임신부에서 항히스타민제와 알레르기용약의 국가별 안전정보 일치도 분석 : 한국, 미국, 영국, 일본 허가사항을 중심으로 (Agreement of Label Information of Antihistamine, Anti-allergy Medications in Pregnancy among Korea, the USA, the UK, and Japan)

  • Background: Antihistamine and anti-allergy medications are widely used during pregnancy. Reading label information is one of the easiest ways to get safety information. But there are content gaps among countries. Objective: To compare the risk level and the recommendation level of antihistamine/anti-allergy drug's label information in pregnant women among Korea, the USA, the UK, and Japan. Method: Study drugs of antihistamine/anti-allergy medications were selected according to Korea drug classification codes. Based on the label information of selected product, risk level was classified into 5 categories as follows: 'Definite', 'Probable', 'Possible', and 'Unlikely', 'Unclassified' according to the level of evidence. Recommendation level was classified into 4 categories as follows: 'Contraindicated', 'Cautious', 'Compatible', and 'Unclassified'. Frequency and proportion were presented according to the each category. To estimate agreement of each category among 4 countries, percent agreement and kappa (k) coefficient were calculated. Results: Total 13 drug ingredients were selected for antihistamine/anti-allergy medications. In risk level, Korea (46%) and Japan (69%) were mostly classified in the category of 'Unclassified', but 'Unlikely' category was more frequent in the UK (62%) and the USA (46%). In recommendation level, the proportion of 'Contraindicated' was highest in Korea (46%) compared to other countries. In contrast, the category of 'Cautious' was 77%-85% in the USA, the UK, and Japan. The percent agreement for risk level was highest in the USA-UK (54%). The recommendation level of Korea-USA showed lowest agreement for percent agreement (46%) and kappa coefficient (k=0.02). Conclusion: We confirmed the differences among safety information provided by four different countries. 'Contraindicated' was more likely in Korea compared with other countries.

Effect of the anti-IL-17 antibody on allergic inflammation in an obesity-related asthma model

  • Background/Aims: The co-occurrence of obesity aggravates asthma symptoms. Diet-induced obesity increases helper T cell (TH) 17 cell differentiation in adipose tissue and the spleen. The 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitor pravastatin can potentially be used to treat asthma in obese patients by inhibiting interleukin 17 (IL-17) expression. This study investigated the combined effects of pravastatin and anti-IL-17 antibody treatment on allergic inflammation in a mouse model of obesity-related asthma. Methods: High-fat diet (HFD)-induced obesity was induced in C57BL/6 mice with or without ovalbumin (OVA) sensitization and challenge. Mice were administered the anti-IL-17 antibody, pravastatin, or both, and pathophysiological and immunological responses were analyzed. Results: HFD exacerbated allergic airway inflammation in the bronchoalveolar lavage fluid of HFD-OVA mice as compared to OVA mice. Blockading of the IL-17 in the HFD-OVA mice decreased airway hyper-responsiveness (AHR) and airway inflammation compared to the HFD-OVA mice. Moreover, the administration of the anti-IL-17 antibody decreased the leptin/adiponectin ratio in the HFD-OVA but not the OVA mice. Co-administration of pravastatin and anti-IL-17 inhibited airway inflammation and AHR, decreased goblet cell numbers, and increased adipokine levels in obese asthmatic mice. Conclusions: These results suggest that the IL-17-leptin/adiponectin axis plays a key role in airway inflammation in obesity-related asthma. Our findings suggest a potential new treatment for IL-17 as a target that may benefit obesity-related asthma patients who respond poorly to typical asthma medications.

Mouse 모델 알레르기 반응에서 삼백초(三白草)가 미치는 영향 (Inhibitory Effect of Saururus chinensis (Lour.) Baill Extracts on Allergy in Mouse Models)

  • We investigated the effect of Saururus chinensis (Lour.) Baill (SCB) on allergy in mice. We conformed compound 48/80-induced mesenteric mast cell degranulation, active systemic anaphylatic shock and histamine release. Also observed acetic acid-induced vascular permeability and anti-dinitrophenyl (DNP) IgE-mediated passive cutaneous anaphylaxis. SCB inhibited mesenteric mast cell degranulation and active systemic anaphylatic shock induced by compound 48/80 dose-dependently. When SCB was pretreated by intra-peritoneal injection, the plasma histamine levels were reduced. SCB also significantly inhibited acetic acid-induced vascular permeability and anti-DNP IgE-mediated passive cutaneous anaphylaxis. In addition, SCB reduced IL-10 mRNA expression of the lung on ovalbumin-induced allergy. These results indicate that SCB inhibits allergy.

Mixed Infection of Mycobacterium abscessus subsp. abscessus and Mycobacterium tuberculosis in the Lung

  • A mixed infection of Mycobacterium abscessus subsp. abscessus (Mab) and Mycobacterium tuberculosis (MTB) in the lung is an unusual clinical manifestation and has not yet been reported. A 61-year-old woman had been treated for Mab lung disease and concomitant pneumonia, and was diagnosed with pulmonary tuberculosis (PTB). Despite both anti-PTB and anti-Mab therapy, her entire left lung was destroyed and collapsed. She underwent left pneumonectomy and received medical therapy. We were able to successfully treat her mixed infection by pneumonectomy followed by inhaled amikacin therapy. To the best of our knowledge, thus far, this is the first description of a mixed Mab and MTB lung infection.

전신과민성 식품 알레르기 마우스 모델에서의 도두(刀豆) 추출물의 항염 효과 (Anti-inflammatory Effects of Canavaliae Semen(Canavalia gladiate) Extracts in a Systemic Anaphylaxis Food Allergy Mouse Model)

  • Objective : An allergy to peanuts is a major cause of fatal food-induced anaphylaxis, with food allergies becoming an increasingly important health research issue. Food allergy as clinical entity has been recongnized for many years, although there is yet no general concord as to the incidence of this symptom.1) Methods : This study was undertaken to verify the effect of seeds of Canavalia gladiata (Jacq.) DC. extract (CGE) on the inhibition of allergic reactions using a cholera toxin and peanut extract-immunized food allergy mouse model. We determine whether the changes in rectal temperature were related to energy consumption owing to heat production in the body. Mast cell distribution and degranulation in the dermis and epidermis were observed with an optical microscope. Subsequently, Ara h1 levels in serum and interleukin (IL)-4, IL-10, and $IFN-{\gamma}$ levels in cultured supernatants of splenocytes were measured. Results : CGE treatment significantly attenuated the secretion of the Ara h1 antibody in serum and splenocytes. Ara h 1 was undetected in the cholera toxin and peanut extract-immunized food allergy mouse model. Improvement in ear tissue inflammation symptoms was the CGE experimental group. In the control group and peanut extract control group, the expression of mast cells was higher, whereas that in the CGE experimental group was significantly lower. Conclusion : CGE causes suppression in a food allergy mouse model via the inhibition of Ara h1 secretion, and might be useful for developing functional health foods.

Intravenous Mesenchymal Stem Cell Administration Modulates Monocytes/Macrophages and Ameliorates Asthmatic Airway Inflammation in a Murine Asthma Model

  • Although asthma is a common chronic airway disease that responds well to anti-inflammatory agents, some patients with asthma are unresponsive to conventional treatment. Mesenchymal stem cells (MSCs) have therapeutic potential for the treatment of inflammatory diseases owing to their immunomodulatory properties. However, the target cells of MSCs are not yet clearly known. This study aimed to determine the effect of human umbilical cord-derived MSCs (hUC-MSCs) on asthmatic lungs by modulating innate immune cells and effector T cells using a murine asthmatic model. Intravenously administered hUC-MSCs reduced airway resistance, mucus production, and inflammation in the murine asthma model. hUC-MSCs attenuated not only T helper (Th) 2 cells and Th17 cells but also augmented regulatory T cells (Tregs). As for innate lymphoid cells (ILC), hUC-MSCs effectively suppressed ILC2s by downregulating master regulators of ILC2s, such as Gata3 and Tcf7. Finally, regarding lung macrophages, hUC-MSCs reduced the total number of macrophages, particularly the proportion of the enhanced monocyte-derived macrophage population. In a closer examination of monocyte-derived macrophages, hUC-MSCs reduced the M2a and M2c populations. In conclusion, hUC-MSCs can be considered as a potential anti-asthmatic treatment given their therapeutic effect on the asthmatic airway inflammation in a murine asthma model by modulating innate immune cells, such as ILC2s, M2a, and M2c macrophages, as well as affecting Tregs and effector T cells.

알레르기 후기 반응 염증 억제 효과에 관한 태음조위탕(太陰調胃湯)의 실험적 연구 (An Experimental Study on the Anti-inflammatory Effect of Taeeumjowui-tang (Taiyintiaowei-tang) in Allergic Late Inflammation)

  • Objective : Allergic disease, a very common chronic illness that affects all ages of patients, has been well characterized as an IgE-dependent immunologic response. Recently, interest has grown about the late inflammatory reaction as well as the early one characterized by IgE and mast cells in allergic disease. The purpose of this study was to find the anti-inflammatory effect of Taeeumjowui-tang (Taiyintiaowei-tang) in allergic reaction. Methods : The experiment was performed using Raw 264.7 cells pretreated with Taeeumjowui-tang contents extracts. The results were measured for different concentrations of Taeeumjowui-tang extracts (100, 200, $300{\mu}g$/ml): the toxicity and proliferation of cells by MTT analysis, LPS-induced NO production using Griess reagent and IL-6/ TNF-${\alpha}$ production, which is a significant criteria for diagnosing allergic reaction. Results : No toxicity of Taeeumjowui-tang (100, 200, $300{\mu}g$/ml) on Raw 264.7 cells was found after 24 hours incubation. LPS-induced NO production was reduced after treatment with Taeeumjowui-tang (100, 200, $300{\mu}g$/ml)(P<0.001). IL-6 decreased only at $100{\mu}g$/ml(P<0.05). TNF-${\alpha}$ production decreased only at $300{\mu}g$/ml, but still statistically insignificant. There was no relationship between any components of Taeeumjowui-tang alone and inhibition of NO production. Conclusions : These data suggest that Taeeumjowui-tang has anti-inflammatory effects in allergic reaction.

Study on Anti-allergic Effects of Electroacupuncture in Allergic Mouse Model

  • Electroacupuncture(EA) is commonly used in various diseases. In the present study, the effect of EA in the allergic mouse model was examined. Allergy is generated via immunological mechanism and non-immunological mechanism. Mast cells activated dy those mechanisms get to release various substances such as histamine, leukotrienes, prostaglandin, TNF-$\alpha$, IL-4, IL-6, etc. which induce allergic reactions and the following inflammatory responses. To evaluate the anti-allergic effects of EA, mortality, ear swelling response, vascular permeability and cytokine secretion were investigated in EA group and non-EA group of which mice were compound 48/80-induced allergy model or PCA model. Compound 48/80 induces allergic reaction via non-immunological mechanism and PCA model is generated through the same mechanism with immediate-type(Type1) allergic reaction, one of immunological allergic reactions. EA inhibited compound 48/80-induced ear swelling response but did not inhibit the systemic anaphylaxis. EA also inhibited passive cutaneous anaphylaxis(PCA) activated dy anti-dinitrophenol IgE. In addition, EA inhibited IL-6 and TNF-$\alpha$ secretion from 48 h PCA in mice. These results indicate that EA may be used for the treatment of mast cell-mediated allergic diseases, especially immediate-type(Type 1) allergy and non-immunologically mediated allergy.

호장근추출물이 피부질환 및 피부미용에 미치는 영향 (The Effects of Polygonum Cuspidatum on the Skin Functions)

  • Objectives : This study was carried out to investigate the effects of Polygonum cuspidatum extract on several skin functions including inflammation and wrinkle formation. Methods : To investigate in vitro anti-oxidant activity assay, ethanol extracts of medicinal plants tested by DPPH method. In the next experiment, to investigate anti-inflammatory test, the RAW 264.7 macrophage cells was cultured using DMEM including the 10% FBS. To study anti-allergic effect, we blended cultured Human Mast Cells(HMC-1), and then observe $TNF-{\alpha}$, IL-8 by ELISA Results : Polygonum Cuspidatum extract has the effects of anti-inflammation and anti-allergy, which may be due to its inhibitory potential on the macrophage activation. Furthermore, Polygonum Cuspidatum extract has the anti-wrinkle effects through the inhibitory potential on the collagnease, elastase and gelatinase activities. Conclusions : The above results suggest that Polygonum Cuspidatum extract could be applicable for improvement of several skin functions.
