Effects of fibrin glue on bone formation in combination with deproteinized bone xenografts in humans

  • Thirty-six sinus grafts were performed in 34 patients with an alveolar crest bone height in the posterior maxilla of 3 to 5 mm before grafting. The sinuses were grafted using Bio-Oss alone or mixed with fibrin glue. Group 1 was the control group and included 25 patients who received a xenograft mixed in saline. Group 2 comprised 9 patients who received a xenograft and fibrin glue. The study was further subdivided at the time of 9 months. This histologic study evaluated by hematoxylin-eosin (H&E) and histomorphometric analysis whether fibrin glue in combination with Bio-Oss enhances bone regeneration in sinus floor elevation in humans. The new bone formation was better in Group 2 than in Group 1, but the difference was not significant. The absorption of the graft material was faster in Group 2 than in Group 1, in the short term, but better in Group 1 over the long term, although the difference was not significant. Lamellar bone was formed earlier in Group 1 compared to Group 2, but the difference was not significant. Overall, the surgery site stabilized earlier with new bone formation in Group 2 than in Group 1, but the difference was not significant. Combining a fibrin sealant and Bio-Oss could lead to improved scaffolds for bone tissue engineering based on the synergistic effects of the biomaterials. Therefore, Bio-Oss or Bio-Oss plus Tisseel may be used depending on the situation.

전악 무치악 환자에서 골이식술을 선행한 임플란트 고정성 보철 수복 증례 (Full mouth rehabilitation of fully edentulous patient with implant-supported fixed prosthesis preceding bone graft: A case report)

  • 완전 무치악 환자에서 임플란트를 이용한 보철 치료는 임플란트 고정성 보철수복, hybrid 고정성 보철수복, 임플란트 유지 혹은 지지 피개의치 등이 있으며 임플란트 고정성 보철 수복은 환자의 심리적 안정감, 발음 등의 측면에서 장점을 보인다. 전악 임플란트 고정성 보철 수복을 계획한 경우 발음, 심미, 구강 위생 등을 고려한 임플란트 식립이 필요하며 이를 위해서는 임상적 및 방사선학적 진단이 필수적이다. 진단 과정을 통해 파악된, 보철물이 필요한 곳에 임플란트를 식립(prosthetic-driven)하기 위해서는 충분한 양의 치조골과 연조직 지지 등이 필요하며 이를 충족시키지 못할 경우 광범위한 골이식술이 선행되어야 한다. 본 증례는 치주질환으로 인한 치아 상실로 인해 치조골 흡수가 심하게 진행된 완전 무치악 환자에서 상악동 골이식술 및 치조골 증대술을 선행하고 임플란트 식립 후 전악 임플란트 고정성 보철로 수복하여 기능 및 심미적으로 만족할만한 결과를 얻었기에 이를 보고하고자 한다.

  • Platelet-rich plasma is an autologous source of platelet-derived growth factor and transforming growth factor beta that is obtained by sequestering and concentrating platelets by gradient density centrifugation. We have used platelet-rich plasma for bone graft, especially allobone graft, at implant surgery, sinus lift procedure, and cyst enucleation. This article is retrospective study from October 1999 to November 2000. All cases were good healing and clinical success.

두개골 조기 유합증 수술 시 두개골막 피판의 역할 (The Role of Pericranial Flap in Surgery of Craniosynostosis)

  • Reconstruction of calvarial bone defects from congenital anomaly or from bone loss due to traumatic or neoplastic processes remains a significant problem in craniofacial surgery and neurosurgery. To facilitate bone regeneration, there have been many trials such as autologous bone graft or allograft, and the addition of demineralized bone matrix and matrix-derived growth factor. Guided bone regeneration is one of the methods to accelerate bone healing for calvarial bone defects especially in children. Pericranium is one of the most usable structure in bone regeneration. It protects the dura and sinus, and provides mechanical connection between bone fragments. It supplies blood to bone cortex and osteoprogenitor cells and enhances bone regeneration. For maximal effect of pericranium in bone regeneration, authors used pericranium as a flap for covering calvarial defects in surgeries of 11 craniosynostosis patients and achieved satisfactory results: The bone regeneration of original cranial defect in one year after operation was 74.6%(${\pm}8.5%$). This pericranial flap would be made more effectively by individual dissection after subgaleal dissection rather than subperiosteal dissection. In this article, we reviewed the role of pericranium and reported its usefulness as a flap in surgery of craniosynostosis to maximize bone regeneration.

구강악안면 영역의 재건을 위한 경골 근위부 내측 골이식술의 후향적 분석 (A RETROSPECTIVE ANALYSIS OF THE MEDIOPROXIMAL TIBIAL BONE GRAFT FOR ORAL AND MAXILLOFACIAL RECONSTRUCTION)

  • Tibial bone grafts provide an adequate volume of cancellous bone with cortical bone, high biologic value of bone, minimal gait disturbance and complications, and no special contraindications, and offer a superior clinical results than any other donor sites. Lateral appoach in tibial bone graft was used to gain large bone volume traditionally, but medial approach provides low morbidity associated with the tibial anatomic structure, simple and safety surgical procedure, and better comfortable to patients recently. We have undertaken clinical and retrospective studies on patients in Dept. of Oral and Maxillofacial Surgery, Inha University Hospital from April 2004 to January 2008. 50 patients have maxillofacial bony defect as resection of bening tumor, cyst enucleation, alveolar bone resorption, sinus pneumatization were received the tibial proximal autogenous particulated cancellous bone grafts. They were analyzed sex, age, diagnosis of recipient site, lesion size, dornor site, cortical bone repositioning, complications and we concluded favorable following results. 1. Medial approach for proximal tibia is safer and technically easier than lateral approach, associated with the proximal tibial anatomic structures, and short operative times. 2. Proximal tibia provides an adequate bone volume with predictability for oral and maxillofacial reconstruction. 3. Patients rarely complain of pain, swelling, discomfort and dysfunction such as gait disturbance. In conclusion, medial approach for proximal tibial graft seems to be a valuable tool for oral and maxillofacial reconstruction.

한국인에서의 상악동 골이식술과 관련된 상악동 동맥 분포에 대한 예비 연구 (ARTERIAL ARCADE OF THE MAXILLARY SINUS RELATED TO SINUS BONE GRAFT IN KOREAN POPULATION ; A PRELIMINARY STUDY USING COMPUTED TOPOGRAPHIES)

  • Aim: The aim of this study was to investigate the arterial arcade of maxillary sinus by determining the distance between the alveolar crest and the inferior border of the vessels in Korean population. Materials and methods: 76 patients and 87 available sinuses were determined by CT scan (Implagraphy, Vatech, Seoul, Korea). The distance between alveolar crest and inferior border of the vessel was determined considering the tooth area of posterior maxilla. Results: The lowest distance was observed on first molar area and the average distance is 16.9 mm. Discussions: it is important in sinus lift surgery that the vessels not to be damaged and that the anastomosis is situated in the lateral wall of maxillary sinus, which has to be exposed to gain access to the maxillary sinus. The knowledge of arterial arcade of maxillary sinus is important to perform the sinus floor elevation procedure effectively.

Primary sinonasal mucosal melanoma simulated as cystic lesions: a case report

  • Sinonasal mucosal melanoma (SNMM) in the maxillary sinus is a rare disease condition. Compared to oral mucosal melanoma, SNMM has a bulky, exophytic, and polypoid appearance, is weakly pigmented, and associated with unspecific symptoms. Due to these features, SNMM in the maxillary sinus has been misdiagnosed as nasal polyps and chronic sinusitis. In this case report, we described SNMM occurring in the right maxillary sinus simulated as a cystic or benign lesion. Cortical bone thinning and expansion were observed around the mass. The excised soft mass was encapsulated and weakly pigmented. The mass was clearly excised and covered with a pedicled buccal fat pad graft. Diagnosis using immunohistochemistry with S-100 and homatropine methylbromide-45 (HMB-45) is critical for proper treatment.

외상성 상악동 안구탈출의 치험례: 증례보고 (Traumatic Displacement of the Globe into the Maxillary Sinus: Case Report)

  • Purpose: Globe displacement due to a blowout fracture is a rare clinical phenomenon. The authors present reduction of a globe displacement to the maxillary sinus due to trauma suffered in a fall and the reconstruction of a large defect left in the medial and inferior orbit. Methods: A 39-year-old male patient was unable to open his left eye after being struck on the periorbital area by a metal edge. Laceration was not noted in that area but we were unable to observe the intraorbital globe. A facial computed tomography (CT) scan showed that the globe was displaced through the maxillary sinus. A transconjunctival approach was used to access the infraorbital margin and the globe entrapped in the inferior margin of the orbit was successfully reduced. A large defect in the medial and inferior orbit was reconstructed using a graft from the iliac bone. Results: In 5 months after the operation, no atrophy of the globe was seen. Both sides retained a similar shape. A satisfactory functionality outcome in terms of improved extraocular muscle movement, and a satisfactory aesthetical outcome were achieved. Conclusion: The authors report the reduction of a globe displaced to the maxillary sinus following a fall and the reconstruction of the large defect left in the medial and inferior orbit.

Bio-Oss와 β-TCP를 이용한 토끼 상악동 거상술 후의 조직학적 비교 연구 (Histological Comparative Study of Rabbit Maxillary Sinus Augmentation with Bio-Oss and β-TCP)

  • 본 연구는 토끼의 상악동에 Bio-Oss와 ${\beta}-TCP$를 이식한 후 골 재생 과정을 조직학적으로 비교하기 위하여 수행되었다. 12마리의 수컷 토끼를 4마리씩 3군으로 구분하였다. 토끼의 양쪽 상악동 점막을 거상한 후 한쪽은 Bio-Oss를 이식하였고, 다른 쪽은 ${\beta}-TCP$를 이식한 후 본래의 골편으로 창을 봉합하였다. 2주, 4주 및 8주 후에 토끼를 희생시키고 조직절편을 만든 후 Bio-Oss군과 ${\beta}-TCP$군의 골 재생 양상을 비교하였다. 조직계측학적 분석을 위하여 각각의 조직절편에 hematoxylin-eosin, Masson trichrome 및 tartrate-resistant acid phosphatase 염색을 하였고, proliferating cell nuclear antigen (PCNA), type I collagen 및 osteocalcin의 발현 양상을 비교하기 위하여 면역조직화학 염색을 하였다. 광학현미경 시야에서 계측프로그램을 이용하여 계측하고 분석하여 다음과 같은 결과를 얻었다. Bio-Oss군과 ${\beta}-TCP$군 모두 2주군에서 초기 골 형성이 시작되었고, 4주군에서는 각 이식재 입자의 표면에 형성된 다량의 신생골이 관찰되었으며, 8주군에서는 신생골의 골량이 더 증가되어 있었고 층판골과 골수조직도 관찰되었다. 신생골의 양은 4주군까지 두 군이 비슷하였으나, 8주군에서는 ${\beta}-TCP$군에서 유의한 증가가 계측되었다. 이식재의 양은 4주군부터 8주군까지 ${\beta}-TCP$군에서 유의하게 감소되었으며, 파골세포의 수는 4주군에서 8주군까지 ${\beta}-TCP$군에서 유의하게 증가되어 있었다. PCNA에 대한 면역반응성은 8주군에서 감소되었지만 두 군간의 유의한 변화는 없었다. Type I collagen의 발현은, 2주군에서는 ${\beta}-TCP$군에서 유의하게 증가되었지만, 8주군에서는 Bio-Oss군에서 유의한 증가가 관찰되었다. Osteocalcin에 대한 면역반응성은 8주까지 증가되었으며 두 군간의 유의한 차이는 없었다. 이러한 조직학적 결과들은 임플란트를 위한 이식재의 선택에 도움을 줄 수 있다. 이상의 결과들을 종합하면 Bio-Oss와 ${\beta}-TCP$는 모두 검증된 이식재 이지만, 토끼의 상악동 거상술에서는 ${\beta}-TCP$가 좀 더 우수한 골 재생 결과를 보였다.

자가치아를 이용한 골이식재의 임상적 유용성: 일차 보고 (Clinical Effectiveness of Bone Grafting Material Using Autogenous Tooth: Preliminary Report)

  • Purpose: The purpose of this study was to evaluate the effectiveness of a novel bone grafting material using an autogeneous tooth (AutoBT) and provide the basis for its clinical application. The AutoBT contains organic and inorganic mineral components and is prepared from autogenous grafting material, thus eliminating the risk of immune reactions that may lead to its rejection. AutoBT can be used as bone material as is has both osteoinduction and osteoconduction activities at guided bone regeneration for implant placement and maxillary sinus graft. Methods: In a total of 63 patients, guided bone regeneration surgery was performed at the time of implant placement, and tissue samples were harvested at the time of the second surgery with the patient's consent. Results: There were no complications in guided bone regeneration using autogeneous tooth. Conclusion: We concluded that AutoBT underwent gradual resorption and was replaced by new bone of excellent quality via osteoinduction and osteoconduction.
