Promising evidence suggests that amyloid beta peptide ($A{\beta}$), a key mediator in age-dependent neuronal and cerebrovascular degeneration, activates death signalling processes leading to neuronal as well as non-neuronal cell death in the central nervous system. A major cellular event in $A{\beta}$-induced apoptosis of non-neuronal cells, including cerebral endothelial cells, astrocytes and oligodendrocytes, is mitochondrial dysfunction. The apoptosis signalling cascade upstream of mitochondria entails $A{\beta}$ activation of neutral sphingomyelinase, resulting in the release of ceramide from membrane sphingomyelin. Ceramide then activates protein phosphatase 2A (PP2A), a member in the ceramide-activated protein phosphatase (CAPP) family. PP2A dephosphorylation of Akt and FKHRL1 plays a pivotal role in $A{\beta}$-induced Bad translocation to mitochondria and transactivation of Bim. Bad and Bim are pro-apoptotic proteins that cause mitochondrial dysfunction characterized by excessive ROS formation, mitochondrial DNA (mtDNA) damage, and release of mitochondrial apoptotic proteins including cytochrome c, apoptosis inducing factor (AIF), endonuclease G and Smac. The cellular events activated by $A{\beta}$ to induce death of non-neuronal cells are complex. Understanding these apoptosis signalling processes will aid in the development of more effective strategies to slow down age-dependent cerebrovascular degeneration caused by progressive cerebrovascular $A{\beta}$ deposition.
심뇌혈관 질환은 국내 사망률 2위를 차지하고 있는 주요 만성질환이다. 이 질환은 생활습관의 개선으로 예방할 수 있어서 평소 건강관리가 중요하다. 하지만 국내에서 대부분 예방관리 프로그램은 안내 책자, 강의 등 소극적인 방법으로 대응하고 있어 질환 예방관리의 실효성이 떨어지고 있다. 현재 ICT 기술의 발달로 스마트 헬스케어 시장이 발달함에 따라, 개인의 건강상태를 측정하고 신체 데이터를 수치로 나타내는 수치화된 자아(Quantified self)가 웨어러블 디바이스를 통해 확산하고 있다. 이에 본 연구는 심뇌혈관 질환의 적극적인 예방과 관리를 위해 헬스케어 웨어러블 디바이스를 탐색하고 논의하였다. 먼저 현재 상용화되고 있거나 심뇌혈관 질환과 관련된 헬스케어 웨어러블 디바이스들의 사례들을 부착 형태에 따라 손목형, 의복형, 부착형으로 분류한 뒤 기능 분석했다. 이를 종합해 웨어러블 디바이스를 통한 심뇌혈관 질환의 예방법과 관련 있는 건강활동 트래킹, 생체신호 트래킹, 의료진과 피드백 기능으로 분류해 비교분석 하였다. 이를 통해 질환에 대한 적극적인 자가 헬스케어의 행태가 개선될 수 있음을 기대할 수 있으며, 향후 본 연구를 통해 질환의 예방 연구의 다학제적인 활성 기제로 작용할 수 있을 것이다.
본 연구는 뇌혈관질환의 위험요인을 규명하고, 특히 혈청 콜레스테롤 값 및 코골음과 뇌혈관질환과의 관련성을 검정하기 위하여 시도된 짝지은 병원 환자-대조군 연구이다. 환자군인 뇌혈관질환군은 1993년 12월부터 1995년 3월까지(16개월간) 경상대학교 병원 신경과에 처음 입원하는 환자로 뇌전산화 단층촬영으로 확진되는 뇌경색 74명, 뇌출혈 53명 총 127명으로 하였다. 대조군은 환자군과 비슷한 시기에 입원하는 경상대학교 병원의 신경과 이외 병동의 입원 환자중 환자군과 성(性)이 같고 연령이 5세 이내인 대상자를 환자군과 1:1 개별 짝짓기(individual matching)하여 127명을 선정하였다. 대조군은 과거에 뇌혈관질환의 병력이 없고 원칙적으로 뇌혈관질환의 잠재적 위험요인과 관계없는 질병으로 입원한 경우로만 하였다. 본 연구결과 논란이 있는 코골음과 저콜레스테롤혈증이 뇌혈관질환의 위험요인이라고 할 수 없었다. 유의한 위험요인은 심전도의 이상(좌심실비대 및 심방세동)이 가장 중요하였고, 그 외에도 뇌혈관질환의 가족력, 안저검사의 이상소견, 과거 일과성 허혈성 발작의 경험 및 혈청 콜레스테롤치의 상승이었다. 뇌경색의 위험요인은 역시 심전도의 이상이 가장 중요하였고, 그외에도 안저검사의 이상소견, 흡연 및 혈청 콜레스테롤치의 상승이었다. 이상의 위험요인을 고려할 때 뇌혈관질환을 예방하기 위해서는 고혈압의 치료가 중요하며, 특히 안저검사와 심전도 검사에서 이상소견이 나타나지 않도록 철저히 관리해야 한다. 가족력이 있는 사람들과 과거 일과성 허혈성 발작 경험자는 특히 고위험군으로서 다른 위험요인을 가지지 않도록 교육해야 한다. 또한 흡연과 고콜레스테롤 혈증의 건강에 대한 위해성도 다시 한번 강조되어야 한다. 이렇게 함으로써 보건당국은 단일 사망원인으로 우리나라에서 제1의 사망원인인 뇌혈관질환을 예방하기 위하여 더욱 노력해야 할 것이다.
본 연구는 G시에 소재한 D와 S 조선소에 근무하는 폐경 후기 여성 근로자 268명을 대상으로 2017년 11월 1일부터 11월 30일까지 설문조사를 실시하였다. 심뇌혈관질환 발병 위험인자 유무에 따라 심뇌혈관질환 발병 위험군과 건강군으로 분류하여 이들 두 군 간의 일반적 특성, 심뇌혈관질환 예방관련 지식, 태도, 건강행위 이행 정도 및 관계를 비교 분석하였다. 건강군과 발병 위험군에서 통계적으로 유의한 차이가 있었던 변수는 연령(F=92.239, p<.001), 동거유형($x^2=20.056$, p<.001), 소득수준($x^2=39.023$, p<.001), 주당 근무시간(F=32.217, p<.001), 근무년수(F=12.310, p=.001), 심뇌혈관질환 가족력($x^2=233.442$, p<.001), 주관적 건강상태($x^2=19.058$, p=.001)였다. 두 군에서 심뇌혈관질환 예방관련 지식 정도(F=2.679, p=.008)와 건강행위 이행 정도(F=4.339, p<.001)가 유의한 차이가 있었다. 심뇌혈관질환 예방관련 건강행위 이행 정도의 관계에서 건강군은 지식이 정적 상관관계를 보였고(r=.348, p<.001), 발병 위험군은 지식(r=.279, p=.002)과 태도(r=.194, p=.030)가 정적 상관관계를 보였다. 이를 바탕으로 심뇌혈관질환 발병에 취약한 집단의 특성을 고려한 중재 프로그램의 개발 및 적용이 필요할 것으로 사료된다.

Objective : Cerebral vessels, such as intracerebral perforating arterioles isolated from rat brain, have been widely used as an ex vivo model to study the cerebrovascular function associated with cerebrovascular disorders and the therapeutic effects of various pharmacological agents. These perforating arterioles, however, have demonstrated differences in the vascular architecture and reactivity compared with a larger leptomeningeal artery which has been commonly implicated in cerebrovascular disease. In this study, therefore, we developed the method for studying cerebrovascular function utilizing the olfactory artery isolated from the mouse brain. Methods : The olfactory artery (OA) was isolated from the C57/BL6 wild-type mouse brain. After removing connective tissues, one side of the isolated vessel segment (approximately $-500{\mu}m$ in length) was cannulated and the opposite end of the vessel was completely sealed while being viewed with an inverted microscope. After verifying the absence of pressure leakage, we examined the vascular reactivity to various vasoactive agents under the fixed intravascular pressure (60 mm Hg). Results : We found that the isolated mouse OAs were able to constrict in response to vasoconstrictors, including KCl, phenylephrine, endothelin-1, and prostaglandin $PGH_2$. Moreover, this isolated vessel demonstrated vasodilation in a dose-dependent manner when vasodilatory agents, acetylcholine and bradykinin, were applied. Conclusion : Our findings suggest that the isolated olfactory artery would provide as a useful ex vivo model to study the molecular and cellular mechanisms of vascular function underlying cerebrovascular disorders and the direct effects of such disease-modifying pathways on cerebrovascular function utilizing pharmacological agents and genetically modified mouse models.
Purpose: This study was to evaluate the effects of visiting nursing intervention on self care behavior and blood pressure with high risk of cerebrovascular diseases. Method: A one-group pretest-posttest was designed. Data collection was done in 150 participants from March. 2003 to November, 2003 at a public health center. Self care behavior was assessed by the scores of smoking, alcohol, diet, exercise, stress and medication compliance. The scores of knowledge related cerebrovascular diseases, blood pressure, blood glucose were estimated. The Participants were intervened in with basic assessment, emotional support, education. This program took a period ranging from 3 months to 7 months depending on the cerebrovascular risk level. The mean number of visiting times was 1.67 per month. Result: 1. Knowledge level was improved(t=-2.542, p= .012). 2. Systolic and diastolic pressure were lower(t=5.439, p<.001; t=4.966, p<.001) 3. Self care level was higher (t=-12.981, p=.001) after the intervention. Conclusion: The visiting nursing intervention was found to have an effect on the scores of knowledge, self care behavior and blood pressure with high risk of cerebrovascular diseases. The results provided evidences for the importance of visiting nursing intervention in the high risk factor of cerebrovascular disease for self care.
Purpose: This study was conducted to examine the effects of individual counseling for bus-drivers on cerebrovascular and cardiovascular disease prevention. Methods: The one-group pretest-posttest design was used. This study presents a secondary analysis of data collected in 'contents of cerebrovascular and cardiovascular disease prevention program for bus-drivers in one workplace in Seoul, Korea. The data were collected from 56 bus drivers, using questionnaires from September 10th to November 20th, 2014. The analysis was conducted with ${\chi}^2$ test and paired samples t-test using SPSS/Win 21.0. Results: After the intervention, the participants showed a significant decrease in the level of systolic blood pressure (p=.003) and a significant increase in the total cholesterol level (p=.030). The distribution of cardiovascular risk groups changed after the intervention: 5.3% decreased in the high risk group, 16.1% decreased in the medium risk group, and 3.5% decreased in the low risk group, while 25.0% increased in the normal group. Conclusion: The comprehensive individual counseling including simple screening, tailored education and counseling is effective to manage their lifestyle risk factors, resulting in better maintenance of their health as well as preventing cerebrovascular and cardiovascular disease.
The moyamoya disease is a type of cerebrovascular disease which produces thin abnormal blood vessels like haze in the brain base because the end of internal carotid artery which supplies about 80% of blood is blocked. Regarding this moyamoya disease, the shearing stress and thrombus generation are mentioned as its main causes. This study three-dimensionally implemented the ICA, ACA, and MCA parts of the cerebrovascular configuration related to the moyamoya disease, and analyzed the hydrodynamic phenomenon with the commercial program ADINA. In particular, the correlations between shearing stress and speed distribution according to the branch angle of ACA and MCA. A numerical analysis found that the greater the branch angle of ACA and MCA, the lower the shearing stress and the greater the stationary area of the flow.. Put Abstract text here.

Objective : Glutathione S-transferase polymorphism (GST) were examined in 120 cases with ischemic cerebrovascular disease (ICVD) to test the hyperthesis that GST polymorphisms confer a risk to an individual to develop ICVD. Tobacco smoking is a major cause of both cancer and vascular disease. Methods : therefore We were stratified the subjects with ICVD for smoking status, and then examined whether polymorphisms in this detoxification enzyme gene, GST, influence risk of ICVD Results : Neither GSTM1 nor GSTT1 genotypes in the ICVD group was significantly different from the control group (n=207), even in smokers. We attempted the combined analyses for GSTM1 and GSTT1 genotypes in ICVD for smoking status. No significant association observed between the combined genotypes and ICVD Conclusion : Our observation do not confirm the effect of the GSTM1 and GSTT1 genotypes as a risk factor for ICVD, even in smokers.
