Evaluation of the Accuracy and Precision Three-Dimensional Stereotactic Breast Biopsy (3차원 입체정위 유방생검술의 정확도 및 정밀도 평가)

  • This research was study the accuracy of three-dimensional stereotactic breast biopsy, using a core Needle Biopsy and to assess the accuracy of Stereotactic biopsy and Sono guided biopsy. Using Stereotactic QC phantom to measure the accuracy of the 3D sterotactic machine. CT Scan and equipment obtained in the measured X, Y, Z and compares the accuracy of the length. Using Agar power phantom compare the accuracy of the 3D sterotactic machine and 2D ultrasound machine. Z axis measured by the equipment to compare the accuracy and reliability. Check the accuracy by using visual inspection and Specimen Medical application phantom. The accuracy of the 3D sterotactic machine measured by Stereotactic QC phantom was 100%. Accuracy as compared to CT, all of X, Y, Z axis is p > 0.05. The accuracy of the two devices was 100% as measured by Agar powder phantom. There was no difference between t he t wo d evices as C T and p > 0.05. 3D sterotactic machine of the ICC was 0.954, 2D ultrasound machine was 0.785. 2D ultrasound machine was different according to the inspector. Medical application phantom experiments in 3D sterotactic machine could not find the Sliced boneless ham. 2D ultrasound machine has not been able to find a small chalk powder group. The reproducibility of the three-dimensional stereotactic breast biopsy was better than effect of Sono guided biopsy.

  • The egg white phantom is a thermal lesion visualization phantom able to illustrate a thermal lesion. It is often used to evaluate the performance of HIFU and is less expensive than the BSA phantom. This study determined the optimal phantom composition for evaluated therapeutic ultrasound machines by varying the egg white concentration in the egg white phantom and demonstrated its utility as a therapeutic ultrasound phantom. The egg white phantom at varying egg white concentrations (10-40% in 10% intervals) was fabricated, and its thermal properties and acoustic properties were assessed. In addition, the size and shape of the formed lesion were compared between the egg white phantom and bovine liver tissue according to the electrical power. The results showed that 30% egg white phantom was optimal for the performance evaluation due to its thermal and acoustic properties. The generated thermal lesions formed sequentially as a cigar, ellipse, tadpole, and cone shapes according to the electrical power; a similar tendency was observed in the liver tissue. Hence, we conclude that the egg white phantom will prove useful in quantitatively evaluating the thermal effects of therapeutic ultrasound.

  • This study was purpose to assessment of the resolution characteristics by using ATS 535H Basic quality assurance (QA) phantom for ultrasound. The ultrasound equipment was used Logiq P6 (Ultrasound, GE Healthcare System, Chicago, IL, USA). And the ultrasound transducer were used Convex 4C (4~5.5 MHz), Linear 11L (10~13 MHz), Sector 3SP (3~5.5 MHz) probe. As for the noise power spectrum (NPS) comparison results by using ATS 535H Basic QA ultrasound phantom and Convex 4C, Linear 11L, Sector 3SP probe. The NPS value of the Convex 4C probe image was 0.0049, Linear 11L probe image was 0.0049, Sector 3SP probe image was 0.1422 when the frequency is 1.0 mm-1. The modulation transfer function (MTF) comparison results by using ATS 535H Basic QA ultrasound phantom and Linear 11L probe the MTF value of the 3 cm focus image was 0.7511 and 4 cm focus image was 0.9001 when the frequency is 1.0 mm-1. This study was presented characteristics of spatial resolution a quantitative evaluation methods by using ultrasound medical images for QA of ultrasound medical QA phantom. The quality control (QC) for equipment maintenance can be efficiently used in the clinic due to the quantitative evaluation of the NPS and MTF as the standard methods. It is meaningful in that it is applied mutatis mutandis and presented the results of physical resolution characteristics of the ultrasound medical image.

  • This research is to measure real-time temperature distribution inside a tissue-mimicking phantom for the safety and effectiveness evaluations of focused ultrasound (FUS) device capable of linear scanning stimulation. Since the focusing area of the FUS stimulation device is smaller than diameter of conventional thermal probe and keeps moving, it is impossible to monitor temperature distribution inside the phantom. By using the phantom with a thin film temperature sensor array inserted, real-time temperature change caused by the FUS device was measured. The translation of the measured temperature peak was also tracked successfully. The present phantom had been experimentally proven to be applicable to validate the performance and safety of the therapeutic ultrasound devices.

Technical Report: A Cost-Effective, Easily Available Tofu Model for Training Residents in Ultrasound-Guided Fine Needle Thyroid Nodule Targeting Punctures

  • Objective: To establish a cost-effective and easily available phantom for training residents in ultrasound-guided fine needle thyroid nodule targeting punctures. Materials and Methods: Tofu, drinking straws filled with coupling gel, a urine tube, and 21-gauge needles were used to generate a phantom thyroid with nodules for training. Twelve radiology residents were involved in the study. The puncture success rates were recorded and compared before and after phantom training using the Wilcoxon signed-rank test. Results: On ultrasonography, tofu mimicked the texture of the thyroid. Drinking straws filled with coupling gel mimicked vessels. The urine tube filled with air mimicked the trachea, and 21-gauge needles mimicked small nodules in the transverse section. The entire phantom was similar to the structure of the thyroid and surrounding tissues. The puncture success rates of radiology residents were significantly increased from 34.4 ± 14.2% to 66.7 ± 19.5% after training (p = 0.003). The phantom was constructed in approximately 10 minutes and materials cost less than CNY 10 (approximately $ 1.5) at a local store. Conclusion: The tofu model was cost-effective, easily attainable, and effective for training residents in ultrasound-guided fine needle thyroid nodule targeting punctures in vitro.

Tissue Mimicking Phantom for Visualization of Temperature Elevation Caused by Ultrasound (초음파에 의한 온도상승 가시화용 생체 모의매질)

  • To probe the temperature elevation effect caused by ultrasound, a tissue mimicking phantom was newly suggested. A carrageenan gel was adopted to realize not only the required transparency for visualization but also acoustic characteristics similar to human tissue. To visualize the temperature elevation inside phantom, thermochromic film with a critical temperature of discoloration was introduced. Acoustic characteristics of the tissue mimicking phantom were examined when the concentrations of carrageenan and sucrose changed. As the results, the attenuation coefficient of the phantom could be controlled in the range of 0.44~0.49 dB/cm/MHz, and the acoustic impedance in the range of 1.52~1.77 Mrayls. We could control the acoustic characteristics of the phantom by different concentration of carrageenan and sucrose, and it was possible to examine the temperature elevation caused by ultrasound in the phantom. The suggested method was verified by noninvasively visualizing the temperature elevation due to planar and focused ultrasound using the fabricated phantom.

The Use and Educational Effect of Ultrasonic Phantom in Korean Medicine Obstetrics and Gynecology Clinical Practice Education (한방부인과 임상실습교육에서 초음파 팬텀의 활용과 효과)

  • Objectives: The purpose of this study is to analyze educational effect of ultrasound examination training with ultrasonic phantom in the department of Korean Medicine Obstetrics and Gynecology. Methods: All 4th grade students in 2021 and 2022 of school of Korean Medicine, Dong-Shin University must be trained in the department of Korean Medicine Obstetrics and Gynecology according to ultrasound examination training guideline including Objective Structured Clinical Examination (OSCE). After completing ultrasound examination training, we distributed questionnaires to them about a confidence before OSCE and after OSCE, difficulty in using ultrasound, ultrasound reading, difficulty with probe manipulation and computer operation, ultrasound understanding, clinical usefulness. And then, we analyzed the related factors including descriptive statistics, frequency analysis, student's t-test and paired t-test by SPSS 12.0. Results: Confidence in using ultrasound showed differences among students by gender and year, but it was not statistically significant. The difficulty of ultrasound use and ultrasound reading was at a moderate level, and the gender difference was not significant, and the difference between the practical students by year was statistically significant. The difficulty of the operation of the ultrasonic probe was at a normal level, and the difference between the students in practice by gender and year was not statistically significant. Although they answered that they had a very good understanding of the use of ultrasound, there was a gender difference and it was statistically significant, and practical students by year was not statistically significant. They answered that the effect on clinical use was very sufficient, but it was not statistically significant. The appropriateness of the practice time was evaluated at a moderate level. Conclusions: The use and educational effect of ultrasound examination using ultrasonic phantom in the department of Korean Medicine Obstetrics and Gynecology clinical practice education was very effective.

Manufacture and Image Characteristic Changes Observation by Temperature of Ultrasound Tissue Mimicking Phantom (초음파 Tissue Mimicking 팬텀의 제작과 온도 변화에 따른 영상 특성 변화 관찰)

  • The purpose of this study is that in measures the acoustic propagate characteristics and temperature sensitivity of ultrasound tissue mimicking phantom(TM phantom). TM phantom manufacture according to the International Electronical Committee(IEC) guidelines for acoustic propagate characteristics of soft tissue. TM phantom was observed to have the image brightness and the image depth penetration decreases changes convergence which was the subject of ultrasound image characteristics in accordance with an external temperature that the change is reduced in temperature below $22^{\circ}C$. This study provide a basis t o create another TM Phantom and TM Phantom has been determined that it is appropriate for use in more than $22^{\circ}C$.

Ultrasound Phantoms to Protect Patients from Novices

  • With the growing use of ultrasound for pain management, we are interested in how to teach and practice ultrasound-guided procedures. Ethically, we should not insert a needle in a patient until after much practice on a phantom. Several types of phantoms have been introduced for ultrasound training, including water, agar/gelatin, elastomeric rubber, and meat phantoms and cadavers. The ideal phantom is similar to human tissue, is readily available and inexpensive, can be used repeatedly, provides tactile feedback, will hold a needle in place, does not generate needle tracks, and is not a health hazard. Several studies have shown the effectiveness of phantoms for improving the proficiency of novices. We hope that the application of phantoms in education leads to improved proficiency and increased patient safety.

Development of Human-Head-Mimicking Phantom for Brain Treatment Using Focused Ultrasound (집속 초음파 뇌 질환 치료를 위한 두부 유사 팬텀의 개발)

  • In this study, human head-mimicking phantom was developed for brain disease treatment study using focused ultrasound. Acoustic parameters of skin, skull and brain were investigated through literature investigation and adequate substitutes according to each tissue were suggested. In the case of skin phantom, construction ratio of glycerol-based TMM phantom was controlled to mimic real skin. The suitability of skull substitutes was evaluated through measurement of acoustic parameters. In the case of brain phantom, transparent egg white phantom was used to observe thermal properties of focused ultrasound. Combined human-head-mimicking phantom using each substitutes was fabricated for development of brain disease treatment protocol. Denaturation of brain phantom according to ultrasonic condition was observed for validation.
