This study was performed to investigate the effect of kangaroo care on body weight, physiological responses and behavioral states in premature infants. The subjects were 32 premature infants, fifteen for the kangaroo care group and seventeen for the control group, who hospitalized in a neonatal intensive care unit at a university-affiliated hospital. The kangaroo care was applied 8 times during the twenty five days with 40 minutes for each session. The kangaroo care provides the skin-to-skin contact during which a premature infant wearing a diaper and a hat is placed on its mother's chest. As for the measures, body weight was measured everyday. The levels of epinephrine, norepinephrine and 17-OHCS were measured twice, before beginning the first intervention and after finishing the last intervention. While each session of the care is undergoing, such physiological responses were measured periodically as heart rate, respiration rate, oxygen saturation, core temperature and skin temperature. The results were as follows : 1. The weight gain was significantly greater in the kangaroo care group than that in the control group during the period of performing the kangaroo care. 2. No significant difference was revealed between the two groups in heart rate, respiration rate, oxygen saturation and core temperature. The kangaroo care group also showed significant increases in the skin temperature. 3. The differences in the levels of epinephrine, norepinephrine and 17-OHCS were not significant between the kangaroo care and the control groups. The level of norepinephrine in the two groups was significantly increased over time. 4. Sleep pattern changed significantly in the kangaroo group from a very restless sleep to a very quiet sleep. These results suggest that kangaroo care is an effective nursing intervention for premature infants in gaining weight, achieving stable physiological responses and facilitating a quiet sleep.
Background: We aimed to analyze the factors predicting the diagnostic performance of flexible bronchoscopy without guidance in peripheral lung lesions that are endoscopically invisible. Methods: This was a retrospective study conducted in St. Paul's Hospital, The Catholic University of Korea, between January 2007 and March 2013. We included all patients who received bronchoscopy during this period. The analyzed variables were age, sex, the etiology of the lesion, lesion size, distance from the pleura, and presence of the bronchus sign. We used multiple logistic regression analysis to identify the significant independent factors associated with diagnostic yield. Results: We included 151 patients in this study. The overall diagnostic yield was 58.3%. The sensitivity was 43.2% for malignant disease and 78.1% for benign disease. The benign lung lesions (p<0.001), lesion size (p=0.015), presence of the exposed type of bronchus sign (p<0.001), and presence of cavitary lung lesions (p=0.005) were factors influencing the yield of flexible bronchoscopy by univariate analysis. In a multivariate logistic regression analysis, the exposed type of bronchus sign and benign lung lesions were independent predicting factors (odds ratio [OR]: 27.95; 95% confidence interval [CI], 7.56-103.32; p<0.001 and OR, 4.91; 95% CI, 1.76-13.72; p=0.002). Conclusion: The presence of the exposed type of bronchus sign and benign lung lesions are determining factors of the diagnostic yield in flexible bronchoscopy in evaluating peripheral lesions that are not endoscopically visible.
Sounds that have been evaluated to be effective in inducing sleep are helpful to reduce sleep disorders. Generally, several sounds have been verified the effects by brainwave experiments, but it cannot be considered on all users because of individual variation for effects. Moreover, the effectiveness for inducing sleep is not known for all new sounds made by creative activities. Therefore, new classification system is required to collect new effective sounds with considering personal brainwave characteristics. In this paper, we propose a new sound classification method by applying improved MinHash cluster to brain waves. The proposed method will classify them through whether it is effective for sleep care by evaluation his brainwave during listening for each sound. In order to prove effectiveness of the proposed classification method, we conducted accuracy experiment for sleep sound classification using verified sleep induction sound. In addition, we have compared time for existing method and proposed method. The former is scored 85% accuracy in the experiment. We confirmed the latter one that the average processing time was reduced to 70%. It is expected to be one of method for pre-screening whether it is effective when a new sound is introduced as a sound for sleep induction.
Purpose: The purpose of this study was to examine the relationship between sleep duration, quality of sleep and depression, and to identify the factors associated with depression in middle school students. Methods: This study used a cross-sectional design with secondary data, middle school student panel data from the Korean Children and Youth Panel Survey (2018), and data from a total of 2,590 students was used for analysis. For the statistical analysis, t-test, ANOVA, and hierarchical multiple regression were performed using SPSS ver. 26. Results: The mean depression score in adolescents was 17.99±6.38. Depression had significant differences according to sleep duration and quality. Adolescents with less than eight hours of sleep showed the highest depression, and poor sleepers showed higher depression scores than good sleepers. Gender, school achievement, school satisfaction, economic status, and exercise time were found to significantly relate to depression. After controlling for general characteristics, depression explained 17.2% of the variance in quality of sleep (β=-.15, p<.001) and sleep duration (β=-.04, p=.022). Conclusion: These findings indicate that it is necessary to develop intervention strategies to enhance the quality of sleep and appropriate sleep duration for preventing depression in adolescents.

Purpose: The purpose of this study was to examine effects of music intervention on environmental stress and sleep quality in liver transplant patients who are receiving care in one-person isolation rooms of an ICU. Methods: The study was a quasi-experimental design pre-and-post nonequivalent control group. Participants were 37 patients (18 in the experimental group and 19 in the control group) who, after receiving liver transplant, were hospitalized in one-person isolation rooms of the ICU. The study covered patients admitted between August 2016 and December 2017. Earplugs and sleep shades were provided as ordinary care to both experimental and control groups, and music intervention was provided to the experimental group three times a day for 30 minutes each from the first day of hospitalization. Results: The first hypothesis, "The experimental group who received music intervention will experience a lower environmental stress level than the control group" was supported (Z=-3.212, p<.001). The second hypothesis, "The experimental group who received music intervention will experience a higher sleep quality than the control group" was also supported (t=3.715, p=.001). Conclusion: Findings show that music intervention is an effective nursing intervention to reduce environmental stress and improve sleep quality in liver transplant patients in the ICU.
Purpose : This study examined the prevalence of delirium-related factors in a surgical intensive care unit (SICU). Methods : This retrospective study enrolled 73 patients who were admitted to an SICU from October 1, 2016 to March 20, 2017 and who had been hospitalized for more than 72 hours. Data was collected by reviewing electronic medical records. Results : Delirium occurred in 46 (63.0%) patients. Its related factors were age, education, mechanical ventilator, sleep, narcotics, physical restraint, and central line catheters. Conclusion : The results indicate that sleep and physical restraint are significant factors related to delirium occurrence. The results of this study can help in developing guidelines for the prevention of delirium.
Obstructive sleep apnea (OSA) has been definitively shown to be a risk factor for the development of cardiovascular disease and mortality. Recent reports have indicated that obstructive sleep apnea is associated with insulin resistance and impaired glucose metabolism, also have type 2 diabetes. The potential mechanisms leading to the development of type 2 diabetes in OSA patients are likely to be various. Reduced physical activity resulting from daytime somnolence, sympathetic nervous system activation, intermittent hypoxia, sleep fragmentation and sleep loss, dysregulation of the hypothalamic-pituitary axis, alteration in adipokine profiles, and activation of inflammatory pathways have been proposed. Based on the current evidence, clinicians should assess the risk of OSA in patients with type 2 diabetes and, conversely, consider that possibility of glucose intolerance in patients with OSA. Further large-scale and long-term follow-up studies in patient populations with selected by reliable but inexpensive diagnostic measures, controlled for potential confounder factor, are needed.
Purpose: The study aimed to examine the effects of aroma hand massage on state sanxiety, depression, quality of sleep, and blood pressure levels of palliative patients in hospice. Methods: This study was conducted using a nonequivalent control group with a pretest-posttest design. The participants were 48 palliative patients in hospice (experimental group: 24, control group: 24). The aroma hand massage was given to the experimental group once a day for 5 days. Data were collected between December 2017 to February 2018. Data were analyzed using SPSS 20.0 program with the chi-square test, Fisher's exact test, independent t-test, and repeated measurement analysis of variance(ANOVA). Results: There were significant differences in state anxiety levels (t=2.41, p=.020) and quality of sleep (F=14.29, p<.001). However, significant differences in the levels of depression (t=1.59, p=.119), systolic blood pressure (F=0.37, p=.695), and diastolic blood pressure (F=0.37, p=.695) were observed. The aroma hand massage was effective in improving the quality of sleep and state anxiety levels of palliative patients in a hospice. Conclusion: The aroma hand massage was effective in improving the quality of sleep and state anxiety in a hospice.
Purpose: This paper was aimed to investigate the effect of laugher therapy on the pain, depression and sleep disturbance in elderly patients who admitted in long term care facility. Method: This study used a nonequivalent control group pretest-posttest design. A sample of 40 elderly patients (20: experimental group, 20: control group, 65 or older) were recruited. Visual analogue scale was used to measure level of pain level, Geriatric Depression Scale Short Form Korea Version (GDSSF-K) for depression. Sleep Scale for sleep. The laugher therapy was given twice a week for four weeks (8 times). Each therapy consists of preparatory, action and finishing stage. Data were analyzed by PASW 18.0. Results: The pain score in experimental group was significantly different from that in control group (t=4.17, p<.001). The level of depression in experimental group was significantly different from that in control group (Z=4.12, p<.000). The level of sleep in experimental group was not significantly different from that in control group (Z=-1.43, p<.152). Conclusion: A laugher therapy is expected to be practical used an efficient method of a nursing intervention to elderly patients in long term care facility.
Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.
