other evaluation indicators, such as symptom score, symptom disappearance time, and recurrence rate also demonstrated statistically significant improvement. Of the 16 studies which reported safety, 5 studies reported no adverse reactions in either group, and 4 studies reported no statistically significant differences in the incidence of adverse events between two groups. Also, reported adverse events were mostly mild. Conclusions Herbal medicine may help improve symptoms of FGIDs in children. However, due to limited types of studies on sub-diseases of FGIDs and small sample sizes in each study, additional large scale clinical studies on various other FGIDs are necessary.

  • Objectives The objective of this study was to investigate the impact of applying acupuncture as intervention of peripheral facial paralysis in children. Methods We conducted searches on China National Knowledge Infrastructure and Wanfang Data using the keywords "facial paralysis", "children", and "acupuncture". Results Six randomized controlled trials were included in the analysis. The acupoints most frequently utilized were Hapgok (LI4), Jichang (ST4), Yangbaek (GB14), Sabaek (ST2), Hyeopgeo (ST6), Yeonghyang (LI20), Yepung (TE17), and Taeyang (EX-HN5). The Stomach Meridian (ST) was the most targeted. Acupuncture treatment was found to be as effective as or more effective than Western medicine treatment in certain cases. Conclusions Acupuncture therapy shows promise for treating pediatric peripheral facial paralysis. However, further clinical randomized controlled trials are necessary to validate the efficacy and safety of acupuncture treatment.

골관절염에 대한 추나요법을 포함한 중의학치료의 효과 : 체계적 문헌 고찰 (Chinese Medicine including Chuna Manual Therapy for Osteoarthritis : A Systematic Review)

  • Objectives: To evaluate the evidence supporting the effectiveness and safety of Chinese medicine, including Chuna manual therapy, for the treatment ofosteoarthritis. Methods: We conducted a search of 8 electronic databases(Pubmed, Web of Science, EMBASE, Cochrane Library, CAJ, KISTI, NDSL, and RISS) to find all randomized controlled clinical trials that discuss Chuna manual therapy forosteoarthritis till May 2018. Results: Our inclusion criteria selected 7eligible RCTs involving 633 participants. The meta-analysis of 6 studies showed positive results for the useof Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. There was no evidence of worsening symptoms or any adverse events. Conclusions: Our systematic review found favorable evidence regardingthe effectiveness of Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. However, our systematic review had several limitations owingto the low quality of original articles andadditionalclinical trials would be necessary for stronger evidence.
