보건소 금연교육 프로그램이 흡연 고등학생의 흡연행동과 태도에 미치는 영향 (Effects of Smoking Cessation Programs in Public Health Centers on High School Smokers' Smoking Behavior and Attitude)

  • Objectives: Subjects of this study were technical high school students. It researched students' smoking status, smoking attitude, smoking perception and willingness to quit smoking, providing them with a five-day-smoking cessation program. Methods: Data collection proceeded from April to August 2008. The subjects were the whole number, 38 students, of 1EA technical high school located. The subjects who attended the smoking cessation program were the first and second grade students of the high school whom classroom teachers recommended. As to the subject, I comparatively analyzed differences between the before and after instruction, between after instruction and after consecutive consulting, and between before instruction and after consecutive consulting. The five-day-smoking cessation program took 2 successive hours a day for five days. Results: Average age for them to start smoking was 13.6 and to form smoking habits was 15.5. Average smoking period was 31.6 months and average smoking quantity was 11.2 cigarettes. As a result of the five-days smoking cessation program, 10.5% of the participants quit smoking and smoking quantity also significantly reduced. After instruction, willingness to quit smoking was generally higher than before. Smoking attitude, especially toward adolescent smoking, changed to be more negative than before. Perception of smoking also changed to be positive. Conclusions: The results from the comparative analysis of before and after instruction revealed a decrease on smoking quantity of the participants who attended the smoking cessation program. It also showed a significant change on smoking attitude, willingness to quit smoking and smoking perception.

Associations between the Frequency and Quantity of Heated Tobacco Product Use and Smoking Characteristics among Korean Smoking Adolescents

  • Purpose: Although heated tobacco product (HTP) use among adolescents is an emerging public health problem, little is known about the frequency and quantity of HTP use. Thus, we investigated the associations between the frequency and quantity of HTP use and smoking characteristics (i.e., combustible cigarette [CC] and electronic cigarette [EC] use, and attempts to quit smoking) among CC-smoking adolescents. Methods: We analyzed nationally representative data from 2,470 Korean adolescents who were current CC smokers. To investigate our aim, we conducted multinomial logistic and logistic regression analyses. Results: We found that daily and heavier CC users had greater likelihoods of more frequent and heavier HTP use. In addition, dual users of CCs and ECs were more likely to use HTPs more frequently and heavily than CC users who did not use ECs. Moreover, daily EC users had the highest risk of frequent and heavy HTP use. The frequency and quantity of HTP use were not associated with attempts to quit smoking. Compared to CC-only use, dual use of CCs and HTPs was not associated with quitting attempts, and triple use of CCs, ECs, and HTPs was associated with a lower likelihood of quitting attempts. Conclusion: HTP use was less likely to displace CC use and promote attempts to quit smoking. Thus, strict regulations are required to prevent the promotion of HTPs as a substitute for CCs or as a means of quitting smoking. Additionally, health professionals should consider preventive interventions for HTP, as well as CC and EC use among adolescents.

청소년 금연 프로그램에서 추후관리의 효과 (The Effects of Follow-up Support in the Adolescent Smoking Cessation Program)

  • The purpose of this study is to investigate the effects of follow-up support in the adolescent smoking cessation program. In this study, a model for an effective adolescent anti-smoking program, which includes follow-up support, is based on the method of cognitive-behavioral therapy. The study lasted for 7 weeks, while information on smoking including materials to be analyzed, were provided to the subjects. Meanwhile, the state of the smokers was studied while consultations on anti-smoking was also carried out in the 7th week. At first, the quantity of cigarettes smoked between groups made no difference, but there was an obvious difference in the rate of people who stopped smoking and in the quantity of cigarettes smoked between group I(managed for 7 weeks) ,group II (managed for only 4 weeks) and group III (not managed at all after anti-smoking class). According to the results of the study, we can conclude that the follow-up support plays a big role not only in raising the rate of success in the level of reformed smokers, but also in lowering the quantity of cigarettes smoked.

Prognostic Significance of Cigarette Smoking in Association with Histologic Subtypes of Resected Lung Adenocarcinoma

  • Background: Smokers with lung adenocarcinoma have a worse prognosis than those who have never smoked; the reasons for this are unclear. We aimed to elucidate the impact of smoking on patients' prognosis and the association between smoking and clinicopathologic factors, particularly histologic subtypes. Methods: We reviewed the records of 233 patients with pathologic stage T1-4N0-2M0 lung adenocarcinomas who underwent surgery between January 2004 and July 2015. The histologic subtypes of tumors were reassessed according to the 2015 World Health Organization classification. Results: In total, 114 patients had a history of smoking. The overall survival probabilities differed between never-smokers and ever-smokers (80.8% and 65.1%, respectively; p=0.003). In multivariate analyses, the predominant histologic subtype was an independent poor prognostic factor. Smoking history and tumor size >3 cm were independent predictors of solid or micropapillary (SOL/MIP)-predominance in the logistic regression analysis. Smoking quantity (pack-years) in patients with SOL/MIP-predominant tumors was greater than in those with lepidic-predominant tumors (p=0.000). However, there was no significant difference in smoking quantity between patients with SOL/MIP-predominant tumors and those whose tumors had non-predominant SOL/MIP components (p=0.150). Conclusion: Smoking was found to be closely associated with SOL/MIP-predominance in lung adenocarcinoma. Greater smoking quantity was related to the presence of a SOL/MIP component.

중학생의 흡연에 대한 지식 및 태도 (Knowledge and Attitude about Smoking in Middle School Students)

  • This study was done to provide basic data to develop smoking prevention and non-smoking education program. Questionnaire were collected from 438 male, 474 female middle school students at Chunchon, Kwangwon-do from Sep 2, 2000 to Feb 28, 2001. The Questionnaire used to measure subjects' knowledge and attitude about smoking was Guideline for the content of tobacco smoking surveys of the general population by WHO. The collected data were analyzed by SPSS-Win program. The results were as follows. 1.The average score of knowledge about smoking of subjects was 57.01 on the basis of 100. 2. The average score of attitude about smoking of subjects was 63.54 on the basis of 100. 3. In knowledge about smoking, there were statistically significant difference according to subjects' grade(F=3.218, p=.041), thinking about smoking behavior(F=3.424, p=.017), harmfulness of smoking(F=17.202, p=.000), present drinking(F=3.555, p=.029) and in attitude about smoking, there were statisti cally significant difference according to subjects' grade(F=20.520, p=.000), sex(t= -5.073, p=.000), mother's smoking(t= -2.405, p=.035), brother's smoking(t= -5.022, p=.000), mother's schooling(F= 3.730, p=.024), having smoking friend or not (t=-9.357, p=.000), subjects' smoking experience (t=-12.363, p=.000), present smoking(F= 76.643, p=.000), quantity of smoking(F= 6.808, p=.001), period of stop smoking(F= 4.685, p=.004), beginning time of smoking (F=2.286, p=.049), thinking about smoking behavior(F=17.933, p=.000), harmfulness of smoking(F=58.360, p=.000), present drinking (F=58.071, p=.000). 4. There were significant relationship between subjects' knowledge and attitude about smoking(r=.469, p=.000).

일부 남자 중학생의 흡연에 대한 지식 및 태도 (Knowledge and Attitude toward Smoking Behavior in Male Middle School Students)

  • This study was conducted to provide basic data to develop a smoking prevention and non-smoking education program. Data were collected, using a questionnaire, from 438 male middle school students living in Chunchon, Kwangwon- do. The data collection period was from September 2 to 30, 2000. The questionnaire used to measure the subjects' knowledge and attitude toward smoking was based on the guideline for the contents of tobacco smoking surveys for the general population designed by WHO. The collected data were analyzed using the SPSS-Win program. The results were as follows. 1. The subjects' average score for smoking knowledge was 58.52 out of 100 points. 2. The subjects' average score for attitude toward smoking was 63.54 out of 100 points. 3. Smoking knowledge of the subjects was significantly different by their grade (F=9.187, p=.000), father's smoking behavior (t=2.261, p= .024), aware of harmfulness of smoking (F=8.911, p=.000). The subjects' attitude toward smoking was significantly different by mother's smoking behavior (t=-2.557, p=.011), brother's smoking behavior (t=-3.959, p=.000), having a smoking friend or not (t=-4.422, p=.000), subjects' smoking experience (t=-8.304, p=.000), present smoking behavior (t=7.001, p=.000), quantity of smoking (F=3.297, p= .041), abstinence period of smoking (F=3.858, p=.013), thinking about smoking behavior (F=20.999, p=.000), aware of harmfulness of smoking (F=20.964, p=.000), and amount of drinking (F=8.316, p=.000). 4. There was a significant correlation between subjects' knowledge and attitude toward smoking (r=.514, p=.000).

일부 여대생의 흡연실태와 그 관련요인 분석 (A Survey on Smoking Status and Its Related Factors for Female Students of A Women's University in Seoul)

  • In order to find smoking status and its related factors for female students, a survey was conducted for 1,045 female students of a women's university in Seoul from 22nd of May to 18th of June, 1995. The results were as follows: 1. Of the 1,045 respondents, 7.4% of them were current smokers and 20.6% of them were ex-smokers. The rate of smoking by the major field of study was 18.5% in art and physical education, 10.6% in liberal art and social science, 1.9% in natural science, and 1.2% in medical science. The rate of smoking by the grade was 4.1% in the freshman, 5.9% in the sophomore, 10.9% in the junior, and 10.0% in the senior grade. 2. 7.2% of respondents experienced first smoking at junior high school, 16.8% at senior high school, 4.1% after high school, and 69.2% at college. 3. The major motivation to start smoking was 'curiosity'(57.9%), 'releasing stress'(21.6%), and 'temptation from friend or senior'(12.0%). 4. The first smoking place was 'entertainment place'(49.6%), 'own room'(20.5%), and 'school '(11.3%). And the first smoking companion was 'friend'(62.3%), 'alone'(19.9%), and 'senior or junior'(5.1%). 5. The average quantity of a daily smoking was 6.16±5.29 cigarettes in current smokers and 31.2% of them had experience to quit smoking at least one or more time. 6. Most of the reasons to quit smoking in ex-smoker were due to 'bad taste '(60.5%), 'health concern'(27.4%) and 'religion'(2.3%). 7. Current smoker and ex-smoker had more positive attitude for the female smoking than nonsmoker. And 76.7% of nonsmokers answered that they would not smoke in the future. 8. Most correspondents(89.8% of nonsmoker, 83.7% of ex-smoker, 80.5% of current smoker) strongly supported the antismoking movement. 9. The major information source in smoking were from 'radio, TV'(51.0%), 'newspaper, magazine'(38.8%) and 'school'(7.7%). 10. The multiple logistic regression analysis showed that the related factors for smoking status were satisfaction of home life, coffee intake, alcohol drinking, sister's smoking, girl friend's smoking, and knowledge about smoking.

흡연이 구강환경에 미치는 영향 (The Effects of Smoking on Oral Environment)

  • 구강질환의 국소적인 원인이자 중 하나로 생각되는 흡연이 구강환경에 미치는 영향을 알아보기 위하여 흡연자군과 비흡연자군에 있어서 구강환경관리능력지수와 간이구강위생지수를 비교해 본 결과 비흡연자군에 비해 흡여자군의 구강환경관리능력이 낮고, 간이구강위생지수가 높은 것으로 나타나 유의한 정도의 차이는 나타나지 않았으나 각 지수들간의 차이를 보였다. 흡연기간, 흡연량, 연간흡연갑수가 많을수록 구강환경관리능력이 낮고, 간이구강위생지수가 높게 나타나 흡연이 구강환경상태에 미치는 영향이 높음을 제시하고 있다. 연간흡연갑수는 1일 평균 흡연량과 흡연기간의 곱이므로, 1일 평균흡연량과 흡연기간이 개별적으로 미치는 영향보다 더 큰 영향을 미치는 것으로 나타났다.

금연/흡연예방 프로그램의 방법에 따른 효과성 평가: 최근 3년 간 국내학술지 논문에 대한 체계적 문헌고찰 (Efficacy of Smoking Cessation and Prevention Programs by Intervention Methods: A Systematic Review of Published Studies in Korean Journals during Recent 3 Years)

  • Objectives: The purpose of this study was to evaluate program methods for effective smoking cessation and prevention through systematic review of studies published during recent 3 years in Korea. Methods: Electronic searches were conducted in DBPIA, RISS, KISS and other databases looking for studies published from 2010 to 2012 that dealt with the effects of intervention on smoking prevention and cessation. A total of 25 studies were included in the final analysis. Three independent reviewers critically evaluated the study quality and evidence of effectiveness based on The Community Guide methodology. Results: Multi-component intervention showed strong evidence of effectiveness for adolescents and sufficient evidence for patient smokers. Cognitive-behavioral intervention for adolescents and child-parent interaction program for adults demonstrated sufficient evidence of effectiveness on the smoking cessation and its determinants. There was insufficient evidence showing effectiveness of smoking prevention programs. Conclusion: Improving the quality and quantity of the intervention studies on smoking could help to expand evidence base for effective intervention methods and strategies on smoking.

전국 간호교육기관의 금연관련 교육내용 분석 (Tobacco-Related Education in Graduate, Baccalaureate and Associate Degree Programs in Korea)

  • Purpose: The purpose of this research was to find out tobacco control education taught graduate, baccalaureate and associate degree-nursing programs in Korea. Method: A valid and reliable questionnaire previously used to assess tobacco use in medical and nursing schools in the United States was translated and revalidated. Surveys were sent out to all the graduate, baccalaureate and associate nursing programs in Korea. All 6-tobacco curricula content areas recommended by the United States National Cancer Institute and the Agency for Health Care Policy and Research was included. Result: Majority of BSN (93.2%-75%) and AD (92.3%-64.1%) programs were teaching health hazards, cancer risk, health effects of tobacco, effects of ETS, contents of cigarette, withdrawal symptoms and high risk groups of smoking as a part of required courses. However, 87.5% (14/16) of graduate, 50.0% (22/44) in baccalaureate, 53.8% (21/39) in associate degree programs were not teaching about the 5 A's. Of those who reported teaching smoking cessation strategies, they were covered most frequently in Community Nursing (50%), Adult Health Nursing (43.8%) on the graduate level, and in Community Nursing (65.9%-BSN, 76.9%-AD) and Adult Health (54.5%-BSN, 74.4%-AD) on the undergraduate level. 31.1% (5/16) in Graduate, 18.2%(8/44) in BSN, 10.3% (4/39) in Associate Degree programs reported that there are no smoking students in their schools and rest of the schools reported either do not know or they have 1 to more than 21% of students smoking. Conclusion: The study results identifies the quantity and level of tobacco control content in three different nursing programs and will help in developing curricular guidelines in the future. Further survey needs to be done on nursing students' knowledge, attitude and belief on smoking and smoking cessation.
