Purpose : To figure out the degree of leisure satisfaction of elderly's life. Method : A total of 1,015 persons were randomly selected from cities. This survey was performed by college students from July 1 through October 31, 2008. Results : 1. The level of health status of the subjects was $8.58{\pm}0.24$ of 15 points(57.2points of 100 points). 2. The average satisfaction degree of leisure was $2.92{\pm}39$(58.8points of 100). Of the study subjects, females, a group over 65-69, singles, the persons spending petty cash over 1.1 million won a month, the persons living with 3-4 family members and the subjects living with children had higher degree of satisfaction than other groups. 3. The factors affecting the degree of leisure satisfaction by regression analysis were age, gender, marital status, amount of petty cash, education level, the number of family members, utilization of social service agency, current health status. The influence level of those variables was 36.1%. Conclusion : It is very important to develop special activity program for their pleasant leisure time. The recreation and leisure program for the elderly should be implemented to provide by the authorities concerned. Children should be encouraged to live with their parents. All elderly welfare related agencies such as government, NGOs and professionals have to work together to improve the health of the elderly.
The degree of income inequality deepened by health care expenditure was useful in assessing the health security level. This exploratory study was conducted to provide a basic evidence to prove the necessity of reinforcement the benefit coverage of South Korea's health security systems. Data from the Household Income and Expenditure Survey of Korea and Luxembourg Income Study were used. Income inequality indices before and after deduction of health care expenditure were computed, and the degree of the increase in the indices was compared among 13 countries. The degree of decrease against the effect of income inequality reduction policies by health care expenditure was determined. The relationships between the national characteristics and the increase in income inequality were examined. In South Korea, all income inequality indices increased after deducting health care expenditure, but the difference was not high compared to the mean of 13 countries. However, the degree of decrease against the effect of income inequality reduction policies by health care expenditure was high, compared to the mean of 13 countries. The proportion of public sector spending on health care proved to be statistically significant with the increase of income inequality indices (p<0.05). In the context of the continuous increase in health care expenditure, if benefit coverage of health security systems is not reinforced, income inequality will all the more increase due to health care expenditure. In the establishment of the policies for reinforcement of the benefit coverage, income inequality after deduction of health care expenditure should be continuously monitored.
Background: Chronic obstructive pulmonary disease (COPD) is a serious chronic condition with a global impact. Symptoms of COPD include progressive dyspnea, breathlessness, cough, and sputum production, which have a considerable impact on the lives of patients. In addition to the human cost of living with COPD and the resulting death, COPD entails a huge economic burden on the Chinese population, with patients spending up to one-third of the average family income on COPD management in some regions is clinically beneficial to adopt preventable measures via prudent COPD care utilization, monetary costs, and hospitalizations. Methods: Toward this end, this study compared the relative effectiveness of six indices in predicting patient healthcare utilization, cost of care, and patient health outcome. The six assessment systems evaluated included the three multidimensional Body mass index, Obstruction, Dyspnea, Exercise capacity index, Dyspnea, Obstruction, Smoking, Exacerbation (DOSE) index, and COPD Assessment Test index, or the unidimensional measures that best predict the future of patient healthcare utilization, cost of care, and patient health outcome among Chinese COPD patients. Results: Multiple linear regression models were created for each healthcare utilization, cost, and outcome including a single COPD index and the same group of demographic variables for each of the outcomes. Conclusion: We conclude that the DOSE index facilitates the prediction of patient healthcare utilization, disease expenditure, and negative clinical outcomes. Our study indicates that the DOSE index has a potential role beyond clinical predictions.
Purpose: The aim of this study was to examine body attitude, body mass index, and perceived stress among women's college students and decide the relationships among them. Method: A cross-sectional study design and convenient sampling method was utilized. Ultimately, 393 students participated in the study from October 8 to October 30, 2008. The measurements administered were comprised body attitude questionnaire and stress scale for college students and the students had filled out the questionnaires by themselves. The final data were analyzed with SPSS Win 16.0 statistics program, which was used to calculate the frequencies, percentages, ANOVA, and Pearson's correlations of the variables. Results: The major findings of this study were as follows; female students were mostly stressed by 'schooling task.' Stress scores were significantly different in accordance with spending money per month. Overall, significant differences were observed in body attitude scores according to each BMI group. There were significant relationships between body attitude, BMI, and perceived stress. Conclusion: Stress managements for women's college students should be developed and a program for modifying body attitude should be designed.
This study focuses on the healthcare sector in Vietnam which is promoting universal health insurance for the achievement of Universal Health Coverage (UHC) under Sustainable Development Goals (SDGs). The purpose of this study is to examine the characteristics of the reform process of the health care system and the law on health insurance through the historical and cultural contexts and its implications from the perspective of development. Based on the three dimensions of UHC - extension of protection for population, provision of various medical services, and financial protection, the current status of the Vietnam healthcare sector is summarized respectively as follows. First, according to the revised Health Insurance law which came into effect in 2015, the mandatory health insurance premiums are calculated based on household units. Second, there is a medical network that can provide preventive and healthcare services centered on primary health care facilities, for example commune health stations (trạm y $t{\hat{e}}$$X{\tilde{a}}$). Third, out-of-pocket expenditure is still a large proportion although public spending has increased and private spending has decreased since the enforcement of the health insurance law and various schemes. Vietnam is currently striving towards a universal health care system. The development of institutions and systems should be designed in a way that is appropriate for the members of the society rather than efficiency. This article findings shed light on the role of social values, family culture, and informal institutions.
Objectives : To estimate the economic burden of osteoporotic vertebral fracture (VF) from a societal perspective. Methods : From 2002 to 2004, we identified all National Health Insurance claims records for women ${\geq}50$ years old with a diagnosis of VF. The first 6-months was defined as a "clearance period," such that patients were considered as incident cases if their first claim of fracture was recorded after June 30, 2002. We only included patients with ${\geq}$ one claim of a diagnosis of, or prescription for, osteoporosis over 3 years. For each patient, we cumulated the claims amount for the first visit and for the follow-up treatments for 1 year. The hospital charge data from 4 hospitals were investigated to measure the proportion of the non-covered services. Face-to-face interviews were conducted with 106 patients from the 4 study sites to measure the out-of-pocket spending outside of hospitals. Results : During 2.5 years, 131,453 VF patients were identified. The patients had an average of 3.38 visits, 0.40 admissions and 6.36 inpatient days. The per capita cost was 1,909,690 Won: 71.5% for direct medical costs, 20.6% for direct non-medical costs and 7.9% for indirect costs. The per capita cost increased with increasing age: 1,848,078 Won for those aged 50-64, 2,084,846 Won for 65-74, 2,129,530 Won for 75-84and 2,121,492 Won for those above 84. Conclusions : Exploring the economic burden of osteoporotic VF is expected to motivate to adopt effective treatment options for osteoporosis in order to prevent the incidence of fracture and the consequent costs.
In this, analyzing the type of subjectivity in which people would have about home visit nursing services originating from public health care centers. I tried to research more effective ways to improve home visit nursing care services. and later. for the development of home visit nursing care. to supply basic data. The method for this study was the Q-method. created by William Stephenson. and was adequate for the study of subjectivity. For this study. through the deep interview. literature inquiry, and the discussion course. 206 Q-statement sentences were abstracted. and based on them, after Q-sample-selection. I then collected the Q-categorized-result from 32 subjects from Mar. 10. 2000 to Mar. 25. 2000. Through the statistic a analysis of PC-Qunal program. the subjectivity species were categorized and analyzed. The study results show that there are 3 sorts of recognition types. and they are analyzed in the following; The first type: the positively receiving type shows that they feel thankful and a trusting feeling about home visit nursing. The second type: the negatively mistrusting type shows that they had doubtful attitudes about the specialty of home visit nursing: they wanted medicine or nutrition remedies rather than health education and concerning the their own health care, they prefered the hospitals or clinics. The third type: the conditional receiving type shows that even though they had a positive receiving attitude about home visit nursing wanting to consult with the home visit nurses about the difficult problem which could not easily be settled, hoping that the home visit nurses could visit them more often, in their actual lives. they strongly indicated their attitudes concerning money as more important than home visits. The subjects in these 3 types commonly had a good feeling about the kindness of the home visit nurses: the first and third types also had a positive recognition about home visit nursing; however. in aspects of the evaluation and receiving attitudes, they showed a big difference. When all the above results are integrated. in the case of the first type the home visit nursing service, which satisfied the demand for health care of the medically weak people. should be continuously supplied. Additionally in case of the second type (negatively mistrust). continuous education and support should be supplied with enough interest to lead their concerns about their own health care as well as lead medical spending in a productive and effective direction in order to change their impressions. Through this study. I learned that the recognition of the objectives of home visit nursing services can be categorized in to 3 types and could be analyzed. Thus I wish that this study helps to present basic data which contributes to the development of the home visit nursing field.

First, the average scores of personal financial management ability in the college students' financial management attitude were 72.24 for male and 71.65 for female, so that male students showed a little higher score than female. College students who had an experience of opening a bank account and who kept a record of spending revealed higher scores. Those whose parents were elementary or middle school graduates unfolded the highest score, 73.67, and the lowest score, 63.35, from those whose parents had M.A. or Ph.D. The average scores of personal financial management ability in the college students' financial management knowledge indicated almost no difference with 63.26 for male and 63.35 for female. Only whether they had an experience of opening a bank account, keeping a record of spending, or a credit card revealed score differences. Depending on the students' majors, there were also score differences. Students from engineering department came up with the highest score, an average of 66.88, and students from art & physical education department with the lowest score, 53.39, in the financial management knowledge. The personal financial management ability in the college students' financial management function showed that the score of male students was a little higher than that of female, 64.26 versus 63.58. Students who kept a record of spending and whose parents' income was between three million won and four million won marked the highest score, an average of 65.26, and students whose parents' income was below two million won marked the lowest score, an average of 60.43, in the college students' financial management function. There were score differences in the college students' financial management function depending on the students' majors. Students from engineering department came up with the highest score, an average of 69.67, and students from public health department with the lowest score, 63.21.
Local autonomy of Korea's lower-level local council has been reinstated following elections last March for the first time in thirty years. Last June, we had elections for the upper-level local council. Mayors, governors, and administrative chiefs of cities, provinces and other local government bodies are slated for elections in the first half of next year. The impacts of local autonomy are taking effect in not only the political sphere, but also the administrative and economic spheres. In fact, it seems that some modification of all economic policy making and administration is inevitable. Since the initiation of local autonomy, in order to make the economy work more efficiently, it has become quite important to examine the impact of local autonomy on the national economy. The areas of local autonomy include independent legislative power, administrative power, organizational power, and most important of all, the independent public financial power of the local governments. The purpose of this paper is to examine the effects of local autonomy on the national economy and ways of enhancing the role of local public finance to facilitate settlement and development of the local autonomy system. Local autonomy will contribute to the continuous growth of our economy, allow balanced development, and generate greater efficiency. However, local autonomy can also incur economic costs causing at times short-term price instability, inefficient resource allocation, through tax competition and tax exporting, and insolvency of local government due to abusive fiscal operation. To reduce these side effects, different alternatives must be considered. Local autonomy systems generally provide more efficient resource allocation than centralization. But in the model used in Chapter 3 of this paper, the relative efficiencies of both local autonomy and centralization are determined by comparing the elasticity of substitution between national public goods and local public goods. If the elasticity of substitution is bigger than one, centralization provides a more efficient resource allocation. The development of local autonomy could be attained through democratization of the local public finance system including the following three propositions. I) The independence of public financial power of local governments should be established over central government. Furthermore, a democratically operated scheme of intergovernmental fiscal coordination is especially necessary. 2) In the operation of local finance, direct democracy is needed to induce the voluntary participation of local residents. The residents can take part in planning both the local budget and the development of the community. To attain this goal, all the results of local finance operations should be made public. 3) Among economic ill-effects of the local autonomy system, the most serious one is the possibility of insolvency of local governments. Therefore, measures to limit abusive spending by the local governments should be introduced, such as the fiscal restraints system adopted in the United States.
The purpose of this research is to investigate the factors which effect the intention to reemploy after the retirement and the mediation effect of the social capitalism for retired and prospective retired baby boomers. Starting from 2010, baby boomers have been retiring at a large scale. However retirement at a large scale without securing provision for fiancial wise in later life can lead to many issues regarding social and fiancial wise such as instability of labour market and increase of spending in society welfare cost and such on. Change of individual's self prospective and institutional support to help prepare find new career for next 3decade after the retirement needs to be concretized. Also, while in the work force, potential retiring baby boomer workers should be systematically supported with education, training and reemployment program based on individual's career orientation and allowing them to stay for a longer period in the labour market can be the implication to the solution for the economic stability and increasing social cost such as public pensions.
