A Study on the Perception of Students in the Radiation Study on the Clinical Practices (임상실습에 대한 방사선 전공 학생들의 인식에 관한 연구)

  • The clinical practices provide the students with a good opportunity to study the practical experiences in their field through the clinical training education in hospital. Now, in this study, comparing the perceptions of the clinic teachers with those of students at the clinical site. The study was conducted to the students attending universities located in Seoul and who finished the clinical practices in 2013. The questionnaires were distributed to the student and collected from them. The study were conducted to 275 questionnaires with frequency analysis, crosstabs, chi-square test and McNemar test. The major motivation was of the select radiography course was high employment rate(44.0%) and the satisfaction of radiography course was general(53.1%). 51.3% of the study answered 8 weeks current duration of clinical practices is proper. The 3-year course students answered that the period of clinical practices would be proper if it is performed in the winter vacation in their second year in college(47.3%). The 4-year course students answered that the first semester in their third years is proper for clinical practices( 27.7%). The students answered that they felt the lack in their knowledge on the professional field(32.4%) during the clinical practices and some of the practical training is different from the education performed at school(68.4%). Most of answered that they were satisfied with the clinical practices and among them they recognized the importance of the clinical practices ($3.94{\pm}0.89$). After the clinical practices, their desire for getting job as a radiography has changed from 84.1% to 82.9%. The reason why they want the job related to the radiation is because the job is stable (changed from 49.0% to 46.0% after the clinical practice) while the reason why they don't want be a radigrapher because that job is not proper for them (changed from 37.0% to 40.7% after the clinical practice) The effort should be made to enhance the position of radiation professionals through the improved education system to the students, rather giving them education for just employment.

  • Both bronchial leiomyoma and pulmonary sequestration are rare conditions, and to the best of our knowledge there are no reports of the two conditions coexisting. We report a female patient with bronchial leiomyoma with acquired pulmonary sequestration who presented with dyspnea, cough and purulent sputum. The patient had been treated for pneumonia at a local medical clinic. but was transferred to our clinic beacausr there was no clinical improvement. A 3-D computed tomography scan revealed a 1.5 cm sized mass near the distal portion of the left main bronchus and an anomalous artery arising from the aorta. The patient showed clinical improvement after a left lower lobectomy and a ligation of the anomalous artery.

Effects of skin temperature change, cold pain and muscle activity by Cold Air Application type on the induced delayed onset muscle soreness (지연성 근육통 유발 후 냉기 적용 방법이 피부온도의 변화, 냉각 통과 근육 기능에 미치는 효과)

  • The purpose of this study was to after induced delayed onset muscle soreness, the purpose of this study is to present the effective way on skin temperature changes and cold pain for 14 subjects during to applied with only Cold-jet stream and Cold-jet stream with infra red. 14 healthy men and women who delayed onset muscle soreness eccentric contractions induced by exercise and then the biceps alone was applied to the Cold-jet stream. We measured the time that skin temperature fall from room temperature to $10^{\circ}C$(first period), the time rewarmed from $10^{\circ}C$ to $20^{\circ}C$(second period), the time fall again to $10^{\circ}C$(third period) and the time rewarmed again to $20^{\circ}C$(fourth period). Cold-jet stream with infrared combination therapy was performed with the same method. Results of this study were 1st and 2nd experimental cooling experiments in the to fall to $10^{\circ}C$ Cold-jet stream with infrared combination therapy than in the Cold-jet stream was longer(p<.05). At second period, It took longer in Cold-jet stream with infrared rewarmed than Cold-jet stream to rewarm skin(p<.05). Cold-jet stream with infrared combination therapy than Cold-jet stream had less incidence of cold pain(p<.05). Thickness of biceps brachii were found significant difference related measurment each group was consistent. In this study, Cold-jet stream with infrared combination therapy more effective than Cold-jet stream in reduced cold pain and lowering skin temperature. The Effects of Education and Test Information Provision on Anxiety, Discomfort, and Satisfaction of Patients During Double Contrast Barium Enema (대장 조영검사에 대한 교육 및 검사정보 제공이 환자의 불안, 불편감, 검사 만족도에 미치는 영향)

  • The purpose of this study was to evaluate the effects of education and test information provision on anxiety, discomfort, and satisfaction of patients during double contrast barium enema. The experiments were conducted from October 11, 2010 to November 5, 2010. Among all patients who visited the hospital to receive colon study in the outpatient radiology clinic of one general hospital located in Seoul, a total of 50 patients fit for selection criteria were divided into an experimental group (25) and a control group (25). After providing education and test information to the experimental group, we evaluated patients' levels of anxiety and discomfort, as well as satisfaction. The experimental group and the control group had different anxiety scores: 3.76 versus 6.04 respectively (P<0.05). In addition, the levels of abdominal pain and anal pain in the experimental group were lower than those of the control group (P<0.05). Lastly, the scores of test satisfaction between two groups were 89.6 and 67.4 respectively (P<0.05). Education and test information provision about the colon study decreased the levels of patients' anxiety and discomfort and increased satisfaction level of the test.

Performance Evaluation of the Tumor Tracking Method Using Beam on/off Interface for the Treatment of Irregular Breathing (호흡이 불규칙한 환자의 치료를 위한 Beam on/off Interface를 이용한 종양 추적 치료 방법의 성능 평가)

  • Dose rate regulated tracking is known to be an efficient method which adaptively delivers tracking treatments when patient breathing is irregular. The Motion Management Interface (MMI, Varian Medical System, CA), which provides beam on/off switching during treatment is available for clinic. Study is to test if delivering the adaptive tumor tracking is feasible for irregular breathing using beam switching with MMI. 55 free breathing RPM traces acquired from lung cancer patients are used. The first day RPM traces of the patients are utilized to design preprogrammed tracking MLC patterns, of which periods are intentionally reduced by 20% in order to catch up the variation of patient breathing irregularity in the treatment day. Eligibility criteria for this technique are the variation of amplitude and period less than 20%. An algorithm which determines beam on/off every 100 ms by considering the preprogrammed (MLC) positions and current breathing positions is developed. Tracking error and delivery efficacy are calculated by simulating the beam-switching adaptive tracking from the RPM traces. Breathing patterns of 38 patients (70%) met the eligibility criteria. Tracking errors of all of the cases who meet the criteria are less than 2 mm (average 1.4 mm) and the average delivery efficacy was 71%. Those of rest of the cases are 1.9 mm and 48%. Adaptive tracking with beam switching is feasible if patient selection is based on the eligibility criteria.

Transcatheter arterial chemoembolization and radiation therapy for treatment-na$\ddot{i}$ve patients with locally advanced hepatocellular carcinoma

  • Purpose: To evaluate the safety and efficacy of transcatheter arterial chemoembolization (TACE) followed by radiotherapy (RT) in treatment-na$\ddot{i}$ve patients with locally advanced hepatocellular carcinoma (HCC). Materials and Methods: Eligibility criteria were as follows: newly diagnosed with HCC, the Barcelona Clinic Liver Cancer stage C, Child-Pugh class A or B, and no prior treatment for HCC. Patients with extrahepatic spread were excluded. A total of 59 patients were retrospectively enrolled. All patients were treated with TACE followed by RT. The time interval between TACE and RT was 2 weeks as per protocol. A median RT dose was 47.25 $Gy_{10}$ as the biologically effective dose using the ${\alpha}/{\beta}$ = 10 (range, 39 to 65.25 $Gy_{10}$). Results: At 1 month, complete response was obtained in 3 patients (5%), partial response in 27 patients (46%), stable disease in 13 patients (22%), and progressive disease in 16 patients (27%). The actuarial one- and two-year OS rates were 60.1% and 47.2%, respectively. The median OS was 17 months (95% confidence interval, 5.6 to 28.4 months). The median time to progression was 4 months (range, 1 to 35 months). Grade 3 or greater liver enzyme elevation occurred in only two patients (3%) after RT. Grade 3 gastroduodenal toxicity developed in two patients (3%). Conclusion: The combination treatment of TACE followed by RT with two-week interval was safe and it showed favorable outcomes in treatment-na$\ddot{i}$ve patients with locally advanced HCC. A prospective randomized trial is needed to validate these results.

  • The purpose of this study is to confirm the safety of the clinical application of image co - registration in steteotactic radiosurgery by evaluating the 3D positioning of magnetic resonance imaging using image co-registration. We performed a retrospective study using three-dimensional coordinate measurement of 32 patients who underwent stereotactic radiosurgery and performed magnetic resonance imaging follow-up using image co-registration. The 3 dimensional coordinate errors were $1.0443{\pm}0.5724mm$ (0.10 ~ 1.89) in anterior commissure and $1.0348{\pm}0.5473mm$ (0.36 ~ 2.24) in posterior commissure. The mean error of MR1 (3.0 T) was lower than that of MR2 (1.5 T). It is necessary to minimize the error of magnetic resonance imaging in the treatment planning using the image co - registration technique and to confirm it.

Analysis of the cause by Pre Exposure Tube Voltage and Actual Exposure Tube Voltage deviation in Mammography Examination (유방 촬영검사에서 사전조사 관전압과 실제조사 관전압 편차에 따른 원인 분석)

  • The purpose of this study is to investigate the cause analysis according to the difference between the pre exposure tube voltage and actual exposure tube voltage in mammography in connection with breast pressure thickness, breast size and body mass index and to find the improvement. The study tracked 377 women age 40 and older among the mammography examiners conducted by the National Health Insurance Corporation. It was analyzed that breast pressure thickness, breast size and body mass index according to the difference between the pre exposure tube voltage and actual exposure tube voltage among the parameters of dose report been sent to the picture archiving communication system with reference to the image with cranio-caudal projection in mammography. As are result, it shows that the thicker the breast thickness, smaller the breast size and lower body mass index, the higher the difference of tube voltage. In conclusion, the minimum tube voltage of mammography machine should be reset in order to set the tube voltage according to breast pressure thickness and breast size that are suitable for our country in mammography, in addition, it was considered that radiologist should make an effort to reduce radiation exposure and make a good quality image with reducing the difference of mammography condition by making a correct exposure condition in case of examining the patients with thin breast pressure and small breast size.

  • Chronic kidney disease can be treated if it is detected early, but as the disease progresses, it becomes impossible to recover. Finally, renal replacement therapy such as transplantation or dialysis should be used. Ultrasonography is used to diagnose kidney cancer, inflammatory disease, nodular disease, and chronic kidney disease. It is used to identify information about degree of inflammation using information such as kidney size, internal echo characteristics. Currently, the degree of disease in the clinic uses the value of glomerular filtration rate. However, even in ultrasound, changes in the degree of inflammation and disease can be observed. In this study, we used ultrasound images to quantify the changes in brightness, size, cortex, and subclinical changes of the kidney with progression of the disease, and compared them with the glomerular filtration rate used in clinical practice. In 105 cases, we performed 35 cases of normal kidney, 35 cases of early kidney disease, and 35 cases of terminal kidney. The brightness of the cortex of the image was obtained and the difference in brightness between the cortex and the proximal portion was obtained by the slope. The graph of the portion which was not smooth due to the ultrasonic characteristics was used as the function regrass. The size reduction was obtained from the original data. The results were as follows: It was proportional to the glomerular filtration rate. It is considered that the algorithm can be applied to the disease if the algorithm study continues.

The Effect of Botulinum Toxin A on Calf Reduction (Type A Botulinum Toxin이 장딴지근 퇴축에 미치는 영향)

  • Botulinum toxin type A is widely used for anti-wrinkling therapy, and correction of the square face. The toxin ultimately prevent the release of membrane-bound acetylcholine at the neuromuscular junction of striated muscles and thus produce chemical denervation and paralysis of the muscles. Our purpose of study is to know if application of botulinum toxin type A on calf reduction is effective, how much dosage is effective, and what are the possible complications. We reviewed data of 30 consecutive patients subjected to calf reduction in Dong-A University Hospital from February 2003 to April 2003. We injected normal saline 2cc on both calves region in 15 control group patients, and the other patients was divided 3 group. Group 1, Group 2, Group 3 was injected 50U, 100U, 150U botulinum toxin A on each calf region and followed up for 6 month. Maximal circumference of calf was not changed in the control group but an average of 0.7 cm reduction was noted in group 1, average 1.34 cm(right calf) and 1.26cm(left calf) in group 2, average 1.44cm(right calf) and 1.58cm(left calf) in group 3. Maximal area of calf was not changed in the control group but average reduction of 12.5%(right calf) and 12.7%(left calf) was obtained in group 1, average 19.4% (right calf) and 19.9%(left calf) in group 2, average 24.8%(right calf) and 21.07%(left calf) in group 3, as measured on CAT scan. Total fat amount and fat amount in the lower extremity was no change in all the groups, but lean body mass was decreased average 1.27%(right calf) and 1.15%(left calf) in group 1, average 3.47%(right calf) and 2.98%(left calf) in group 2, average 3.58%(right calf) and 3.95%(left calf) in group 3. Photography of the preoperative and postoperative 6 month state revealed higher satisfaction in Group 2, 3 compared to Group 1. Use of botulinum toxin type A in calf reduction is a very simple, safe, non-invasive method and effective in terms of calf contouring rather than reduction of calf circumference.
