병의원 금연치료프로그램 참여자의 금연 경험: 질적 연구 (Experience of Smoking Cessation Treatment Program in Korea: Results from a Qualitative Study)

  • Background: The objectives of this study were to identify the reasons of quitting smoking, to determine the factors that make it difficult or helpful to maintain smoking cessation and to confirm the changes after the smoking cessation. Methods: This study was conducted an in-depth interview with people who participated in the smoking cessation treatment program. There were 10 participants and they were interviewed thoroughly for 14 times. Using the directed content analysis, we analyzed the transcript which was written by recording the interview and the researchers' note. Results: The results of this study are summarized as 'the experience while at the verge of smoking cessation and stepping over the verge of smoking cessation,' 'the changing experience due to smoking cessation,' and 'maintaining the changes through overcoming re-smoking.' The smoking cessation treatment program induced the smokers to quit smoking. Participants mentioned drinking alcohol was the major obstacle to maintain smoking cessation. Also, they noted that the money and the time that were already spent to quit smoking helped their smoking cessation as well as the social relations which helped to prevent re-smoking. Participants felt well-cared through the smoking cessation treatment program and that it helped them to maintain smoking cessation. Conclusion: Smoking cessation treatment program has a positive effect on the smoking cessation trial and maintenance. In order for smokers to overcome re-smoking, educations regarding drinking problem, formation of supportive social relationship, and the applications for smoking cessation will be beneficial.

Use and perception of Smoking Cessation in Traditional Medicine: A Survey of Korean Medical Practitioners

  • Objectives : This survey assessed Korean medical doctors' (KMDs') use and perception of smoking cessation therapies in traditional Korean medicine (TKM) and their drawbacks. Methods : A total of 14,485 KMDs affiliated with the Association of Korean Medicine were sent surveys via email. They were asked the use and perception of smoking cessation therapies in TKM and their drawbacks: Present use of Korean medicine treatment for smoking cessation, ideal treatment for smoking withdrawal symptoms, advantages and disadvantages of smoking cessation treatments in Korean medicine were asked with closed-ended questionnaire. Results : Two hundred fifty-three KMDs (1.75%) responded to the questionnaire. According to the results of the study, more than half of KMDs (51.4%) answered that they have practiced smoking cessation therapy. The most frequently used treatments for smoking cessation were ear acupuncture (EA) (74.6%) and acupuncture (15.4%), and the most of TKM doctors said that they were the most effective treatments. The advantages of smoking cessation treatment in TKM were the fewer adverse events (53.4%), availability to stimulate acupoints continuously in everyday life (48.2%), and the possibility of controlling withdrawal symptoms. Disadvantages included the long treatment duration and the necessity of frequent visits to the doctor. The greatest disadvantage was the lack of sufficient evidence for the effectiveness of TKM smoking cessation protocols. Conclusions : Despite of the low response rate, the survey results show that the main treatment modalities for smoking cessation are acupuncture, and this was also estimated effective for stop smoking. However, to improve smoking cessation rate, the more various smoking cessation treatments should be developed.

청소년에 대한 금연침 효과 (The Acupuncture Effect on Juvenile Smoking)

  • Objectives : Because of the recent well-being syndrome, concerns about smoking cessation, as well as dietary improvement and exercise, are being increased. There are various methods for smoking cessation. Among them, recently acupuncture treatment is verified through diverse attempts. This research was conducted to examine the effects of acupuncture treatment especially on juvenile smoking cessation. Methods : From April 04 to Oct 04, this study had investigated 93 middle and high school students. 5 times of acupuncture treatment were given to each participant, and the survey was executed before and after each treatment. The acupuncture was treated to the ear alternately twice a week. The evaluation was taken before the treatment after the 1st treatment, and the 5th treatment with 6 questions of amount of daily smoking, desire to smoke, taste of smoking, subjective symptoms, smoking cessation proportion, Fagerstorm Tolerance Questionaire point. Results : Regarding amount of daily smoking, the significant tendency between before and after the treatment varied, but between the 1st and the 5th treatment, there was no difference. The 1st treatment had caused desire to smoke reduction in 46 students(49.5%), but the 5th treatment didn't have much effects on desire to smoke(43 persons; 46.2%). After treatment, 37.6% had no effect in taste of smoking, while the rest of them showed various reactions. Also, there were no difference of significant tendency between the 1st and 5th treatments. In respect to the amount of smoking, 70% of them achieved smoking cessation, or reduction in smoking, but about 25% of them showed no changes. Various subjective symptoms such as lethargy, decrease of appetite, headache were shown, and especially desire to smoke was shown pervasively. Regarding Fagerstorm Tolerance Questionaire point, the average of significant tendency in point after the treatment had decreased(3.1+-2.4 to 2.1+-2.1, p<0.0001). but for those students who had tried smoking cessation less than 5 times, Fagerstorm Tolerance Questionaire point had decreased significantly, but for the others who had tried smoking cessation more than 5times, there were no difference. Conclusion : In regard to 6 questions of amount of daily smoking, desire to smoke, taste of smoking, subjective symptoms, smoking cessation proportion, Fagerstorm Tolerance Questionaire point, various conclusion is drawn. These results suggest that acupuncture treatment on juvenile may have the tendency of smoking cessation.

Adherence to Varenicline and Abstinence Rates for Quitting Smoking in a Private Health Promotion Center-Based Smoking Cessation Clinic

  • Background: Varenicline is an effective smoking cessation aid. However, smokers prescribed with varenicline do not always receive varenicline for 12 weeks, as recommended. This study analyzed the subjects who received varenicline and investigated the effect of varenicline treatment duration on the success rate of 6-month smoking cessation. Methods: This study retrospectively analyzed 78 subjects, who received varenicline, out of the 105 smokers that had visited the smoking cessation clinic after medical examination from September 2007 to December 2009. Results: The subjects were all males. Twenty-two subjects (28.2%) had varenicline treatment for 12 weeks or longer; 18 subjects (23.1%) for 8~12 weeks; 22 subjects (28.2%) for 4~8 weeks; and 16 subjects (20.5%) for less than 4 weeks. The total success rate of the 6-month smoking cessation was 47.4%. The success rate of the 6-month smoking cessation was 63.6% in the group that received varenicline for 12 weeks or longer, which was higher than 41.1% of the group that early terminated the varenicline treatment (p=0.074). The period of varenicline treatment was extended for one more week, the odds ratio of the 6-month smoking cessation success increased to 1.172-folds (p=0.004; 95% confidence interval, 1.052~1.305). Adverse events occurred in 30.8% of the subjects who received varenicline, but no serious adverse events were found. Conclusion: If varenicline treatment period is extended, the odds ratio of the success rate for the 6-month smoking cessation increases. Therefore, an effort to improve drug compliance for varenicline in clinical practices could be helpful for the long-term success of smoking cessation.

Smoking Cessation Treatment and Outcomes in Medium to Heavy Cigarette Smokers being Treated for Cancer in Jordan

  • Background: Studies evaluating smoking cessation treatment outcomes in cancer patients are scarce, despite smoking cessation importance in cancer care. We sought to add to the literature by evaluating smoking cessation in a challenging group of cancer patients (medium-to-heavy smokers) visiting an out-patient smoking cessation clinic (SCC) in a cancer center in Amman, Jordan. Materials and Methods: Patients smoking >9 cigarettes per day (CPD) and referred to the SCC between June 2009 and May 2012 were studied. Clinic records were reviewed to measure demographic and baseline clinical characteristics, and longitudinal (3-, 6- and 12- month) follow-up by phone/clinic visit was conducted. At each follow-up, patients were asked if they experienced medication side-effects, if they had returned to smoking, and reasons for failing to abstain. Descriptive and multivariable logistic regression analyses were performed. Results: A total of 201 smokers were included in the analysis. The 3-month abstinence was 23.4% and significantly associated with older age, being married, and presenting with lower (${\leq}10ppm$) baseline carbon monoxide (CO) levels. On a multivariable level, lower CO levels, a higher income (relative to the lowest income group), being older, and reporting severe dependence (relative to dependence reported as 'somewhat' or 'not') were significant predictors of higher odds of abstinence at three months. Reasons for failing to quit included not being able to handle withdrawal and seeing no value in quitting. Long-term ARs did not reach 7%. Conclusions: In a sample of Jordanian smokers (>9CPD) with cancer and receiving smoking cessation treatment, ARs were low and further declined with time. Results underscore the need for more aggressive patient management and rigorous follow-up during and after smoking cessation treatment, particularly when this takes place in challenging settings. Observed reasons for failure to abstain should be used to tailor counseling practices.

자아존중감 및 금연 자기효능감 증진 프로그램이 흡연 고등학생에게 미치는 효과 (The Effects of a Self-esteem and Smoking Cessation Self-efficiency Improvement Program on Smoking High School Students)

  • Purpose: The purpose of this study is to understand the effects of a program to improve self-esteem and smoking cessation self-efficiency on smoking high school students' self-esteem, smoking cessation self-efficiency, amount of smoking, cotinine in urine, and carbon monoxide while exhalation. Methods: This research was conducted as a quasi-experimental pretest and posttest control and experimental group methodological comparison study. The subjects were 45 smoking high school students (Exp.=22, Cont.=23) in U City. Data were collected from October 19 to December 7, 2010, and analyzed using the SPSS/WIN 12.0 program by frequency, Mann-Whitney test, means, standard deviations, and Willcoxon signed rank test. Results: After the treatment, those belonging to the experimental group showed significantly increased self-esteem and smoking cessation self-efficacy and also showed decreased amount of smoking, cotinine in urine, and carbon monoxide while exhalation compared to the control group. Conclusion: The self-esteem and smoking cessation self-efficacy improvement program was effective in improving self-esteem and smoking cessation self-efficacy and in diminishing the amount of smoking, cotinine in urine and carbon monoxide while exhalation. Therefore, this program is recommended as a smoking cessation strategy for smoking adolescents.

금연프로그램이 흡연근로자의 흡연량과 니코틴의존도 및 자기효능감에 미치는 효과 (Effects of a Smoking Cessation Program on Amount of Smoking and Nicotine Dependence and Self-efficacy of Smoking Cessation for Smoking Workers)

  • Purpose: The purpose of this study was to identify the effects of a smoking cessation program on the amount of smoking and nicotine dependence and the self-efficacy of smoking cessation for smoking workers. Methods: The total number of subjects was 38 smokers and was divided into two: 16 smokers were placed in the experimental group and 22 in the control group. A nonequivalent control group pretest-posttest design was used. The smoking cessation program consisted of two sessions and was implemented for two hours daily for five days. Data was analyzed by using the SPSS/WIN 11.0 program with $X^2-test$, t-test or Mann-Whitney U and ANCOVA. Results: After the treatment, those belonging to the experimental group showed a significantly decreased amount of smoking, a decreased nicotine dependency and increased self-efficacy compared to the control group. Conclusion: The smoking cessation program was effective for diminishing the amount of smoking, decreasing nicotine dependence and improving self-efficacy. Therefore, this program is recommended as a smoking cessation strategy for adult smokers.

흡연행위의 자조적 변화기전에 대한 요인분석 (Factor Analysis on Behavior Change Mechanisms in Self-help Smoking Cessation)

  • Smoking is considered to be one of the important risk factors associated with various health problems. Thirty seven million people in America quit smoking in the past decade, with 90% quitting on their own(Americal Heart Association, 1986). According to McAlister(1975), 70% of smokers reported that they adopted a self-help smoking cessation approach rather than a formal treatment program. In our country, although, auccessful exsmokers have used, are not available, concerns about smoking cessation have increased, recently, due to changes in social and political attitudes to health promotion. This study was conducted to help smokers who had made a decision to adopt a self-helf smoking cessation approach instead of depending on a formal treatment program. It provided a structurd way for sucessful smoking cessation by identifying change mechanism factors which have been commonly used by smokers. The subjects selected for this study were 155 university students between 20 and 29. Years of age the results from the analysis of the collected data showed that seven factors in the self-help change processes significant, and they are named, Stimulus Control, Self-Determination, Information Management, Reinforcement Management, Helpful Relationships, Dramatic Relief, and Cognitive Restructuring. All of the factors except Self-Determination represent the same change mechanisms which are proposed by Prochaska et at.(1988). Self-Determination uniformly includes items belonging to various change processes. Therefore, it might be an transitional factor including strategies related to both cognitive and behavioral dimensions. In spite of this, noted that one item which gives meaning of Self-Determination shows the highest factor loading, it is named Self-Determination.

경피적 관상동맥 중재술을 받은 환자의 금연교육이 흡연에 미치는 효과 (Effects of Smoking Cessation Education for Patients with Percutaneous Coronary Intervention)

  • Purpose: The purpose of this study was to investigate the effects of a smoking cessation education for patients with percutaneous coronary intervention. Methods: The study was a quasi-experimental study with a non-equivalent control group non-synchronized design. The Sixty subjects were assigned to either the treatment group. The treatment intervention was a onetime program during hospitalization. Data were collected during several time periods: before treatment, immediately after treatment, two weeks later, and one month after the treatment. Data were analyzed with the SPSS 14.0 program with an independent t-test, $x^2$ test, repeated measures ANOVA. Results: There were significant differences between the two groups in terms of knowledge and attitude. The significance was absent for the testing two weeks post group intervention and then one month later the significance was again detected in the amount of smoking and smoking behavior. Conclusion: The results of this study suggest that smoking cessation education could be utilized for inpatients with percutaneous coronary intervention as nursing intervention in hospitals effectively. Further research is recommended in order to understand the change in significance in the different time periods.

아로마테라피 마사지를 적용한 금연프로그램이 여대생의 금연에 미치는 효과 (Effects of Smoking Cessation Program applying Aromatherapy Massage on Smoking Cessation in Female College Students)

  • Purpose: The purpose of this study was to examine the effects of smoking cessation program applying aromatherapy massage on female college students' quitting of smoking. Methods: Forty-eight female college students were assigned to a smoking cessation program applying aromatherapy massage or a control group. The treatment group was taught to conduct a self hand massage with aroma essence oil. Lavender extra, Camomile roman and Bergamotte aroma essence oil were used for massage. The results of this study showed that the smoking cessation program applying aromatherapy massage resulted in significant effects on the number of cigarettes smoked per day, anxiety, smoking cravings and withdrawal symptoms. Conclusions: The findings presented above suggest that the smoking cessation program applying aromatherapy massage may be an effective intervention for female college students attempting to quit smoking to alleviate smoking-related anxiety and to reduce the number of cigarettes smoked, smoking cravings and withdrawal symptoms.
