The purpose of this study is to analyze the characteristic of living environment on nursing homes and to examine the current status and problems of the nursing homes by looking into the differences in the spatial uniqueness of living environment in relation to the characteristics of the nursing home. This study utilized a mail survey during July 29 - August 31, 2006 that included a structured questionnaire. The questionnaire largely dealt with the general feature of the facility and the elderly who live there, and the spatial feature of the facility. The study limited its survey to those facilities that were registered to the Ministry of Health and Welfare, which opened after 1995 and that had the capacity of more than 30 people. As a result, the questionnaire was sent to the directors of 344 facilities who were in charge of the nursing home operation and planning. 182 facilities returned the questionnaire and the study used'182 questionnaires for data analysis. The study used the SPSS 12 program to draws the frequency, percentage of the answers and utilized the cross-tab, t-test, ANOVA to analyze the differences.
The purpose of this study was to provide basic information about the facility planning to reduce elderly isolation and to improve their rehabilitation ability. Four re%archers observed the using behavior of the elderly residents in the corridor and lounge of 5 facilities. The results of the study were as follows. The using rate of the lounge wis higher than that of the corridor since most of the elderly were controlled by the staffs at the lounge. The facility could decrease the crowd of the lounge by arranging the sofa at the corridor-especially surrounding the atrium. The use rate in the lounge was very dependent on the time of the day, because its use was influenced mainly by daily schedule, and this resoled in it being largely inactive most of the time. A counter plan is needed to encourage natural interaction among the elderly. Individual or small group activities are required in addition to the regular programs. Since the furniture arrangements_(sofas and tables) at the corridor could induce elderly isolation and inactivity, various equipments should be arranged for the self-controlled activities. The indirect participation in the regular activities, which are usually conducted in the lounge, should also be considered in the corridor so that the elderly could observe them at any places.

The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in nursing homes. The thorough investigation and observation works were made from the view points of daily living behaviors and behavioral places of the recuperating elderly with dementia in nursing homes. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in 3 nursing homes. The results of this study are summarized as follows; There is no wide difference between skilled nursing facilities on the characteristic of living behaviors of the elderly with dementia. Daily living activities and the use of spaces are largely influenced by the daily program and the operation policy of each nursing homes in addition to human relationship of the elderly with dementia. Common spaces such as day room, dining room plays an important role for their daily living activities, because that most of the elderly with dementia stay long during daytime. Layout of each private room and common space seems very crucial point in designing these facilities, in order to enable demented people to live their lives more independently.
Purpose: Nursing homes are a type of residential care that provide diverse services such as planned activities, convalescent care, skilled nursing and memory care. Nursing homes are good facilities for old people with dementia to improve their quality of life. On the other hand, the subjectivity toward nursing homes can vary to greater or lesser degrees. The purpose of this study was to identify the subjective attitudes and viewpoints toward nursing homes in undergraduate nursing students. Methods: The Q methodology, which is a research method to study the people's subjectivity, was used. The 40 Q-statements selected from each of 43 participants were classified into the shape of a normal distribution. The data collected were analyzed using the QUANL program. Results: Three types of subjectivity toward nursing homes emerged: 'Type I, in terms of usage of nursing homes-hesitative perspectives;', 'Type II, in terms of running of nursing homes-progressive perspectives;', and 'Type III, in terms of admission to nursing homes-constructive perspectives'. Conclusion: The findings of this study suggest that nursing intervention programs should be developed for the three types. The study results will lead to further research.
The purpose of this study was to provide basic information about efficient space use in the dining and bathing area through the analysis of service flowing. Four researchers observed the service flowing and the using behavior at those areas. The results of the study were as follows: Dining service was proceeded as resident moving, waiting, meal serving, dining, moving and arranging in order. The waiting stage was one of the problematic processes since the staffs made the residents wait to) long at a fixed position. The program right before the meal serving will be helpful for reducing tediousness of the elderly residents. Another problem was that the area was not big enough for the meal sowing. The legal regulation Is needed to prescnbe for the size of dining area per resident. The flowing of bathing service was proceeded as staff preparation, moving, waiting, undressing, bathing, drying, dressing, moving and arranging in order. There were more problems in the dressing area than in the bathing area. The elderly with stretcher or wheelchairs had difficulty in entering the narrow doorway. The dressing area was so crowded with the staffs, undressed elderly, dressed elderly, and other laundries. The division of dressing and undressing area is required to avoid the confusion of the users in the area.
Purpose: The purposes of this study were to identify the factors influencing service quality in nursing homes, and to develop an evaluation instrument for service quality. Methods: A three-phase process was employed for the study. 1) The important factors to evaluate the service quality in nursing homes were identified through a literature review, panel discussion and focus group interview, 2) the evaluation instrument was developed, and 3) validity and reliability of the study instrument were tested by factor analysis, Pearson correlation coefficient, Cronbach's ${\alpha}$ and Cohen's Kappa. Results: Factor analysis showed that the factors influencing service quality in nursing homes were healthcare, diet/assistance, therapy, environment and staff. To improve objectivity of the instrument, quantitative as well as qualitative evaluation approaches were adopted. The study instrument was developed with 30 items and showed acceptable construct validity. The criterion-related validity was a Pearson correlation coefficient of .85 in 151 care facilities. The internal consistency was Cronbach's ${\alpha}$=.95. Conclusion: The instrument has acceptable validity and a high degree of reliability. Staff in nursing homes can continuously improve and manage their services using the results of the evaluation instrument.

People are living longer than ever before. As a result, life expectancy is going up and the demand of long-term care facilities is increasing in most countries. The facilities provide rehabilitative, restorative, and skilled nursing care to patients or residents in need of assistance with activities of daily living. This study deals with the capacitated location and allocation problem of long-term care facilities in a city that consists of a finite number of regions. Assuming that in each region candidate locations for three types of facilities are already given, two integer programming models are developed under the closest assignment rule reflecting the demand characteristics of the facilities. Both the location and type of the facilities to be built become decision variables. To show the validity of the models, numerical problems are solved with commercial software, CPLEX. Also, sensitivity studies were conducted to identify relationships between the system parameters.

The purpose of this study is to clarify the daily living activities of the elderly with dementia and user oriented spatial needs of the staff in the common space of nursing home in Japan. Data was taken from six units of the 2 skilled facilities. The results revealed as follows; 4 types of elderly activities were founded, and physical environmental characteristics affected residents' space usage. And it was perceived like as living oriented space, social interaction space, facility space, staff oriented space. Conclusively it was suggested to plan dining space as semi-private space, day corner as semi-public space.
The purpose of this study is to measure the resource use of the elderly in long-term care services and to examine the effects of patient and facility characteristics on their use of resources. The data were collected from 510 old people over sixty years of age, residing in five long-term care hospitals and two skilled nursing homes during the period between December 1, 2000 and February 28, 2001. For a full sample, when the first level of RUG(Resource Use Group)-III categories were employed as the proxy of patient severity, facility characteristics, such as location, size and ownership, have large effects on the resource use measured by service intensity, whereas patient characteristics such as severity have little or no effect. The resource use is significantly high if the facility: (1) is located in rural areas (gun): (2) has mare than 200 beds; (3) is a long-term care hospital; (4) is private; and (5) has a low percentage of medical aid patients. The analysis of the resource use in each RUG-III categories, for which ADL(Ability of Daily Living) were employed as the prosy of patient severity, shows a similar result. The loose relationship between the needs of residents and the resource use seems to be closely associated with the ineffective reimbursement system for providers. The current reimbursement system has no provision for quality improvement and reimburses facilities simply according to their types: fee-for-service for long-term care hospitals, and monthly-flat-rate or full-coverage-national-aid for skilled nursing facilities. It will be necessary to develop a more reasonable reimbursement system that takes patient's severity into account and gives incentives for long-term care providers to offer cost-effective services.
This study is about major symptoms of elderly and medical services for elderly in long-tenn care facilities. The subject of this study was 298 patients over 00 years old staying in two geriatric hospitals and two nursing homes. The symptoms and medical services were level of patient classification from RUG(Resource Utilization Group)-III which is applied for both Medicare and Medicaid for skilled nursing facilities reimbursement system in US and designed for measuring patient characteristics and medical staff time. This classification is explained by each patient resource(staff time) utilization level which is called CMI(Case-Mix Index). In this study, the symptoms and services were compared by facility type and they were categorized by level and compared by CMI. Major findings are as follows; 1. There were more elderly who have cognitive function problems in nursing homes than patients in geriatric hospitals. There were more patients with behavioral problems in geriatric hospitals than residents in nursing homes. These results were both statistically significant. 2. The patients in geriatric hospitals received significantly more nursing rehabilitation services, rehabilitation services and extensive services than residents in nursing homes. Other hands, special care services were provided significantly more to residents in nursing homes than elderly in geriatric hospitals. 3. ADL and depression variables had higher CMI when the symptoms were heavier condition. The CMI were not matched with levels of cognitive function problems and behavioral problems. 4. The CMI matched well significantly with levels of nursing rehabilitation services, special care services, and clinically complex services provided for the patient in geriatric hospitals and only nursing rehabilitation services in nursing homes. The CMI for rehabilitation services level and extensive services had regular trends. From the result of this study, the resource utilization level and services provided for elderly in each long-term care facilities were figured out. For the further study, it needs to have more concern about RUG-ill which classification variables were just analyzed.
