Insomnia is one of the most common symtoms of Neuropsychiatry patients. These are many approaches to treat insomnia, but it is difficult to be resolved completely. Acupuncture and Auricular acupuncture have been used for relief insomnia. But it is not sufficient to compare the effect of Acupuncture treatment and Auricular acupuncture treatment. Thus we investigate the effect of Acupuncture treatment group and Auricular acupuncture treatment group. A clinical analysis was carried out for 40 patients who was treated insomnia with Acupuncture(20 patients) and Auricular acupuncture(20 patients) in Seoul Dong-Seo Oriental Hospital. 1. Total duration of sleeping time showed $1.55{\pm}1.1574$ hours increased in Acupuncture group and $0.925{\pm}1.184$ hours increased in Aricular acupuncture group. especially Acupuncture were much more effective method than Aricular acupuncture. P-value was 0.033 (P <0.05) 2. Delaying time before the onset of sleep showed $0.7375{\pm}0.8940$ hours decreased in Acupuncture group and $0.7969{\pm}1.3298$ hours decreased in Auricular acupuncture group. (P <0.05) 3. The wake times showed $0.9167{\pm}1.7299$ times decreased in Acupuncture group and $1.2308{\pm}1.1658$ times decreased in Auricular acupuncture group in sleep maintenance insomnia. (P <0.05) 4. Change in quality of sleep showed Excellent 30%, Good 50%, Fair 15%, Poor 5% in Acupuncture group and Excellent 20%, Good 45%, Fair 30%, Poor 5% in Aruricular acupuncture group.
Shihogayonggolmoryo-tang (ST) is a Korean medical herb cocktail which has been used to treat anxiety induced insomnia. In this study, we will examine sleep-promoting and anti-anxiety effects of ST, and investigate its mechanism. ICR mice were divided into three groups for the first examination : control group (n=11), ST50 group (50 mg/kg, po, n=11), ST200 group (200 mg/kg, po, n=11). Sleep-promoting effect was confirmed by measuring the sleeping duration time and sleeping onset time after thiopental sodium treatment (50 mg/kg, ip). ICR mice were divided into five groups for the second examination : control group (n=11), ST200 group (200 mg/kg, po, n=11), ST200+Flumazenil group (ST 200 mg/kg, po, flumazenil 0.3 mg/kg, ip, n=11), diazepam group (1 mg/kg, ip, n=11), diazepam+flumazenil group (diazepam 1 mg/kg, ip, Flumazenil 0.3 mg/kg, ip, n=11). Anxiety behavior and sleep-promoting effect was confirmed by open field test and measuring the sleeping duration time and sleeping onset time. Expression levels of c-fos in tuberomammillary nucleus (TMN) and ventrolateral preoptic nucleus (VLPO) were analyzed by immunohistochemistry. ST treated group showed significantly decreased anxiety behavior and enhanced sleeping duration time and sleeping onset time concentration dependently. The expression of c-fos was significantly upregulated in VLPO as sleep-inducing center and TMN as downregulated in arousal center by ST treatment. In addition, all effects of ST were reversed by flumazenil. Our results suggest that ST has sleep-promoting and anti-anxiety effects through regulating arousal center (TMN) and sleep-inducing center (VLPO).
The sleeplessness in childhood is quite different from that in adulthood in terms of causes, developmental process, and treatment. Sleep behavior in childhood is strongly influenced by parental personality and familial and cultural background. In understanding and management of sleeplessness of children, it is especially important to understand the separation anxiety and the ways of its management in bedtime because bedtime routine with children one of separation process from parents. Co-sleeping, parental intervention, transional object and bedtime routines can be appeared in order to reduce the anxiety from bedtime separation. Causes of sleeplessness in infant and toddler are bad sleep-onset association, nocturnal drinking, colic, and food allergy. In preschool and school aged children, limit-setting sleep problem and fears and nightmare can be causes of sleeplessness. When good sleep environment and habits are established sound sleep and more mature personality can be developed.
Purpose: Sleep onset association disorder (SOAD) is a form of behavioral insomnia observed in children that is caused by inappropriate sleep training. SOAD typically disturbs the sleep of not only infants and children but also their parents. We investigated levels of depression and marital intimacy among parents of infants with typical SOAD, to understand the influence of SOAD on family dynamics, as well as examine ways for improving depression and marital intimacy through behavioral training. Methods: Depression and marital intimacy were assessed using the Beck Depression Inventory (BDI) and Waring Intimacy Questionnaire (WIQ). These measures were administered to 65 parents of infants (n=50) diagnosed with SOAD. We conducted sleep education and behavioral training for the parents and compared levels of depression and marital intimacy after 2-6 weeks of training. Results: The 65 parents consisted of 50 mothers and 15 fathers. Depressive symptoms were higher among mothers than fathers (P=0.007). Marital intimacy was negatively correlated with depressive symptoms. Twenty-six parents were assessed again after sleep training. We found that mothers' depressive symptoms and marital intimacy improved post training. Conclusion: SOAD can be detrimental to both infants and parents, especially for parents who sleep with their infants. For instance, disruption of sleep patterns in such parents can reduce marital intimacy. However, behavioral modification is an effective treatment for infants with frequent nighttime waking, as well as for diminishing the depressive symptoms of sleep-deprived parents.
목 적 : 한국 사회의 고령화와 동반되어 노인 인구의 불면증은 증가되는 추세이나 이로 인해 초래되는 개인 및 사회경제적 손실에 비해 적절한 치료를 받는 환자의 수는 많지 않다. 불면증 인지행동치료가 부작용을 최소화할 수 있는 일차적인 치료로서 추천되고 있으나, 시간 및 경제적 제약, 인지적인 자원이 필요한 부담으로 인해 치료에 대한 접근성이 제한된 실정이다. 불면증 단기 치료 프로그램은 이러한 부담을 최소화할 수 있는 대안이 될 수 있을 것이다. 본 연구의 목적은 지역사회 거주 노인을 대상으로 불면증 단기 치료 프로그램의 효과를 검증하는 것이다. 방 법 : 2016년 5월부터 2018년 1월까지 서울 소재 3개의 노인 종합복지관을 방문한 65세 이상의 불면증을 호소하는 노인들 47명을 대상으로 3주에 걸쳐 불면증 단기 치료 프로그램을 시행하였다. 프로그램의 전, 후의 수면의 질을 측정하는 피츠버그 수면의 질 지수(Pittsburgh sleep quality index, PSQI), 수면 위생 척도, 정서 관련 평가를 실시하였으며 치료 기간 중 매일 수면 일지를 작성하도록 지시하였다. 임상적 효과는 치료 전후의 총 수면 시간, 수면 잠재기, 수면 후 각성 시간 및 수면 효율로 평가하였다. 치료 전, 후의 변화를 비교하기 위해 비모수검정 방법인 윌콕슨 부호 순위 검증(Wilcoxon's signed-ranks test)을 이용하여 분석을 시행하였다. 결 과 : 불면증 단기 치료 프로그램을 시행한 뒤, 치료 전보다 대상자들의 PSQI 총점이 감소하였으며 PSQI 로 측정한 주관적 수면의 질이 유의하게 호전되었다. 치료 전후에 시행한 수면 일지를 비교한 결과에서도 개운함 및 수면 후 각성 시간에 대해서 유의한 수준의 호전을 보였다. 결 론 : 개발된 불면증 단기 치료 프로그램은 지역 사회 노인의 불면증에 대해 긍정적인 치료 효과를 보이는 것으로 나타났다 기존의 불면증 인지행동치료에 비해 시간적, 인지적 자원 부담이 적음을 고려하였을 때 불면증 단기 치료 프로그램은 지역 사회 내에서 노인 불면증 치료에 대한 접근성을 확대하는 데 사용할 수 있을 것이다.
Stroke is a leading cause of death in most developed countries and some developing countries including South Korea. It is well known that stroke has is related in some way with several sleep disorders. At first, the onset time of stroke varies according to circadian rhythm. Early morning is the most prevalent time and late evening the least. The changes of blood pressure, catecholamine level, plasminogen activity and aggregation of platelet during sleep have been suggested as possible mechanisms. Sleep apnea (SA), a representative disorder in the field of sleep medicine, is found in more than 70% of acute stroke patients compared to 2-5% of the general population. Various sleep related breathing disorders occur after stroke and snoring is a distinct risk factor for stroke. So the relationship between stroke and SA is obvious, but the cause and effect are still not clearly known. Also, stroke may cause many sleep related problems such as insomnia, hypersomnia, parasomnia and changes in sleep architecture. Patients, family members and even medical personnel often ignore stroke-related sleep problems, being concerned only about the stroke itself. The clinical impacts of sleep problems in stroke patients may be significant not only in terms of quality of life but also as a risk factor or prognostic factor for stroke. More attention should be paid to the sleep problems of stroke patients.
배 경:인간에 있어서 애착은 각종 정신생리적 증상과 관계가 깊다고 알려져 있다. 그러나, 현재까지 성인기 애착 유형과 수면과의 관계에 대한 연구가 부족하며, 애착 유형과 불면증과의 관계에 대한 연구는 거의 이루어지지 않았다. 따라서, 본 연구에서는 지역사회 구성원을 대상으로 성인기 애착 유형과 불면증과의 관계에 대해 알아보려고 하였다. 방 법:지역사회에 거주하는 19세 이상 성인177명(남자 74명, 여자 103명, 평균연령 $41.23{\pm}8.44$세)이 연구에 참여하였다. 연구참여자들의 애착 유형(안정형, 거부형, 의존형, 두려움형)을 평가하기 위해 자기보고형 대인관계 유형 설문지를 작성하게 하였다. 또한, 불면증의 임상양상을 조사하기 위해 지난 한달간 불면증의 존재, 형태, 빈도, 기간에 대한 설문도 작성하게 하였다. 결 과:불면증이 있는 경우 두려움형 애착 점수가 높았으며(t=2.87, p=0.005), 이러한 상승은 모든 유형의 불면증에서 나타났다(초기 불면, t=2.33, p=0.021;중기 불면, t=2.92, p=0.004;후기불면, t=2.89, p=0.004). 또한, 주 3회 이상의 잦은 불면증이 있는 경우 간헐적인 불면증에 비해 두려움형 애착 점수가 더 높았다(t=2.57, p=0.012). 반면, 6개월 이상 지속된 만성 불면증이 있는 경우 일과성 불면증에 비해 의존형 애착 점수가 더 높았다(t=2.57, p=0.012). 결 론:본 연구에서는 불면증 유무에 따라 애착 유형의 차이가 있었다. 두려움형 애착 유형은 다양한 형태의 불면증과 관련이 있었으며, 의존형 애착은 불면증상의 만성화와 관련이 있었다. 이는 성인기 애착 유형과 불면증의 임상 양상과 서로 관련이 있음을 시사한다.
목 적 : 불면증은 대표적인 수면 질환이다. 최근 연구에 의하면 인지적 신체적 각성이 불면증을 야기하는 주요 역할을 한다. 아울러 대뇌 피질의 과각성으로 인한 정보처리과정 장애가 정상적인 입면과 수면의 연속성을 방해한다는 연구 결과도 있다. 뉴로피드백은 행동치료의 한 방식으로 피검자의 뇌파에 영향을 미쳐서 대뇌 과각성을 감소 시킬 수 있다. 이에 본 연구에서는 불면증 환자에서 뉴로피드백 치료가 뇌파 특성에 미치는 영향을 분석하고자 한다. 방 법 : 본 연구는 불면증 진단기준을 만족하는 피검자 13명과 성별 및 연령이 매칭된 대조군 14명을 대상으로 진행하였다. 뉴로피드백 치료와 Sham 치료를 무작위로 각각 30분씩 시행하였다. 각각의 치료 세션 중 뇌파를 측정하고 스펙트럼 분석을 시행하여, 뉴로피드백 치료가 뇌파 스펙트럼에 미치는 영향을 비교 분석하였다. 결 과 : 불면증 환자에서 치료적인 1회기 뉴로피드백을 한 경우, Sham 치료를 한 경우에 비해서 세타 및 시그마 파워($13.9{\pm}2.6$ vs. $12.2{\pm}3.8$ and $3.6{\pm}0.9$ vs. $3.2{\pm}1.0$ in %, respectively ; p < 0.05)가 통계적으로 유의한 수준으로 증가하였다. 그 외 뇌파상으로 통계적으로 유의미한 변화는 없었다. 결 론 : 본 연구는 국내 최초로 불면증 환자에서 1회기 뇌파 뉴로피드백을 통해 입면에 도움이 되는 세타파의 비율이 증가하는 것을 확인하였으며, 이는 입면주기를 앞당길 수 있는 새로운 방법에 대한 제안을 줄 수 있다. 불면증 자체의 치료 반응을 평가하지 못한 제한점은 있으며, 향후 불면증상의 변화까지 평가할 수 있는 후속 연구가 이어져야 한다.

${\gamma}$-Aminobutyric acid (GABA), a major inhibitory neurotransmitter in the mammalian central nervous system, is involved in sleep physiology. Caffeine is widely used psychoactive substance known to induce wakefulness and insomnia to its consumers. This study was performed to examine whether GABA extracts from fermented rice germ ameliorates caffeine-induced sleep disturbance in mice, without affecting spontaneous locomotor activity and motor coordination. Indeed, caffeine (10 mg/kg, i.p.) delayed sleep onset and reduced sleep duration of mice. Conversely, rice germ ferment extracts-GABA treatment (10, 30, or 100 mg/kg, p.o.), especially at 100 mg/kg, normalized the sleep disturbance induced by caffeine. In locomotor tests, rice germ ferment extracts-GABA slightly but not significantly reduced the caffeine-induced increase in locomotor activity without affecting motor coordination. Additionally, rice germ ferment extracts-GABA per se did not affect the spontaneous locomotor activity and motor coordination of mice. In conclusion, rice germ ferment extracts-GABA supplementation can counter the sleep disturbance induced by caffeine, without affecting the general locomotor activities of mice.

This study was investigated to know whether pachymic acid (PA), one of the predominant triterpenoids in Poria cocos (Hoelen) has the sedative-hypnotic effects, and underlying mechanisms are mediated via ${\gamma}$-aminobutyric acid (GABA)-ergic systems. Oral administration of PA markedly suppressed locomotion activity in mice. This compound also prolonged sleeping time, and reduced sleep latency showing synergic effects with muscimol (0.2 mg/kg) in shortening sleep onset and enhancing sleep time induced by pentobarbital, both at the hypnotic (40 mg/kg) and sub-hypnotic (28 mg/kg) doses. Additionally, PA elevated intracellular chloride levels in hypothalamic primary cultured neuronal cells of rats. Moreover, Western blotting quantitative results showed that PA increased the amount of protein level expression of $GAD_{65/67}$ over a broader range of doses. PA increased ${\alpha}$- and ${\beta}$-subunits protein levels, but decreased ${\gamma}$-subunit protein levels in $GABA_A$ receptors. The present experiment provides evidence for the hypnotic effects as PA enhanced pentobarbital-induced sleeping behaviors via $GABA_A$-ergic mechanisms in rodents. Taken together, it is proposed that PA may be useful for the treatment of sleep disturbed subjects with insomnia.
