연조직 결손부 회복을 위한 인공진피 이식의 조직학적 및 임상적 연구 (HISTOLOGICAL AND CLINICAL STUDY OF ARTIFICIAL DERMIS IMPLANTATION FOR RESTORATION OF SOFT TISSUE DEFECTS)

  • The present study was aimed to compare the resorption rate and the histological change of the autogenous dermis and the artificial dermis (Terudermis$^{(R)}$) after the transplantation, and to report the clinical results of the use of Terudermis$^{(R)}$ in order to restore the soft tissue defect. Twenty mature rabbits, weighing about 2 kg, were used for the experimental study. The autogenous dermis and the Terudermis$^{(R)}$ size 1${\times}$1 cm were transplanted to the space between the external abdominal oblique muscle and the external abdominal oblique fascia of the each rabbits. They were divided into 4 groups (n=5 each) and gathered at 1, 2, 4, and 8 weeks after the transplantation. The resorption rate was calculated, and H-E stain was preformed to observe the histological changes. The chart review of the 17 patients who received Terudermis$^{(R)}$ graft to the facial soft tissue defects was conducted for the clinical study. The resorption rate at 8 weeks after the transplantation was 21.5% for the autogenous dermis, and 36.4% Terudermis$^{(R)}$. In microscopic examinations, the infiltration of the inflammatory cells and the epidermal inclusion cyst were observed in the autogenous dermis graft. The neovascularization and the progressive growth of the new fibroblast were shown in the Terudermis$^{(R)}$ graft. In clinical data of 17 patients, the size of the grafted Terudermis$^{(R)}$ was from 1.5$cm^2$ to 7.5$cm^2$ (average 3.5$cm^2$). Follow-up ranged from 5 to 25 months. Fourteen patients with cleft palate demonstrated stability of the graft and unremarkable complications. But unstability of the graft and the partial relapse were observed in three patients received the vestibuloplasty. These results indicate that Terudermis$^{(R)}$ can be available substitute of autogenous dermis because of the stability about resorption, the histocompatibility, and the unremarkable clinical complications.

소아 얼굴 연조직의 3차원 입체영상의 정확성 및 재현성 평가 (Accuracy and Precision of Three-dimensional Imaging System of Children's Facial Soft Tissue)

  • 이 연구의 목적은 소아 얼굴 연조직의 선형 길이를 비교하여 삼차원 입체영상의 정확성과 재현성을 평가하는 것이다. 연구는 7 - 12세 아동 15명을 대상으로 하였다. 환자의 안모에 23개의 계측 점을 표시하고, 전자 캘리퍼를 이용하여 16 개의 선형길이를 직접 2번 계측하였다. 3차원(3D) 스캐너를 이용하여 2개의3D 얼굴 영상을 만들었으며, 직접계측과 동일한 16개의 선형 길이를 계측하였다. 연구결과 정확성 평가에서 전체 평균 차이 값은 0.9 mm였다. 3D 영상의 재현성은 직접계측의 재현성과 거의 유사한 정도로 나타났다. 이에 따라 이 3D 영상장비는 소아에서 임상적으로 충분한 정확성과 재현성을 가지고 있었다. 3D 영상 시스템은 정확한 이미지를 위하여 대상의 협조가 필요하다. 소아의 정확한 3D영상을 얻기 위한 구체적인 지시사항을 바탕으로 촬영을 한다면, 이 3D 영상 장비는 향후 소아의 성장과 발달을 조사하고, 소아교정치료에서 결과를 평가하는데 유용하게 사용될 수 있을 것이다.

청소년 및 성인 환자에서 II급 부정교합 치료시 연조직 변화에 관한 비교 (Comparison of Soft Tissue Changes between Adolescents and Adults in Class II Malocclusion Treatment)

  • 본 연구의 목적은 II급 청소년, 성인 환자에서 발치와 비발치 후 연조직 변화를 비교하기 위한 것이었다. 원광대학교 치과병원 교정과에 내원한 환자 68명을 각각 청소년발치군, 청소년비발치군, 성인발치군, 성인비발치군으로 분류하여 치료 전후의 측모 두부방사선사진을 계측하였다. 각 군의 치료 전후를 비교하고 각 군 간의 변화량을 비교하였으며, 그 중 통계적으로 유의한 변수들의 상관성과 회귀분석을 하여 다음과 같은 결과를 얻었다. 청소년과 성인의 발치군에서 비순각은 유의하게 증가하였고 비발치군에서는 유의하게 감소하였다. 발치군에서는 vertical-U1과 E line-upper lip의 변화량 간에, vertical-U1과 E line-lower lip의 변화량 간에, vertical-L1과 vertical-Li의 변화량 간에 양의 상관관계를 보였다. 또한 발치군에서 회귀분석을 통해 E line-upper lip, E line-lower lip, Li에 대한 단순회귀방정식이 도출되었다.

유치열기 정상교합아동의 측모두부방사선 계측연구 (STUDY ON LATERAL CEPHALOGRAM OF CHILDREN WITH NORMAL OCCLUSION IN THE PRIMARY DENTITION)

  • 본 연구의 목적은 유치열기 부정교합의 진단자료로 사용할 수 있는 정상 계측치를 알아보는 것이다. 4세에서 6세 사이 유치열기 정상교합아동 32명의 측모두부방사선 계측사진 및 안모사진을 이용하여 악골 및 연조직의 전후방적, 수직적 관계를 분석하였다. 수평적인 골격성 계측항목 중 SNA는 $83^{\circ}$, SNB는 $78.72^{\circ}$, ANB는 $5.28^{\circ}$를 나타내어 하악골이 상악골에 비해 후방에 위치하고 있었으며 수직적인 골격성 계측항목 중 Gonial angle은 $127^{\circ}$를 나타내어 성인에 비하여 High angle pattern을 보였다 연조직 분석에서는 성인에 비해 convex한 soft tissue profile이 관찰되었다. 치성 계측항목 중 U1 to SN은 $91.04^{\circ}$, IMPA는 $86.57^{\circ}$를 나타내어 성인에 비해 retroclination된 상하악 유중절치가 관찰되었다. 정상교합아동의 골격성 분석에서 여자가 남자에 비하여 평균 나이가 많음에도 불구하고 linear measurement는 전제적으로 남자가 여자보다 큰 수치를 보였다.

  • 저등급의 섬유점액성 육종은 매우 드문 연부 종양이다. 젊은 성인에서 발생하는 경향이 있으며, 국소 재발이나 원격 전이가 가능하다. 이들 종양 대부분은 하지 연부 조직에 발생하는 것으로 알려졌다. 조직학적으로 방추상 세포와 점액성 기질과 섬유성 기질이 혼재된 양상으로 관찰되는 것이 특징이며, 면역 조직학적 검사상 다른 양성 종양이나 악성 연부 조직과의 감별에 도움이 된다. 견갑부의 저등급의 섬유점액성 육종을 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

백서에서 혈소판 풍부 혈장의 제작과 유효성에 대한 실험적 연구 (FABRICATION OF PLATELET-RICH PLASMA IN A RAT MODEL AND THE EFFICACY TEST IN VITRO)

  • Purpose: Platelet-rich plasma (PRP) is known to accelerate and/or enhance hard and soft tissue healing and regeneration. As such, PRP has been used in various clinical fields of surgery. Recently there have been several attempts to use PRP in the field of tissue engineering. However, some controversies still exist on exact mechanism and benefits of PRP. Therefore various animal experiments are necessary to reveal the effect of the PRP. However, even if animal experiment is performed, the efficacy of the experiment could not be validated due to absence of an animal PRP model. The purpose of this study is to establish rat PRP model by comparing several PRP fabricating methods, and to assay growth factor concentration in the PRP. Materials and methods: Rat blood samples were collected from nine SD rat (body weight: 600-800g). PRP was prepared using three different PRP fabricating methods according to previously reported literatures. (Method 1: 800 rpm, 15 minute, single centrifuge; Method 2: 1000 rpm, 10 minute, double centrifuge; Method 3: 3000 rpm, 4min and 2500 rpm, 8 min, double centrifuge). Platelet counts were evaluated in an automated machine before and after PRP fabrications. In terms of growth factor assay, prepared PRP were activated with 100 unit thrombin and 10% calcium chloride. Growth factor (PDGF-BB, VEGF) concentrations on incubation time were determined by sandwich-ELISA technique. Results: An average of 3ml (via infraorbital venous plexus) to 15ml (via celiac axis) the rat blood could be collected. By using Method 3 (3000 rpm, 4 min and 2500 rpm, 8 min, double centrifugation), around 1.5ml of PRP could be prepared. This method allowed us to concentrate platelet 3.77-fold on average. PDGF-BB concentration (mean, 1942.10 pg/ml after 1 hour incubation) and VEGF concentration (mean, 952.71 pg/ml after 1 hour incubation) in activated PRP were higher than those in untreated blood. Also PDGF-BB showed constant concentration during 4-hour incubation, while VEGF concentration was decreased after 1 hour. Conclusion: Total 11,000 g minute separation and condensation double centrifuge method can produce efficient platelet-rich plasma. Platelet-rich plasma activated with thrombin has showed higher concentrations of growth factors such as PDGF-BB and VEGF, compared to the control group. Platelet-rich plasma model in a rat model was confirmed in this study.

Micromorphological and Chemical Characteristics of Cengal (Neobalanocarpus heimii) Heartwood Decayed by Soft Rot Fungi

  • The heartwood of cengal (Neobalanocarpus heimii) is known to have a high degree of decay resistance by virtue of its high extractive content. After 30 years in ground contact an utility pole of this tropical hardwood was found to be degraded only in the surface layers by cavity-forming soft rot fungi. The present work was undertaken 1) to characterize the degradation of cengal heartwood from the aspect of ultrastructure and chemistry and 2) to investigate the correlation between soft rot decay and its extractive microdistribution in wood tissues. The chemical analysis of cengal heartwood revealed the presence of a high amount of extractives as well as lignin. The wood contained a relatively high amount of condensed lignin and the guaiacyl units. Microscopic observations revealed that vessels, fibers and parenchyma cells (both ray and axial parenchyma) all contained extractives in their lumina, but in variable amounts. The lumina of fibers and most axial parenchyma were completely or almost completely filled with the extractives. TEM micrographs showed that cell walls were also impregnated with extractives and that pit membranes connecting parenchyma cells were well coated and impregnated with extractives. However, fungal hyphae were present in the extractive masses localized in cell lumina, and indications were that the extractives did not completely inhibit fungal growth. The extent of cell wall degradation varied with tissue types. The fibers appeared to be more susceptible to decay than vessels and parenchyma. Middle lamella was the only cell wall region which remained intact in all cell types which were severely degraded. The microscopic observations suggested a close correlation between extractive microdistribution and the pattern and extent of cell wall degradation. In addition to the toxicity to fungi, the physical constraint of the extractive material present in cengal heartwood cells is likely to have a profound effect on the growth and path of invasion of colonizing fungi, thus conferring protection to wood by restricting fungal entry into cell walls. The presence of relatively high amount of condensed lignin is also likely to be a factor in the resistance of cengal heartwood to soft rot decay.

Adult-onset kaposiform hemangioendothelioma with neurofibromatosis type 1: A case report and literature review

  • Kaposiform hemangioendothelioma (KHE) is a very rare, locally aggressive vascular neoplasm. It occurs mostly in children and is rarely observed in adults. It typically originates on the skin, later affecting the deep soft tissue of the extremities, head or neck, and retroperitoneum by infiltrative growth. It is locally aggressive, does not regress spontaneously, and tends to metastasize locally as well as to the regional lymph nodes. In this article, we report a case of adult-onset KHE with neurofibromatosis type 1. The patient presented to our department with a 2-month history of a painful ulceration in her left popliteal area. Since KHE had not previously been reported in patients with neurofibromatosis, the diagnosis was difficult due to the similarity of the skin manifestation to neurofibromatosis-associated lesions. We share our experience of diagnosing and treating this rare case of adult-onset KHE.

레이저 펄스 에너지를 이용한 무통증 마이크로젯 약물전달시스템 (Painless Microjet Injector Using Laser Pulse Energy)

  • 레이저 기반의 무바늘 액체 약물전달장치는 계속해서 개발되어왔다. 레이저 빔이 고무 챔버 내부의 액체에 모이게 된다. 초점이 맞춰진 레이저 빔은 공기방울을 생성시키고 급격히 팽창하게 된다. 밀봉된 챔버 안쪽에서의 급격한 부피변화는 액체약물을 마이크로 노즐을 통해 빠르게 밀어내어 마이크로 약물젯을 생성한다. 노즐의 출구지름은 100 ${\mu}m$ 이하이며 본 연구팀은 생성된 마이크로 약물젯의 속도가 인체의 연조직으로 침투시키기에 충분함을 확인하였다. 이 실험에서는 사람의 혈전을 모사한 무게 비 5%의 젤라틴 수용액을 냉각시킨 샘플과 돼지 지방층을 사용하여 침투실험을 수행하였다.

천공형 티타늄막과 탈회동결건조골의 신생골 형성에 대한 영향 (The effect of new bone formation of titanium mesh and demineralized freeze-dried bone)

  • This study was performed to evaluate bone formation in the calvaria of rabbit by the concept of guided bone regeneration with titanium mesh membrane and demineralized freeze-dried bone. The animal was sacrificed at 2 weeks, 4 weeks, 8 weeks, and 12 weeks after the surgery. Non-decalcified specimens were processed for histologic analysis. 1. The titanium mesh but the biocompatibility was excellent the cell-occlusiveness was feeble. 2. The cell-occlusiveness was feeble and also the soft tissue growth of the upper part of the newly-formed bone after operating was excellent in early stage. 3. The maintenance ability of the space for the GBR very was excellent. 4. The titanium mesh the tissue-integration was superior the wound fixation ability excellent. 5. The demineralized freeze-dried bone did not promote the bone regeneration. 6. With the lapse of time, formation quantity of the bone some it increased, it increased quantity very it was feeble. Within the above results, the titanium mesh for the guided bone regeneration was excellent, the dεmineralized freeze-dried bone confirmed does not promote bone regeneration.
