Urban Characteristics Affecting Obesity of Elementary, Middle and High School Students (초, 중, 고등학생들의 비만에 영향을 미치는 도시 특성)

  • This study is to identify urban characteristics affecting obesity of elementary, middle and high school students. Most of previous studies in Korea discussed healthy city focusing on adults. We list up possible urban characteristic factors that are considered to affect adolescent obesity from previous studies, and then conduct regression analysis to find policy implications in urban planning. Based on the physical examination data of adolescent students in Seoul from the Ministry of Education, we set the rate of obese students by school as a dependent variable. Urban characteristic variables are set as explanatory variables, and measured by buffer analysis within 500m, 1000m and 1500m. The result of regression analysis shows that the obese rate of students increases with adults' lower education level, higher rate of non-apartment houses, older age of houses and more homogeneous land use. However, the result does not indicate that population density, accessibility to public transportation and the urban park are significant factors for adolescent obesity. Based on this result, urban planning policy for promoting adolescent health should be discussed further.

Combined Influence of Screen based Sedentary Behavior and Sleep Duration on Obesity, Depression, and Suicidal ideation in Korean Adolescents (청소년의 비만도, 우울, 자살생각에 대한 스크린 기반 좌식활동과 수면시간의 수준별 결합효과)

  • Purpose: The purpose of this study was to investigate the combined influence of screen based sedentary behavior and sleep duration on obesity, depression, and suicidal ideation in Korean Adolescents. Methods: This cross sectional study used secondary data from the 2013 Adolescent Health Behavior Online Survey. A total of 27,929 high school students were included in the study. Logistic regression was used for analysis of the combined influence of screen based sedentary behavior and sleep duration on obesity, depression, and suicidal ideation. Results: Obesity was more prevalent among adolescents who had ${\geq}$ 4h of screen based sedentary behavior and <7h of sleep duration compared to the reference group, defined as < 2h of screen time and > 7h of sleep duration. Depression and suicidal ideation were more prevalent among adolescents who had 2-4h of screen based sedentary behavior and <7h of sleep duration compared to the reference group. Conclusion: Based on the results of this study, maintaining a low (< 2h) level of screen based sedentary behavior and appropriate sleep duration over 7 hours per day is essential for prevention of adolescent obesity, depression, and suicidal ideation.

Establishing a Standard Definition for East Asian Adolescent's Obesity (동아시아 청소년의 비만 기준 설정)

  • Objcetive: The purpose of this study was to establish body mass index percentiles and cutoffs for overweight and obesity in East Asian Adolescent. Methods: Based on one's age and gender, subjects were selected and measured their weight and height in order to calculate BMI. For BMI cut off points, data were analyzed and percentile curves were established by the modified LMS method. Results were followed by comparing BMI cut off points and percentiles with one's nationality, age and gender. Results: The prevalence of male's overweight and obesity among East Asian adolescent were as follows: Taiwan (15.4%, 9.0%), Korean (14.5%, 8.5%), Chinese (13.3%, 6.6%), and Japanese (6.0%, 2.1%). And for female, Korean (13.1%, 3.7%), Taiwan (12.6%, 5.1%), Chinese (8.3%, 2.3%) and Japanese (7.9%, 3.1%) were in the order. Conclusion: Corresponded to the whole subjects, 11.0% of men were overweight, 4.7 were obesity and that appeared in women with 9.0% in overweight, 3.5% were obese.

The prevalence of Childhood and Adolescent Obesity Over the Last 18 Years in Seoul Area (서울시내 초.중.고 학생들의 최근 18년간 (1979-1996년) 비만도 변화추이 및 비만아 증가 양상)

  • The purpose of this study is to survey the prevalence of obesity assessed on the basis of height and weight among students in primary and secondary schools in the metropolitan area of Seoul, Korea during the period of 1979 to 1996. The major findings are as follows ; 1) The fiftieth percentile values of BMI have increased more in mid-ranged age group than upper(16-17 years old) and lower(6-7 years old) ranged age groups. 2) The prevalence of obesity by standard weight of height was higher among male students than female students and students in the primary schools showed higher weight increase than adolescent age(12-17 years old) group. As a whole, the prevalence of obesity over last 18 years has increased 4.6 times in males and 3.2 times in females. 3) Judged on the basis of the BMI 90th, 95th percentile values of 1979 , the prevalence of obesity among male students showed higher increases than among female students. Higher increase was also observed among primary school students than among middle and high school students. Increase of weight was higher for the $\geq$95th percentile group than the 90-95th percentile group. 4) Judged on the basis of the BMI, 90th, 95 th percentile values of NHANES-I, the $\geq$95th percentile group showed higher increase in the prevalence of obesity than the 90-95th percentile group. According to the 1996 data, severe obesity group ($\geq$95th percentile) was about two times of NHANES-I in male primary school students, while the number for female middle and high school students was about 1/5 of NHANES-I.

  • Adolescent obesity has a high risk of developing into adult obesity and may cause many psychological problems. This paper aims at giving a mathematical model for the obesity of Korean adolescents and predicting how much the prevalence of obesity among adolescents will increase using real data. We estimate that the obesity rate of boys will increase until about 28 ~ 29% in 2025.

RELATIONSHIP BETWEEN QUALITY OF LIFE AND BODY MASS INDEX IN MIDDLE SCHOOL STUDENTS (중학생들에서의 체 질량 지수와 삶의 질의 관계)

  • Objectives: Obesity is one of risk factors for psychological and social adjustment problems (depression, low self esteem, low school function etc). Quality of life(QOL) is comprehensive and multidimensional construct including physical, emotional, social and school functioning. Some authors reported that child and adolescent obesity is associated with impaired QOL. This study was to examine the association between QOL and body mass index (BMI) in middle school students. Methods : Cross sectional analysis of 471 adolescents between the ages of 13 and 14 years ($mean{\pm}SD,\;13.4{\pm}0.6$), who attended one of middle schools in Seoul, Korea. Participants' QOL were scored by using Korean version of pediatric QOL inventory generic score scale ($PedsQL^{TM}$ 4.0). Participants had a $mean{\pm}SD\;BMI\;of\;21.3{\pm}4.0$. Self reports and parent proxy reports using $PedsQL^{TM}$ 4.0 were administered to measure participant's QOL. Participants were divided into four groups by BMI (Male: obesity group >26.5, at risk group 23.2-26.5, normal weight group 15.8-23.1, underweight group <15.8, Female : obesity group >25.2, at risk group 22.9-25.2, normal weight group 16.0-22.8, underweight group <16.0). Results : Compared with other groups, obese adolescents reported significantly(p<0.01) love. QOL in all domains. Obese adolescents were more likely to have impaired QOL than normal weight adolescents (Odds Ratio, 8.3, 95% confidence interval, 4.0-16.7). Conclusion : Obese adolescents have lower QOL than normal weight adolescents. Psychiatrist, parents, and teachers need to be informed of the risk fur impaired QOL in obese adolescents to target interventions that could enhance health outcomes.

Evaluation of Web Sites on Treatment of Childhood and Adolescent Obesity (국내 인터넷 웹사이트에 소개된 소아 및 청소년 비만치료의 실태 및 문제점)

  • Purpose: The purpose of this study was to evaluate the quality and problems of Web sites for management of childhood and adolescent obesity. Methods: We evaluated 203 Web sites identified from the search engine, Korean Yahoo, using the word of 'childhood and adolescent obesity'. 203 Web sites were classified according to medical institutions, health information Web sites, beauty shops. etc. We surveyed whether childhood and adolescent obesity distinguished with adult obesity was considered, or not. and researched the unique managements of childhood and adolescent obesity including the cardinal treatment. Results: Of the 203 Web sites, 157(77.3%) provided detailed information about treatment of obesity, 46(22.7%) provided only simple information about one. The sites providing detailed information were composed of 52.2% of oriental medicine clinics, 35.0% of clinic & hospitals including pediatric hospitals. Distribution of the sites about management of childhood and adolescent obesity distinguished with adult's one was only 23% of oriental medicine clinics, but 93% of childrens hospitals. Conclusion: Without considering the speciality of childhood obesity, inaccurate information are distributing on internet web sites. It is necessary for concern and development of advertizing system on the internet distributing accurate information about treatment of childhood obesity.

A Study of the Obesity Index and Psychosocial Factors Influencing Obesity among Adolescent Girls (여고생들의 비만도 및 비만과 관련된 사회심리적 요인 분석)

  • The purpose of this study was to investigate the dietary intake, obesity index and psychosocial factors influencing obesity among 200 high school girls in Seoul. The Social Cognitive Theory provided the Conceptual basis for this study. A cross-sectional survey was conducted to examine factors related to obesity, including self-efficacy for controlling overeating, social support for eating behavior, perception of body image and weight control, nutrition knowledge, and attitudes toward obesity. The data were analyzed using t-test and multiple logistic regression. The results of this study are as follows : 1) The mean age of the girls was 16.4 years, and the rate of overweight and obesity(measured by obesity index) was 27.0$\%$. 2) The mean energy intake of subjects was 1832.3$\pm$384.0kcal. The energy derived from carbohydrates, proteins and fats was 62.7$\%$, 13.8$\%$, and 23.5$\%$, respectively. There was no significant difference between the obese and the comparison group in energy intake. 3) The result of multivariate analysis indicated that obesity had a significant relation to the perception of ideal body image, social support for eating behavior, and self-efficacy for controlling overeating(p〈0.01). As subjects preferred thinner body images(OR=0.39) and received less social support(OR=0.93), the odds of being classified as obese increased. The odds of being obese were also associated with self-efficacy, however, the relation was not strong(OR=1.04). 4) Specific social support was related to obesity among adolescent girls. As subjects received more support from family member, the odds of being obese decreased. The emotional support as well as family member's positive nutrition behavior plays a significant role. In addition, instrumental support from friends was associated with obesity. With repect to self-efficacy, the odds of being obese were increased as subjects felt less confident in controlling overeating when tempting food was placed in front of them or after an argument. In contrast, the obese group felt more confident in controlling overeating for the rest of the specific situations examined. These findings suggest that educational interventions for weight control should incorporate strategies to help participants realize their degree of obesity, to reduce the discrepancy between current and ideal body image, to elicit and maintain social support from friends and family, and to increase the self-efficacy for changing eating behaviors.   • Objective: To report the clinical effect of Korean medicine treatment in an adolescent patient diagnosed with nonalcoholic fatty liver disease (NAFLD) with obesity. Methods: The patient was treated from July 9th, 2014 to August 7th, 2014. He was taking Saenggangeonbi-tang (生肝建脾湯) during the treatment period. He also undertook regular exercise and regulated his diet to reduce body weight. Results: The patient’s AST, ALT, and GGT levels were significantly decreased. Total cholesterol, triglyceride, and LDL levels were also decreased, and HDL level was increased. His body weight and body mass index (BMI) were decreased. Conclusions: The results suggest that Saenggangeonbi-tang with exercise and regulated diet could be an effective treatment for adolescent NAFLD with obesity in clinics.

Analysis of Obesity Intervention Programs in Adolescents: Focused on Endothelium Functions (비만 청소년의 비만 중재 프로그램에 대한 국내외 연구 분석: 혈관내피 기능을 중심으로)

  • Purpose: The purpose of this study was to determine the current status of obesity intervention programs, and its effects on endothelium function in adolescents. Method: This was a descriptive research study, that investigated domestic and international literature 2009-2018. Using databases inside and outside of Korea to search for 'adolescent', 'obese adolescent', 'exercise', 'program', or 'intervention', and 'endothelium function', a total of 14 literature have been selected for 6 domestic research and 8 international research, excluding overlapping, case studies, literature reviews, and unrelated studies. Results: Interventions for enhancing endothelium function were exercise program, diet, vitamin D3 treatment, and Orlistat intake relative to international research, and combined exercise and purple sweet potato intake, in most domestic research. Dependent variables included FMD, RHI, NMD, EPCs, and EMPs, for measuring endothelium function in international research, and PWV, FMD and RHI, in domestic research. Conclusion: Results of this study confirmed that application of obese intervention programs, may improve endothelium function in adolescents. Further studies are required, to develop nursing intervention, that would enhance endothelium function in adolescents.
