EFFECT OF PLATELET-RICH PLASMA ON BONE FORMATION IN DISTRACTED AREA OF CANINE MANDIBLE (성견하악골의 신연 부위에서 골형성에 대한 혈소판-풍부 혈장의 효과)

  • Distraction osteogenesis refers to the biological process responsible for new bone formation between bone segments by gradual distraction after osteotomy. For the past several years, various inconveniences including a protracted consolidation period that requires patients to wear a distractor frame longer, as well as higher medical costs, have not been remedied by improvements in osteotomy, distraction rate and monitoring system. Furthermore, side effects such as pin tract infections and soft tissue swelling may arise due to the long treatment period. These drawbacks form the rationale of this study which purports to seek a method by which the consolidation period can be reduced. This paper examines how platelet-rich plasma(PRP), known to facilitate osteogenesis, influences bone formation when applied in distracted area. Ten mongrel dogs, which were made to wear external distractor frames after osteotomy in both sides of the mandible, were used as subjects. After a 7day period of latency, distraction was carried out at a rate of 1mm/day for 14 consecutive days. After the onset of distraction, 2ml of PRP and a mixture of calcium gluconate and thrombine were injected into the center of the distracted callus on the left side of the mandible. The left was injected with PRP while the right side was set as the control site without PRP treatment. Execution at the onset of distraction and in 2 weeks, 4 weeks and 8 weeks after the consolidation period, clinical and radiographic tests, bone mineral density examination, histological examination and histomorphometric analysis were conducted to compare both sides. The results are summarized as follows: 1. Based on the clinical examination at two weeks, more remarkable cortical bone formation was found on the buccal and lingual side of the distracted area in the PRP treatment site than in the control site. No visual difference was found between the PRP treatment site and the control site at four eight weeks. 2. Based on the radiological examination, a distinct increase in the radiopaque appearance of the PRP treatment site was revealed at two weeks, but this increase appeared to slow down at four and eight weeks. 3. Examination of bone mineral density revealed a significant difference at two weeks with the PRP treatment site yielding density two times higher than the control site. This difference lessened after four weeks, and disappeared at eight weeks. 4. The histomorphometric examination revealed that about 20% more bony trabeculae area(20%, higher) was formed in the PRP treatment site than in the control site. In conclusion, it can be said that PRPs effect on stimulating bone formation in the PRP treatment site manifest as early as two weeks. Trabeculae formation likewise increased throughout the whole period. If this result can be applied to humans, the consolidation period can be reduced by injecting PRP into the distracted area.

Evaluation of the Photoprotective Effect of Dongchongxiacao ($Paecilomyces$ $japonica$) Extract against Ultraviolet Radiation-induced Skin Wrinkling and Cancer (자외선 유도 피부주름 및 암발생에 대한 누에동충하초($Paecilomyces$ $japonica$)의 보호효과 평가)

  • To evaluate the ability of Dongchongxiacao ($Paecilomyces$ $japonica$) extract (PJE) to protect the skin from photodamage, the gross and microscopic changes in the skin of hairless mice and PJE-treated mice exposed chronically to ultraviolet (UV) were examined. The skin of the UV-irradiated mice showed characteristic signs of photoaging, such as deep wrinkles across the back. PJE-treated mice showed a significantly decreased wrinkling score. By the 22nd week, 88.9% (i.p. with saline) or 44.4% (topical administration with cream base) of the UV-irradiated mice developed at least one tumor. PJE delayed tumor onset significantly. PJE (i.p.) was also effective in reducing the occurrence of UV radiation-induced skin tumors and reduced the number of tumors per mouse. After 22 weeks of treatment, 80.0% (i.p.) and 75.0% (topical) of the mice treated with PJE were tumor-free. Tumor multiplicity was reduced by 96.2% (i.p.) in the PJE treated groups. It is noted that skin that is chronically exposed to UV is subject to photoaging and photocarcinogenesis and regular use of PJE would prevent these photodamaging effects of UV.

Glutamine Deprivation Inhibits Invasion of Human Prostate Carcinoma LnCap Cells through Inactivation of Matrix Metalloproteinases and Modulation of Tight Junctions (글루타민 결핍에 따른 Tight Junction 및 MMPs 활성 조절을 통한 전립선 암세포의 침윤 억제 현상)

  • Cancer cells exhibit increased demand for glutamine-derived carbons to support anabolic processes. Indeed, the spectrum of glutamine-dependent tumors and the mechanisms through which glutamine supports cancer metabolism remain areas of active investigation. In the present study, we investigated the effects of glutamine deprivation on the correlation between tightening of tight junctions (TJs) and anti-invasive activity in human prostate carcinoma LnCap cells. Glutamine deprivation markedly inhibited cell motility and invasiveness in a time-dependent manner. The anti-invasive activity of glutamine deprivation was associated with an increased tightness of the TJ, which was demonstrated by an increase in transepithelial electrical resistance (TER). The activities of matrix metalloproteinase (MMP)-2 and MMP-9 were inhibited in a time-dependent fashion by glutamine deprivation, which was correlated with a decrease in expression of their mRNA and proteins and up-regulation of tissue inhibitors of metalloproteinases (TIMPs) expression. Furthermore, glutamine deprivation repressed the levels of the claudin family members, which are major components of TJs that play a key role in the control and selectivity of paracellular transport. Moreover, the levels of E-cadherin, a type I transmembrane glycoprotein, and snail, an epithelial to mesenchymal transition regulator and zinc finger transcription factor, were markedly modulated by glutamine deprivation. Taken together, these findings suggest that TJs and MMPs are critical targets of glutamine deprivation-induced anti-invasion in human prostate carcinoma LnCap cells.

What Effects Does Necrotic Area of Contrast-Enhanced MRI in Osteoporotic Vertebral Fracture Have on Further Compression and Clinical Outcome?

  • Objective : The objective of this study was to analyze the correlation between further compression and necrotic area in osteoporotic vertebral fracture (OVF) patients with contrast-enhanced magnetic resonance imaging (CEMRI). In addition, we investigated the radiological and clinical outcome according to the range of the necrotic area. Methods : Between 2012 and 2014, the study subjects were 82 OVF patients who did not undergo vertebroplasty or surgical treatment. The fracture areas examined on CEMRI at admission were defined as edematous if enhancement was seen and as necrotic if no enhancement was seen. The correlation between further compression and the necrotic and edematous areas of CEMRI, age, and bone mineral density was examined. Also, necrotic areas were classified into those with less than 25% (non-necrosis group) and those with more than 25% (necrosis group) according to the percentages of the entire vertebral body. For both groups, further compression and the changes in wedge and kyphotic angles were examined at admission and at 1 week, 3 months, and 6 months after admission, while the clinical outcomes were compared using the visual analog scale (VAS) and Eastern Cooperative Oncology Group (ECOG) performance status grade. Results : Further compression was $14.78{\pm}11.11%$ at 1 month and $21.75{\pm}14.43%$ at 6 months. There was a very strong correlation between the necrotic lesion of CEMRI and further compression (r=0.690, p<0.001). The compression of the necrosis group was $33.52{\pm}12.96%$, which was higher than that of the non-necrosis group, $14.96{\pm}10.34%$ (p<0.005). Also, there was a statistically significantly higher number of intervertebral cleft development and surgical treatments being performed in the necrosis group than in the non-necrosis group (p<0.005). Moreover, there was a statistical difference in the decrease in the height of the vertebral body, and an increase was observed in the kyphotic change of wedge angle progression. There was also a difference in the VAS and ECOG performance scales. Conclusion : The necrotic area of CEMRI in OVF had a strong correlation with further compression over time. In addition, with increasing necrosis, intervertebral clefts occurred more frequently, which induced kyphotic changes and resulted in poor clinical outcomes. Therefore, identifying necrotic areas by performing CEMRI on OVF patients would be helpful in determining their prognosis and treatment course.

High Resolution Time Resolved Contrast Enhanced MR Angiography Using k-t FOCUSS (k-t FOCUSS 알고리듬을 이용한 고분해능 4-D MR 혈관 조영 영상 기법)

  • Purpose : Recently, the Recon Challenge at the 2009 ISMRM workshop on Data Sampling and Image Reconstruction at Sedona, Arizona was held to evaluate feasibility of highly accelerated acquisition of time resolved contrast enhanced MR angiography. This paper provides the step-by-step description of the winning results of k-t FOCUSS in this competition. Materials and Methods : In previous works, we proved that k-t FOCUSS algorithm successfully solves the compressed sensing problem even for less sparse cardiac cine applications. Therefore, using k-t FOCUSS, very accurate time resolved contrast enhanced MR angiography can be reconstructed. Accelerated radial trajectory data were synthetized from X-ray cerebral angiography images and provided by the organizing committee, and radiologists double blindly evaluated each reconstruction result with respect to the ground-truth data. Results : The reconstructed results at various acceleration factors demonstrate that each components of compressed sensing, such as sparsifying transform and incoherent sampling patterns, etc can have profound effects on the final reconstruction results. Conclusion : From reconstructed results, we see that the compressed sensing dynamic MR imaging algorithm, k-t FOCUSS enables high resolution time resolved contrast enhanced MR angiography.

The Associated Factors of Protective Behaviors for Radiation Exposure based on Health Belief Model Honam Province Radiologic Technologists (건강신념모델을 적용한 호남지역 방사선사의 방사선 방어행위 수행도 관련 요인)

  • This study aimed to identify the associated factors of protective behaviors for radiation exposure among some radiology technologists using the Health Belief Model. The subjects of the study were 541 radiology technologists working at hospitals or clinics in Honam Province. Using the SPSS version 18.0 program, data were analyzed using a t-test, ANOVA, Pearson's correlation analysis, and hierarchical multiple logistic regression analysis. To modify the factors, the performance of subjects who had a higher level of education and nuclear medicine rooms were higher than those who worked in simple radiography rooms. The radiation protective behaviors performance of the subjects who had more exercise, medium-level stress, and worked in higher-quality protection facilities was higher. Regarding the personal perceptions, the cues to action (β=.292, p=.0001), and perceived seriousness (β =.075, p=.010) were factors that had effects on the performance of radiation protection behaviors. Regarding the likelihood of action, the benefits (β=.168, p<.0001), self-efficacy (β=.148, p=.007), and the performance of protective behaviors were higher. In conclusion, protection education as a cue to action should be provided to stimulate protective behaviors, and the benefits of protective behaviors should be emphasized. To increase the performance of protection behaviors, self-efficacy should be enhanced, and the subjects are offered appropriate information that helps perceive seriousness.

Production of NO, TNF-α and IL-6 by Squalene, Alkoxy Glycerol, Batyl and Chimyl Solutions in RAW 264.7 Macrophage Cells (RAW 264.7 대식세포에서 NO, TNF-α, IL-6 생산에 관한 Squalene, Alkoxy Glycerol, Batyl, Chimyl의 효과)

  • The purpose of the present study was to evaluate the effect of Squalene, Alkoxy Glycerol, Batyl and Chimyl solutions on the production of nitric oxide (NO), tumor necrosis factor-alpha (TNF- $\alpha$ ) and interleukin-6 (IL-6) in RAW 264.7 macrophage cells after treatment of C.albicans. The cytotoxic effects was evaluated by the lactate dehydrogenase (LDH). All solutions did not affect on the LDH and NO production by itself. At 6th hour, the TNF- $\alpha$ and IL-6 production was not affected by each solutions. However, at 24th hour, the TNF- $\alpha$ and IL-6 production was affected by itself (p < 0.05). Each solution in the presence of C. albicans decreased the C. albicans-induced TNF- $\alpha$ and IL-6 production compared with C. albicans only (p < 0.05, p < 0.01). These results suggest that Squalene, Alkoxy Glycerol, Batyl and Chimyl solutions will increase the immune response on the C. albicans-induced damage.

Research on the Hematological Changes in Accordance with Radiation Dose and Radiation Exposure period of the Medical Radiation Workers (의료 방사선 종사자의 피폭기간 및 피폭선량과 혈액성분 변화에 대한 조사)

  • In this study, we analyzed the effects of radiation exposure, as compared to the hematological parameters change of medical radiation workers and the public. The mean value of all hematological parameters were in the normal range. Eosin mean value of the radiation workers($2.52{\pm}1.79%$) showed that a significantly lower than the control group($2.92{\pm}1.39%$). In the comparison of the results depending on the occupation period, it showed high value that the mean of the radiation workers group WBC, platelet, Lymph, Mono, Baso. Over 20 years of radiation workers WBC, Mono showed low values and less than 10 years of radiation workers mean value of Baso showed low values, there was no statistical significance. In the comparison of the results depending on the 4 years cumulative radiation dose, Over 5.0 mSv of Radiation works RBC($4.61{\pm}0.53$ vs $4.91{\pm}0.38$), Hct($41.51{\pm}4.07$ vs $43.97{\pm}3.40$), Eosin($1.74{\pm}1.14$ vs $2.92{\pm}1.39$) showed low value, it was statistical significance. 0.5~1.0 mSv radiation exposure workers Hb ($13.93{\pm}1.75$) showed a significantly lower value than that of the control group ($14.90{\pm}1.29$).

Protective Effects of Chitosanoligosaccharide on Gamma Ray-Induced Hepatic Damage in Mice (생쥐에서 키토산올리고당의 감마선 저항 효과)

  • The purpose of this study was to evaluate the effect the radiation-resistance of chitosan on the mice. A healthy male ICR mice were used for experiment. The SOD and MDA activity was measured from the liver of mice at 48 and 72 hours after the irradiation. The ultrastructural changes of the liver by irradiation was observed at 24, 48 and 72 hours after irradiation. The experimental groups were divided into three groups. Group 1 was the control group which was not treated with chitosanoligosaccharide before or after iradiation. Group 2 was the prefeeding group which chitosanoligosaccharide solution was supplied by feeding ad libitum for 30 days before irradiation. Group 3 was the postfeeding group which chitosanoligosaccharide solution was supplied by feeding after irradiation. In all groups 10 mice were used. The results were as follow: The SOD and MDA activity of the prefeeding group was decreased significantly (P<0.01). Control group - The nuclei were condensed. The mitochondria and rough endoplasmic reticulum (rER) were extended and the ribosome was dropped from the rER. Prefeeding group - The nuclei were rounded. The mitochondria was elongated. And the rER attached ribosomes. Postfeeding group - The nuclei were slightly condensed. The mitochondria and the rER were extended and the ribosome was dropped from the rER. It was concluded that the chitosanoligosaccharide was effective in the radiation-protection. So, chitosan would have the potential as the radiation-protection materials.

Development of a standardized mucositis and osteoradionecrosis animal model using external radiation

  • Objectives: Although the side effects of radiation therapy vary from mucositis to osteomyelitis depending on the dose of radiation therapy, to date, an experimental animal model has not yet been proposed. The aim of this study was to develop an animal model for assessing complications of irradiated bone, especially to quantify the dose of radiation needed to develop a rat model. Materials and Methods: Sixteen Sprague-Dawley rats aged seven weeks with a mean weight of 267.59 g were used. Atraumatic extraction of a right mandibular first molar was performed. At one week after the extraction, the rats were randomized into four groups and received a single dose of external radiation administered to the right lower jaw at a level of 14, 16, 18, or 20 Gy, respectively. Clinical alopecia with body weight changes were compared and bony volumetric analysis with micro-computed tomography (CT), histologic analysis with H&E were performed. Results: The progression of the skin alopecia was different depending on the irradiation dose. Micro-CT parameters including bone volume, bone volume/tissue volume, bone mineral density, and trabecular spaces, showed no significant differences. The progression of osteoradionecrosis (ORN) along with that of inflammation, fibrosis, and bone resorption, was found with increased osteoclast or fibrosis in the radiated group. As the radiation dose increases, osteoclast numbers begin to decrease and osteoclast tends to increase. Osteoclasts respond more sensitively to the radiation dose, and osteoblasts are degraded at doses above 18 Gy. Conclusion: A standardized animal model clinically comparable to ORN of the jaw is a valuable tool that can be used to examine the pathophysiology of the disease and trial any potential treatment modalities. We present a methodology for the use of an experimental rat model that incorporates a guideline regarding radiation dose.
