Regional Difference of Chronic Periodontal Care Services in Korea (의료기관 종별 만성 치주염 진료의 지역 간 차이)

  • Objectives: This study aimed to investigate the regional difference of chronic periodontal care services in Korea by the analysis of 2010 raw data from Health Insurance Review and Assessment Service. Methods: The subjects were the chronic periodontitis patients over 35 years old from dental care facilities in Korea. The study population was 278,319 including 264,994 claims made by dental clinics, 8,084 by dental hospitals, 3,509 by general hospitals, and 1,732 by tertiary hospitals. Results: There was a significant difference in medical care cost benefit between the provinces(p<0.0001). The age groups showed a clear difference in the patient charge, cost of insurance, and medical care cost benefit(p<0.0001). In consideration of the first visit or revisit, there were differences in the rate of prescription, dental examination, and surgical procedures of the chronic periodontal patients from dental facilities. The radiographic use rate in the tertiary hospitals was 2.6 times higher than that of the dental clinic in the treatment of the new chronic periodontal patients. Conclusions: The use of dental services in the periodontitis is influenced by the types of medical services facility, cost of medical insurance, and patient charge. In consideration of cost benefit analysis, prevention is the most important care for the periodontitis. Regional difference in peridontitis is cause by the use of medical services and quality of treatment.

  • This study compares and analyzes user experiences of intelligent personal assistant services based on the evaluation criteria of human-computer interaction to explore positive elements of user experiences and factors that could be improved. The research was conducted on Apple's Siri, Google's Google Associate, and Samsung's Bixby, which is presently the smartest personal assistant service on the market. The research method was to compare and analyze the concepts and characteristics of the current services through a literature review and by interviewing seven UI/UX design experts for the second 2 weeks using contextual inquiry. The interview reorganized Peter Morville's user experience honeycomb, reducing his seven usability principles down to five, asking questions about usability, convenience, visual attractiveness, reliability, and satisfaction. On the basis of the reconfigured usability principle, the assessment was conducted on the basis of the assessment timing and the system usability scale. This study is meaningful in that it analyzed the user experience of artificial intelligence personal assistant services both quantitatively and qualitatively.

Analysis of Medical Use and Costs of Liver Transplant Patients Using National Patients Sample Data (환자표본자료를 이용한 간이식 환자의 의료이용 특성 및 의료비용 분석)

  • Background: Patients experience significant differences in aspects of mortality, quality of life, and costs between during the year of receiving liver transplant (LT) and the subsequent years (post-LT). This study aimed to estimate the medical utilization and cost of LT for patients compared to post-LT patients by using a recent National Patient Sample (NPS) data provided by the Korean Health Insurance Review and Assessment Service (HIRA). Methods: This study used a subset of the 2015 HIRA-NPS. Patient claims data that included Z944 (Korean Standard Classification of Diseases code for LT status) were selected. Within the selected data, LT patients were identified based on whether the national health insurance number code of Q80 (procedure code for LT surgery) was included, and they were compared to post-LT patients. Results: In the analysis, 330 patients were included. The average cost per patient was $90,066{\pm}36,959$ thousand KRW and $10,557{\pm}9,668$ thousand KRW for LT and post-LT patients, respectively. Especially, LT patients' costs for injection/procedure, surgery/treatment, and examination were higher than other costs, being $35,983{\pm}18,115$ thousand KRW, $28,246{\pm}9,408$ thousand KRW, and $12,131{\pm}6,604$ thousand KRW, respectively. For inpatients, the average number of hospitalized days was $63.5{\pm}66.0$ days for LT patients and $22.3{\pm}35.1$ days for post-LT patients. Conclusion: Compared to post-LT patients, LT patients had higher costs, especially for injection/procedure, surgery/treatment, and examination. Additionally, the LT group had longer hospitalization duration and higher costs for their hospital admission, whereas they did not show a significant difference in number of visits and medical costs for outpatient-care.

Adaptation accuracy and mechanical properties of various denture base resins: a review (다양한 레진 의치상의 적합도와 기계적 특성)

  • This paper reviews the adaptation accuracy and mechanical properties of currently used denture processing systems with base resin materials and introduces the latest research on the development of antimicrobial denture base resins. Poly(methyl methacrylate) has been successfully used as a dental denture base resin material by the compress-molding method and heat polymerization for a long time, but recently, new processing techniques, injection molding-methods or fluid-resin technique are also used for fabricating denture base. However, studies indicated that there was no difference between the injectionmolding and the conventional compression-molding method in terms of adaption accuracy of denture base. The fluid-resin fabrication and one injection-molding systems exhibited better adaptation accuracy than the other processing methods. Resin denture bases in the oral cavity may undergo midline fractures due to flexural fatigue from repeated masticatory loading. For those patients, impact resistant denture base resins are recommended to prevent denture fracture during service. Thermoplastic denture base resins can be helpful for patients suffering from allergic reaction to resin monomers with a soft-fit, however, thermoplastic resins with low stiffness can irritate gum tissues and accelerate abnormal alveolar ridge resorption. Moreover, due to low chemical durability in oral cavity, those should be used for a limited period of time.

Analysis of Medical Resources according to number of beds in Korean General Hospitals (국내 종합병원의 병상규모에 따른 의료자원 분석)

  • Purpose: This study analyzed the status of general hospitals as an expanded concept of medical resources including medical staff and equipment. The purpose of this study is to provide a basic for the feasibility study of the scale and establishment of facility guidelines at the planning stage of general hospitals. Methods: The subjects of this study were limited to general hospitals. The status of medical resources was based on the data of the Health Insurance Review and Assessment Service. The number of beds, doctors, nursing grades and major medical equipment were surveyed in 335 general hospitals. Results: 1) The characteristic of general hospitals varies depending on the number of inpatient beds. To be concrete, there were differences in the number of medical staffs and equipments in general hospitals based on 300 500 800 1,000 beds. 2) As the number of hospital beds increases, the number of medical staff increases more than medical equipment and facilities. Medical equipment and facilities remain constant, even when the number of beds increases. On the other hand, the number of medical staff increased about 1.5 times in each level. Implications: Architectural plans for medical staff should be considered differently depending on the number of beds. In particular, architectural planning and facility guidelines should be applied differently based on 300 and 500 beds.

  • This study aims to investigate the components of group writing through three steps of group writing course consist of collecting information, organizing information, and revision. A total of 19 pre-service teachers who took the science argumentation course participated in the group writing activity. They made up a group of four or five and chose one subject from among socioscientific issues for group writing. The discussion contents and writing were analyzed inductively to find the group writing components at every step. The results of the study are as follows: First, components in the step of collecting information were (1) sharing information (2) understanding information. and (3)judging information. Second, components in the step of organizing information were (1) categorizing information, (2) decentralization, (3) balancing information, and (4) reflection. The last, process components in the step of revision were (1) unification of form, (2) global review, and (3) improving readability.

Study on the Lolita Complex of Korea Girl Group's School look image Fashion (국내 걸그룹 교복이미지 패션에 나타난 롤리타 콤플렉스(Lolita Complex))

  • This study systematically identified the influence of the school look fashion image on public culture, which is used for increasingly sexualized marketing appeal by domestic Girl Groups. We examined and analyzed the school look fashion image of Girl Groups, focusing on the Lolita complex which is particularly influential in the sexual appeal of domestic popular fashion. The method of this study is based on a literature review from the years 2007 to 2016, when the female girl groups began to receive attention. The music videos of the female girl groups in the top 100 charts of 'Melon' from 2006 to 2-16, which provides the largest mobile music service in Korea, were watched and analyzed as primary data. As a result, it was found that the 'school fashion look image' of adolescence which was used as costumes for Girl Groups, plays a role in commercializing the image of a 'girl', and the types and characteristics of school look fashion image are drawn in two ways. First, it is the image of a seductive Lolita complex. This is the case where young girls wear school look fashion image to emphasize their sexual maturity. Second, it is the case that is using the school look fashion image in order to perform with the 'young girl' concept, as an image of the enchanting Lolita complex; in addition, the erotic body image is more explicitly exposed through choreography and nakedness.

Validity and Reliability Test of the Korean Version of the Oncology Patients' Perception of the Quality of Nursing Care Scale (한국어판 '암 환자가 인식하는 간호의 질 측정도구'의 타당도와 신뢰도)

  • Purpose: The purpose of this methodological study was to examine the validity and reliability of a translated Korean version of The Oncology Patients' Perception of the Quality of Nursing Care Scale. Methods: The translated scale was pilot tested and then administered to 360 patients with cancer. Exploratory and confirmatory factor analyses were utilized to assess the factor structure. Internal consistency reliability was determined using coefficient ${\alpha}$. Results: Two of the 40 items were deleted with a principal component method of exploratory factor analysis because they did not meet the factor-loading criterion. The 38 items were again analyzed and, four factors were validated (KMO=.96, Bartlett ${\chi}^2=10809.81$, df=780, p<.001). The four factors accounted for 60.9% of the variance. Factor loadings of the scale on the four subscales ranged from .65 to .91. The scale showed reliable internal consistency with a Cronbach's ${\alpha}$, total 38 items (${\alpha}=.96$) in four subscales: individualization (18 items, ${\alpha}=.96$), proficiency (10, .91), responsiveness (7, .90), and coordination (3, .80). Conclusion: The findings of this study demonstrate that the scale has satisfactory construct validity and reliability to measure quality of cancer nursing care from the patient's perspective in Korea.

Mobile Phone based Asthma Management System (이동전화를 이용한 천식질환 원격관리시스템)

  • Asthma requires continuous long-term management with at least 5% outcome of general population as well as being the second cause of death and disability after cancer. The present study developed an efficient self management system based on the commercial mobile phone network. The spirometric test results are input to the mobile phone through the communication line connected to the portable spirometer. The doctor or the care-giver can search, identify, and review the data accumulated daily by the patient, and feedback to the patient necessary recommendations by short message and color mail services. Patient can also send an emergency call to the doctor and/or the care-giver. User interface was designed as convenient as possible for maximum efficiency of these operations. The present system provides a desired remote medical services, thus would enhance health management of chronic diseased patients.

Overview of Diabetic Peripheral Neuropathy and Need for Therapeutic Strategy using Traditional Korean Medicine (당뇨병성 말초 신경병증의 개괄과 한의학적 연구의 필요성)

  • Objective: This study aimed to analyze the current status of treatments and patients with diabetic peripheral neuropathy, and then map out of a strategy for development of generalized-treatments for diabetic peripheral neuropathy in Traditional Korean Medicine. Methods: We selected research materials from various databases such as PubMed, Google, KStudy, KoreanTK, OIM, KOMS and books. Also, to understand current tendencies of medical examination and treatment related with diabetic peripheral neuropathy, we requested Health Insurance Review and Assessment Service clinical data from 2003 to 2007. Results: It is reported that the incidence of diabetic peripheral neuropathy is increasing in an aging society. The medical fees of National Health Insurance related with diabetic peripheral neuropathy show a year-on-year increase. There are no particularly effective therapies for diabetic peripheral neuropathy in Western medicine, and in some papers, it was reported that treatment of diabetic peripheral neuropathy using Traditional Korean Medicine was effective. However, patients usually visit on Western medical center rather than seek Traditional Korean Medicine. To take charge of clinical fields related with diabetic peripheral neuropathy by Traditional Korean Medicine, we need more studies and experiments of diabetic peripheral neuropathy using Traditional Korean Medicine and should make a standardized protocol. Conclusion: Various studies related with diabetic peripheral neuropathy using Traditional Korean Medicine will have to be undertaken hereafter. We expect that Traditional Korean Medicine will play a vital role in treating of diabetic peripheral neuropathy.
