Properties of a Surface Curvature in Toric Lens (토릭렌즈의 표면 곡률 특성 연구)

  • We obtained the sum of two curvature ($C_x+C_y$) in toric lens which two toroidal surface is the right angle each other. $$C_x+C_y=\frac{x^2+y^2}{2r_1}+\frac{x^2}{2}(\frac{1}{r_2}-\frac{1}{r_1})$$ and the sum of two curvature ($C_a+C_b$) in toric lens about the cross angle. $$(C_a+C_b)=\frac{x^2cos^2{\alpha}_1}{2r_1}+\frac{x^2cos^2{\alpha}_2}{2r_2}+\frac{y^2sin^2{\alpha}_1}{2r_1}+\frac{y^2sin^2{\alpha}_2}{2r_2}$$ and claculated the parameter S, C, ${\theta}$ of a combination power in toric lens of the cross angle including surface curvature ($C_x$, $C_y$) values. $$S=(n-1)\[\frac{C_x}{x^2}+\frac{C_y}{y^2}\]-\frac{C}{2},\;C=-\frac{2(n-1)}{sin2{\theta}}\[\frac{C_x}{x^2}+\frac{C_y}{y^2}\]$$ $${\theta}=\frac{1}{2}tan^{-1}\[-\frac{{C_xy^2sin2{\theta}_1}+{C_yx^2sin2{\theta}_2}}{{C_xy^2cos2{\theta}_1}+{C_yx^2cos2{\theta}_2}}\]$$.

Accuracy of Astigmatic Correction Using Toric Intraocular Lens by Position and Size of Corneal Incision (각막절개 위치와 크기에 따른 난시교정인공수정체의 난시교정의 정확성)

  • Purpose: To assess the accuracy of toric intraocular lens (IOL) implantation by the location and size of the corneal incision. Methods: We retrospectively reviewed the medical records of 98 patients (98 eyes) who underwent phacoemulsification with toric IOL implantation from January 2014 to March 2017. The patients were divided into two groups: group 1 got an incision of the superior side of the cornea (n = 54) and group 2 received an incision on the temporal side of the eye (n = 44). For both groups, incisions were made at their steep corneal astigmatism axises. Each group was further divided into subgroups for whom different sized blades were employed (2.75 vs. 2.2 mm widths). We measured the refractive index and autokeratometric parameters. We postoperatively assessed residual astigmatism and any reduction thereof. Results: In both groups, uncorrected and best-corrected visual acuity, refraction cylinder astigmatism, and autokeratometric astigmatism improved statistically. Between two groups, corneal astigmatism decrease was not significant. Residual astigmatism also showed no significant differences between the two. Patients in both groups treated using 2.75 mm wide blades exhibited greater increases in corneal astigmatism. Conclusions: During cataract surgery, precise correction of astigmatism via toric IOL implantation is possible when surgically induced astigmatism is minimized by careful choice of the location and size of the corneal incision.

A Correlation between Axis-Rotation and Corneal Eccentricity in Toric Soft Contact Lens Fitting in With-the-rule Astigmatism (직난시안에서의 토릭소프트렌즈 피팅 시 축 회전과 각막이심률과의 상관관계)

  • Purpose: A correlation between the rotating direction and rotation amount according to the eccentricity was analyzed in the present study when fitting the toric soft contact lens. Methods: One hundred fourteen eyes of with-the-rule astigmatism in 20s and 30s were appropriately applied toric soft contact lens by the guideline of manufacturer and fitting evaluations and analyzed the rotating direction and rotation amount when fixating at primary position and 8 different directions of gaze. The speed of reorientation and the correlation between the rotation amount and cornea eccentricity were also analyzed when rotating $45^{\circ}$ each toward the temporal and nasal direction. Results: The cornea eccentricity and rotation amount was shown positive correlation. For the gaze of direction, it was identified that the gaze of directions other than the gaze of direction towards the nasal and temporal had correlation, and among them, the gaze of directions to superior and the superior-temporal showed the tendency of decreasing rotation amount as the cornea eccentricity increased. On the contrary, the gaze of direction towards the inferior, inferior-temporal, superior-nasal, and inferior-nasal showed tendency of increasing rotation amount as the cornea eccentricity increased thus, it was identified that the cornea eccentricity showed different correlation according to the gaze of direction. For the case of re-orientational speed, the speed of lens reorientation in the group having the smallest corneal eccentricity was fastest in both direction but was not significantly different. Conclusions: Considering cornea eccentricity to current method of prescribing toric soft contact lens which is based on total astigmatism, it will be helpful to establish the optimal axis stabilization.

Change of Corneal Shape with Soft Contact Lens Type (소프트콘택트렌즈의 유형에 따른 각막형태의 변화)

  • Purpose: This study was to investigate changes in the thickness of cornea, curvature of cornea, and aberration depending on the water contents, materials, and refractive power of contact lens. Methods: The differences in the corneal thickness between pre- and post-wearing the lenses were compared using 5 kinds of lenses. The changes in the corneal thickness, the curvatures of the anterior and posterior cornea, and high order aberration (HOA) before and after wearing the lenses were investigated at the center of the cornea, and the different distance and the direction away from the center of the cornea. For the equipments of measurement, ORB ScanII (Bausch & Lomb Inc, ver 3.14) was used to measure the corneal topography and thickness, and Zywave (Bausch & Lomb Inc, ver 5.20) was used to analyze the high order aberration. Results: Five (S1, S2, S3, T1, T2) of the lens was used for this study, excluding the lens T2 lens has four lenses and the thickness of the corneal shape, but the impact is minimal. In the case of the hydrogel soft contact lenses (T2 lens) with low oxygen permeability, the corneal thickness showed distinct increasing patterns. The high order aberration and coma aberration were most changed in the silicon hydrogel toric lens, while the depth of anterior was most changed in the hydrogel toric lens. Conclusion: Among the 5 kinds of contact lenses with different water contents, materials, and refractive power used for this study, the corneal shape change was small for the lenses with an oxygen permeability (Dk) of more than 28, and the largest for the lenses with a very low oxygen permeability.

  • To manufacture the lens mold used in producing polycarbonate (PC) lenses, the core manufacturing is needed and this core manufacturing is generally performed by diamond turning machine (DTM) or computer numerical control (CNC) lathe. The numerical data about the lens core feature is necessarily needed for operating of these devices. Therefore, we developed the program which calculate the numerical data about the lens core feature. The program was composed to be able to input aspherical coefficients of lens feature, display the graph of lens feature, and save the numerical data file.

Clinical Evaluation of Prism-stabilized Toric Soft Contact Lens Fit (프리즘 안정화 디자인 토릭 소프트 콘택트렌즈 피팅에 대한 임상 평가)

  • Purpose: This study was to evaluate the clinical performance of 1-Day disposable toric soft contact lenses (TSCL) and to compare the visual performance with glasses and TSCL. Methods: In a two-week investigation of 'prism ballast' designed TSCL, occular refractions in 15 (30 eyes) healthy subjects were within the range of -2.75 D TO -8.00 D spherical power and -0.75 DC to -2.25 DC cylinderical power (with-the-rule). Subjects were assessed the clinical performance (orientation, centration, movement, tightness, rotation recovery, symptoms) of TSCL and compared binocular vision and contrast sensitivity with corrected glasses and TSCL at initial and two-week follow-up visits. Results: The prism-ballast design TSCL achieved better orientation, rotation recovery and symptoms over 2 week period. There was no statistically significant difference between spectacles and TSCL with respect to measuring binocular vision and contrast sensitivity. Conclusions: The clinical performance was good with prism-ballast design toric soft contact lenses. Evaluation of binocular vision and contrast sensitivity did not show the differences between spectacles and TSCL correction. This study suggests that TSCL wearers can be achieved similar visual performance for spectacles wearers.

Contact Lens Prescribing Pattern in Korean During 2010 to 2013 (대한민국의 최근 4년간(2010-2013) 콘택트렌즈 처방 추세)

  • Purpose: To understand the pattern of prescribing contact lenses in Korea from 2010 to 2013. Methods: Four hundred survey questionnaires were randomly posted to Korean Optometric clinics for 4 years. The questionnaire was the same form as a form used for International Contact Lens Prescribing Trend. It was also designed to collect information about the contact lenses prescribed to the first 10 patients after its receipt. Results: 1937 fits were used for the analysis among the four hundred survey questionnaires. The average patient age was $26.2{\pm}7.0$ in 2010, $26.1{\pm}7.6$ in 2011, $24.7{\pm}6.6$ in 2012 and $24.8{\pm}7.2$ in 2013. The patients were mainly women (75%). Of all the soft contact lenses, silicon hydrogel lenses were 18% of fits (17% in 2010, 10% in 2011, 20% in 2012, 24% in 2013). By degine, the percentage of toric design lenses accounted for 17% of soft lenses fits and had increased during 4 years (10% in 2010, 18% in 2011, 16% in 2012, 22% in 2013), and cosmetic contact lens made up for 29% (15% in 2010, 20% in 2011, 40% in 2012, 41% in 2013). Conclusions: The contact lenses wearers were mostly women and the average patient age becomes slightly younger. In relation to the increase in the mumber of younger female contact lens wearers,the percentage of cosmetic lenses prescription was much higher (29%) than the global average (7%), and it has gradually increased for last 4 years. This trend could be related to the growth of beauty market in Korea. By emphasizing on toric lens marketing in manufactures, the prescription rate has been gradually increased.

  • Ftheta Lens, whose image height is proportional to its field view angle, is one of the most important parts in Laser Scanning Unit(LSU). In this paper $f{\theta}$ lens design, mold production and modification method of lens design and mold are introduced. Lens design was carried out with Zemax and Special Toric surfaces were applied for lens surfaces to minimize distortion both in main and sub scanning directions. And a high precision machine with 1nm resolution was used to fabricate lens mold cores. After injection the lens was evaluated and the difference from design was examined. This difference was compensated by modifying lens design and new lens mold cores were made according to modified lens design to obtain the quality of original design.

Usefulness of Rotation for Toric Soft Lenses Using Objective Refraction (타각적 굴절검사를 이용한 토릭 소프트 렌즈 회전 평가의 유용성)

  • Purpose: The clinical usefulness of rotation evaluation using objective refraction in toric soft lenses fitting was investigated. Methods: Toric soft lenses were fitted for 32 subjects (64 eyes; mean age of 24.69 ${\pm}$ 1.65 years) with astigmatism and both eyes of each subject were fitted with toric soft lenses. Objective refraction-based lenses rotation was evaluated from refraction and over-refraction data by indirect calculating technique. These calculated data were compared with the measured data from slit lamp with direct measuring technique. Results: Orientation of toric soft lenses around zero position (within ${\pm}$ 5$^{\circ}$ vertical line) was investigated. The orientations to the direction of nose of measured and calculated values were 69.78% and 63.64%, respectively, which showed similar values between two techniques. Agreement frequency between measured and calculated values in the magnitude of lenses rotation 54.69% and 82.82% for 10$^{\circ}$ and 20$^{\circ}$ of vertical line, respectively. The 95% limits of agreement between calculation and measurement were from -10.08$^{\circ}$ to 12.65$^{\circ}$, and mean difference was 1.29$^{\circ}$ within ${\pm}$ 10$^{\circ}$. The result showed there was no significant difference (p = 0.1984) and high correlation (r = 0.56, p = 0.0004) between two techniques. But the 95% limits of agreement was widen in ${\pm}$ 20$^{\circ}$ of vertical line. The magnitude of lens rotation between two methods was 9.66 ${\pm}$ 6.16$^{\circ}$, 16.17 ${\pm}$ 12.38$^{\circ}$ and 10.58 ${\pm}$ 12.02$^{\circ}$ for normal, loose and tight fitted conditions. Conclusions: From the results with smaller difference between two techniques, it was found that higher availability of subjective over-refraction data can be used as a supplementary tool for subjective refraction. An application using objective refraction with direct measuring could be provide high success in prescription on toric soft lenses.

Generation of Lens surface by moving mask lithography (가변 속도 이동식 마스크를 이용한 렌즈 곡면 형성)

  • We propose a fabrication method for refractive lens by variable velocity moving mask lithography and slit pattern. Distribution of exposure dose should be controlled for the curved photoresist surface that works as a refractive surface. We analyze theoretically the distribution of exposure dose by change of moving velocity, moving direction of mask and the shape of mask pattern, and confirm for the curved surface experimentally. The lens could have sag height of a few of hundreds ${\mu}m$, by using thick photoresist or Deep RIE process.
