Safety of Various Types of Cheese manufactured from Unpasteurized Raw Milk: A Review (비살균 원유로 만든 다양한 치즈의 안전성에 관한 연구: 총설)

  • Before the advent of pasteurization and other microbiological controls in the dairy industry, milk product-borne diseases such as scarlet fever, typhoid fever, septic sore throat, and tuberculosis were globally widespread. Pasteurization was invented by Louis Pasteur, and it has been considered as one of the most effective ways to control milk product-borne diseases since the 20th century. Nevertheless, till date, various types of cheese in the EU, the USA, and other countries are made from unpasteurized milk as artisan cheese, following the specific food regulations of each nation. Furthermore, after the effectiveness of the Free Trade Agreement (FTA) negotiations between Korea and many nations, the import of various types of cheese made in different conditions and influenced by acidity, preservatives, temperature, competing flora, water activity, and salt concentration increased yearly. Hence, the objective of this review was to describe (1) the 60-day aging rule of cheese, (2) characteristics of the outbreaks linked to cheese manufactured from unpasteurized milk since 1998 to 2011 in USA, and (3) serious health risks from unpasteurized milk, to ensure food protection and safety and to use this basic information for risk assessment.

Assessment of Substantial Equivalence and Environmental Risk for Event Selection of Genetically Modified Chrysanthemum (국화의 GM 이벤트 선발을 위한 실질적 동등성 및 환경위해성 평가)

  • As an assessment of substantial equivalence and environmental risks of branchless chrysanthemum GM lines (LeLS69, LeLS76, LeLS80) developed in Korea, we investigated their degree of branchlessness and developmental characteristics including growth and flowering, as well as the stability of populations of insect pests and soil microbes. The GM lines were compared with a control variety, 'Jinba'. LeLS80 showed the highest level of branchlessness among the three GM lines, whereas abnormal axillary buds developed in lines LeLS76 and LeLS69. There were no significant differences between the growth characteristics of 'Jinba' and the GM lines. The branchlessness LeLS80 was increased during the summer season, suggesting that it is a useful germplasm for developing elite branchless cultivars. These GM lines showed no significant differences in the occurrence of five insect pests including aphids, mites, leaf miners, thrips, and whiteflies. Similarly, for soil microbes including bacteria and actinomycetes, there were no significant differences between 'Jinba' and the GM lines. However, there was a higher occurrence of fungi in the GM lines compared to 'Jinba'. Our findings suggest that 'Jinba' and the GM lines are substantially equivalent, although further monitoring studies are needed to confirm this.

Ecological Risk of Alien Apple Snails Used in Environmentally-friendly Agriculture and the Urgent Need for Its Risk Management in Korea (친환경농법용 외래 왕우렁이의 생태위해성 및 위해성 관리의 필요성)

  • Alien apple snails (Pomacea canaliculata, Pomacea insularus) used in environmentally-friendly agriculture are different from indigenous snails found in Korea. Due to high herbicidal effects and cost-effectiveness, the number of farmers using the snails has been growing every year since 2000. Moreover, in 2008, because of the outbreaks of avian influenza throughout the country from March to May, 2008, central and local governments recommended the use of alien apple snails in agriculture as an alternative to the ducks-oriented environmentally-friendly agriculture. Therefore, it is expected that the use of alien apple snails in agriculture should be expanded in a near future. Since alien apple snails lay eggs with 95.8% of eclosion rate, they are considered to be potential pests unlike indigenous snails. In addition, Japan, Taiwan and most of the southeast Asian countries had already experienced severe ecological and agricultural damage by the alien apple snails. Subsequently, International Union for Conservation of Nature and Natural Resources (IUCN) designated P. canaliculata as one of "the 100 of the world's worst invasive alien species". It seems highly likely that the alien apple snails in Korea pose a potential threat to conservation of ecosystem and biodiversity since the snails were either found or invaded into the natural environments in some regions of Gangwon-Do and southern parts of Korean Peninsula. However, just recently, agricultural authorities and farmers using alien apple snails in agriculture opposed a proposition of designating the alien apple snails as an ecosystem-disturbing animal described by the Wildlife Protection Act. This is because there has been no concrete evidence of the ecological risk imposed by the alien snails up to now in Korea. Subsequently, in this paper, we analysed the ecological and agricultural risks imposed by the alien snails from the studies done in domestic and abroad. In addition, we proposed an urgent need and reasoning for ecological risk management of the alien snails at the national level as well as using the snails in agriculture.

Radiation Dose and Estimate of Lifetime Attributable Risk of Cancer from Coronary Angiography and Percutaneous Coronary Intervention (관상동맥조영술과 경피적관상동맥중재술에서 환자 선량과 암 발생 생애귀속위험 평가)

  • The Purpose of this study was to determine the effective dose to an average patient from Coronary Angiography (CA) and Percutaneous Coronary Intervention (PCI). And to estimate the lifetime attributable risk (LAR) of cancer associated with radiation exposure from CA and PCI. The dose-area product (DAP) values to the patient were recorded from 60 CA and 58 PCI. A Monte Carlo based program PCXMC was used to calculate the effective dose from DAP values for each patient. Lifetime attributable risks were estimated with models developed in the National Academies' Biological Effects of Ionizing Radiation VII report. The mean DAP values was $53.76\;Gy{\cdot}cm^2$ for CA and $165.82\;Gy{\cdot}cm^2$ for PCI. Mean effective dose were 1.28 mSv in CA, 3.94 mSv in PCI. Results of Calculate organ dose, lung doses was 2.17 mSv in CA and 6.71 mSv in PCI. Female breast doses was 5.45 mSv in CA and 16.82 mSv in PCI. LAR estimates for CA varied from 1 in 1,508 for man to 1 in 1,357 for women. In PCI procedure varied from 1 in 553 for man to 1 in 482 for women. DAP can be used as the dose indicator to calculate the organ dose and effective dose of patient based on Monte Carlo simulation. These dose estimates derived from our simulation models suggest that CA and PCI are associated with a nonnegligible LAR of cancer. This risk varies markedly and is considerably greater for women, PCI than for man, CA.

The Cosmetic Operation without Healing Purpose - A comparative insight into the ruling of BSG and BGH - (미용성형의료 - 우리 판결례와 독일 판결례의 비교·분석적 소고 -)

  • This paper is concerned in the theme of the liability for the breach of duty to inform(Haftungszurechnung der $Aufkl{\ddot{a}}rungspflichtverletzung$) and the malpractice of cosmetic surgery. Here, the terms, treatments for healing purposes and purely medical-technical cosmetic operations are well integrated in the category of "medical conduct(medizinische Handlung)" within the meaning of the public and administrative 'Medical Law'. In the judgment of 6. 13. 2013 Az. 2012DA94865 provides the KHGH(Korean Highest Court of Justice) to inform the patient about the prospects and risks of cosmetic surgery(Infrabrow Excision Blepharoplasty) stringent requirements, similar to the judicature of BGH(cf. BGH, Urt. v. 6. 11. 1990, Az.: VI ZR 8/90). Even in the judgment of 5. 12. 2014 Az. 2013GASO865646 the SZLG(Seoul Central Regional Court) recognizes the physician contract for 'cosmetic septoplasty' as a sort of contract for work. The medical treatment(${\ddot{a}}rztliche$ Heilbehandlung) is still regarded as a prototype of the medical activity, therefore in the meaning of the 'Civil Law(KBGB)', its term needs to be used immediately for healing purposes. The cosmetic operation, desired by a patient, differs from the healing treatment by the element of "indication" and the fact that the "healing purpose(Heilzweck)" itself is missing. In comparative context - methodically fully aware that the unreflective term transfer between different laws might contradict their legal purposes - a series of judgments BSG(BSGE 63, 83, BSGE 72, 96, BSGE, 82, 158, BSGE 93, 252 etc.) and some judgments of LSG are reviewed. In addition, also the dogmatic topic for the "legal natur of a medical treatment contract" is to reconsider by comparative introducing BGHZ 63, 306. Now in view of the current state of greater popularity of artificial cosmetic surgery still indeed is the sentences: The doctor is minister naturae, a helper of nature. A doctor promises regularly only the proper treatment of the patient, but the contractual liability for work should not be excluded in medical conditions for cosmetic surgeries altogether. Issues and Considerations surrounding Revocation Physician's Medical License Arising from Criminal Offenses (의사의 형사범죄에 따른 면허취소처분의 쟁점과 고려사항)

  • In recent years, there have been opinions in which physicians are liable to the revocation of their medical license if they are sentenced to above a certain level for criminal charges regardless of the types of offenses. Accordingly, a revised bill of law was submitted in the National Assembly, and related discussions are thus expected to commence. Considering the morality and ethics or the level of the rule of law that the general public expects of physicians, as well as the license revocation system in other professional sectors, it is assessed that medical license revocation due to criminal convictions of physicians is appropriate to some degree. However, if a poorly devised system is established based on unrefined inferences or emotional judgements, unexpected side-effects are likely to arise. With regard to serious criminal acts that society generally perceives as unacceptable, it can be assessed that the revocation of physicians' licenses would appropriately protect the general public from threats. However, given the life-saving characteristics of high-risk medical practices, higher malpractice exposures, and social values, it is difficult to assess charges of professional negligence resulting in death(or in injury) and minor offences in the same manner as anti-social criminal offences are handled. Physicians need to be treated the same as any other professions. At the same time, they are engaged in administering medical treatment to patients in the face of great risks as professionals. Under the circumstances, a discussion on the introduction of a more specific and empirical system is needed by considering the intrinsic characteristics of medical treatment and the need for an equitable health and medical policy. Accordingly, based on the above judgment and perception, this study explores the code of ethics for physicians and medical license revocation related to criminal offences at home and abroad, and examines various legislative alternatives appropriate for the Republic of Korea. In doing so, the purpose of the study is to contribute to the development of a reasonable system for handling criminal offences by physicians.

The Effects of Sight Alignment Method on Pistol Shooting Records of Security Guard (경호요원 권총사격시 조준선정렬방식과 기록변화와의 관계)

  • Some of individuals working at various fields of security guard are given the legal right to use a gun to cope with expected risks and crimes. Not only in Korea but also throughout the world, a pistol is being used as a device for the workers of security guard such as the army, the police and intelligence agencies to keep the law and social order. A pistol, which is regarded as the last safety measure of security guard, is easy to carry but, because of its short barrel for high portability, its hit ratio and long-range shooting performance are much lower than ordinary guns. Despite such weak points, a pistol can be a great tool because of its high portability and if the users make steady efforts to improve their skills and, for this reason, it has been being used widely until now. National agency workers' pistol shooting skills depend on several important factors, but for the fast and accurate shooting, the mot important factor is how to aim at the target. Because it takes a relatively longer time to align the sight on the target, the change of the sight alignment method is expected to improve pistol shooting skills a lot. In Korea, however, most of national agency training centers teach the basic aiming method that crosses the foresight and the backsight and aligns the crossing to the target. That is, they teach the sight alignment method for fixed targets. Some agencies in Korea and foreign countries that have used guns more frequently and longer than us are teaching different sight alignment methods as well. Representative ones of them are aiming only with the foresight and pointing shooting. These aiming methods are expected to produce good results against moving targets. While the basic sight alignment method has to cross the foresight and the backsight and then to align the crossing to the target, so takes a longer time than other aiming methods, the other two aiming methods are expected to be effective in fields where both swiftness and accuracy are require. Because domestic agencies are teaching mainly the basic sight alignment method, the present study purposed to examine the effects of different aiming methods on shooting records and ultimately to contribute to national agency workers' operation. For this purpose, we selected 20 policemen from local police commandos under the National Police Agency who were accustomed to pistol shooting and measured their pistol shooting. According to the results, the pointed shooting method showed the best record in 5m short.range shooting. As to hit ratio in 15m shooting, which is relatively long.range for pistol shooting, the basic aiming method showed the best record, but the results of statistical analysis showed that pointed shooting is most superior. Of course, the results were obtained from a group who were used to and skillful in pistol shooting, but they are considered meaningful in that all the police and national agency workers should have such high shooting skills.

Study on Recognition Attitudes of Residents on Safety Management against Disasters of Local Governments: Focused on Chungcheongbuk-do (지방자치단체의 재난안전 관리에 대한 주민 인식태도 연구 - 충청북도 지역을 중심으로 -)

  • This study analyzed safety management system against disasters perceived by local residents of Chungcheongbuk-do and then examined the policy directions to be considered in order for local governments to improve the safety level of residents and build an effective safety management system against disasters. The findings were as follows. First, in their recognition of risks of safety against disasters, recognition on the possibility of the occurrence of natural disasters was higher than that of social disasters or safety accidents. Secondly, also in the aspect of the importance of category of safety management against disasters, they recognized that of natural disasters far higher than others. Third, they showed satisfaction higher than average with basic job performance of local governments related with safety management, whereas they showed relatively less satisfaction with the aspects of check and publicity of risk factors, and short-term restoration system out of phased job performance. Fourth, in the aspect of capability of local governments for safety management against disasters, they rated positively capability of the responsible departments and the professionality, whereas they relatively underestimated the scale or budget of safety-related organizations. Fifth, the policy directions to be taken for safety against disasters by local governments included strengthening of regular education like experience-based training, expansion of education among local residents, more support for relevant facilities and resources, activation of residents-participating campaigns, improvement of apparatus and personnel treatment related with firefighting and security, frequent patrol and oversight, more exercises against disasters. So, to strengthen safety management system against disasters in local governments and build a effective responding system may need to extend programs assisting vulnerable class to safety against disasters, build a community-friendly safety management system, extend the cooperation system by participation of residents, enhance collaboration and support system with safety-related bodies like police, firefighters.

The Clinical Experiences of Patch Angioplasty in Isolated Critical Left Main Coronary Artery Stenosis (첨포를 이용한 좌주관상동맥 협착증의 치료)

  • The conventional surgical treatment of isolated critical stenosis of the left main coronary artery restores a less physiologic perfusion of the myocardium, leads to occlusion of the left coronary ostium, and consumes an appreciable length of bypass material. From June 1994 to February 1996, eleven patients, three male and eight female, underwent patch angioplasty and additional bypass graft to left anterior descending artery (10 internal mammary artery, 1 saphenous vein) in isolated critical left main coronary artery stenosis. Their ages ranged from 34 to 62 years, mean 44 years. All had 60% to 90% stenosis of the left main coronary artery and Class III angina. The angiogram showed nine osteal lesion and three main stem stenosis. The operation was performed with conventional cardiopulmonary bypass and cold blood cardioplegia. We approached anteriorly and used bovine pericardium as onlay patch in all patients. There were one leg wound dehiscence, but no operative deaths and infarctions. All patients are free of symptoms after a mean follow-up of 15.5 months. Angiographic restudy at an average 14.4 months was obtained in five patients and showed widely patent left main coronary artery with excellent runoff. But additional graft to left anterior descending coronary artery were stenosed in two patients and showed diminutive flow in others. Our preliminary results suggest that angioplasty of the left main coronary artery can be carried out with low operative risks. But additional bypass graft to left anterior descending coronary artery may be unnecessary. The technique appears to be a promising alternative to conventional coronary artery bypass grafting in isolated left main coronary artery stenosis.

The Effects of Glucose, Insulin and Angiotensin II on Plasminogen Activator Inhibitor-1 Expression and Growth of Aortic Vascular Smooth Muscle Cell in Rats (포도당, 인슐린 및 Angiotensin II가 흰쥐 대동맥평활근세포의 Plasminogen Activator Inhibitor-1 발현 및 성장에 미치는 영향)

  • Background: Plasminogen activator inhibitor-1(PAI-1) is known as the primary physiological inhibitor of tissue-type plasminogen activator(t-PA) in the plasma, and is present within the atherosclerotic vessels. Increased plasma levels of PAI-1 are one of the major disturbances of the hemostatic system in patients with diabetes and/or hypertension, and may have multiple interrelations with the important risk factors in the development of atherosclerosis. This study was performed to determine whether altered gene expression of PAI-1 occurs within the arterial wall, and thereby potentially contributing to the increase of cardiovascular risks associated with diabetes and/or hypertension. Material and Method: The aortic vascular smooth muscle cells of the rat were exposed to 22 mM glucose, angiotensin II, and insulin increased PAI-1 mRNA expression with the use of Northern blotting were examined. Also examined were the effects of 22 mM glucose, angiotensin II and insulin on the growth of the rat's aortic smooth muscle cells by using MTT assay. Result: Twenty-two mM glucose treatment increased the PAI-1 mRNA expression in a time- and dose-dependent manner. Aniotensin II treatment synergistically increased the glucose-induced PAI-1 mRNA expression. In contrast, addition of insulin attenuated the increase of 22 mM glucose and angiotensin II induced PAI-1 mRNA expression. Furthermore, treatment of 22 mM glucose, angiotensin II and insulin resulted in a significant increase in cell numbers. This study demonstrated that 22 mM glucose and angiotensin II have a synergistic effect in stimulating the PAI-1 mRNA expression and in the cell growth of the rat's aortic smooth muscle cells. Conclusion: Elevation of glucose and angiotensin II may be important risk factors in impairing fibrinolysis and developing atherosclerosis in diabetic patients.
