Objectives: The purpose of this study was to investigate the differences of capacity of local health organization to regional characteristics and the influence of organizational capacity on organizational performance. Methods: The study used the secondary data for 160 local public health organizations from $5^{th}$ Community Health Plans and 2009 Community Health Survey. The collected data were analyzed using one-way ANOVA and multiple regression analysis. Results: Work force and budget showed differences in regional size and elderly population rate. And consumer satisfaction and health care utilization showed differenced in work force and budget. The regression model with total number of employee, number of registered nurses, number of doctors and budget against consumer satisfaction was statistically significant (F=14.70, p=<.001), and number of registered nurses was identified as a factor influencing consumer satisfaction. This model also explained 20.5% of service satisfaction. The regression model for consumer satisfaction was statistically significant (F=45.98, p=<.001), and total number of employee nurses was identified as a factor influencing health care utilization. This model also explained 53.1% of utilization. Conclusions: The findings of this study imply that organizational capacity as work force and budget should be increased to improve the organizational performance as consumer satisfaction and health care utilization.
Oriental public health programs have been introduced for the purpose of providing comprehensive oriental health care services to community people including vulnerable classes, increasing the accessibility of oriental medicine and the public benefit, and further more elevating the health promotion and the quality of life of community people. Promoting these programs since 2002 in earnest, it is evaluated that it has made a lot of performances. In the other side, it showed many problems and policy issues also. This study is accomplished to analyze the problems made since the system was introduced and until now, 2007, and with this analysis, to examine policy issues and the reasonable recommendations for its development Major problems are as follows. First, fundamental notions and identity of oriental public health programs are not positioned accurately. Second, the infra-structure construction for effective propulsion of business is insufficient. Third, it is short of the capacity for program implementation of oriental public health doctors, related manpower, and health centers. Fourth, oriental health promotion programs that can fulfill the various health needs of community people are deficient. Fifth, active aid of the government and the oriental medical world as well as legal and systematical support for oriental public health programs is insufficient. As a result, to solve the problems and induce the successful settlement of the program, the policy recommendations such as (i) the fundamental notions of the program and establishment of approach strategies, (ii) reinforcement of the foundation of the program implementation, (iii) capacity enhancement of the manpower of the program implementation, and (iv) effective building of supportive system of the program are presented.

Dengue infection is becoming a serious global health threat. Public awareness is a pre-requisite for the successful implementation of dengue prevention programs. The main purpose of this study was to investigate dengue-related information needs and seeking behavior of the general public in Singapore. Some areas covered by this study were: importance of dengue-related information needs, preferred channels for seeking information, and respondents' perceptions of using dengue-related information. A questionnaire was used for data collection and 152 individuals participated in this study. Data analysis showed that the most sought after information concerned: dengue-related medicines, primary symptoms of dengue infection, and different possible treatments. The popular channels for seeking information were: websites of hospitals and other health agencies, the social media, television, and newspapers. Medical staff, such as doctors and nurses, were trusted for providing accurate information. Although credibility of social media was considered low, respondents were using it due to its easy accessibility. The findings of this study will be useful to government health departments in Singapore as well as in other countries suffering from dengue, hospitals, and public welfare agencies involved in public health awareness campaigns.
Objectives : The aim of this study was to introduce the development process of Korean medicine health promotion program for short stature children that is suitable for applied at public health center. Methods : The draft of health promotion program was developed through literature search of previous similar programs and advice of several experts. A small conference targeted public health Korean medicine doctors was also held to introduce the developed program and discuss for improvements. Results : The details of 12-week of Korean medicine health promotion program for children growth are as follows: 4-week of taking herbal medicines (Yukmijihwang-tang or Yukgunja-tang), contactless counseling with Korean medicine doctor, self-care home kit consisting of Sogeonjung-tang tea leaf, Sogeonjung-tang with Cervi Parvum Cornu extract, ear acupressure stick, finger chuna manual, growth diary, and jump rope. Conclusions : This health promotion program can help to increase the height of children and self-esteem of children with short stature as well as decrease the stress of parents.

Objectives : This study is retrospective chart review research on the combined-prescription of Western and Korean medicine in Public Health Center in rural area. Methods : Researchers reviewed medical records of patients who was prescribed Korean medicine and Western medicine from 1st, Jan, 2019 to 31st, Dec, 2019 in Public Health Center. 50 patients' medical records were included. Results : Total number of treatment is 3,808 cases and 1.3% of them is prescribed Korean medicine and Western medicine simultaneously. Prescription of Korean medicine is 153 cases and Western medicine is 160. Jowiseunggi-tang and Diroba tablet were the most frequently prescribed Korean medicine and Western medicine. Furthermore, Korean Medicine Doctors of Public Health Center use muscular skeletal disease system code(M code among KCD code) for prescription and Western Medicine Doctors use diseases of the circulatory system(I code among KCD code) frequently. Conclusions : We analyze 50 patients who were prescribed both Korean medicine and Western medicine. Mostly, patients were prescribed medicine for different diseases in each clinic. In western medicine clinic, drugs about circulatory or endocrine disease were prescribed frequently and in Korean Medicine clinic, drugs about muscular skeletal disease were most frequently prescribed. This result imply the real world's combination of prescription status that was different from result of National health insurance corporation database. Senior patients in rural area take medicine long period and have various underlying disease. We call for some attention about senior and rural area patients' prescription status in interaction studies of Korean medicine and Western medicine.

Background : The principal diagnosis has been used in many different fields such as hospital statistics, medical research, insurance claim, national health statistics and so on. Some principal diagnoses have a relatively low level of reliability in the medium-sized hospitals. The purpose of this study is to identify the reliability level of principal diagnoses and to suggest ways to improve reliability of the principal diagnosis. Method : Data were collected from a medium-sized hospital located in Pusan. The discharge summaries on 323 patients who were discharged in January, 2008 and the outpatient summaries on 251 patients who visited the hospital on March 28, 2008 were collected, and descriptive analysis was performed using SPSS version 12.0K. Result : The findings are the followings: (1) the diagnostic consistency rate between medical records and doctors' was 92.0%; (2) the diagnostic consistency rate between medical records and insurance claims was 86.1%; (3) the diagnostic consistency rate between doctors' diagnoses and insurance claims was 80.2%. The evidence seems to indicate that some principal diagnoses have reliability problems in the medium-sized hospitals. Conclusion : The results of this study suggest the followings: (1) employees should be trained and supervision of hospital activities are needed; (2) network systems should be constructed for each department; (3) professions need to be fostered (4) doctors' awareness of medical records should be changed.
Objectives: Self-reported disease history is often used in epidemiological studies. In this study, we acquired the hospital records of subjects who self-reported stroke or myocardial infarction (MI) and evaluated the validity of the participants' self-reported disease history. We also determined the level of agreement between specialists and non-specialists. Methods: Among the participants in the Health Examinees study, 1488 subjects self-reported stroke or MI during 2012-2017, and medical records were acquired for the 429 subjects (28.8%) who agreed to share their medical information. Each record was independently assigned to 2 medical doctors for review. The records were classified as 'definite,' 'possible,' or 'not' stroke or MI. If the doctors did not agree, a third doctor made the final decision. The positive predictive value (PPV) of self-reporting was calculated with the doctors' review as the gold standard. Kappa statistics were used to compare the results between general doctors and neurologists or cardiologists. Results: Medical records from 208 patients with self-reported stroke and 221 patients with self-reported MI were reviewed. The PPV of self-reported disease history was 51.4% for stroke and 32.6% for MI. If cases classified as 'possible' were counted as positive diagnoses, the PPV was 59.1% for stroke and 33.5% for MI. Kappa statistics showed moderate levels of agreement between specialists and nonspecialists for both stroke and MI. Conclusions: The validity of self-reported disease was lower than expected, especially in those who reported having been diagnosed with MI. Proper consideration is needed when using these self-reported data in further studies.
Objectives : This study was conducted to investigate the levels of psychosocial stress, job stress and their related factors among medical doctors practicing at local clinics. Methods : A survey using a self administered questionnaire was administered to 1,456 doctors practicing at private clinics via post for 2 months (2006. 1 - 2006. 3). Psychosocial stress, job stress, demographic factors, job related factors and health related behaviors were investigated. Among the eligible study population, the respondents were 428 doctors (29.4%). Results : The average scores of psychosocial stress and job stress were 2.19 and 3.13, respectively. The levels of psychosocial stress and job stress were statistically lower in older respondents, those who worked shorter or who were more satisfied with their job, and those with higher socioeconomic status. The level of psychosocial stress was related with smoking status, drinking status and exercise. The level of job stress was related with smoking status and exercise. In multiple linear regression analysis using psychosocial stress as a dependent variable, age, working hours per day, job satisfaction and perception on socioeconomic status were significant independent variables. In analysis using job stress as a dependent variable, age, working hours per day and job satisfaction were significant independent variables. Conclusions : Stress affects the doctor-patient relationship, productivity and overall health level of people. Therefore, it is important to manage and relieve the stress of doctors. It is suggested that more advanced studies on stress level and related factors and ways to improve the stress and health related behaviors of medical doctors should be conducted.
Objectives: We aimed to identify the factors related to depression and quality of life in patients with hypertension by using multilevel regression analysis. Methods: In 2019, 229 043 participants in the Korean Community Health Survey were selected as the study group. Individual factors were identified using data from the 2019 Community Health Survey. Regional factors were identified using data from the National Statistical Office of Korea. Multilevel regression analysis was conducted to find individual and local factors affecting depression and quality of life in patients with hypertension and to determine any associated interactions. Results: As individual factors in patients with hypertension, women, those with lower education-levels, recipients of basic livelihood benefits, and those with poor dietary conditions showed stronger associations with depression and quality of life. As regional factors and individual-level variables in patients with hypertension, lower gross regional personal income, fewer doctors at medical institutions, and lower rates of participation in volunteer activities presented stronger associations with depression and quality of life. In addition, the associations of depression with gross regional personal income, the number of doctors at medical institutions, and dietary conditions were significantly stronger in patients with hypertension than in patients without hypertension. The associations of gender and employment status with quality of life were also significantly greater. Conclusions: Policy interventions are needed to adjust health behaviors, prevent depression, and improve quality of life for patients with hypertension, especially for those with the risk factors identified in this study.
