Objective : Management strategies for pediatric chiasmatic-hypothalamic gliomas(CHG) include surgery, irradiation, chemotherapy and a combination of these modalities. This study was performed in order to compare the efficacy of various methods of treatment and to describe its optimal management. Material and Method : We have reviewed the results of management of 6 children with a diagnosis of CHG, who were observed closely during the last 8 years. The patients were aged 7 months to 15 years. Our patients presented with diencephalic syndrome, endocrine dysfunction, and progressive visual loss. None of these had evidence of neurofibromatosis-1. Treatment consisted of surgery alone(2), surgry and irradiation(2), surgery, irradiation and chemotherapy(1), and surgery and chemotherapy(1). Results : Four children had large exophytic suprasellar tumors and two showed diffuse midline lesions. Obstructive hydrocephalus was present in all patients. Pathologic examination revealed anaplastic astrocytoma in 1 and lowgrade astrocytoma in 5. Two patients, recently treated with radiation therapy following radical subtotal resection, showed significant tumor reductions and good clinical status. Four patients had partial tumor resection. Of these patients, two developed disease stabilization during follow-up period of 7 and 8 years, respectively. Life-threatening complications were noted in remaining two patients. Conclusion : CHG may follow an unpredicatable course and show a various reponse to each treatment modality. Further studies are indicated to define the optimal method of treatment of CHG in childhood.

Objective: This study is designed to compare the effects of Sulguanjul-Bang No.1 on osteoarthritis in knee joint according to formulation. Methods: 47 patients with symptomatic osteoarthritis of the knee joint were enrolled in a open clinical trial. The 47 patients were devided into two groups at random and taken Sulguanjul-bang No.1 with decoction type, suspension type respectively for8 weeks. After 8-week treatment period, the following parameters were analyzed ; Visual analogue Scale(VAS), Lysholm index score, patients' global assessment. Results: Sulguanjul-bang No.1 treatment led to significant improvement in the pain and symptoms of osteoarthritis as determined by all efficacy measures. After 8 weeks of therapy, there was significant improvement in VAS, Lysholm index and patients' global assessment. But there is no significant difference between two groups. Conclusions: Sulguanjul-bang No.1 was very beneficial pain relief effect and functional improvement irrespect of formulation, decoction type or suspension type.

Objectives : The purpose of this study is to investigate the effect of Chuna treatment for neck pain patients caused by traffic accident. Methods : 64 patients were divided into two groups, experimental group and control group, of 32 patients each. Control group was treated with general korean medical treatment three times per week for 2 weeks. Experimental group was treated with Chuna treatment and general korean medical treatment three times per week for 2 weeks. We evaluated the treatment effect of each group with the visual analog scale(VAS) and neck disability index(NDI). Results : 1. In both two groups, VAS were decreased significantly in statistics after treatment. 2. In both two groups, NDI were decreased significantly in statistics after treatment. 3. There were significant reduction of the VAS in experimental group than control group, but, there were not significant reduction of the NDI in experimental group than control group. Conclusions : We suggest that Chuna treatment could be recommended as a useful therapy to neck pain patients caused by traffic accident.

Purpose : To evaluate effects of McKenzie exercise on the functional recovery and forward head posture of chronic neck pain patients. Methods : The subjects were consisted of fifteen patients who had abnormal neck posture, mild neck pain (28 males, 17 females; mean aged 21.9) from 19 to 33 years of age(mean age 21.9). All subjects were received McKenzie exercise for 35 minutes with clinical massage per day three times a week during 4 weeks period. Neck disability index was used to measure functional disability level. Visual analogue scale(VAS) was used to measure subjective pain level. craniovetebral angle(CVA), cranial rotation angle(CRA) was used to measure forward head posture with digital camera. All measurements of each patients were measured at pre-treatment and after 2 week, after 4 week, post-treatment on 2 week. Result : The VAS of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The Neck Disability Index of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CVA of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CRA of McKenzie exercise was not significantly reduced between pre-treatment and post-treatment (p<.05). Conclusion : McKenzie exercise improved pain and function of Chronic neck pain patients.
The optic nerve often suffers regenerative failure after injury, leading to serious visual impairment such as glaucoma. The main inhibitory factors, including Nogo-A, oligodendrocyte myelin glycoprotein, and myelin-associated glycoprotein, exert their inhibitory effects on axonal growth through the same receptor, the Nogo-66 receptor (NgR). Oncomodulin (OM), a calcium-binding protein with a molecular weight of an ~12 kDa, which is secreted from activated macrophages, has been demonstrated to have high and specific affinity for retinal ganglion cells (RGC) and promote greater axonal regeneration than other known polypeptide growth factors. Protamine has been reported to effectively deliver small interference RNA (siRNA) into cells. Accordingly, a fusion protein of OM and truncated protamine (tp) may be used as a vehicle for the delivery of NgR siRNA into RGC for gene therapy. To test this hypothesis, we constructed OM and tp fusion protein (OM/tp) expression vectors. Using the indirect immunofluorescence labeling method, OM/tp fusion proteins were found to have a high affinity for RGC. The gel shift assay showed that the OM/tp fusion proteins retained the capacity to bind to DNA. Using OM/tp fusion proteins as a delivery tool, the siRNA of NgR was effectively transfected into cells and significantly down-regulated NgR expression levels. More importantly, OM/tp-NgR siRNA dramatically promoted axonal growth of RGC compared with the application of OM/tp recombinant protein or NgR siRNA alone in vitro. In addition, OM/tp-NgR siRNA highly elevated intracellular cyclic adenosine monophosphate (cAMP) levels and inhibited activation of the Ras homolog gene family, member A (RhoA). Taken together, our data demonstrated that the recombinant OM/tp fusion proteins retained the functions of both OM and tp, and that OM/tp-NgR siRNA might potentially be used for the treatment of optic nerve injury.
Background: Isolated infraspinatus tear is very rare and clinical features are not as well known, therefore the purpose of this study was to evaluate clinical characteristics and outcomes of isolated infraspinatus tear that authors experienced. Methods: Authors reviewed 288 cases of full-thickness rotator cuff tear involving infraspinatus between 2010 and 2015, and retrospectively analyzed six cases of isolated infraspinatus tear. Perioperative clinical characteristics, postoperative functional outcomes of 6 months were investigated. Functional evaluation included visual analogue scale (VAS), range of motions, American Shoulder and Elbow Surgeons (ASES) score, and Constant score. Results: Calcific tendinitis was accompanied in 4 cases (66.7%). Three of them received steroid injection or aspiration or extracorporeal shockwave therapy. Mean preoperative pain VAS was 7.1 (range, 5-9), and mean postoperative pain VAS at 6 months later was 1.6 (range, 0-5). Preoperative muscle strength by isokinetic muscle performance test showed 52% deficit of abduction and 37.6% deficit of external rotation. All 6 patients had arthroscopic repair of the infraspinatus tendon. All the patients at the 6 months follow-up exhibited clinical improvement in the Constant score (67.8 [range, 45-77] to 89.3 [range, 81-100], p=0.029), and ASES score (52.3 [range, 30-77] to 90.0 [range, 80-100], p=0.002). There was no healing failure on imaging. Conclusions: Isolated infraspinatus tendon tear was frequently accompanied by calcific tendinitis, but pathophysiologic relationship should need more study. To rule out neurogenic etiology, magnetic resonance imaging and electromyography would be helpful. Arthroscopic infraspinatus tendon repair and supraspinatus debridement showed relatively good result in painful shoulder.

Objectives : The purpose of this study is to report the effect of Korean medicine treatment on limitation of eye movement in Miller-Fisher Syndrome. Methods : A patient was treated with herbal medicines, acupuncture(including electropuncture, phamacopuncture), Moxibustion and cupping therapy. To evaluate the result of this treatment, we used photographs of eye movement, Scott and Kraft score of both eye, length of eyeball movement and visual analogue scale(VAS) for subjective symptoms. Results : After treatment, the limitation of eye movement and diplopia were remarkably improved. Also, other symptoms such as dizziness, gait disturbance, facial nerve disorder were disappeared. Conclusions : This study suggests that Korean medicine treatments are effective for patient with limitation of eye movement in Miller-Fisher Syndrome.

Objectives: The purpose of this study was to report Four cases of Failed Back Surgery Syndrome (FBSS) patients after spinal fusion surgery who showed significant improvement in pain and function with Complex Korean medical treatment. Methods: This study was a retrospective observational study. We reviewed medical records of Four patients with lumbar pain or radiating leg pain, who have received spinal fusion surgery in the past. All Four patients took complex treatments of Mokhuri Neck and Back hospital which involes Acupuncture, Pharmaco-acupuncture, Gangchuk herbal medicine, Chuna and Physical therapy during about four-week of admission treatment. Visual Analogue Scale (VAS), Oswestry disability index (ODI), Pain Free Walking Distance (PFWD) scores were assessed before and after treatments. Results: The average of hospitalization period was 28.5 days. Mean VAS scores decreased from 6.5 to 2.3, Oswestry Disability Index (ODI) scores decreased from 56.25 to 38.25 and Pain Free Walking Distance (PFWD) also improved from 10m to 166.6m. Conclusion: This study implies that a combination of Korean medical treatments might be effective in relieving pain, and improving the functional status of FBSS patients. Further studies are needed to fully understand the mechanisms underlying the effects.
Objectives The purpose of this study is to understand effect on respiratory disease and satisfaction of parents about acupoint sticking in hottest period of summer. Methods A survey was conducted to the parents of their children who took acupoint sticking in Dog-days for two years in a row. 17 children (10 boys, 7 girls, 4.0-12.11 years) were attached Socheongo to BL-13, BL-15, BL-17 for 3 hours. In terms of the effectiveness of the therapy, the treatment only considered as effective when there is any improvement in respiratory symptoms. Results 11 children (64.7%) from the study group have shown positive effect. There were significant improvements in frequency of having a cold or duration of a cold; duration of rhinitis; frequency of having empyema or its duration; frequency of having bronchitis; frequency of having pneumonia; frequency of having otitis media. Respiratory health VAS (Visual Analog Scale) had significant increase from 5 to 6.71 (p<0.05). 7 parents (41.2%) were satisfied with acupoint in hottest period of summer and 16 parents (94.1%) wanted to have their children get treated again in next year. From the survey, inconveniences of acupuncture were weak attachment of the acupoint (23.5%), long attaching time (11.8%), pressure about number of treatments (5.9%), worry about side effects (5.9%). Side effects have been reported; 2 pruritus (11.8%), 1 pruritus and erythema (5.9%). Conclusions Acupoint sticking in hottest period of summer improves lung symptoms in children older than three years old. However, the ways to reduce the side effects and inconveniences are needed.

The purpose of this study was to compare the balance ability at different foot positions using K A T 2000 (Berg, Inc, vista, CA.1994). Thirty (male 15, female 15) normal subjects participated in this study. All subjects were tested at a one leg position or a two leg position that were toe-in $25^{\circ}$, toe-out $25^{\circ}$, and $45^{\circ}$. The starting position was where the subject crossed their arms across the chest and flexed knees slightily. The results of each test were displayed on a screen in a score format, which indicated balance index. These collected data were analyzed by using one way ANOVA, and t-test. The results of this study were as follows: When changing the angle of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated by $25^{\circ}$ was better than that with the foot rotated by $45^{\circ}$. When changing the direction of the foot in the one-foot and two-foot standing positions, there was no statistically significant difference, but the balance performance with the foot rotated laterally was better - except for the case when the foot was medially rotated by $25^{\circ}$ in the right-foot static standing position. When the feet were medially rotated by $25^{\circ}$ in the two-foot static standing position, and were medially rotated by $25^{\circ}$ and $45^{\circ}$ in the one-foot static standing position, the balance performance of females was better than that of males. In this study, it was found that the balance performance of the subjects changed when the position of the foot was shifted on an unstable base of support. However, there was little correlation between balance performance and the height, weight and foot length of the subjects. It is necessary to conduct a follow-up study targeting various age groups and those with various diseases using an unstable platform or applying different physical or visual conditions, such as the length of the legs. Physical therapists need to consider the position of the foot in clinical settings for a better balance training or assessment.
