Correction of Dose Distribution at Total Body Irradiation using Compensator

  • The using of compensator is required to adjust the irregular dose distribution due to irregular thickness of the body in Total Body Irradiation. Aluminuim, copper or lead is generally used as compensator. In our study, we would like to introduce a result of the attenuation and compensation effect of radiation use compensator made by duralumin and its clinical use. The thickness of compensator was calculated by the attenustion of radiation, which was measured by polystyrene phantom and ionization chamber(farmer). The compensation effect of radiation was measured by diode detector. All of conditions were set as in real treatment, and the distanc from source to detector was 446 cm. We also made fixation of device to easily attach the compensator to LINAC. Beam spoiler was menufactured and placed on the patient to irradiate sufficient dose to the skin. diode detector were placed on head, neck, chest, umbilicus. pelvis and knee with each their entranced exit points, and datas of dose distribution were evaluated and compared in each points for eleven patients(Feb. 96-Feb. 97). The attenuation rate of irradiation by duralumin compensator was measured as $1.4\%$ in 2mm thickness. The mean attenuation rate was $1.3\%$ per 2mm as increasing the thickness gradually to 50 mm. By using duralunim compensator, dose distribution in each points of body was measured with ${\pm}2.8\%$ by diode detectior. We could easily calculate the thickness of compensator by measuring the attenuation rate of radiation, remarkably reduce the irragularity of dose distribution duo to the thickness of body and magnify the effect of radiation therapy.

  • 우리나라에서 수행된 국책연구개발기술과 관련된 기존 연구는 주로 기술개발에 초점이 맞추어진 관계로, 개발기술의 상용화는 연구개발 결과평가요인의 하나로서 다루어졌을 뿐, 그 자체를 효율화하기 위한 체계적인 연구는 없었다고 해도 과언이 아니다. 이러한 결과로 실제 국책연구개발의 기술적인 이전이 부족하고, 여러 분야에로의 2차 확산을 통한 상용화에 대한 성공률이 상대적으로 낮은 수준에 머무르고 있다. 또한 국내에서 출원된 특허를 포함한 산업재산권의 휴면 율이 55%-65% 수준에 이르고 있다. 이와 같이 연구개발기술의 산업계로의 확산이 부족한 관계로 국제적인 기술경쟁력의 제고가 어려우며, 기술발전과 산업발전이 상호 작용하는 선 순환적 구조를 이루지 못하고 있다. 따라서 본고에서는 기술집약형 중소기업 또는 기술벤처기업을 통한 정보통신산업분야의 정부지원 연구개발 기술의 상업화를 촉진하기 위한 효율적 방안의 도출을 위하여, 일차적으로 연구개발 기술의 상용화프로세스 모형을 도출하고, 다음으로 도출된 모형의 단계별 특성에 대한 기술개발자, 기술사용기업, 지원기관간의 중요도 차이를 분석하고자 한다. 이를 위하여 기존의 기술상용화 관련문헌 분석과, 우리나라의 정보통신 분야의 상용화 특성분석을 통하여, 우리나라 기술개발특성에 맞는 6 단계의 상용화프로세스 모형, 즉 1) 연구개발 계획 및 제안서 작성, 2)연구개발의 수행, 3)개발기술의 인큐베이팅과 시작품제작, 4)테스팅, 엔지니어링 및 시험생산, 5) 공장입지 결정 및 대량생산, 6) 시장판매, 마케팅 및 확산 등을 도출하였다. 아울러 동 모형의 단계별 특성을 설문지로 작성하여, 기술사용기업(벤처기업 및 기술집약형 중소기업), 기술개발자(국책연구소, 대학, 및 일반기업), 관련기관(정부, TBI운영기관, 정책 및 지원기관)을 대상으로 설문조사를 하였다. 회수 및 사용된 142개 설문지 분석을 통하여, 일차적으로 정보통신 국책개발기술의 상용화에 관한 변수들의 중요도 순위로 본 중요도 실태를 분석하였으며, 나아가 이들 변수에 대하여 앞에서 제시한 기술사용기업-기술개발자-관련기관 즉 세 집단 간의 차이분석 즉 ANOVA 분석을 하였다. 이러한 분석을 통하여 우리나라 정보통신 국책연구개발기술의 상용화와 관련된 특성을 도출하는 동시에, 이를 촉진하기 위한 인프라조성과 관련한 중요한 특성 및 지원정책 대안을 도출하였다.

Effect of Motor Training on Hippocampus after Diffuse Axonal Injury in the Rats (운동훈련이 미만성 축삭손상을 일으킨 흰쥐의 해마에 미치는 영향)

  • Diffuse axonal injury(DAI) is a common form of traumatic brain injury and thought to be a major contributor to cognitive dysfunction. Physical activity has been shown to beneficial effects on physical health and influences in hippocampus which is an important location for memory and learning. The purpose of this study was to investigate the effect of motor training on motor performance and axonal regeneration in hippocampus through the immunoreactivity of GAP-43 after diffuse axonal injury in the rats. The experimental groups were applied motor training(beam-walking, rotarod, and Morris water maze) but control groups were not. The time performing the motor tasks and GAP-43 immunohistochemistry were used for the result of axonal recovery. There were meaningful differences between experimental groups and control groups on motor performance and GAP-43 immunohistochemistry. The control groups showed increasing tendency with the lapse of time, but experimental groups showed higher. Therefore, Motor training after DAI improve motor outcomes which are associated with dynamically altered immunoreactivity of GAP-43 in axonal injury regions, particularly hippocampus, and that is related with axonal regeneration.

A Study on the Optimum Composition of Rose Bengal Reagent using Phase Transfer Catalyst (상이동 촉매 기법(phase transfer catalyst)을 이용한 rose bengal 시약의 최적 조성에 관한 연구)

  • The rose bengal reagent, based on the phase transfer catalysis technique, is a calcium-targeting reagent that forms an insoluble salt that does not dissolve in a neutral or basic solution. It is expected to be effective in developing a latent fingerprint in a wet sample. However, many previous studies did not observe the fluorescence of the developed fingerprints, nor were the proposed methods of producing the reagents the optimal develop conditions. The aim of this study was to investigate the optimum reagent composition of rose bengal by varying the concentration of rose bengal dye and phase transfer catalyst based on maximum emission fluorescence of rose bengal. As a result, it was confirmed that rose bengal and surfactant concentration were the most effective when 0.01M: 0.008M, respectively.

Is Routine Repeated Head CT Necessary for All Pediatric Traumatic Brain Injury?

  • Objective : Repeated computed tomography (CT) follow up for traumatic brain injury (TBI) patients is often performed. But there is debate the indication for repeated CT scans, especially in pediatric patients. Purpose of our study is to find risk factors of progression on repeated CT and delayed surgical intervention based on the repeated head CT. Methods : Between March, 2007 and December, 2013, 269 pediatric patients (age 0-18 years) had admitted to our hospital for head trauma. Patients were classified into 8 subgroups according to mechanisms of injury. Types, amount of hemorrhage and amount changes on repeated CT were analyzed as well as initial Glasgow Coma Scale (GCS) scores. Results : Within our cohort of 269 patients, 174 patients received repeat CT. There were progression in the amount of hemorrhage in 48 (27.6%) patients. Among various hemorrhage types, epidural hemorrhage (EDH) more than 10 cc measured in initial CT was found to be at risk of delayed surgical intervention significantly after routine repeated CT with or without neurological deterioration than other types of hemorrhage. Based on initial GCS, severe head trauma group (GCS 3-8) was at risk of delayed surgical intervention after routine repeated CT without change of clinical neurologic status. Conclusion : We suggest that the patients with EDH more than 10 cc or GCS below 9 should receive repeated head CT even though absence of significant clinical deterioration.

Effects of Gastrodiae Elata Pharmacopuncture at GB20 on Motor Control and Cognitive Function in Mild TBI Rats (중등도 외상성 뇌손상 흰쥐에서 천마약침(天麻藥鍼)이 운동조정 및 인지 기능회복에 미치는 효과)

  • This study was designed to investigate the effects of Gastrodiae Elata Pharmacopuncure at GB20 on motor control and cognitive dysfunction recovery after mild traumatic brain injury in rats. Rats were divided into three groups; (1) no treatment after traumatic brain injury(experiment I), (2) Treatment with NPA after traumatic brain injury(experiment II), (3) Treatment with GEP after traumatic brain injury(experiment III). In our study, we carried out behavioral test(Rotarod, Morris water maze) and immunohistochemistry study of the change BDNF in the hippocampus(pre, $7^{th}$, $14^{th}$ day). In Rotarod test(motor control function) was significantly increased in the experimental group III as compared with experimental group I, II on $7^{th}$(p<0.01) and $14^{th}$ day(p<0.001). In Morris water maze test(cognitive function) was significantly decreased in the experimental group III as compared with experimental group I, II on $14^{th}$ day(p<0.001). In immunohistochemistric response of BDNF in the hippocampus, the experimental group III was more immune response than the other groups on $14^{th}$ day. These results imply that Gastrodiae Elata Pharmacopuncure at GB20 can play a role in facilitating recovery of motor control and cognitive function after mild traumatic brain injury in rats.

Quantifying the seismic resilience of two tall buildings designed using Chinese and US Codes

  • With ongoing development of earthquake engineering research and the lessons learnt from a series of strong earthquakes, the seismic design concept of "resilience" has received much attention. Resilience describes the capability of a structure or a city to recover rapidly after earthquakes or other disasters. As one of the main features of urban constructions, tall buildings have greater impact on the sustainability and resilience of major cities. Therefore, it is important and timely to quantify their seismic resilience. In this work, a quantitative comparison of the seismic resilience of two tall buildings designed according to the Chinese and US seismic design codes was conducted. The prototype building, originally designed according to the US code as part of the Tall Building Initiative (TBI) Project, was redesigned in this work according to the Chinese codes under the same design conditions. Two refined nonlinear finite element (FE) models were established for both cases and their seismic responses were evaluated at different earthquake intensities, including the service level earthquake (SLE), the design-based earthquake (DBE) and the maximum considered earthquake (MCE). In addition, the collapse fragility functions of these two building models were established through incremental dynamic analysis (IDA). Based on the numerical results, the seismic resilience of both models was quantified and compared using the new-generation seismic performance assessment method proposed by FEMA P-58. The outcomes of this study indicate that the seismic resilience of the building according to the Chinese design is slightly better than that according to the US design. The conclusions drawn from this research are expected to guide further in-depth studies on improving the seismic resilience of tall buildings.

Allogeneic Hemopietic Stem Cell Transplants for the Treatment of B Cell Acute Lymphocytic Leukemia

  • Objective: Explore the feasibility of allo-hemopietic stem cell transplants in treating patients with B cell acute lymphocytic leukemia. Methods: Between september 2006 and February 2011, fifteen patients with B cell acute lymphocytic leukemia (ALL) were treated by allo-hemopietic stem cell transplants (HSCT). Stem cell sources were peripheral blood. Six patients were conditioned by busulfan (BU) and cyclophosphamide (CY) and nine patients were conditioned with TBI and cyclophosphamide (CY). Graft versus host disease (GVHD) prophylaxis regimen consisted of cyclosporine A (CSA), methotrex ate (MTX) and mycophenolatemofetil (MMF). Results: Patients received a median of $7.98{\times}10^8{\cdot}kg^{-1}$ ($5.36-12.30{\times}10^8{\cdot}kg^{-1}$) mononuclear cells (MNC). The median time of ANC> $0.5{\times}10^9/L$ was day 12 (10-15), and PLT> $20.0{\times}10^9/L$ was day 13 (11-16). Extensive acute GVHD occurred in 6 (40.0%) patients, and extensive chronic GVHD was recorded in 6 (40.0%) patients. Nine patients were alive after 2.5-65 months follow-up. Conclusion: Allogeneic stem cell transplant could be effective in treating patients with B cell acute lymphocytic leukemia.

  • Reliability prediction of an electronic arm fire device(EAFD) was studied which is applied to prevent accidental ignition in a solid rocket motor. For predicting the reliability, the main components of the EAFD were first defined(Control unit, LEEFI, TBI) and the operating principle of each component was analyzed. Performance modeling of each part is established using selected input variables through system analysis. When complex analysis is required, we approximated it with polynomial equation using response surface method. Monte-Carlo simulation is applied to performance modeling to estimate the design reliability.

Herbicidal Effect of 5-Aminolevulinic Acid, a Biodegradable Photodynamic Substance (생분해성 광활성 물질 5-aminolevulinic acid의 제초활성)

  • Laboratory and greenhouse experiments were conducted to determine the herbicidal effect of two types of ${\delta}$-aminolevulinic acid (ALA), microbiologically-produced ALA (Bio-ALA) and synthetically produced ALA (Synthetic-ALA), on plant growth and chlorophyll content of Chinese cabbage. ALA effect on early plant growth was greatly concentration dependant, showing significant inhibition at higher concentrations. Both pre- and post-emergence application of ALA exhibited significant degree of photodynamic phytotoxicity. Older plants with many leaves were more tolerant to ALA than younger plants, showing less injury. No significant difference in herbicidal activity of two types of ALA, Bio-ALA and Synthetic-ALA, on plant height and chlorophyll content of Chinese cabbage was observed. However, residual biological activity and physico-chemical properties of Synthetic-ALA were more stable than those of Bio-ALA. Our results suggest that ALA had herbicidal potential with both pre- and post-emergence application, and that the chemical may be a valuable mean of eco-friendly weed control based on natural microbial substance.
