  • Purpose: Feeding children is a problem in pediatric intensive care units (PICU) and it is difficult to know the correct amount. The purpose of this study is to evaluate if prealbumin or retinol binding proteins (RBP) are effective relative to daily enteral nutrition, without being affected by severity of diseases or infections and can be used to follow up nutritional amount. Methods: This is a prospective observational study that includes 81 patients admitted to PICU in Akdeniz University with estimated duration >72 hours, age between 1 month and 8 years. Daily calorie and protein intake were calculated and prealbumin, RBP and C-reactive protein (CRP) levels were measured on the first, third, fifth and seventh mornings. Results: We find moderate correlation between daily calorie intake and prealbumin levels (r=0.432, p<0.001), RBP levels and daily protein intake (r=0.330, p<0.001). When we investigated the relationship between changes of prealbumin, RBP, CRP, calorie and protein intake during intensive care stay, we found that increase of Prealbumin and RBP levels are explained by decrease of CRP levels (r=-0.546 and -0.645, p<0.001) and not with increase of nourishment. Conclusion: Even adjusted for PRISM3, age and CRP, prealbumin and RBP are correlated with last 24 hours' diet. However, it is not convenient to use as a follow up biomarker because increase of their levels is related with decrease of CRP levels.

  • Background: The Korea Interstitial Lung Disease Study Group has made a new nationwide idiopathic pulmonary fibrosis (IPF) registry because the routine clinical practice has changed due to new guidelines and newly developed antifibrotic agents in the recent decade. The aim of this study was to describe recent clinical characteristics of Korean IPF patients. Methods: Both newly diagnosed and following IPF patients diagnosed after the previous registry in 2008 were enrolled. Survival analysis was only conducted for patients diagnosed with IPF after 2016 because antifibrotic agents started to be covered by medical insurance of Korea in October 2015. Results: A total of 2,139 patients were analyzed. Their mean age at diagnosis was 67.4±9.3 years. Of these patients, 76.1% were males, 71.0% were ever-smokers, 14.4% were asymptomatic at the time of diagnosis, and 56.9% were at gender-age-physiology stage I. Occupational toxic material exposure was reported in 534 patients. The mean forced vital capacity was 74.6% and the diffusing capacity for carbon monoxide was 63.6%. Treatment with pirfenidone was increased over time: 62.4% of IPF patients were treated with pirfenidone initially. And 79.2% of patients were treated with antifiboritics for more than three months during the course of the disease since 2016. Old age, acute exacerbation, treatment without antifibrotics, and exposure to wood and stone dust were associated with higher mortality. Conclusion: In the recent Korean IPF registry, the percentage of IPF patients treated with antifibrotics was increased compared to that in the previous IPF registry. Old age, acute exacerbation, treatment without antifibrotics, and exposure to wood and stone dust were associated with higher mortality.

Standardized questionnaire were sent out to look into the following: 1) the attitude of working mothers, who have infants below 4 years of age, regarding their job, care-taker and group settings. 2) the status of taking care of their infants 2. Subjects Women teachers of Seoul and Kyung Ki primary school districts, who have infants below age 4. The selection of these subjects were done by random sampling. III. Results 1. Attitudes regarding their jobs: The first reason for retaining their jobs are for financial reasons; whereas, their intents of self-actualization and contributions to society comes next. 2. The Status of Caretakers 60.7% of the care-takers are family-related members, and 38.3% are not. The educational level of caretakers are as follows, with 19.6% of the caretakers without any formal education, 44% with primary education, and 18.7% having high school education. The reasons for the dissatisfaction by the caretakers are low education, a frequent change of caretaker, and an attitude without affection. The cost of child caring is high, the main reason being the heavy expense spent on the caretakers. 3. Attitudes of care in group settings: 85.9% of the subjects responded with positive reactions for the necessity of having group-settings. The reasons given for the importance of having a group setting are early experience, social interaction with peer group and educational functioning, in addition to protective care. They desire for a family-style care which can raise infants under high quality of care.

  • Background: The diffusing capacity of the lung is influenced by multiple factors such as age, sex, height, weight, ethnicity and smoking status. Although a prediction equation for the diffusing capacity of Korea was proposed in the mid-1980s, this equation is not used currently. The aim of this study was to develop a new prediction equation for the diffusing capacity for Koreans. Methods: Using the data of the Korean National Health and Nutrition Examination Survey, a total of 140 nonsmokers with normal chest X-rays were enrolled in this study. Results: Using linear regression analysis, a new predicting equation for diffusing capacity was developed. For men, the following new equations were developed: carbon monoxide diffusing capacity (DLco)=-10.4433-0.1434${\times}$age (year)+0.2482${\times}$heights (cm); DLco/alveolar volume (VA)=6.01507-0.02374${\times}$age (year)-0.00233${\times}$heights (cm). For women the prediction equations were described as followed: DLco=-12.8895-0.0532${\times}$age (year)+0.2145${\times}$heights (cm) and DLco/VA=7.69516-0.02219${\times}$age (year)-0.01377${\times}$heights (cm). All equations were internally validated by k-fold cross validation method. Conclusion: In this study, we developed new prediction equations for the diffusing capacity of the lungs of Koreans. A further study is needed to validate the new predicting equation for diffusing capacity.

  • Most special health care needs (SHCN) people suffer from oral health problems due to high incidence and severity of oral disease. The aim of the present study was to investigate the characteristics of dental treatment provided in this patient population. The study population comprised 485 SHCN patients who visited Kyung Hee University Dental Hospital from 2006 to 2014. Based on the medical record, the date were evaluated according to age, gender, type of disorder, reason for visit, type of behavior management, and type of dental treatment. The most common age group was 7-12 year olds (26%). The chief compliant was dental caries (39%), as followed by oral exam (12%) and trauma (12%). Approximately half of SHCN patients (45.9%) was treated under behavior management. Restorative treatment was the most common procedure with the average of 2.9 teeth treated per patient. The results of the present study may be valuable for establishing the data of SHCN patients for primary dental care.
