Factor Analysis of the Seriously-ill Patient인s Family Needs (중환자 가족의 요구 요인 분석 연구)

  • A group of studies revealed that family members of the seriously ill patient had some needs during the period of patient's hospitalization. Needs of the seriously-ill patient family could be classified into three aspect, i. e. needs on the prognosis of patient, needs on the well-being of family members themselves and needs on hospital environment. Several instrument were developed to measure the needs of the seriously ill patient, but their content tended to overemphasize the aspect of the prognosis of patient. The purpose of this study was to develop a new instrument to measure the needs in the aspect of the seriously ill patient's family themselves in details to increase the cummulative Percentage of the scale. Subjects were 134 family members of the seriously-ill patients, who were bung cared in ICU of seven university hospitals and data were collected from march 16, 1987 to April 11, 1987. The instrument used in this study was made by the author on the basis of results of literature review. Content valitity of the instrument was tested by a professor majoring in nursing and reliability by calculation of Cronbach's α with data of the respondents. Data was analyzed as follows, using SAS, computer system. Factor structures of the seriously ill patients' family needs were elicited by factor analysis. The programme was the Principal Component Factor Analysis Method of factoring employing Varimax Orthogonal Rotation. The influences of the demographic variables on the degree of the seriously-ill patients' family need were analyzed by t-test and P-test. Results were as follows. 1. Needs of the seriously-ill patients' family elicited and their cummulative percentage were: Needs to be supported emotionally, 29.2% Needs to be informed on facilities available, 9.6% Needs to be informed on the patient's prognosis, 7.7%. Needs to be supported spiritually, 5.1%. Needs to be informed on hospital environment, 4.2%. Needs to be helped to spare time, 3.9%. Needs to be informed on their role, 3.5%. Needs to be present near patient, 3.3%. 2. Educational background was found to influence on some factors. College-graduate group had higher Bevel of needs to be . informed on facilities available to be informed on the patient's prognosis, needs to be supported spiritually, needs to be helped to spare time, needs to be informed on their role than high school graduate group. 3. Among the parents, sons and daughters and the relative groups, the parents of the seriously ill patient had highest level of needs to be present near patient. Suggestions for further studies were as follows. 1. As the instruments used in the previous studies had high cummulative percentages in the aspect of the prognosis of patient and that in this study in the aspect of the prognosis of patient and that in this study in the aspect of the seriously ill patient's family themselves, development of a new instrument which combined the items of both of them is needed. 2. A study to clarify the influence of type and number of admission on need to be supported emotionally is needed.

Open Source-Based Surgical Navigation for Fracture Reduction of Lower Limb (오픈소스 기반 수술항법장치의 하지 골절수술 응용검토)

  • Minimally invasive intramedullary nail insertion or plate osteosynthesis has shown good results for the treatment of long bone fractures. However, directly seeing the fracture site is impossible; surgeons can only confirm bone fragments through a fluoroscopic imaging system. The narrow field of view of the equipment causes malalignment of the fracture reduction, and radiation exposure to medical staff is inevitable. This paper suggests two methods to solve these problems: surgical navigation using 3D models reconstructed from computed tomography (CT) images to show the real positions of bone fragments and estimating the rotational angle of proximal bone fragments from 2D fluoroscopic images. The suggested methods were implemented using open-source code or software and evaluated using a model bone. The registration error was about 2 mm with surgical navigation, and the rotation estimation software could discern differences of $2.5^{\circ}$ within a range of $15^{\circ}$ through a comparison with the image of a normal bone.

Emission Characteristics of Methane and Nitrous Oxide by Management of Water and Nutrient in a Rice Paddy Soil (논에서 물과 양분관리에 따른 메탄CH4), 아산화질소(N2O)배출 특성)

  • Emission of methane and nitrous oxide affected by nitrogen fertilizer materials were measured simultaneously in rice paddy fields under flooding and intermittent irrigation in 2000. Studies focused on mitigating $CH_4$ emission from rice paddy fields are summarized and the possibilities and limits applied to world's rice cultivation are discussed. The mitigation options are water management, soil amendments, organic matter management, different tillage, rotation, and cultivar selection. Altering water management, in particular promoting midseason aeration by short-term drainage, is one of the most promising strategies, although these practices may be limited to the rice paddy fields where the irrigation system is well prepared. The test site was divided into two water managements: a continuously flooded plot which was maintained flooded by constant irrigation from May to September, and an intermittently drained plot in which short-term (20days) draining practices were performed one times during the flooding period. By total emission of GHGs converted by global warming potential (GWP), flooding plots were higher 170$\sim$208% than interimittent irrigation plots. For emission of GHGs in fertilizer materials, it was high in the order of Swine slurry>Urea+Rice straw>Urea>LCU. Basing on GHGs emission of urea fertilization under flooding as baseline GWP of urea fertilization and Latex-coated urea under intermittent irrigation showed lower GHGs emission by 41.4% and 55.8 respectively. In this case fertilizer use efficiency (kg unhulled rice/ of applied N) were 18.2$\sim$20.2 and 18.7$\sim$19.0 and 9.3 and 5.8$\sim$6.6 for Swine slurry and LCU and Urea+Rice straw and Urea in the continuously flooded and intermittently drained plot.

Effect of Winter Rye Cultivation to Improve Soil Fertility and Crop production in Alpine Upland in Korea (동계호밀재배가 고랭지 밭토양의 비옥도 증진에 미치는 영향)

  • Soil erosion is one of the most serious problems in alpine upland in Korea. Soil fertility has continuously decreased due to serious soil erosion. To increase soil fertility, new sources of organic matter should be inputted. Therefore, the objectives of this research were to select winter cover crop as new sources of organic matter and to investigate the effect of winter cover crop on soil property changes, major crop productivity (Chinese cabbage, potato) production in highland, and disease occurrence with different cropping systems. Among 17 candidates for winter coverage crop, rye was most suitable due to it's soil covering rate, and over-wintering rate. The optimum sowing period for rye ranged from late August to late September. Soil porosity and organic matter content increased with rye cultivation. Rye cultivation during winter increased amounts of crop (both Chinese cabbage and potato) productivity up to 8%. There was little difference on amount of crop productivity depending on cropping systems such as monoculture (Chinese cabbage or potato) and Chinese cabbage-potato rotation.

  • The basis of capacity design has been explicitly or implicitly regulated in most bridge design specifications. It is to guarantee ductile failure of entire bridge system by preventing brittle failure of pier members and any other structural members until the columns provides fully enough plastic rotation capacity. Brittle shear is regarded as a mode of failure that should be avoided in reinforced concrete bridge pier design. To provide ductility behavior of column, the one of important factors is that flexural hinge of column must be detailed to ensure adequate and dependable shear strength and deformation capacity. Eight small scale circular reinforced concrete columns were tested under cyclic lateral load with 4.5 aspect ratio. The test variables are longitudinal steel ratio, transverse steel ratio, and axial load ratio. Eight flexurally dominated columns were tested. In all specimens, initial flexural-shear cracks occurred at 1.5% drift ratio. The multiple flexural-shear crack width and length gradually increased until the final stage. The angles of the major inclined cracks measured from the vertical column axis ranged between 42 and 48 degrees. In particular, this study focused on assessing transverse reinforcement contribution to the column shear strength. Transverse reinforcement contribution measured during test. Each three components of transverse reinforcement contribution, axial force contribution and concrete contribution were investigated and compared. It was assessed that the concrete stresses of all specimen were larger than stress limit of Korea Bridge Design Specifications.

  • In recent years, non-welded building hardware has been installed by bolt assembly is used. The non-welded building hardware method can reduce accidents caused by welding, and can be constructed by bolt assembly, which can reduce labor costs and shorten the construction period. However, there is a need for a method to compensate for the occurrence of buckling at the time of construction. The purpose of this study is to evaluate the behavior of joints between steel pipe and fastener and to evaluate the behavior of joints of non-welded and welded hardware frame. As a result, it was found that the foundation steel structure without welded joints was deformed to a rotation angle of member much larger than the allowable interlayer displacement angle 0.01 to 0.02 required according to the seismic load rating in the seismic load resistance system.

Isolation and Purification of Berberine in Cortex Phellodendri by Centrifugal Partition Chromatography (Centrifugal Partition Chromatography에 의한 황백으로부터 Berberine의 분리 및 정제)

  • Cortex Phellodendri (CP) is derived from the dried bark of Phellodendron amurense. It has been widely used as a drug in traditional Korea medicine for treating diarrhea, jaundice, swelling pains in the knees and feet, urinary tract infections, and infections of the body surface. Many analytical methods have been used to study oriental herbal medicines, such as thin-layer chromatography, column liquid chromatography, and high performance liquid chromatography (HPLC). In this study, preparative centrifugal partition chromatography (CPC) was successfully carried out in order to separate pure compounds from a CP methanol extract. The optimum two-phase CPC solvent system was composed of n-butanol: acetic acid: water (4:1:5 v/v/v). The flow rate of the mobile phase was 3 mL/min in ascending mode with rotation at 1,000 rpm. The CPC-separated fraction and purification procedures were carried out by preparatory HPLC. The $^1H$ NMR spectrum revealed that the resonances at ${\delta}$ 4.10 and 4.20 ppm corresponded to three protons ($-OCH_3$), whereas those at ${\delta}$ 6.10 ppm corresponded to two protons ($-OCH_2O-$). Further, two aromatic protons (H-11 and H-12) conveys a doublet-doublet pattern. The H-11 doublet and H-12 doublet appear at ${\delta}$ 7.98 and 8.11, respectively. The $^{13}C$ NMR. spectrum showed a tetrasubstituted with a methylenedioxy group at C2 and C3, and two methoxy groups at C9 and C10. The chemical structure of the berberine was identified by $^1H$, $^{13}C$-nuclear magnetic resonance and electrospray ionization-mass spectroscopy spectral data analysis.

  • Motility and chemotaxis are crucial for disease development in many motile pathogens, including spirochetes. In many bacteria, motility is provided by flagella rotation, which is controlled by a chemotaxis-signal-transduction system. Thus, motility and chemotaxis are inextricably linked. Spirochetes are a unique group of bacteria with distinctive flat-wave morphology and corkscrew-like locomotion. This unusual motility pattern is believed to be important for efficient motility within the dense tissues through which these spirochetes preferentially disseminate in a host. Unlike other externally flagellated bacteria-where flagella are in the ambient environment-the flagella of spirochetes are enclosed by the outer membrane and thus are called periplasmic flagella or endoflagella. Although motilityand chemotaxis-associated genes are well studied in some bacteria, the knowledge of how the spirochete achieves complex swimming and the roles of most of the putative spirochetal chemotaxis proteins are still elusive. Recently, cutting-edge imaging methods and unique genetic manipulations in spirochetes have helped to unravel the mystery of motility and chemotaxis in spirochetes. These contemporary advances in understanding the motility and chemotaxis of spirochetes in a host's persistence and disease process are highlighted in this review.

STUDY ON MONITORING UNIT EFFICIENCY OF FLATTENING-FILTER FREE PHOTON BEAM IN ASSOCIATION WITH TUMOR SIZE AND LOCATION

  • To investigate monitoring unit (MU) efficiency and plan quality of volumetric modulated arc therapy (VMAT) using flattening-filter free (FFF) photon beam in association with target size and location. A virtual patient was generated in Eclipse$^{TM}$ (ver. A10, Varian Medical Systems, Palo Alto, USA) treatment planning system. The length of major and minor axis in axial view was 50 cm and 30 cm, respectively. Cylindrical-shaped targets were generated inside that patient at the center (symmetric target) and in the periphery (asymmetric target, 7.5 cm away from the center of the patient to the right direction) of the virtual patient. The longitudinal length was 10 cm and the diameters were 2, 5, 10 and 15 cm. Total 8 targets were generated. RapidArc$^{TM}$ plans using TrueBeam STx$^{TM}$ were generated for each target. Two full arcs were used and the axis of rotation of the gantry was set to be at the center of the virtual patient. Total MU, homogeneity index (HI), target mean dose, the value of gradient measure and body mean dose were calculated. In the case of symmetric targets, averaged total MU of FFF plan was 23% and 19% higher than that of flattening filter (FF) plan when using 6 MV and 10 MV photons, respectively. The difference of HI, target mean dose, gradient measure and body mean dose between FF and FFF was less than 0.04, 2.6%, 0.1 cm and 2.2%, respectively. For the asymmetric targets, total MU of FFF plan was 21% and 32% was higher than that of FF when using 6 MV and 10 MV photons, respectively. The homogeneity of the target was always worse when using FFF than using FF. The maximum difference of HI was 0.22. The target mean dose of FFF was 3.2% and 4.1% higher than that of FF for the 6 MV and 10 MV, respectively. The difference of gradient measure was less than 0.1 cm. The body mean dose was higher when using FFF than FF about 4.2% and 2.8% for the 6 MV and 10 MV, respectively. No significant differences between VMAT plans of FFF beam and FF beam were observed in terms of quality of treatment plan. The HI was higher when using FFF 10 MV photons for the asymmetric targets. The MU was increased noticeably when using FFF photon beams.

Influence of Tungsten Carbide/Carbon Coating on the Preload and Tightening Torque of Implant Abutment Screws (임플랜트 지대주 나사의 텅스텐 카바이드/탄소 코팅이 전하중 및 조임회전각에 미치는 영향)

  • The purpose of this study was to clarify the clinical efficiency of tungsten carbide/carbon coating on obtaining optimal preload of abutment screw compared with non-coated screw in external-hex implant system. In the present work, rotational value and the compressive force between abutment and fixture of abutment screws without coating and with coating tightened to 30Ncm were measured. Mean compressive force of coated screw was 504.6N. Then uncoated screw was tightened to clamping the abutment and the implant to 504.6N, and the tightening torque value and the rotational value was recorded. The following conclusions were drawn within the limitation of this study. Compressive force of coated screw was higher than that of uncoated screw after tightening at 30Ncm. The tightening torque that was nedeed to clamping the uncoated screw equal to coated screw tightened to 30Ncm was 55.6Ncm. In case of equal compressive force, there was no significant difference in rotation value between coated and uncoated screw.
