Combination Therapy with Ciclosporine, Ketoconazole and 0.1% Tacrolimus Ointment for Fast Reduction of Perianal Fistulae Lesions in Three Military Working Dogs (싸이클로스포린, 케토코나졸, 0.1% 타크로리무스 연고제제로 병용 치료한 군 사역견의 항문주위 누공 3례)

  • Three military working dogs (MWDs) were diagnosed with perianal fistulae after a complete physical examination and rectal palpation. The initial treatment protocol was used with short-term oral ciclosporine (CyA) and ketoconazole and 0.1% topical tacrolimus ointment. When lesions were nearly healed, treatment was continued with 0.1% tacrolimus ointment only at a lower frequency. The lesions improved significantly within 2-8 weeks with no noticeable side effects. Complete recovery was determined at weeks 13, 6, and 4 for cases 1, 2, and 3, respectively. This combination protocol was effective at resolving or reducing perianal fistulae (PAF) lesions even communicating with anal sacs in three dogs. The major benefit of this protocol was the rapid reduction of fistulae lesions and this could be helpful to extend the pain-free period. Moreover, this protocol appeared cost-saving as well as time-saving option for PAF dogs.

A Study on the Enamel Surface Texture and Caries Susceptibility in Interdentally Stripped Teeth (치간삭제 후의 법랑질 표면조도와 치아우식 감수성에 관한 연구)

  • To investigate the difference of Texture exhibited on interproximal enamel surface with each different stripping method and the susceptibility of proximal enamel to demineralization after stripping and the application of a topical fluoride go] and sealant, one hundred human premolars, which were Previously extracted for orthodontic reasons were evaluated by means of Scanning electron microscopy and laser fluorescence. The results were as follows : 1. No matter what the initial stripping instrument was the furrows that resulted from all the stripping methods were not completely removed by careful polishing. 2. Among the enamel surfaces that were treated with three different initial abrasive instruments, followed by the same polishing method (Sof-$Lex^{(r)}$ disks), the enamel surfaces that were treated with 700 crosscut carbide bur showed the smoothest surfaces. 3. The stripped teeth, no matter what the initial stripping instrument was, were less resistant to initial demineralization than untreated teeth. But no difference in caries susceptibility according to differently stripped methods was found (p<(0.001). 4. Teeth treated with APF-gel or sealant were mote resistant to demineralization than those treated without other treatment after stripping (p<0.001). 5. Comparing groups treated with APF-gel to groups treated with sealant, the former was more resistant to demineralization than the tatter (p<0.05). In conclusion, enamel surfaces that were stripped jnterproximally were less resistant to demineralization even though various attempts were made to produce smooth, self-cleaning enamel surfaces. Therefore, additional treatment-sealant or calcifying/ fluoridating solution To the stripped enamel surfaces is recommended.

Synergistic Effect of Notopterygium incisum with Clematis manshurica in the Anti-inflammatory Activity (강활(羌活)과 위령선(威靈仙)의 항염증 상승작용에 관한 연구)

  • Objective : Oriental medicines have been combined oriental medical theory which based on the seven modes of emotions. Notopterygium incisum (N. incisum) and Clematis manshurica (C. manshurica) have been used as an anti-rheumatic and analgesic medicine for the treatment of rheumatism, headache, cold, etc. In this study, we evaluate the synergistic anti-inflammatory effect of N. incisum and C. manshurica. Method : To evaluate the synergistic anti-inflammatory effect of a herbal mixture N. incisum and C. manshurica, we examined the changed ear thickness in 12-O-tetradecanoyl-phorbol-13-acetate (TPA)-induced mouse ear edema model after topical application of herbal mixture. In addition, the levels of markers for inflammation, such as tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$, prostaglandin $G_2$ ($PGE_2$), and nitric oxide (NO) were measured by ELISA assay and Griess reagent in lipopolysaccharide-stimulated Raw 264.7 cells. Results : Our results showed that aqueous extracts of N. incisum and C. manshurica combination significantly inhibited the mouse ear edema induced by TPA. Moreover, the aqueous extracts of N. incisum and C. manshurica combination exhibited synergistic effects in down-regulating TNF-${\alpha}$, IL-$1{\beta}$, $PGE_2$ level, but not NO. Conclusions : This study suggested that combined treatment of N. incisum and C. manshurica, based on seven methods in prescription compatibility, has a synergistic effect in down-regulating inflammatory response both in vitro and in vivo models.

Effects of Persimmon leaf on the Photoaging Skin Improvement(1) (감잎의 광노화 피부 개선에 미치는 효과(1))

  • This study was performed to investigate the anti-photoaging effects of Persimmon leaf tea(PLT) in hairless mice(SKH-1) exposed to UVB irradiation. The animals were divided into non-treated group (normal, N) and UV-radiated groups. UV-radiated groups were divided into only UV-radiated group(control, C) and UV-radiated and PLT treated experimental groups[first extraction treated group(PLT-I), second extraction treated group(PLT-II), and third extraction treated group(PLT-III)]. Three PLT treated experimental groups of mice were treated with both oral administration(300 mg/Kg B.W./day) and topical application (100 ul of 2% conc./mouse/day) for 4 weeks. Anti-photoaging effects of Persimmon leaf were evaluated by anti oxidative reaction, stereomicroscopic and microscopic observations. The expression of photoaging skin related factors including mast cell tryptase, proliferating cell nuclear antigen (PCNA) and vascular endothelial growth factor (VEGF) was examined by immunohistochemical staining. Treatment of PLT-I, -II, -III prevented the wrinkle formation as well as epidermal hyperplasia, inflammatory cells, disruption of collagen in photoaged skin induced by UVB radiation. It also reduced the PCNA and VEGF expression in the UVB irradiated dorsal skin. Furthermore, it significantly decreased the number of mast cells in the UVB irradiated dermis(p<0.05 and p<0.01). On the effects of oxidative stress and antioxidant function on the treatment with water extract from Persimmon leaf tea(PLT), the activity of superoxide dismutase(SOD) was significantly increased in PLT-III group(p<0.05), and catalase(CAT) was significantly increased in PLT-I and PLT-III groups(p<0.05), and PLT-II group(p<0.001). These extracts showed relatively antioxidant activity and protective effect on UVB-induced oxidative stress in hairless mice(SKH-1). Our results suggest that Persimmon leaf tea may serve as an useful radical scavenging antioxidant and anti-photoaging skin agents in the UVB irradiated skin.

Effect of Capsaicin on Causalgiform Pain in the Rat

  • The purpose of this study is to obtain the effective concentration of capsaicin to relieve pain with no change in the number of C-fibers and its effective duration for pain relief. Capsaicin has been used extremely as a experimental tool and as topical medications for acute or chronic tissue injuries and partial nerve injury is the main cause of causalgiform pain disorders in humans. Here, the left sciatic nerve was ligated unilaterally at the high level of the thigh to prepare an animal model of this pain condition. The rat developed guarding behavior of the ipsilateral hind paw within a few hours after the operation and this behavior was maintained for several months thereafter, suggesting the possibility of spontaneous pain. These animals were divided into two groups(4-week & 8-week) and each group was subdivided into five groups by different concentration (0.05, 0.1, 0.5, 1.0 & 2.0%). Each capsaicin concentration was treated locally on the spinal cord-side of the ligated nerve and the foot withdrawal latency was measured. Thereafter, the dorsal roots of L5 were removed from both sides immediately after in tracardial perfusion for the counting of C-fibers by the histological procedure. There were no significant differences in the foot-withdrawal latency and the number of C-fibers between the left side treated with 0.05% capsaicin and the right side treated with the vehicle. However, latencies of the left sides treated with 0.1, 0.5, and 1.0% capsaicin increased significantly throughout 4-6 weeks with almost no change in the number of C-fibers, and the latencies showed the trends to approach slowly to those of the conditions after operation. The latency of subgroup treated with 2.0% increased by approximate 10% more than that of the right side throughout 8 weeks, and the number of C-fibers decreased by about 30% or more These results suggest that the elevated latency with capsaicin(0.1-1.0%) treatment is due to the inhibition of impulse transmission throughout the primary afferent fiber and the data from 2.0% are due to partial destruction of C-fibers. Therefore, capsaicin concentrations from 0.1% to 1.0% are probably very effective for the treatment of causalgiform pain with almost no destruction of C-fibers.

Enhanced Transdermal Delivery of Procaine Hydrochloride by lontophoresis -Comparison of Synergic Effect of High Voltage Current and Ultrasound- (이온도입에 의한 염산프로카인의 경피전달 증대 -고전압전류 및 초음파 병행의 상승효과 비교-)

  • The purpose of this study was to determine the effects of iontophoresis on transdermal delivery of procaine hydrochloride in healthy volunteers, as well as to the synergic effect of high voltage current or ultrasound on the efficacy of transdermal delivery of iontophoresis. Forty healthy volunteers were randomly assigned to four groups topical application group (TA), iontophoresis group (IT), pre-treatment of high voltage current stimulation with iontophoresis (HVS + IT), and pre-treatment of ultrasound application with iontophoresis (US + IT). All subjects received procaine iontophoresis on the forearm using direct current with 4 mA f3r 15 minutes. All subject was measured the duration of local anesthesia, pressure pain threshold, pain perception threshold using rectangular wave at 0.2 ms, 1 ms, 50 ms of rectangular current stimulation after procaine iontophoresis. For comparisons of the sensory characteristics and efficacy of iontophoresis between the groups, an one-way ANOVA and Kruskal-Wallis were used. The significant difference the duration of local anesthesia were found between the groups (p<0.001). The local anesthetic duration of IT, HVS+IT were significantly longer than TA. Meanwhile, the local anesthetic duration of US+IT was significantly longer than HVS+IT, IT and TA group (p<0.05). Also, the pressure pain threshold, pain perception threshold at 0.2 ms, 1 ms, 50 ms were significant difference between the groups (p<0.001). All sensory characteristics including pressure pain threshold, pain perception threshold of IT, HVS+IT was significantly increased than TA, whereas, US+1T was significantly increased HVS+1T, IT and TA (p<0.05). This study showed that the procaine iontophoresis have increase the duration of local anesthesia concomitantly pressure pain threshold and pain perception threshold of sensory nerve fibers such as $A-{\beta}$, $A-{\delta}$ and C fiber. This findings suggest that the iontophoresis enhanced the transdermal delivery of drug ions in vivo. The combination of ultrasound application and iontophoresis synergized the transdermal delivery of drug ions. It is suggests that an electric field, mechanical and heating property of ultrasound may contribute to synergic effect due to temporary changes of structure in the stratum corneum.

Sedum sarmentosum Enhances Hyaluronan Synthesis in Transformed Human Keratinocytes and Increases Water Content in Human Skin (돌나물추출물에 의한 사람 각질형성세포에서의 Hyaluronan Synthesis 촉진과 인체 피부의 보습력 증진)

  • In this study, we investigated the effects of Sedum sarmentosum extract on the expression of hyaluronan synthase (HAS) genes and hyaluronan (HA) production in HaCaT keratinocytes. We also assessed water content (electrical capacitance) and transepidermal water loss (TEWL) in human skin after topical treatment with Sedum sarmentosum extract. Sedum sarmentosum extract increased expression of HAS-2 and HAS-3 genes in HaCaT cells, when assayed by real-time reverse-transcriptase polymerase chain reactions (RT-PCR). Sedum sarmentosum extract induced HA production in HaCaT cells, when determined by enzyme-linked immunosorbent assay (ELISA). Finally, treatment of Sedum sarmentosum extract on human skin increased the skin hydration and decreased TEWL when measured using Corneometer and Tewameter. Our study suggests that Sedum sarmentosum extract should be a very useful cosmetic ingredient, as a skin moisturizer.

The Inhibition of UVA-induced Matrix Metalloproteinase-1 in Human Dermal Fibroblasts and the Improvement of Skin Elasticity by Cirsium setidens Extract (고려엉겅퀴 추출물의 사람 섬유아세포에 있어서 자외선으로 유도된 MMP-1발현 저해와 피부 탄력 개선 효과)

  • In this study, we measured the anti-oxidative activity of Cirsium setidens and investigated its effect on UVA-induced MMP-1 expression in human dermal fibroblats. And then we examined possible improvement in skin elasticity by topical treatment with fomular including Cirsium setidens extract. The ethanol extract of C. setidens showed free anion radical scavenging effect(87.47 % at 1 mg/mL) and superoxide anion radical scavenging effect(61.71 % at 1 mg/mL) in the xanthine/xanthine oxidase system, respectively. At the concentration of 100 ${\mu}g/mL$, C. setidens extract showed 95.54% inhibition on lipid peroxidation of linoleic acid. UVA-induced MMP-1 expression in human dermal fibroblasts was reduced to 54.69 % by treatment with 100 ${\mu}g/mL$ of C. setidens extract. A human clinical study, in which oil-in-water emulsion with C. setidens extract was topically applied, showed significant increase in skin elasticity. These results suggest that the C. setidens extract can be effective anti-aging ingredient for cosmetics applications.

Hand-Foot syndrome induced by sorafenib, a multitargeted tyrosine kinase inhibitor, in a patient with advanced renal cell carcinoma (진행된 신세포암 환아에게 sorafenib 투여시 발생한 Hand-Foot syndrome 1예)

  • Renal cell carcinoma (RCC) arising from epithelial cells of the renal tubules is a highly aggressive and malignant tumor in all ages; however, it rarely occurs in children. the standard treatment for RCC is radical nephrectomy with lymph node dissection when the tumor is localized and can be completely resected. Adjuvant chemotherapy, radiotherapy, and immunotherapy are used for pediatric patients with advanced RCC involving lymph nodes or metastatic lesions. Sorafenib is an oral, multikinase inhibitor that has recently been approved for use in metastatic RCC. Common toxicities that have been reported include dermatologic changes such as rash or desquamation and hand-foot skin reaction, diarrhea, fatigue, alopecia, and hypertension. In particular, hand-foot syndrome (HFS) an erythematous skin lesion of the palms and solesis most often caused by cytostatic chemotherapeutic agents. In this report, we have studied a 14-year-old female patient with hand-foot syndrome that occurred in association with sorafenib for the treatment of metastatic RCC. Furthermore, this case demonstrates that reversal of complications can be achieved by discontinuing the drug and intervention with topical steroids, vitamin E, and high-dose pyridoxine.

Evaluation of Stability in the Purified Wood Vinegar and Its Hair Growth Effect (목초액의 안정성 및 모발 성장 촉진 효과)

  • Generally, it takes a long time to purify wood vinegar, and it contains toxic compounds such as tar, methanol, phenol and benzopyrene. To reduce the toxicity of wood vinegar itself, we have developed a new purification method of wood vinegar using an oxidation-cohesion reaction and distillation with an active carbon. We have investigated the physico-chemical change (pH, specific gravity, refractive index and dissolved tar), the change of amount of toxic compounds (carbonyl group, phenol, benzopyrene and residual solvents) and organic acids (formic acid (FA), acetic acid (AA), propionic acid (PA)) of the purified wood vinegar under the long term and accelerated storage conditions. Also, we have evaluated the effect of the purified wood vinegar on hair growth using an alopecia model of C57BL/6 mice. As a result, we could find out that the purified wood vinegar was stable and remained without decay under the storage conditions and benzopyrene, a carcinogenic agent, was not detected in the purified wood vinegar. After topical treatment of the purified wood vinegar solution or minoxidil (MXD) for 2 weeks to dorsal skin, the hair regrowth of the mice accelerated faster than that of the control, with no clinical signs. In conclusion, we could suggest a guideline for quality control of process to reduce the toxic compounds in wood vinegar and it might be a useful hair growth promoter in the treatment of baldness or alopecia.
