Combination Treatment of Korean Medicine with Ohrim-san for Lower Urinary Tract Symptoms (LUTS) after Laparoscopic Myomectomy Patient : A Case Report (복강경하 근종적출술 후 발생한 하부요로증상에 대한 오림산을 포함하는 복합한의진료의 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on a post-laparoscopic myomectomy patient's lower urinary symptoms (LUTS). Methods: The patient who diagnosed with uterine myoma had Laparoscopic myomectomy. After the operation, the patient complained of dysuria, nocturia, frequent urination, urgent urination and urinary retention. The patient received combination treatment of Korean medicine during 14 days of hospitalization. The treatment included Ohrim-san, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through the Numeric Rating Scale (NRS), Overactive Bladder Symptom Score (OABSS), Frequency-Volume chart, and 5-Level version of EuroQol-Five Dimension (EQ-5D-5L). Results: After the treatment, the clinical symptoms such as dysuria, nocturia, frequent urination, urgent urination, urinary retention were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the combination treatment of Korean Medicine with Ohrim-san may be effective for treating lower urinary tract symptoms (LUTS) after laparoscopic myomectomy.

A Review of Ovarian Cyst Management in Oriental Medicine (난소낭종에 대한 한의 임상 연구의 종설)

  • Purpose: Ovarian cysts are fluid-filled sacs or pockets within or on the surface of an ovary. Most cysts present little or no discomfort and are noncancerous and harmless. So non-surgical treatments with herbal medicine or acupuncture and moxa therapy are important and suitable in this pathology. But there are few RCT articles about ovarian cyst in Korean Medicine(KM) clinical and research field. Methods: To review treatment method and other research tendency about ovarian cyst in journals related with KM and Traditional Chinese Medicine(TCM) through web-searching. Searching was done through web site #http://omis.ksucc.ac.kr# and pub-med(http://www.ncbi.nlm.nih.gov/sites/entrez) with key words #ovarian cyst#, #TCM#, #herb#, #CAM#, and #acupuncture#. Results: Seven articles were searched in journals related with KM, then six of them were simple case report and the other one was about diagnosis for ovarian cyst and another gynecologic disease such as uterine myoma and infertility through the iris diagnosis. And two articles were searched in journals related with TCM, then one of them was simple case report but the other is RCT article. Conclusion: KM and TCM therapies have some beneficial effect to resolve cyst, but objective power as a evidence is weak in the viewpoint of EBM. So continuous clinical trials such as RCT and multi center trials are needed.

A Total Spinal Anesthesia Developed during an Induction of an Epidural Block -A case report- (경막외차단 유도중 발생한 전척추마취 -증례보고-)

  • Total spinal anesthesia is a well documented serious life threatening complication which results from an attempted spinal or epidural analgesia. We had an accidental total spinal anesthesia associated with a cranial nerve paralysis and an eventual unconsciousness during epidural analgesia. A 45-year-old female with an uterine myoma was scheduled for a total abdominal hysterectomy under the epidural analgesia. A lumbar tapping for the epidural analgesia was performed in a sitting position at a level between $L_{3-4}$, using a 18 gauge Tuohy needle. Using the "Loss of Resistance" technique to identify the epidural space, the first attempt failed; however, the second attempt with the same level and the technique was successful. The epidural space was identified erroneously. However, fluid was dripping very slowly through the needle, which we thought was the fluid from the normal saline which was injected from the outside to identify the space. Then 20 ml of 2% lidocaine was administered into the epidural space. Shortly after the spinal injection of lidocaine, many signs of total spinal anesthesia could be clearly observed, accompanied by the following progressing signs of intracrainal nerve paralysis: phrenic nerve, vagus nerve, glossopharyngeal nerve and trigeminal nerve in that order. Then female was intubated and her respiration was controlled without delay. The scheduled operation was carried out uneventfully for 2 hours and 20 minutes. The patient recovered gradually in th4e reverse order four hours from that time.

  • Because interventional procedure operates looking at premier as real time when perate intervention enemy, by patient is revealed during suitableness time in radiation, side effect such as radiation injury of skin is apt to happen. It established by purpose of study that measure exposure dose that patient receives about these problem, and find solution for radiation injury and repletion method. In this study, we used Rando phantom of identical structure with the human body which becomes accomplished with 4 branch ingredient of the attempt and system equivalent material them and absorbed dose were measured by TLD. According to the laboratory, it shows that operations such as TFCA procedure or uterine myoma embolization are more dangerous than TACE procedure. If both operations are inspected during a short time, it is not affected in being bombed. However, it can lead to palliative agenesis or depilate, definitive agenesis only if operations are repeated more than three times. Dose distibution based on experiment, to reduce radiation exposure to patients result from reduction of scatter ray as we control field size of radiation and protection of side organs except for tumor. also we knew that we can protect patients form radiation exposure, if we increas SOD and decrease SID.

A Clinical Study on the Effect of Dysmenorrhea Clinic for Female College Students (여대생을 대상으로 한 월경통 클리닉에 대한 임상 보고)

  • Objectives: The purpose of this study is to identify the clinical effects of korean medicine treatment of dysmenorrhea patients. Methods: We studied twenty-six patients who visited Oriental Hospital of ${\bigcirc}{\bigcirc}$ University from September 2011 to December 2011. Women with organic disease such as endometriosis, uterine myoma and pelvic inflammatory disease were excluded from this study. We treated them with herb, acupuncture and moxibustion for one menstrual cycle. The severity of dysmenorrhea was measured by VAS(Visual analog scale) and MVRS(Multidimensional Verbal Rating scale). Results: The mean${\pm}$Standard Error of mean(S.E) of VAS on after treatment was decreased significantly compared with that of before(p=0.00). The mean${\pm}$S.E of MVRS on after treatment was decreased significantly compared with that of before(p=0.00). Conclusions: This study shows that korean medicine treatment such as herb, acupuncture and moxibustion has remarkable effects on dysmenorrhea patients. Further study will be needed.

  • Objectives: This study was to investigate clinical usage of Guibi-tang based on actual application in obstetrics and gynecology. Methods: This study investigated the number of patients who were prescribed Guibi-tang-gami-bang and what herbal medicines were adjusted according to their chief complaint from January 1st, 2007 to August 31th, 2008. Results: The number of patients who were prescribed Guibi-tang for obstetrics and gynecological diseases at the Department of Obstetrics and Gynecology was 721. Guibi-tang has been used to treat menstrual irregularity, postpartum general weakness, postpartum pantalgia, perimenopausal symptoms and postmenopausal syndrome most frequently. And Guibi-tang has been widely used in obstetric and gynecologic diseases such as general weakness after abortion or gynecological surgery, pelvic pain, dysmenorrhea, uterine myoma, endometriosis, infertility, premenstrual syndrome, urinary incontinence and breast disease. Conclusions: Guibi-tang can be used in obstetric and gynecologic diseases as described in classical oriental medicine literatures, and it is necessary to research clinical usage of Guibi-tang through randomized controlled trial.

A Review of the Domestic Study Trends on Korean Medicine Treatment after Hysterectomy (자궁적출술 후 한의약 치료에 대한 국내 임상 연구 고찰)

  • Objectives: The purpose of this review is to analyze the domestic study trends on Korean medicine treatment for patients after hysterectomy. Methods: We investigated the studies on Korean medicine treatment for patients who undertaken hysterectomy via searching 4 online databases up to October 2020. After searching studies, we analyzed selected studies. Results: 12 clinical studies were selected and all studies were case reports. The most common cause of hysterectomy was myoma of uterine, followd by adenomyosis. The major complaints of post-hysterectomy were abdominal pain, low back pain and fatigue, general weakness. Herbal medicine was used in all studies. In addition, acupuncture, moxibustion and cupping were performed. The duration of inpatient treatment ranged from 5 to 39 days. All cases showed that chief complaints and accompanying symptoms were improved. Conclusions: Korean medicine treatment is an effective treatment option for patients after hysterectomy. Further large, well-designed studies are needed to establish the foundation of Korean medicine treatment after hysterectomy.

Radiation Therapy Results of Invasive Cervical Carcinoma Found After Inappropriate Hysterectomy (부적절한 수술루 침윤성 자궁경부암의 방사선치료 결과)

  • Purpose : Hysterectomy without lymph node dissection was considered an inadequate treatment method for invasive uterine cervix cancer. Usually the procedure was performed inadvertently on patients who were thought to have benign or premalignant conditions preoperatively. We analysed radiotherapy results of such patients to evaluate survival rates, failure patterns and prognostic factors according to various conditions. Materials and Methods : Sixty one patients undergoing hysterectomy in the presence of invasive cervical carcinoma were reviewed retrospectively. Preoperative diagnosis were carcinoma in situ (38 cases), severe dysplasia (2), myoma (6), uterine bleeding (4), uterine prolapse (2). and early invasive cervix cancer (10) (One patient had myoma and carcinoma in situ coincidently). Patients received postoperative megavoltage radiotherapy from August 1985 to December 1993, and minimum follow-up period was 24 months. Eight patients received ICR only, 6 patients ICR and external radiation, and 47 patients received external radiation therapy only. Results : Overall 5-year survival rate and relapse-free survival rate werer $83.8\%$, $86.9\%$ respectively. For patients with retrospective stage IA, IB, IIB (gross residual after surgery), and vaginal cuff recurrence were $90.9\%$, $88.8\%$, $38.4\%$, and $100\%$ respectively There were 8 cases of treatment failure, most of them (5/8) were in patients with gross residual disease, other patients were full thickness involvement of cervix wall (2/8) except one. Patients with early vaginal cuff recurrence and microinvasive cervical cancer (stage IA) had no treatment related failure Prognostic factors affecting survival by univariate analysis were status of residual disease, tumor histology and retrospective stage. Conclusion : Adjuvant radiotherapy appeared to be effective treatment method for patients with presumed stage IA, IB and early local recurrent disease after inadvertent hysterectomy Survivals for patients with gross disease remained after inappropriate hysterectomy was poor, So, early cancer detection and Proper management with precise pretreatment s1aging is necessary to avoid inadherent hysterectomy especially in the cases of gross residual disease.

A Study on the Change of Digital Infrared Thermographic Imaging and Skin Resistance Variability before and after Treatment of Dysmenorrhea (월경통 치료 전과 후의 적외선체열영상 및 피부저항변이도 변화에 대한 연구)

  • Purpose: The aim of this study was to examine the effect of oriental diagnosis and treatment for dysmenorrhea patients on lower abdominal temperature and Skin Resistance Variability(SRV). Methods: We studied twenty-five dysmenorrhea patients, who visited Oriental Medicine Hospital of ${\bigcirc}{\bigcirc}$ University for 10 months(Sep. 2011~Jun. 2012). Patients with organic disease such as endometriosis, uterine myoma and pelvic inflammatory disease were excluded from this study. We prescribed 3 types of treatments, herbal medicine, acupuncture, and moxibustion, for one menstrual cycle. Digital Infrared Thermographic Imaging(DITI), 7-Zone-Diagnostic System(CP-6000A), VAS(Visual analog scale) and MVRS(Multidimensional verbal rating scale) were conducted before and after the treatment. The result of DITI, 7-Zone-Diagnostic System, VAS and MVRS before and after treatment were compared in order to objectively assess the therapeutic effect. Results: When we compared the results of DITI, CV4 temperature after treatment was increased compared with that of before, and temperature difference between CV12 and CV4 after treatment was decreased compared with that of before. But there was no significant difference. When we compared the results of 7-Zone-Diagnostic System at 1st and 2nd trial, the height of 1~5 area was lower than that of before. And at 1st and 2nd trial, the height of 6, 7 area was higher than that of before, but there were no significant difference in most. When we compared the results of VAS and MVRS, VAS and MVRS after treatment was significantly decreased compared with that of before(p<0.01). Conclusions: This study shows that oriental diagnosis and treatment for dysmenorrhea patients affects the results of DITI. Further study will be needed.

  • Objectives: To observe the effects on liver and kidney of gynecology patients' during long term Korean Medicine. Methods: This research was conducted as a panel study on 29 female gynecology patients at Korean medicine clinic and compares their conditions before, 3 months after, and 6 months after Intake of Korean medicine. This analysis was conducted to frequency(%), repeated measure ANOVA and significantly statistics was p<0.05, confidence interval. Results: The most common conditions among the subjects were polycystic ovary syndrome, endometriosis, adenomyosis, and uterine myoma. Except in a few instances, liver functions (AST, ALT, γ-GTP) were within the normal range before and after the intake of Korean medicine. Further, while there was a slight change in liver functions when three months and six months after the intake, but there was no statistical significance (p>0.05). As for the kidney functions (BUN, Creatinine, BUN/Creatinine), they were within the normal range before and after the intake of Korean medicine. Further, while there was a slight change in kidney functions three months and six months after the intake, there was no statistical significance(p>0.05) either. Conclusions: Based on these results, it can be concluded that Korean medicine does not have a negative impact on the gynecology patients' liver and kidney.
