The fractal structures, which include Sierpinski and Pascal triangular fractals, have provided many mathematical interests. In this study, the hydrodynamic and mechanical properties of the triangular fractals were investigated, and their application to the design of various artificial bone scaffolds has been implemented via CAD modeling, computational analysis and mechanical testing. The study proved that the Sierpinski and Pascal triangular fractal structures could effectively be applied to bone scaffold design and manufacturing regarding permeability and mechanical stiffness.
This study examined effects of calcium supplemented milk on bone loss in ovariectomized rats. Twenty four Sprague-Dawley female rats, 7 weeks-old, were divided into 4 groups, ovariectomized and fed diets containing: 1) control, no Ca supplemented milk, 2) ovx 1, Ca carbonate supplemented milk, 3) ovx 2, ionized Ca supplemented milk, and 4) ovx 3, nano Ca supplemented milk. All rats were fed 1 ml of milk containing 20 mg supplemented Ca. After 18 wk feeding, body weight gain and food efficiency ratio were significantly different between ovx 1 and ovx 3. Serum concentration of calcium and phosphorus were not different among groups. However, there was a significant difference in calcium content of dry femoral weight in ovx 3 compared with the control and ovx 2. In addition, femoral bone mineral density ($g/cm^2$) was significantly greater in ovx 3 than in other groups (p<0.05). The ovx 3 group showed the highest stiffness (N/mm), maximum energy (N) in femur and trabecular bone area (%). The present study indicated that nano Ca supplementation in milk may be an effective way to enhance bone calcium metabolism for ovariectomized rats.
A modelling scheme for the stress analysis taking into account load transfer characteristics of the osseointegrated interfaces between dental implant and surrounding alveolar bone was investigated. Main aim was to develop a more realistic simulation methodology for the load transfer at the interfaces than the prefect bonding assumption at the interfaces which might end up the reduced level in the stress result. In the present study, characteristics of osseointegrated bone/implant interfaces was modelled with material nonlinearity assumption. Bones at the interface were given different stiffness properties as functions of stresses. Six different models, i.e. tens0, tens20, tens40, tens60, tens80, and tens100 of which the tensile moduli of the bones forming the bone/implant interfaces were specified from 0, 20, 40, 60, 80, and 100 percents, respectively, of the compressive modulus were analysed. Comparisons between each model were made to study the effect of the tensile load carrying abilities, i.e. the effectivity of load transfer, of interfacial bones on the stress distribution. Results of the present study showed significant differences in the bone stresses across the interfaces. The peak stresses, however, were virtually the same regardless of the difference in the effectivity of load transfer, indicating the conventional linear modelling scheme which assumes perfect bonding at the bone/implant interface can be used without causing significant errors in the stress levels.
Chitosan is abundant polysaccharide polymer in nature Chitosan has been the subject of much research regarding its potential as a nutritional adjunct and pharmaceutical ingredient. In the present study, we examined fracture-healing process with chitosan administration and low calcium diet in rats. Left femur was fractured and fixed with intramedullary pin. The rats were fed normal diet or low calcium diet and administered chitosan with the doses of 0, 50, 100 and 150ng/ml orally 5 times a week for 10 weeks. Soft X-ray and mechanical testing of all fractured femora was taken. Radiographical finding showed that the callus formation and process of fracture healing was present in all the fractured femora. Mechanical testing indicated that the maximum load and stiffness of femur in rat fed low calcium diet was lower than those of that in rat fed normal diet. No difference in maximum load and stiffness of fractured femora in chitosan treated rat were observed as compared to vehicle treated rat. Chitosan or/and low calcium diet did not affect the ratio of fractured/unfractured femur about maximum load and stiffness. The results suggest that chitosan dose not affect the bone mechanical strength and the process of fracture healing. Low calcium diet does decrease the bone mechanical strength.

Purpose : The purpose of this study was to provide an effective method of exercise therapy for patients with cervicogenic headache. Methods : The subjects were divided into the following two groups according to the intervention received: cervical stretch exercise (n=15, control group) cervix-stabilizing exercise (n=15, experimental group) tone (Hz) and stiffness (N/m) of the suboccipital and upper trapezius muscles were measured. T1 slope angle and neck tilt angle were measured. After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Results : After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Conclusion : The neck-stabilizing exercise were shown to be effective in decreasing the tone of the cervical muscles by stabilizing the cervical bone and improving muscle activity, and in improving the posture by decreasing muscle tone and stiffness.
Age-related changes in the geometry of human lumbar spine would lead to changes of its mechanical behaviors. To investigate the effects of the geometric changes, no age-related changes in the material/mechanical properties were considered. Using the finite element method. two age-related models of lumbar spine segments (L3-L4) were constructed. The annulus of the models was modeled as laminate composite elements with 16 layers and 6 materials. The spinal stiffness and facet reaction of the lumbar spine increased with the age-related geometric changes in various combined loadings. Fiber and transverse tensile strains of the inner annulus. cancellous bone stress and end-plate stress decreased with the age-related geometric changes whereas fiber/layer compressive strains of the annulus. facet reaction. ligament reaction and end-plate rigidity increased. Consequently, it appears that in the normal age-related deterioration of discs, the age-related geometric change contributes to the increase of spinal stiffness (the decrease in range of the motion segment), preventing an excessive deformation of the disc.
Purpose: We analysis the outcome and complications of treatment of lesser toe brachymetatarsia. Materials and Methods: We analysed 28 patients 35 cases of lesser toe brachymetatarsia. Mean post operative follow up period was 2 years 8 months. All of the patients were female and mean age at operation was 21 years old. 2 cases of third metatasal bone and 33 cases of fourth metatarsal bone were operated. 8 metatarsal bones were treated using one staged lengthening with tricortical bone graft and 27 metatarsal bones were treated using callotasis with monofixator. Results: The average amount of lengthening was 13.3mm(12mm-15mm) in one staged lengthening, while 14.4mm(4mm-23mm) in callotasis. Average percentile increase was 28.9%(26%-34%) in one staged lengthening and 32%(18%-46%) in callotasis. The average healing index of callotasis was 76 days/cm (41 days/cm-166 days/cm). Satisfied outcomes in 4 cases of 8 cases (50%) after one staged lengthening and 17 cases of 27 cases (63%) after callotasis. 6 complications in 4 cases were occurred after one staged lengthening; insufficient length gain in 3 cases, fracture on the junction of graft bone and metatarsal bone in 1 case, plantar bowing deformity in 1 case and bony fusion of metatarsophalangeal joint in 1 case. 17 complications in 10 patients were occurred after callotasis ; metatarsophalangeal joint stiffness in 8 cases, metatarsophalangeal joint subluxation in 2 cases, overlengthened metatarsal bone in 2 cases, tapering of callus in 1 case, fracture of callus in 1 case, premature consolidation of callus in 1 case, osteomyelitis of metataral head in 1 case and plantar bowing deformity in 1 case. Conclusion: Although one staged lengthening and gradual lengthening using callotasis are effective treatment for lesser toe brachymetatarsia, complications not rarely occured after lengthening. Insufficient lengthening are most common complication after one staged lengthening while metatarsophalangeal joint stiffness lire most common complication after callotasis.
In fracture treatment with external fixators, the inter-fragmentary movements at the fracture site affect the fracture healing process, and these movements are highly related to the stiffness of external fixation systems. Therefore, in order to provide the optimal fracture healing at the fracture site, it is essential to understand the relationship between the stiffness and the system configurations in external fixation system. In this study we investigated the influences of system configuration parameters on the stiffness in the finite element analysis of an external fixation system of a long bone. The system alignment, the geometric and the material non-linearity of the pin, the joint stiffness and the callus formation were considered in the finite element model. In the first, the system stiffness of the developed finite element model was compared with the experiment data for model validation. The consideration of the joint stiffness and nonlinearity of the model improved the system stiffness results. The joint stiffness, the non-alignment of the system decreased the system stiffness while the callus formation increased the system stiffness. The present results provided the biomechanical basis of rational guidelines for design improvements of external fixators and pre-op. planning to maximize the system stiffness in fracture surgery.
Statement of problem. Cortical bone plays an important role in the primary implant stability, which is essential to immediate/early loading. However, immediate load-bearing capacity and primary implant stability according to the change of the cortical bone thickness have not been reported. Purpose. The objectives of this study were (1) to measure the immediate load-bearing capacity of implant and primary implant stability according to the change of cortical bone thickness, and (2) to evaluate the correlation between them. Material and methods.48, screw-shaped implants (3.75 mm$\times$7 mm) were placed into bovine rib bone blocks with different upper cortical bone thickness (0-2.5 mm) and resonance frequency (RF) values were measured subsequently. After fastening of healing abutment. implants were subjected to a compressive load until tolerated micromotion threshold known for the osseointegration and load values at threshold were recorded. Thereafter, RF measurement after loading, CT taking and image analysis were performed serially to evaluate the cortical bone quality and quantity. Immediate load-bearing capacity and RF values were analyzed statistically with ANOVA and post-hoc method at 95% confidence level (P<0.05). Regression analysis and correlation test were also performed. Results. Existence and increase of cortical bone thickness increased the immediate load-bearing capacity and RF value (P<0.05) With the result of regression analysis, all parameter's of cortical bone thickness to immediate load-bearing capacity and resonance frequency showed significant positive values (P<0.0001). A significant high correlation was observed between the cortical bone thickness and immediate load-beating capacity (r=0.706, P<0.0001), between the cortical bone thickness and resonance frequency (r=0.753, P<0.0001) and between the immediate load-bearing capacity and resonance frequency (r=0.755, P<0.0001). Conclusion. In summary, cortical bone thickness change affected the immediate load-baring capacity and the RF value. Although RF analysis (RFA) is based on the measurement of implant/bone interfacial stiffness, when the implant is inserted stably, RFA is also considered to reflect implant/bone interfacial strength of immediately after placement from high correlation with the immediate load-baring capacity. RFA and measuring the cortical bone thickness with X-ray before and during surgery could be an effective diagnosis tool for the success of immediate loading of implant.

Numerous studies reported the significant association between genetic polymorphisms in the vitamin D receptor (VDR) gene and various bone phenotypes such as bone mineral density (BMD) and bone quality, although conflicting results were produced. The objective of this study was to investigate the association between a TaqI RFLP in the VDR gene and calcaneal BMD in Korean vegetarian men, and its interaction with nutrition status as an environmental factor. BUA (broadband ultrasound attenuation), SOS (sound of speed) and stiffness index of the calcaneus were measured using an ultrasound bone densitometer in 266 Korean men (age: $mean{\pm}SD;\;50.9{\pm}12.0$ year), and a TaqI RFLP in the VDR gene analysed by PCR-RFLP method. In total subjects, the distribution of TT, Tt and tt genotypes occurred with frequencies of 90.8%, 8.8% and 0.4%, respectively. There were no significant associations between this polymorphism and osteopenia-osteoporosis or several bone phenotypes in our subjects irrespectively of nutrition status (P>0.05). Therefore, our results suggest that a TaqI RFLP in the vitamin D receptor gene does not contribute to the susceptibility to the calcaneal BMD in Korean men.
