The aim of this study is the development of a fee - based model day care center for the elderly by inquiring into the current condition of facilities in America and in Korea, and in surveying the opinion of domestic elderly about day care facilities. A field trip to U.S. day care services was held between July 5 and July 15 in 1997, and an on-the-spot study for domestic facilities took place during March in 1998. Our research reveals that the overall supply of day care facilities can not meet future demand in terms of quality and quantity. Therefore a model must be created for day care centers of a that consists of a director from a professional group. an adequate environment, and a standardized in order to offer a qualified public health service linked to the home and community in Korea. The director of a day care center is a critical variable in determining the quality of service. Professional skills related to the needs of the elderly and the person's quality of service should be considered in appointing director for the center. This study belleves that a professional nurse should be the director of a day care center. The operating environment of a day care facility should be made up of considerable space comparable to the number of residents, should be in a comfortable and safe location, and should have equipment that provides a qualified, safe service to the elderly. Our model is designed for 20 persons and allocates 4 Peng per person. This model is comprised of a reading room. a craft room, a health room, a room for physical therapy, a dining room, a staff office, and a multi -purpose room connected to other rooms. Day care service should be a comprehensive service program meeting the multidimensional needs of the elderly. A comprehensive service program needs a team of various professionals made up of the elderly family, participants, nurses, social workers, physical therapists, nutritionists, and medical doctors. The program will also include health care service, physical therapy, speech therapy. diet, occupational therapy, transportation service, health and an education program, etc. In conclusion, a model of a day care center is developed with the following components: a professional director and an environment and program, that considers the physical, mental, and social characteristics of the elderly. A model should also motivate self-reliance self-fulfillment in the elderly in order to fulfill their health needs and to prevent isolation from society and mental depression. Furthermore, This facility will be a beneficial factor in reducing a family's burden on caring for the elderly that includes unnecessary hospital expenses. The following is a suggestion based on results this study: A service program should be developed to fit the conditions of the elderly in Korea by specifically analyzing the needs of the elderly.
Objectives : This study develops a salutogenic program and examines its effects on the sense of coherence, acculturative stress, and quality of life of married immigrant women. Methods : The salutogenic program was developed based on Antonovsky's Salutogenic Model, and the study design was a nonequivalent control group pre-post test design. The participants were married immigrant Chinese (n=30) and Vietnamese (n=31) women, who were assigned to either an experimental group (n=29) or a control group (n=32). The data were analyzed using the ${\chi}^2$-test, t-test, Fisher's exact test, and repeated measures ANOVA, and the IBM SPSS for Windows (version 20.0) program was used to perform the analysis. Results : Compared to the control group, the sense of coherence (F=52.05, p <.001) and quality of life (F=6.95, p=.002) were significantly improved in the experimental group, and the acculturative stress (F=24.64, p<.001) in this group significantly decreased after implementing salutogenic program. Conclusions : These findings indicate that a salutogenic program is an effective intervention for married immigrant women. The study suggests that such programs can be applied to married immigrant women at public health or multicultural family support centers to improve their sense of coherence and quality of life.

As the medical market has gradually changed from supplier-centered market to consumer-centered one, it makes hospital managers seek the consumer's satisfaction and the various marketing strategies for survival. Under the these changes, this study was accomplished to measure the consumer's satisfaction about the medial services which a general hospital provided, to identify the marketing strategies that a general hospital could establish for survival. For these purposes, a questionnaire was developed and distributed to 200 patients in general units and 100 patients in oriental units of a general hospital in Seoul. Among them, 163 reponses in general units and 71 reponses in oriental units were turned out to be useful. they were used for final analysis. The results of the survey were as follows : First, among all items of medical services which the hospital provided, patients were most satisfied by nursing services. Second, to Improve the hospital's medical service quality, it was very important for hospital staffs to be courtesy and to explain about a process of treatment or care sufficiently. Third, to decrease cost of medical service, it was very important to develop the various items and premiums of medical insurance especially in field of oriental medicine. Fourth, to make people come to the hospital easy, it was very important of fully utilize the public transportation such as subway. Fifth, to make hospital's image good, it was very important to develop well-prepared public relationship and to continuously provide the diverse medical service in the community.
This study was conducted to analyze the Job Satisfaction, Nursing Professionalism, and Nursing Work Environment of Comprehensive Nursing Care Ward nurses and General Ward nurses and provide a convergent basic data. The participants of this study were clinical nurses in the Seoul and Gyeonggi provinces in May 1st.~July 30th., 2016. The data was analysed using SPSS WIN 23.0. When comparing General Ward 100 nurses and Comprehensive Nursing Care Ward 92 nurses, the Nursing Work Environment levels (t=3.027, p=.003) and Job Satisfaction levels (t=2.451, p=.015) were higher in General Wards. Among the General Ward nurses, higher Nursing Professionalism levels had positive correlations to Job Satisfaction levels(r=.605, p<.001). Among the Comprehensive Nursing Care Service Ward nurses, higher Nursing Professionalism levels also had positive correlations to Job Satisfaction levels(r=.642, p<.001). In conclusion, a strategy for the establishment of Nursing Work Environment and Nursing Professionalism is necessary in order to guarantee a higher quality of comprehensive nursing care service.
Implementation of patient feedback is considered as a critical part of effective and efficient management. This study is a retrospective survey after to ensure the medical convergence analysis of contents on customer dissatisfaction using medical services. The data were collected from dissatisfaction 319 case at a affiliated hospital. The result indicate that a repeat visit than the initial visit, outpatients were more dissatisfied than the inpatients. The type of dissatisfaction were more dissatisfied of the in communication and explanation of medical service, nursing service and procedure of administrative services. In the classification of disease the percentages of dissatisfaction in the digestive system and neoplasm were high in both groups(p<0.05). Based on the study to improve the quality of medical service. Prevent the recurrence of dissatisfaction and to establish a customer-oriented business strategy. Characterized by dissatisfaction factors and contents of each patient and should be structured to specific disease types of services through continuous medical convergence research.
Nursing as it is practiced and taught, historically has been viewed as natural science. There are new movements to create a paradigm of nursing in the human sciences. A natural science methodologies elicit quantitative data from observable phenomena and reveal causal relationships. Human science methodology is the study of Unitary Man's participative experience in a situation, the simultaneity paradigm. In a theory of nursing rooted in human science assumptions about man and health are synthesized, and the practice of nursing is continuously expanded through research. To find independent nursing interventions especially cognitive nursing intervention models it has been necessary to consult a multitude of journals and text sources for references, and no one nursing textbook can be used in help patients achieve nursing goals. The goals of nursing in the simultaneity paradigm focus on the quality of life from the person's perspective. Cognitive interventions based on the person's perspective were selected from those that colleagues deemed appropriate to nursing and those that were identified in the nursing literature. They were supportive nursing care, reminiscence, meditation, relaxation and imagery. Nurses have been reluctant to make diagnoses, implement actions, and assume responsibility for this actions and this is of concern but can be understood because nurses have had little exposure to action and lack an intervention armamentarium from with which to choose actions to achieve nursing goals. Efforts in nursing education and nursing service are required to remedy this problem. Nurses must be convinced of the challenge and excitement associated with autonomous functioning. It is a characteristic of the true professional. Traditionally, nursing has prescribed one method for handling a situation. Fundamental nursing texts usually only present one way to handle a situation, because alternative interventions to achieve a client goal may be available. Considerably more research is necessary before these can be prescribed. However, unless a first step is taken, progress will not be made. The quality of health care or nursing care is enhanced when nurses transform dilemas into commited action. This is apparent from widespread experiences of nurses.
Community mental health management system emphasizing on the rehabilitation and the return to the community has been established and carried out for many years. The study has been demanded to prove that the decreasing rate of the recurrence of the mentally ill resulted to lower their medical costs, to enrich the quality of life, and to reduce the psychological burden of their family. This study tried to prove that the mental health services to the mentally ill which were registered in community mental health center of A city have an influence on the medical cost, the quality of their lives. the family burden. The subject group of this study were 39 home-based mentally ill patients and their 37 family members, totally 76 people registered in mental health center of A city and participated in its program. This research had been measured twice, the first before the intervention and the second after at least a year. The measuring tools in the research were the medical cost measurment tools developed by the researcher, the quality of life index by Yoo ja, Noh(1988) and the family burden by Montgonery(1985). The methods were modified and supplemented in this study. This research made use of SPSS Win 10.0. The results of this study are the same as followings. 1) There were the significant difference in the medical cost before and after the mental health service delivery. 2) The quality of lives of the mentally ill, after the mental health services delivered were significantly higher than before. 3) The family burden were significantly reduced after the delivery of community mental health services. Community mental health services brought out efficient results to the social return and rehabilitation. And these results means that the mentally ill changed highly the quality of life and their burden of family and medical cost were reduced. So the public organization and the private society should help positively the mentally ill and their family through mental health policy and social service agency to live healthy lives and to be valuable member of society.

Purpose: This study was conducted to determine the validity and reliability of performance measurement tools based on the nursing process for prevention and management of pressure ulcers, falls and pain. Methods: The performance measurement tools were reviewed by a panel of experts and refined on the basis of the panel's suggestions. The validity of the performance measurement tools was measured by surveying hospital nurses. The reliability of these tools was tested by having nursing experts use the tools in five nursing units to assess nursing performance in prevention and management of pressure ulcers, falls and pain. Results: The performance measurement tools in this study were found to be acceptable as tools to evaluate quality of nursing care in pressure ulcers, falls and pain. The reliability of the performance measurement tools was acceptable. Conclusion: These results indicate that the performance measurement tools developed in this study are valid and reliable instruments to monitor and improve quality of nursing care in prevention and management of pressure ulcers, falls and pain.

To improve the quality of healthcare and build up the health delivery system, electronic health record system is important for healthcare institutions providing health services. However, the systems currently operated in domestic healthcare institutions use independent software interface with distributed environment for data search and process. Therefore, it raises an additional expense to buy or develop each interface module when there is a link to new system and the problem of complexity. To solve these problems, this study implemented the service oriented architecture basis for home nursing service and carried out an assessment. The study defined the process modeling and business requirements based on the scenario of service and drew 17 suitable services based on five verification items for service design. To draw the final service, seven services were selected by service litmus test (SLT). The study found that, by the improvement of business process for healthcare service support department, SOA based information system has an effect of time reduction of patient processing time. In conclusion, to enable the hospital information system to cope with various demands of healthcare consumers, it is necessary to consider the application of SOA technology which has excellent interoperability, reusability and maintenance.
Objectives: The aim of this study was to examine factors influencing health-related quality of life of waged workers. Methods: This study used data from the Seventh Korea National Health and Nutrition Examination Survey. Data were analyzed with the $x^2-test$, t-test, ANOVA, and multiple regression. Results: The significant predictors of health-related quality of life were depression, subjective health status, age, activity limitation, education, worker status, monthly income, drinking, smoking, physical injury, stress, and moderate physical activity. Conclusions: Personal health characteristics, psychological characteristics, socioeconomic characetrictics and working characetrictics affect health-related quality of life. Health-related quality of life is also affected by social structural problems such as socioeconomic factors and employment instabilitye. In addition to the development of health care programs to improve health-related quality of life of twaged workers, policy changes are needed to improve the social structure.
