Purpose: The purpose of this study was to determine the effects of 5 weeks self-help management program developed by Kim et al. (2000-b) and reinforced with music therapy for post stroke patients. Method: This was pre-experimental study and the subjects were 14 post stroke hemiplegic patients at home who were registered at the Gunsan Health Center. The program was applied for five weeks and two hours for each session composed of the preparation step, the main step and the finishing step. The contents of the program were ROM exercise, daily activity training, risk factors and aftereffects, nutrition management and stress management. To collect data, all subjects were questioned before and after the application of the program. Collected data were analyzed through frequencies, percentages, Wilcoxen signed rank test and Cronbach's alpha using SPSS-WIN program. Result: After the application of the self-help management program, a statistically significant increase was observed in the subjects' ability to perform activities of daily living (p=.039), ability to perform instrumental activities of daily living (p=.005), self-efficacy (p=.001), self-care behavior (p=.001) and quality of life (p=.001), and a statistically significant decrease was observed in depression (p=.012). Conclusion: The present self-help management program was found to be helpful in improving the subjects' physical and psychological functions after they were attacked by stroke, so to be an effective nursing intervention strategy for post stoke patients. Future researches need to reinforce and materialize music therapy and to develop and apply a self-help management program that includes not only post stroke patients but also their families. In addition, it is necessary to expand the scope of subjects and apply follow-up management in order to continue self-help meetings.
This study was uncertaken to find an effective management modality for patients with hypertension. Patients 60years or older were selected for this purpose from a general hospital and a welfare center in Pusan and from a public health clinic in Cheju Island. During the month of October in 1984. the management modality for hypertension. the incurred medical expenses. and knowledge about the hypertension were analyzed and compared. The following results were obtained: 1. The a erage total medical expense per year was highest at the general hospital followed by the public health clinic and the welfare center. 2. It was found that the patients chose the public health clinic for its low medical cost. the welfare center for its proximity as well as for its low medical cost. and the general hospital for its quality medical services Also. the study has shown that the prescription of drugs was the major service provided by the public health clinic whereas the blood pressure measurement. the prescription of drugs. and family planning were the primary service offered by the welfare center. Drug prescription and blood pressure measurement were two of the most important services by the general hospital. 3. It was shown that the knowledge of hypertension was highnest at the welfare center whereas the management of hypertension was highest at the public health clinic. Based on above results and considering the chronic nature. it is believed that family awareness of the illness was one of the most important aspects for effective management of the hypertension for the elderly patients. In addition. the health care clinics at every level should come forward to execition of the educational programs and home nursing schomes for the effective management of the hypertension patients.

Purpose: This study examined the effects of heat therapy combined with aromatherapy on pain, flexibility, sleep, and depression in elderly women with osteoarthritis. Methods: This study was designed as a nonequivalent control group quasi-experimental study based upon data acquired through a pre-post test. The experimental group (n=21) was treated using an aroma-permeated hot compress applied to the knee. The cotton towel was heated in a microwave to $45^{\circ}C$, and 2~3 drops of lavender were applied on it. The towel was wrapped with plastic bag to prevent aroma leakage and retain heat. The treatment was applied the towel for 20 minutes, three times a day for 3 weeks. The control group (n=24) was not given any treatment. Data were collected using a VAS(100mm) for pain, questionnaire for sleep and depression, and a goniometer for knee flexibility. The data were analyzed with ${\chi}^2$-test and t-test using SPSS/Win18.0. Results: There were significant differences in knee pain (p=.001) and flexibility (p<.001), subjective quality of sleep (p=.020) and depression (p=.003) between the groups. Conclusion: Heat therapy combined with aromatherapy may be an effective nursing intervention and self-care technique to ameliorate pain and depression and to improve flexibility and sleep in elderly suffering from osteoarthritis.
Purpose: The purpose of this study was to explore the relationships among workplace bullying, organizational commitment, and turnover intention of nurses. Methods: A cross-sectional study was conducted. Data were collected using a structured questionnaire from nurses at five public hospitals (N=190). Results: The experience of bullying was 31.29 points (range 16-64) on average: Work-related bullying was the highest, followed by verbal/non verbal bullying and external threats. The consequences of bullying were 30.54 points (range 13-52) on average: The physical/psychological withdrawal was the highest, followed by increasing distrust and the poor quality of patient care. The experiences of bullying had positive correlations with the consequences of bullying (r=.55, p<.001) and the turnover intention (r=.27, p<.001), and had a negative correlation with organizational commitment (r=-.28, p<.001). The consequences of bullying had a positive correlation with the turnover intention (r=.52, p<.001), and had a negative correlation with organizational commitment (r=-.49, p<.001). Organizational commitment showed a negative correlation with the turnover intention (r=.63, p<.001). Conclusion: The findings of this study suggests the needs of a regular monitoring system and organizational management for workplace bullying, which helps nurses to commit to their organization and to be retained in a workplace.
Purpose: The purpose of this study was to investigate QOL and the factors related to family caregivers who care for Amyotrophic Lateral Sclerosis (ALS) patients. Methods: The subjects were 83 family caregivers caring for ALS patients who visited the neurology outpatient section of a tertiary hospital located in Seoul between January and July of 2008. Their demographic characteristics were assessed and data were collected using the amyotrophic lateral sclerosis functional rating scale (ALSFRS), the caregiver burden inventory (CBI), HADS, and SF-36. Results: The mean score of the physical health component (PHC) of the family caregiver's QOL was $62.6{\pm}24.4$, and the mean score of the mental health component (MHC) of their QOL was $57.7{\pm}22.4$. The mean score of caregiver burden was $76.5{\pm}30.7$. Anxious family caregivers amounted to 55.4% and depressed family caregivers accounted for 63.9% overall. In a multivariate analysis, the PHC of QOL was explained by caregiver burden (41.1%), depression (9.4%), caregiver gender (3.8%), and caregiver age (3.1%). Anxiety (39.8%), caregiver burden (10.9%), patient gender (5.4%) and depression (1.7%) were predictive factors of the MHC of QOL. Conclusion: Caregiver burden and depression affected both the PHC and the MHC. Caregiver burden affected the PHC more than it did the MHC of QOL, but anxiety affected the MHC more than it did the PHC of QOL.

The purpose of this study was to compare and analyze the differences before and after certification for internal members of nursing hospitals and Western hospitals that received the certification system of medical institutions after conducting a survey on 297 members of nursing hospitals and Western hospitals, SPSS 26.0 was used, and to understand the effect of improvement of member satisfaction, job stress, and medical service on the hospital operation effect. As for the hospital operation effect of a nursing hospital, it was revealed that the higher the satisfaction of the members and the improvement of the medical service, the higher the effect of the hospital operation. In the case of Western hospitals, the higher the satisfaction of the members and the improvement of the medical service, the lower the job stress, the more the hospital operation effect can be seen to increase. As the basic value system of the medical institution certification system is the improvement of patient safety and quality of care, it is believed that the effect of the certification system was higher than that of Western hospitals due to mandatory certification in nursing hospitals.

The data were collected through a self-report questionnaire and analyzed using t-test and ANOVA. Results : 1) The satisfaction level of inpatient families and bereaved families showed the mean value of 3.5 where the highest value is 5.0. 2) According to age, the level of satisfaction of inpatient families had significant differences in the fields of hospice philosophy, support for the family, medical management and nursing management(P=0.0001). The level of satisfaction of bereaved families showed significant differences in the field of support for the family, medical management, nursing management, and facilities of the hospice unit(P=0.0001). 3) By family relationship, the level of satisfaction of inpatient families had significant differences in the fields of hospice philosophy, support for the family, medical management and nursing management (P=0.0001). 4) According to religion, the level of satisfaction of inpatient families had significant differences in the fields of hospice philosophy, support for the family and nursing management (P=0.0001), but there was no significant difference for the bereaved families. Conclusions : The findings of this study showed that hospice services had positive influence on families with terminal disease such as cancer. To improve the level of satisfaction in the hospice services for families with hospice patients, we need to provide care by an interdisciplinary hospice team approach, and to assess needs of the families according to their socio-psychological characteristics. Further studies need to be conducted with large samples.
Purpose: This study was conducted to develop a measuring tool for spiritual care performance of hospice team members. The tool may be utilized for providing hospice patients with more systematic and standardized spiritual tares. Methods: The concept and questions of the tool were developed, and then its validity and reliability were tested. The purpose of this study was to identify the factors that could overcome the crisis and adversity of the nursing care provider through understanding the effect of job adaptation on the turnover intention of the nursing care provider and to contribute to the various problems of the nursing care provider in the long term. In order to confirm this as an empirical research task, risk factors and protection factors, general characteristics of the survey subjects, job adaptation and turnover intention were selected, and the risk factors and protective factors of caregivers' As a mediator. So Seoul. The results of the questionnaire survey were as follows: 291 caregivers in the elderly medical welfare facilities in Gyeonggi area. First, as the relationship between the risk factors and protective factors of occupational caregivers and occupational adaptation were more severe, the higher the maladjustment of the workplace culture, the more the job satisfaction and organizational commitment were adversely affected. The emotional support, The higher the information support, the more satisfied and satisfied the job. Second, the relationship between the risk factors of the caregiver and the protective factors and the turnover intention, the higher the conflict of caregivers, the more unstable the workplace, the more difficult it is to adapt to work culture, Respectively. Finally, as a result of verifying the mediating effect of occupational adaptation on the relationship between risk factors and protective factors and turnover intention of caregivers, job satisfaction, which is a sub-factor of job adaptation, It is shown that they play mediating roles only in the relationship between stress and turnover intention, and do not play a mediating role in the relationship between protective factor self - efficacy and social support and turnover intention. In other words, if caregivers feel satisfaction about their job, they can be less stressed on their jobs, improve their self-efficacy, and have a positive attitude toward social support. Also, it was found that the more the caregiver 's immersion into the organization, the less job stress and turnover intention decreased, but the self - efficacy and social support perception were not influenced. Based on this, the director of the facility should strive to stabilize the operation of the facility and provide high-quality services by seeking ways to improve conflict resolution and adaptation to the workplace culture so that nursing care workers can adapt to their work. And it is required to develop active management strategies and institutional support for improving job satisfaction and organizational commitment of caregivers.
