Surgical Treatment of Sternoclavicular Joint Dislocation Using a T-plate

  • A 22-year-old man was hospitalized with a sternoclavicular joint (SCJ) dislocation caused by a traffic accident. Surgical reduction and fixation of the SCJ were performed using a T-plate. SCJ dislocation is rare, accounting for less than 1% of all dislocations, and is usually treated conservatively, although severe cases may require surgery. Surgery typically involves joint reduction and fixation using an autologous tendon graft, but this has disadvantages such as the requirement for additional surgery to obtain autologous tissue and an extended operative time. To overcome these issues, here, we performed a simple SCJ reduction and fixation using a T-plate and achieved good results.

  • Clavicle medial end fracture is rare, and it has not been studied extensively. Although there is debate regarding its treatment methods, because of the complications of conservative treatment, surgical treatment has been considered more than conservative treatment. This study describes a surgical method using double-plate fixation for treatment of clavicle medial end fractures in which plates were used on each anterior and superior border according to the anatomical structure of the clavicle. In addition, we report operative results of three patients treated by double-plate fixation.

  • Fractures at the lateral end of the clavicle inevitably require surgical treatment as there is high potential for delayed union or nonunion. Acromioclavicular dislocation also requires stable and solid fixation for healing, and surgical treatment is recommended for the maintenance of joint function. The hook plate maintains the biomechanics of the acromioclavicular joint, enabling early range of motion. Therefore, for the past 10 years, the hook plate has been widely used in distal clavicle fractures and acromioclavicular joint injuries. However, the hook plate is associated with several complications, such as proximal clavicle fractures, widening of the hook hole, rotator cuff tear, subacromial impingement, and often acromial fractures. We report on two unusual cases of acromion fracture after hook plate fixation in patients with distal clavicle fracture and acromioclavicular dislocation alongside a literature review.

  • The dilemma regarding the management of condylar fractures generally revolves around the surgical approach, implant design, and the surgeon's experience. Zide and Kent's guidelines streamlined the decision making process for condylar fractures. However, there exists no standardized protocol for reduction and fixation of condylar fractures. Here, we have described a detailed and stepwise protocol, common to any surgical approach, that would lead to predictable, reproducible, and repeatable results in every surgeon's hands.

  • This study was performed to evaluate the new surgical technique for debarking. Of the 8 mongrel dogs which underwent devocalization, 4 heads (group 1) received complete surgical removal of the vocal cords and another 4 heads (group 2) received fixation of vocal cords and vocal processes onto the thyroid cartilage. Volume of each dogs' voices was measured by a sound level meter(Function A) over 7 months at intervals of 10 days. All experimental animals'hematological values were measured before operation and postoperative at 2i 5 and 10 days. The changes of volume of dogs'voices and hematological values were analyzed by paired t-test. The volume in the dogs'voices in each group after operation was significantly lower than that before operation. At this results we detected that devocalizing effect was the same in each group. Postoperative volume of dogs'voices in group 1 had a tendency to increaser but group 2 had a tendency to decrease. The number of erythrocytes at 2 days after operation was significantly fewer than those before operation in group 1, but the group 2 did not have significant changes. The number of leukocytes at 2 days after operation significantly increased from those before operation in group 11 but the group 2 did not have significant changes. It was concluded that the new surgical technique, fixation of vocal cords and vocal processes onto the thyroid cartilages could be available for the devocalization of dogs and for the prevention of postoperative inflammation and blood loss.

  • 전방십자인대 재건술은 수술의 시기, 수술 기법, 이식건의 선택 및 고정 방법, 재활 등 각각의 수술 과정을 성공적으로 수행해야하는 수술이다. 여러 과정들 중에서 이식건의 선택과 고정 방법은 아주 중요한 과정 중에 하나이지만 아직 술자들 간에 많은 논쟁의 여지가 있다. 이식건의 장단점에 관해 많은 연구들이 진행되고 있지만 여전히 이상적인 건에 대한 결론은 없다. 이와 유사하게 대퇴터널 및 경골터널 고정 방법에 대해서도 최근 많은 방법들이 개발되고 소개되고 있지만 그중에서 가장 좋은 방법을 찾지 못한 상태이다. 그래서 술자는 다양한 이식건과 고정 방법에 대하여 익숙해져야 하며 각각의 방법에 대한 장단점을 잘 알고 있어야 할 것이다. 따라서 본 종설은 전방십자인대 손상에서 이식건의 선택과 고정 방법에 대하여 지금까지 밝혀진 연구들을 분석하고 이에 관해 논의하고자 한다.

Ten technical aspects of baseplate fixation in reverse total shoulder arthroplasty for patients without glenoid bone loss: a systematic review

  • The aim of this systematic review was to collect evidence on the following 10 technical aspects of glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA): screw insertion angles; screw orientation; screw quantity; screw length; screw type; baseplate tilt; baseplate position; baseplate version and rotation; baseplate design; and anatomical safe zones. Five literature libraries were searched for eligible clinical, cadaver, biomechanical, virtual planning, and finite element analysis studies. Studies including patients >16 years old in which at least one of the ten abovementioned technical aspects was assessed were suitable for analysis. We excluded studies of patients with: glenoid bone loss; bony increased offset-reversed shoulder arthroplasty; rTSA with bone grafts; and augmented baseplates. Quality assessment was performed for each included study. Sixty-two studies were included, of which 41 were experimental studies (13 cadaver, 10 virtual planning, 11 biomechanical, and 7 finite element studies) and 21 were clinical studies (12 retrospective cohorts and 9 case-control studies). Overall, the quality of included studies was moderate or high. The majority of studies agreed upon the use of a divergent screw fixation pattern, fixation with four screws (to reduce micromotions), and inferior positioning in neutral or anteversion. A general consensus was not reached on the other technical aspects. Most surgical aspects of baseplate fixation can be decided without affecting fixation strength. There is not a single strategy that provides the best outcome. Therefore, guidelines should cover multiple surgical options that can achieve adequate baseplate fixation.

Akin 절골술의 내측 횡 봉합사 고정: 술기 보고 (Medial Horizontal Suture Fixation of the Akin Osteotomy: A Technical Report)

  • The Akin osteotomy is a complimentary procedure in hallux valgus surgery. Surgical techniques may vary depending on the operators or fixation devices. Suture fixation, for which a removal procedure is not necessary, can often be recommended. However, there is a risk of failure due to the thin cortex of the phalanx. We describe a new technique using Ethibond suture fixation in Akin osteotomy, which can lower the risk of phalangeal cortical failure and articular cartilage irritation. First, the Akin osteotomy was performed on the proximal phalanx 5 to 6 mm distal to the first metatarsophalangeal joint. Then bone holes were drilled from dorsum to plantar parallel to osteotomy with the Kirschner wire. The final procedure involved passing the Ethibond sutures connected to a straight needle through the holes and tying it. This fixation method offers an effective and easy technique for performance of Akin osteotomy.

Orthogonal versus Parallel Plating for Distal Humeral Fractures

  • In orthopedic trauma surgery, treatment of intraarticular distal humerus fractures is a challenge. With development of implants and biomechanical studies, surgical strategies with recommendations including preoperative computed tomography images, proper approaches and open reduction and internal fixation with dual plates have emerged. In addition, as an effort to provide stable fixation to permit early elbow motion, different methods of internal fixation, particularly plate configuration, have evolved. Using dual plates, either oriented parallel to each other or orthogonal, stable fixation has been achieved and satisfactory clinical outcomes have been reported. With rationales and advantages/disadvantages of each plate configuration, both techniques are selected according to surgeons' preference, and, in specific cases, one could be preferred over another. The key to successful fixation by either technique is obtaining anatomical reduction with restoration of two stable columns of the distal humerus.

Surgical Management of a Mandible Subcondylar Fracture

  • Open reduction and anatomic reduction can create better function for the temporomandibular joint, compared with closed treatment in mandible fracture surgery. Therefore, the double miniplate fixation technique via mini-retromandibular incision was used in order to make the most stable fixation when performing subcondylar fracture surgery. Those approaches provide good visualization of the subcondyle from the posterior edge of the ramus, allow the surgeon to work perpendicularly to the fracture, and enable direct fracture management. Understanding the biomechanical load in the fixation of subcondylar fractures is also necessary in order to optimize fixation methods. Therefore, we measured the biomechanical loads of four different plate fixation techniques in the experimental model regarding mandibular subcondylar fractures. It was found that the loads measured in the two-plate fixation group with one dynamic compression plate (DCP) and one adaption plate showed the highest deformation and failure loads among the four fixation groups. The loads measured in the one DCP plate fixation group showed higher deformation and failure loads than the loads measured in the two adaption plate fixation group. Therefore, we conclude that the selection of the high profile plate (DCP) is also important in order to create a stable load in the subcondylar fracture.
