어머니로의 역할 전환 지원을 위한 음악중재 프로그램 사례연구 (Case Study of Music Intervention for Supporting Maternal Transition)

  • 본 연구는 모성전환 지원의 일환으로 노래 부르기를 격려하기 위해 가정을 방문하여 음악중재 프로그램을 교육한 후 2주간 아이에게 노래 부르기를 실시한 어머니들의 경험과 의견에 대한 사례연구이다. 2개월에서 11개월 사이의 건강한 아이를 둔 어머니들 중 참여를 원하는 어머니들을 선정하여 어머니 유아에게 노래 부르기에 대한 교육적인 정보와 함께 노래 부르기를 쉽게 하실 수 있도록 여러 가지 방법을 제공 한 뒤 2주 동안 스스로 모자 상호작용 시 노래를 불렀을 때 느낀 경험과 의견 등을 종합하기 위한 인터뷰가 실시되었다. 구조적, 비구조적 인터뷰를 실시한 결과 매일 아이에게 노래를 부르면서 경험한 것과 실제 노래 부르기가 어떤 도움을 주었는가에 대한 분석이 이루어졌다. 그 결과, 네 명의 어머니 모두 긍정적인 경험과 노래 부르기를 통한 양육활동 및 부부관계의 도움을 보고하였으며 체계적인 프로그램의 가능성이 제시되었다. 모성 전환 시 도움이 필요한 여러 대상군의 어머니들에게로의 적용 가능성이 발견되었으며, 추후 연구를 통한 효과성 검증이 요구된다.

자폐스펙트럼장애 아동의 수면 장애에 대한 국외 작업치료 중재의 체계적 고찰 : 2011년부터 2021년까지 (A Systematic Review of Overseas Occupational Therapy Intervention Study for Sleep Disorders in Children with Autism Spectrum Disorder : 2011~2021)

  • Purpose : This study aims to systematically review the methods of occupational therapy intervention in children with autism spectrum disorders having sleep disorders and evaluate the improvements with intervention effects, based on experimental studies of children with sleep disorders. Methods : Studies published overseas from January 2011 to June 2021 were searched from a total of two databases. Science Direct and OTseeker. The five selected studies were analyzed by dividing them into age, number, intervention period, intervention type, intervention effect, evaluation tool, research design type, and evidence level. Results : The total number of subjects was 182, 95 subjects in ther experimental group, and 87 in the control group. The interventions included weighted blankets, swimming, and sleep education for parents. Ther interventions were found to increase total sleep time, improve mood when waking up, reduce sleep anxiety, reduce sleep time, reduce the number of waking up of during sleep, and reduce sleep resistance behavior. Conclusion : Many people have sleep disorders, with or without disabilities, and the number is gradually increasing. Consequently, research on occupational therapy intervention in children with autism spectrum disorder are actively conducted in foreign countries, and these interventions have a positive effect. Based on the results of this study, it can be concluded that such occupational therapy intervention studies are necessary for children with autism spectrum disorder with sleep disorders in Korea. In addition, further research on the quality of life of parents of children with autism spectrum disorders due to sleep disorders and their methods are required.

초등학생의 읽기이해력 향상 중재프로그램에 대한 체계적 문헌고찰 (Systematic Review of an Intervention Program for the Improved Reading Comprehension of Elementary School Students)

  • 이 연구는 체계적 문헌고찰(systematic review) 연구방법을 활용하여 초등학생의 읽기이해력 향상을 위해 제공된 중재프로그램 실험연구들을 분석한 것이다. Cochrane의 핸드북을 적용하였으며, PICOTS-SD에 따라 핵심질문을 구성하였다. KCI에서 검색된 1,610건의 논문에서 포함기준과 배제기준을 적용하여, 최종 18건을 분석대상으로 선별하였다. Cochrane에서 제공하는 'RevMan5' 도구를 사용하여 비뚤림 위험을 평가하였다. 평가결과 대부분의 영역에서 비뚤림 위험도가 낮게 나타났다. 그리고 이들 논문들에 대한 정성적인 분석을 위해 자료요약표를 정리하였다. 이 결과, 중재프로그램의 유형은 다양하였으며, 고학년생을 적용대상으로 하는 경우가 많았다. 실험집단의 크기는 6명에서 29명이며, RCT 기반 연구보다 QED 기반 연구가 더 많았다. 중재시간은 40분이 가장 많았으며, 중재회기는 8회에서 24회로 나타났다. 그리고 모든 연구에서 중재프로그램은 읽기이해력 향상에 효과가 있는 것으로 나타났다.

인권 침해와 관련한 게임 기반 스크립트 중재가 초등 장애아동의 자기옹호 언어 및 행동에 미치는 영향 (Effects of Game-Based Self-Advocacy Script Intervention on Children with Disabilities)

  • 본 연구의 목적은 인권 침해와 관련한 게임 기반 스크립트 중재가 초등 장애아동의 언어적 행동적 자기옹호의 습득과 유지에 미치는 영향을 알아보기 위함이었다. 지적장애 초등학생 4명을 대상으로 대상자간 중다 간헐 기초선 설계(multiple probe design across subjects)를 사용하여 게임에 기반한 스크립트 중재를 실시한 결과, 도움 요청하기, 의사표현, 폭력의 대응하기의 영역에서 연구 참여자 모두 언어적 자기옹호와 행동적 자기옹호의 긍정적 향상이 나타났으며 그 효과가 유지되었다. 또한 네 학생 모두 행동적 자기옹호보다 언어적 자기옹호에 있어 더 높은 효과가 나타났다. 본 연구는 기존 자기 결정의 하위 요소로만 다루어지던 자기옹호에 대해 인권 요소와 침해 사례를 기반으로 독립적인 자기옹호의 프로그램을 구조화하였다는 점에서 후속 연구에 유용한 정보를 제공한다.

뇌성마비 취학아동 어머니의 양육체험 (The Lived Experience of Mothers about Rearing of School Children With Cerebral palsy)

  • This study is designed to understand the meaning and nature of raising children with cerebral palsy. It researches the experience of mothers of schoolchildren with cerebral palsy by the research method of hermeneutic phenomenology. The study was conducted from November 10, 1999 to December 20, 2000. When children with cerebral palsy usually show symptoms in the early stage of cerebral palsy, mothers do not take children to a doctor for diagnosis. And, most of mothers have a difficult time to accept the reality; they usually respond to the initial diagnosis with shock, reproach, and deny. When mothers start recognizing the reality, they consider that their children have cerebral palsy due to the their mismanagement during pregnancy, delivery, nursing, and initial treatment. They shelter their children from view and feel guilty that they cannot afford to try folk remedies for their children. As time passes, mothers face conflicts between families in diverse ways. Families put the blame on genetic effects. Mothers-in-law give their daughters-in-law a hard time, husbands shift the responsibility of raising children onto their wives, and trouble arises between families-in-law and mothers native families. When children grow up, it is physically difficult for mothers to take care their children. In addition, they suffer from all the troubles in family due to childrens handicap. Mothers try the diverse methods of bringing up children. However, they start getting tired of raising children as they experience failures and financial difficulties. Mothers feel collapsed recalling the ways of raising children. They feel anxiety, miserable, lonely, and worrying when they think how children would attend school, make friends, and live in the future. In this stage, mothers do their best to raise their children with hope. They tend to compare their children with others without handicap and spend money and time in attempting all the treatments. When mothers and children join the society at school, they find that the society does not understand disabled people, teachers show inconsiderate attitude, friends avoid them, and children hardly follow classes. Such experiences make mothers feel angry and frustrated. However, when children adapt to school, mothers see the possibility that children could accomplish schoolwork. They appreciate teachers help and others consideration. Mothers place appropriate expectations on their children and help them to prepare for the future. I would make following suggestions based on the results. 1. As a primary basic course of rehabilitation nursing intervention, solution-centered nursing intervention system should be developed. The intervention needs to be based on the understanding of mothers, who raise children with cerebral palsy, through in-depth interview. 2. Advance researches on the development of individual nursing intervention should be conducted. Individual nursing intervention needs to prevent and release actual pain focusing on mothers raising children with cerebral palsy. 3. Integrated curriculum that help children with cerebral palsy lead a normal school life with ordinary children should be developed. 4. Basic research on using of facilities and effective application of service volunteer to help children with cerebral palsy in school needs to be conducted.

Exploring the Causal Structure of Adolescent Media Addiction and Policy Intervention

  • Adolescent media addiction has emerged as an important social agenda in Korea. However, there has not been enough discussion on the causal structure of media addiction and policy interventions. The objective of this study is to identify and assess the mechanism of the existing and the revised Shutdown policy based on the systems thinking approach. To achieve this purpose, we establish the relationship between media usage, flow, and addiction, and develop a causal loop diagram. Based on the causal loop diagram, we explore the causal structure of two policy scenarios: shutdown policy and deregulation. Our study suggests that policy interventions inducing direct parental control on children's media usage time are ineffective since the time control reduces children's autonomy, which helps alleviate media addiction. Therefore, this study suggests that policy intervention should focus on alleviating addiction itself rather than on controlling media usage time.

광주지역 일부 저소득층 여자노인에서 지역사회 영양중재 프로그램 적용에 따른 식행동 변화 및 영양상태 개선 효과 (The Effect of a Community-Based Nutrition Intervention Program on Dietary Behavior and Nutritional Status of Low-Income Elderly Women in Gwangju City)

  • This study was conducted to measure the effect of a community-based nutritional intervention program on dietary behaviors and nutrient intakes of low-income elderly women in Korea. The subject of the study was a group of 88 elderly women using the free meal service in Dong-gu, Gwangju city in Korea. The study was carried out from April to July 2009, and the pretest, application of nutrition education, and post-test were applied in each stage. This community-based nutrition intervention program consisted of snack supplement and nutrition education provided three times a week and 36 times in total. The nutrition education was led by the nutritionists and professionals using various educational media. Snack supplement included were milk, soymilk, carrot juice, bread, yogurt, and bananas. The result of pretest showed that the nutrient intakes of the subjects were much below the Recommended Nutrient Intake (RNI) level of Dietary Reference Intake for Koreans. Comparison of the dietary behavior score and nutrient intakes before and after the nutritional intervention program indicated that the food behavior score increased from 3.1 to 5.6 (P<0.001) and intake of energy, protein, carbohydrate, thiamin, niacin, vitamin C, iron, and potassium of the subjects all increased significantly (P<0.05). Mean Adequacy Ratio (MAR) of their nutrient intakes was also improved from 0.53 to 0.64 (P<0.01). These results indicated that the nutrition intervention program conducted in this study was effective in improving dietary behavior and nutrient intakes of elderly women in local community.

가상현실 기반 중재가 지적장애학생의 일상생활 기술 및 수업태도에 미치는 효과 (Effects of VR based Intervention on Daily Living Skills and Class Attitudes of Students with Intellectual Disabilities)

  • 이 연구의 목적은 가상현실 기반 중재가 지적장애 학생의 일상생활 기술과 수업태도에 미치는 영향을 파악하는 것이었다. 전남지역에 거주하는 중학교 2-3학년 지적장애학생 30명을 연구대상으로 선정하였고, 중재방법에 따라 세 집단으로 배치하였다. A집단은 가상현실 프로그램을 이용하는 중재, B집단은 직접 체험 활동을 중심으로 하는 중재, C집단은 가상현실 프로그램과 직접 체험 활동을 결합한 중재에 참여하였다. 중재는 7월 4주부터 8월 3주까지 4주 동안 진행되었고, 일주일에 4회씩 총 16회기의 중재가 이루어졌다. 평가는 사전 평가와 사후 평가를 실시하였다. 수집된 자료는 일원분산분석으로 분석하였고, 사후 분석으로는 scheffe'를 사용하였다. 연구 결과, C집단이 A집단과 B집단보다 일상생활 기술을 유의미하게 향상시켰고, C집단은 B집단보다 수업태도를 유의미하게 향상시켰다. 이를 통해 가상현실 프로그램과 체험활동을 결합한 중재가 일상생활 기술 및 수업태도를 향상시킬 수 있는 효과적인 방법 중에 하나라는 것을 확인하였다.

자폐스펙트럼장애 아동들의 효과적인 조기 중재에 대한 체계적 고찰 (Systematic Review on Effect of Comprehensive Early Intervention for Pre-school Children With Autism Spectrum Disorder(ASD))

  • 목적 : 본 연구는 국외 학술지를 대상으로 체계적인 문헌고찰을 통해 자폐스펙트럼장애(autism spectrum disorder, ASD)아동들을 대상으로 사용되는 조기 중재 방법의 효과를 확인하는 것이었다. 연구방법 : 2017년 12월 이전까지의 국외 학술지에 게재된 논문을 Pubmed를 통하여 검색하였다. 주요 검색어로는 'autism', 'autism spectrum disorder', 'ASD', 'high function autism', 'high function ASD', 'Asperger syndrome', 'pervasive developmental disorder', 'PDDNOS', 'intervention'과 'early intervention'를 사용하였다. 최초 검색된 논문은 724편이었으나 포함 및 배제기준을 적용하여 최종 10편의 연구가 선정되었다. 결과 : 자폐스펙트럼장애 아동의 조기 중재는 10편의 선정된 연구 중에서 8편의 연구가 응용행동분석에 기초한 중재 프로그램을 적용하였고, 다른 2편의 연구들은 감각 통합적 접근과 테크놀로지에 기반을 둔 중재접근을 사용하였다. 대부분의 연구가 조기 중재로 의사소통기술과 사회적 상호작용기술, 지능, 적응 행동의 증가, 동시집중력의 향상과 같은 긍정적인 효과를 보고하였다. 결론 : 본 연구는 자폐스펙트럼장애 아동의 조기 중재에 대한 근거를 제시하며, 향후 국내 연구에서 효과적인 조기 중재에 대한 효과를 확인하는 연구가 필요할 것으로 사료된다.

A Qualitative Study on the Process of the Mental Health Assessment and Intervention after the Sewol Ferry Disaster: Focusing on Survivors among Danwon High School Students

  • Objectives: This study aimed to identify the process of psychosocial assessment, the experience of intervention, and the improvement after the disaster that Danwon high school students survived from the sinking of the Motor Vessel (MV) Sewol in South Korea on April 16, 2014. Methods: We conducted in-depth qualitative research using individual interviews from January to February 2017. Twenty-one of 75 Danwon high school students survived by the MV Sewol disaster were studied. Two interviewers participated in the in-depth interview. Interviews were transcribed verbatim, coded, and analyzed through content analysis based on psychosocial assessment and intervention after the disaster. Results: Twenty-one participants were 20 years old, where 10 were male (47.62%), and 11 were female (52.38%). More than 75% of the interviewed students felt that mental health services were needed, and more than 85% required mental health assessments. Regarding psychiatric symptoms, the students reported that they suffered depression and insomnia (19.05%), anxiety (14.29%), nightmares and phobias (9.52%), and difficulties regarding concentration, aggression, and game addiction (4.76%). Conclusion: Despite survivors experiencing the same disaster, there were differences in their responses to mental health assessments and interventions experienced during the three years, and conflicting opinions were reported. In planning future evaluations and interventions, it is necessary for strategies to cope with flexibly to consider the characteristics and symptoms of the survivors while maintaining principles.
