Health Behaviors Before and After the Implementation of a Health Community Organization: Gangwon's Health-Plus Community Program

  • Objectives: Community organization is a resident-led movement aimed at creating fundamental social changes in the community by resolving its problems through the organized power of its residents. This study evaluated the effectiveness of health community organization (HCO), Gangwon's Health-Plus community program, implemented from 2013 to 2019 on residents' health behaviors. Methods: This study had a before-and-after design using 2011-2019 Korea Community Health Survey data. To compare the 3-year periods before and after HCO implementation, the study targeted areas where the HCO had been implemented for 4 years or longer. Therefore, a total of 4512 individuals from 11 areas with HCO start years from 2013 to 2016 were included. Complex sample multi-logistic regression analysis adjusting for demographic characteristics (sex, age, residential area, income level, education level, and HCO start year) was conducted. Results: HCO implementation was associated with decreased current smoking (adjusted odds ratio [aOR], 0.73; 95% confidence interval [CI], 0.57 to 0.95) and subjective stress recognition (aOR, 0.79; 95% CI, 0.64 to 0.97). Additionally, the HCO was associated with increased walking exercise practice (aOR, 1.39; 95% CI, 1.13 to 1.71), and attempts to control weight (aOR, 1.36; 95% CI, 1.12 to 1.64). No significant negative changes were observed in other health behavior variables. Conclusions: The HCO seems to have contributed to improving community health indicators. In the future, a follow-up study that analyzes only the effectiveness of the HCO through structured quasi-experimental studies will be needed.

  • Purpose: The purpose of this paper is to present a service construction plan using multiple complex sensor information to detect abnormal situations in urban life safety that are difficult to identify on CCTV. Method: This study selected service scenarios based on actual testbed data and analyzed service importance for local government control center operators, which are main users. Result: Service scenarios were selected as detection of day and night dynamic object, Detection of sudden temperature changes, and Detection of time-series temperature changes. As a result of AHP analysis, walking and mobility collision risk situation services and fire foreshadowing detection services leading to immediate major disasters were highly evaluated. Conclusion: This study is significant in proposing a plan to build an anomaly detection service that can be used in local governments based on real data. Risk Factors of Primary Nocturnal Enuresis (일차성 야뇨증의 위험 요인)

  • Purpose: The Pathogenesis of primary noctllrnal enuresis(PNE) is still controversial. Genetic factor and maturational delay of micturition reflex including arousal disorder, lack of nocturnal Arginine Vasopressin(AVP) release and functional bladder capacity have been suggested. We analyzed the risk factors of PNE. Methods: Fifty five children with PNE (20 enuretics diagnosed at school physical examination of the first grade students at Mok-Dong Elementary School and 35 enuretics (Age 6-7 year) diagnosed at Ewha Womans University Mok Dong Hospital) and 221 control students without PNE at school physical examination were included. Genetic, stress and developmental factors, arousability, water intake, urine volume, maximun voiding volume and daytime voiding dysfunction were compared. Results: 1) There was no significant difference between PNE and control group in sex ratio, birth order, percentage of working mothers, parental and child personality, age to start walking, school record and duration of sleep. 2) Family history in the PNE group was significantly higher than control group ($20.0\%\;vs\;2.7\%$)(P<0.05). 3) The difficulty in arousal in the PNE group was significantly more common than the control group ($70.9\%\;vs\;54.3\%$)(P<0.05). 4) Nocturnal water intake in the PNE group was significantly greater than the control group ($330{\pm}158.2\;mL\;vs\;235{\pm}129.5\;mL$). Nocturnal urine volume in the PNE group was significantly greater than the control group ( $390{\pm}61.5\;mL\;vs\;140{\pm}43.2\;mL$)(P<0.05). Daily water intake and daily urine volume did not significantly differ between the two groups. 5) Maximum urine volume per void in the PNE group was significantly lower than the control group ($107{\pm}35.9\;mL\;vs\;236{\pm}41.3\;mL$). Daytime voiding dysfunction in the PNE group was significantly more common than the control group ($80.0\%\;vs\;57.9\%$). The voiding frequency in the PNE group was significantly greater than the control group ($7.0{\pm}3.6\;vs\;5.4{\pm}1.6$)(P<0.05). Conclusion: In addition to genetic factors and maturational delay of micturition reflex (difficulty in arousal, nocturnal polyuria and decreased functional bladder capacity) nocturnal polydypsia was found to be the important risk factors fur PNE. So nocturnal fluid restriction should be encouraged as the first-line management of PNE.

The Effects of Exercise Education Programs in Mentally-Handicapped Children (정신지체아의 운동교육 Program 적용효과)

  • In this research, the mentally-handicapped children being able to be trained were investigated the actual condition of train function making 40 mentally-handicapped children in Kummi Hyedang Spacial Education School, to exmine the effect of physical exercise function when training the mentally-handicapped children who can be trained as applying exercise education programs, and trains for 10 weeks by assigning to both experiment group and control group according to children who are similar to training functions from pre-examination. The results are in following: First, the results of test in the exercise ability of mentally-handicapped children with the degree of being able to trained are very delayed in comparison with normal mental children through the both top and bottom examination. The developments of 5 exercise functions classified by domain, have the order of eyesight exercise, softness, physical strength, quickness, parallelism, the interaction of both eye and hand, and, have the exercise function being equal to the level of between 6 and 12 years old. In 13 bottom test, throwing bean-bag is equal to the nomal 12 years old boy. the board jump, sitting position / bending forward / closing are equal to the level of 12 years old boy. standing with only leg is the level of 9 years old, threading pearls is 7 years old, transfering the wood building, picking the upper body up, walking board, balancing one leg with opened, eye, fist / opening palm / palm, bending and opening arm with postrating on chair, are the exercise functions of below 6 years old. Second, there are great effects in carrying out the exercise education program to the mentally-handicapped children with the level of being able to trained. In experiment group, it is elevated to the middle level of 12 years old nomal children. Classified by domain of test the board jump, training, the bean-bag are far higher level than 12 years old normal children, and are elevated the level of 11 years old boy. Balancing only leg with closed eye is below the level of 10 years old boys, fist / opening palm / palm are the level of 9 years old boys. There and back running, picking the upper body is the level of 9 years old girls. Walking board is the level of 8 years old boys. Bending and opening arm with postrating on chair is the level of 7 rears old boy. Balancing one leg with opened eye is elevated to the level of f years old girls. These functions have the more balanced exercise function rather than pre-examination. In control group, they have little change by classified the bottom test, but have the exercise function on the time of pre-examination, go backward in physical strength. quickness. Third, the exercise function being learned by exercise education program on the mentally-handicapped children of the level with being able to train is appeared to maintain continuately. Softness, physical strength, quickness, eyesight training are maintained the learned exercise function, the interaction of both eye and hands, parallelism are delayed a little. Classified by the bottom test. threading pearls, transfering the wood building, throwing the bean-bag, sitting position / rolling forward / reaching, the broad jump and picking upper body up, there and bark running, picking upper body up, balancing with only leg as opened eye, bending and opening arm with postrating on chair, etc. are maintained. Fist / opening palm / palm, balancing with only leg as opened eye are delayed a little. The change of body position is elevated. Seeing these results, it is appeared to the mentally-handicapped children that the exercise education programs, which is suitable their actual condition and acomplishes in voluntary participation, have very positive effect. So, to develop the function of body exercise in mentally-handicapped children with the level of being to able to be trained, the measures must be groped so that the exercise education programs can be practiced positively, and the ,body exercise can be experienced more.

The intervention effects of the Clean Diet program on the health promotion attitudes and the physiological indices of an elderly (노인의 건강증진태도와 생리적 지수에 대한 클린다이어트 프로그램의 중재효과)

  • In this study, 60 seniors aged at least 65 years who attended C Senior College in Daegu were selected and randomly allocated to test and control groups of 30 subjects each. The Clean Diet Program was only applied to the test group, and the differences in self-efficacy, depression, health behavior, subjective perception of health, wellness, and physiological indices (blood pressure, blood glucose levels, hyperlipidemia, and BMI) between the test and control groups were compared. The results show a significant difference in the test group exhibiting improved or mitigated self-efficacy (t=6.003, p<.001), depression (t=4.038, p<.001), health behavior (t=6.196, p<.001), subjective perception of health (t=6.624, p<.001), and wellness (t=7.069, p<.001). Among the physiological indices, there was a significant decrease in BMI (t=5.974, p<.001). The results demonstrate the mediation effects of the Clean Diet Program. Since the Clean Diet Program can be implemented through walking, simple exercises, and eating habits, it provides great economic efficiency and usefulness and provides a very reasonable intervention program for the elderly. Based on the results, we discuss how to improve the elderly's attitude toward health promotion and their quality of life.

The Effects of Trunk Stability Exercise on Knee function, Balance, Gait in patients after Total Knee Arthroplasty (체간 안정화 운동이 무릎관절 전치환술 환자의 무릎기능, 균형, 보행에 미치는 영향)

  • This study was conducted to investigate the effects of trunk stability exercise on knee function, balance and gait in patients who underwent total knee arthroplasty. The subjects of this study were recruited from individuals diagnosed with degenerative arthritis who had undergone total knee arthroplasty. Overall, 24 patients were randomly divided into a control groups and an experimental groups (12 each). The trunk stability exercise was conducted for 4 weeks with three exercises developed in a previous study. The measurement tools used were knee function measurements based on the Lysholm knee score, balance ability measured using a test of TUG and OLS, and a walking ability test measured using a 10MWT. Pre and post test results were within groups were compared using the paired t-test, whole differences between groups were compared using the independent t-test. The experimental group showed significantly enhanced results relative to the control group (p<0.05). Based on these results, trunk stability exercise in parallel with knee joint therapy effectively improves the recovery of patients with total knee arthroplasty.

Effects of Otago Exercise Program on Physical Function and Risk of Falling in the Falls Experienced Elderly (오타고 운동프로그램이 낙상 경험 노인의 신체기능 및 낙상 위험도에 미치는 영향)

  • The purpose of this study was to investigate the effects of the Otago exercise program on physical function and risk of falling in elderly who have experienced falls. Thirty-six elderly people who had experienced falls more than once were randomly classified into an experimental group and control group. Each group exercised 30 minutes three times per week for a total of 8 weeks. Measurements were made using Tetrax to assess risk of falls. In addition, to measure physical function, static balance was measured using Biorescue, dynamic balance was measured using TUG, walking ability was measured using 10 MWT, and lower extremity strength was measured using FTSST. A paired t-test was conducted to verify the difference between before and after exercise in each group, and an independent t-test was performed for comparison between the groups. Regarding the experimental and control groups, static balance ability and dynamic balance ability increased significantly after exercise, and risk of falls decreased (p<.05). Lower extremity strength and gait ability increased significantly after exercise only in the experimental group (p<.05). Based on these results, the Otago exercise program is effective in improving physical function and reducing risk of falls in elderly people who have experienced falls.

A Study on the Criteria Establishment for One-way road using AHP (AHP분석을 활용한 일방통행 선정기준 정립에 관한 연구)

  • Standards suitable for local conditions on deciding one-way road are desperately required to solve traffic congestions at the backside roads in the old downtown areas which were not designated as a road by urban planning. Therefore, this study intends to re-establish a standard to decide one-way road which is regarded to be of the greatest effect among traffic system control methods in order to control one-way road system more efficiently. Also, this paper suggests a standard for such decision to improve efficiency of using backside roads and expand designation of one-way road. AHP (analytic hierarchy process) was carried out among the traffic experts to find out the factors to decide one-way road system. Its result reveals that importance of causing accident to walking quantity and traffic was high. 10,000 cases out of all the possible scenarios of accident by combining detailed evaluation items and scales were extracted to draw the outcomes of analyzing the scenarios, which were schematized in a graph. As a result, division by three sections of point of inflection was verified into $1{\leq}$ section A<1.91, 1.9$1{\leq}$ section B<2.08, and $2.08{\leq}$ section C<3. In other words, priority of deciding one-way road should be given to section C, the highest total point, while posterior to section A, where relatively low points are distributed. The standard on deciding one-way road suggested in this paper may be used for designating one-way road and basic data to re-establish the relevant system in the future.

The long-term effect of Interactive Video Game on Cognitive Information Processing the elderly: P300 (장기간의 상호작용적 비디오 게임이 노인의 인지정보처리에 미치는 영향: P300)

  • The objectives of this study was to examine the effect of Interactive Video Game on cognitive information processing the elderly. Sixty elderly were attended in this study. Their ages ranged from 65 to 70, with a mean age of 67.60 years. The subjects were randomly assigned to one of three experimental conditions: (1) interactive video game group (n=20), (2) aerobic exercise group (n=20), (3) control group (n=20). The experimental design of this study was analyzed using two-way ANOVAs with repeated measures of groups and time. Cognitive function was assessed by neuroelectrical response, and ERP analysis. The results of the study showed that the interactive video game group and aerobic exercise group showed no significant statistical differences in the response time, response accuracy, amplitude and potential of the performance of the exercise in cognitive function and ERP analysis, but improved the interaction video game group and aerobic exercise (walking) group over the control group. It was concluded that long-term aerobic exercise like interactive video game is associated with attenuation of cognitive decline in the elderly.

Effects of Foot Bath and Spray Application of Peppermint and Grapefruit Essential Oils on Lower Extremity Edema, Pain, and Fatigue (페퍼민트, 그레이프후룻 에센셜 오일을 이용한 족욕 또는 스프레이 적용이 하지부종, 통증 및 피로에 미치는 효과)

  • Work involving standing or walking for a long time can cause swelling, pain, and fatigue in the lower extremities due to the effect of gravity on the circulation of the body. This study aimed to confirm the effects of aromatherapy essential oils, as a complementary and alternative therapy, on lower extremity edema, pain, and fatigue when applied as a foot bath or spray. The study was conducted after daytime work over two consecutive days on nurses who volunteered at the E University Hospital in D Metropolitan city between 15 August 2018 and 11 October 2018. This randomized controlled study. The foot bath group received a foot bath in 9 L of water containing 0.5 ml of essential oil blended with grapefruit and peppermint in a 1:1 ratio. The spray group received the same solution in 3% dilution in the form of a spray. Edema was assessed by measuring calf and ankle circumference, while pain and fatigue were evaluated using a numerical rating scale. Data were analyzed by ANOVA and repeated-measures ANOVA. The calf circumference in the foot bath and spray groups significantly reduced compared to that in the control group (F = 14.053, p <.001) after the interventions. Lower extremity pain (F = 42.497, p < .001) and fatigue (F = 20.641, p < .001) also significantly reduced in the foot bath and spray groups compared to those in the control group after the interventions. Foot bath is recommended to relieve swelling and pain in the lower extremities, and aroma spray is highly recommended considering the ease of application.
