  • This case reports the effect of herbal medicine and acupuncture therapy on the scoring atopic dermatitis (SCORAD) index for an adult atopic dermatitis patient. An adult male in his 30s with complaints of severe eczema on the whole body was diagnosed with heat toxin blazing exuberance pattern (熱毒內蘊) and treated with herbal medicine, including an orally-administrated decoction, externally-applied decoction, extracted powder capsule, and acupuncture for a total of 30 days. We observed skin symptoms, SCORAD index, follow-up photographs, and any adverse events. After treatment, SCORAD index decreased from 74.5 to 38.7 (51.9% from baseline). Skin symptoms such as redness, itching, and exudation were improved without any adverse events reported. This case suggests that herbal medicine and acupuncture therapy could be effective in improving eczema in adult atopic dermatitis patients.

  • Objectives The purpose of this study is to identify the characteristics of atopic dermatitis patients based on the personal health records (PHR) according to age and Sasang Constitution, and to find basic data for daily life management using the personal health record. Methods We analyzed the characteristics of atopic dermatitis patients based on PHR (symptoms, emotional state, SCORAD Index, five viscera weakness questionnaire, etc.), and Sasang Constitution collected for 6 weeks in 34 patients (or guardians). Results Each item of the SCORAD Index score in children and adolescents showed that children and adolescents have significantly higher edema than adults. However, repeated measurement of the SCORAD Index showed a significant positive improvement over time regardless of age and constitution. In terms of the symptoms of atopic dermatitis, there was a significant correlation between age and swelling, quality of life and effusion, pruritus (itch) and sleep disturbance in relation to the general characteristics of the subject. In the five visceral weakness scores, adults had significantly higher spleen weakness, liver weakness, kidney weakness scores than in children and adolescents. All results showed a high tendency to have a high lung weakness score, which was more pronounced in children and adolescents. Conclusion This study is meaningful to confirm the correlation between SCORAD Index items related to the symptoms of atopic dermatitis, and to identify trends for specific items by ages and Sasang Constitution.

  • Background : There are findings indicating that the autonomic nervous system (ANS) is affected negatively in atopic dermatitis (AD). Heart rate variability (HRV) is a tool to examine ANS. But, studies investigation the relationship between AD and HRV is rare. Objective : The aim of this study was to investigate the correlation between atopic dermatitis severity and HRV and to find out the role of ANS in AD. Methods : Retrospective chart review was performed. The Severity Scoring Atopic Dermatitis (SCORAD) index and HRV parameter was investigated and analyzed. Results : There were 55 patients (25males and 30 females) who met the criteria. Objective SCORAD had significant positive correlation with HRT and Subjective SCORAD had significant negative correlation with SDNN, TP and VLF. Intensity item of SCORAD index had negative correlation with most HRV parameters, but TP, LF and LF/HF ratio had significant positive correlation with dryness. Conclusion : This study supports the concept that atopic dermatitis is a stress-responsive disorder and involves autonomic nervous system dysfunction.

  • The present study was undertaken to evaluate effects of eco-friendly underwears(three kinds of natural dying underwears) in patients with atopic dermatitis. We recruited 45 patients with atopic dermatitis admitted to Dongshin Oriental Hospital from August to September 2013. These patients were divided into three groups (A, B and C). Each group include 15 patients and three kinds of natural underwears dyed with polygonum tinctorium, loess, and non-dyed general cotton were given, respectively. We also used SCORAD index and digital infrared thermal imaging to investigate the effects of each eco-friendly underwear in patients with atopic dermatitis. In SCORAD index, Group A after wearing loess-dyed underwears for 4 weeks showed statistically significant reduction to 27.96 (P<0.05) compared to 34.20 before wearing loess-dyed underwears. In Group B after wearing polygonum tinctorium-dyed underwears for 4 weeks, SCORAD index and objective severity assessment were markedly reduced into 22.97 (P<0.01) and 18.17 (P<0.01) compared to 35.95 and 27.89 before wearing polygonum tinctorium.-dyed underwears, respectively. In Group C after wearing general cotton (non-dyed) underwears for 4 weeks, there was statistically no difference in SCORAD index, objective severity assessment and body temperature between before and after wearing general cotton (non-dyed) underwears. Taken together, polygonum tinctorium-dyed underwears used to patients with atopic dermatitis shows significant amelioration in atopic symptoms than loess-dyed underwears as well as general cotton (non-dyed) underwears.
