The Effects of the Activity Program for Preventing Dementia Against Depression, Cognitive Function, and Quality of Life for the Elderly (치매예방을 위한 활동 프로그램이 지역사회 노인의 우울, 인지기능 및 삶의 질에 미치는 효과)

  • Objectives: This study is conducted to identify the effects of the activity program for preventing dementia against depression, cognitive function, and quality of life for the elderly. Methods: A non-equivalent control group with pretest-posttest design was used on 50 elderly who were assigned to the experimental and control group (25 for each group). Results: Cognitive function was significantly different between the two groups of the pretest and posttest (Z=4.217, p<.001). There were significant differences between the two groups of the pretest and posttest (Z=-2.324, p=.020) for the quality of life on the EQ-5D index. Conclusions: These findings confirmed that the activity program for preventing dementia was effective for improving cognitive functions and quality of life for the elderly. Therefore, it is further encouraged to perform the activity programs for preventing dementia within other community health posts and community centers. It is necessary to study the emotional supports from community health posts in the future.

Changes in satisfaction and perceptions of employment decisions after clinical training among physiotherapy students

  • Purpose: To investigate the satisfaction of students majoring in physiotherapy and to confirm a change of perception on employment decisions after clinical training. Methods: Structured questionnaires were distributed to 500 undergraduate physiotherapy students in 2014. Data from 462 respondents were analyzed using the Mann-Whitney, Kruskal-Wallis, Wilcoxon's signed-rank, stepwise regression, and independent samples t tests. Design: Cross-section study Results: The satisfaction and perceptions of employment decision increased post-clinical training among physiotherapy students who hoped to find employment. After clinical training, the desired employment venues also changed: fewer students desired to work in rehabilitation centers and secondary hospitals, and more desired to work in university hospitals, general hospitals, public welfare centers, and the industrial company health facilities. There were changes in the preferred fields of those who hoped to find employment in the field, as well. There was decreased interest in clinical electrophysiology, sports, and women's health, and increased interest in neurology, orthopedics, and pediatrics. Conclusion: The results of this study confirmed that clinical training changes student' employment decisions and affects their desire to enter specific fields.

Exploring Nurses' Experience in Long-term Care: Focusing on Role Adaptation and Barriers (노인장기요양분야 간호사의 역할 경험 탐색: 역할적응과 장애요인을 중심으로)

  • Purpose: The purpose of this study was to explore nurses' experience of the role adaptation process and barriers in long-term care (LTC) facilities including nursing homes, daycare centers, and home visit nursing centers. Methods: After in-depth interviews with 13 nurses who were working at the LTC facilities in G province, the data were analyzed using Colaizzi's phenomenological methodology. Results: 17 themes with 6 theme clusters, and 2 categories emerged: nursing role adaptation process in the LTC (role expectation and readiness, role perception and performance, limits of role performance), and barriers to nursing role adaptation in the LTC facility (organizational, situational, and institutional-environmental barriers). Conclusion: The results showed that it is critical to establish the nurses' role in caring for the health of the elderly and maintaining the quality of care in the LTC facilities. In response to the growing demand for quality care in the LTC facilities, more institutional and environmental changes are needed to address the current barriers that prevent nurses from performing their appropriate roles.

Barriers to Promoting Physical Activity among Overweight and Obese Children from Low-income Families Using Community Child Care Centers (저소득가정 비만아동의 신체활동 증진 장애요인 : 지역아동센터 이용 아동을 중심으로)

  • Purpose: The aim of this study was to identify the barriers to promoting physical activity among obese children from low-income families using community child care (CCC) centers. Methods: This study employed an exploratory-descriptive qualitative research using focus group interviews. The participants were recruited from CCC centers, and 4 focus group interviews were conducted with 10 overweight and obese children and 14 primary caregivers. All interviews were recorded and transcribed verbatim. The data were analyzed using content analysis. Results: The barriers to promoting physical activity were classified into three areas: 1) individual, a lack of will for physical activity; 2) interpersonal, a change in communicative mean with the peer group, absence of parental monitoring due to work-family balance, and an absence of consistent rules; and 3) organizational, lack of physical activity programs and human resources, and confined space for physical activity. Conclusion: The physical activity of children in low-income families is influenced by a range of ecological barriers. These findings can be used to develop tailored intervention to prevent and manage childhood obesity in low-income children.

Effect of Perceptions of Local Neighborhood Environments on the Physical Activity of Children from Low-income Families Using Community Child Care Centers (지역아동센터 이용 아동과 주 양육자의 지역사회 환경에 대한 인식이 아동의 신체활동에 미치는 영향)

  • Purpose: The aim of this study was to identify the influence of children's and caregivers' perceptions of local neighborhood environments on children's physical activities (PAs) and screen-based activities (SBAs) among low-income families. Methods: Secondary data analysis was performed using the data of 171 low-income children attending community child care centers and of their caregivers. Descriptive analysis, factor analysis and logistic regression analysis were employed to analyze data. Results: PAs were insufficient and SBAs were excessive in the majority of children. Children and primary caregivers had moderately negative perceptions of their local neighborhood environments. However these perceptions were not found to affect children's PAs or SBAs significantly. Conclusion: Although perceptions of local neighborhoods were not found to significantly influence children's PAs or SBAs, efforts are needed to make community child care centers and neighborhoods safer and more activity-friendly.

Effect of Community Based Participatory Research Nutrition Education Program for 3rd Grade Elementary School Students in the Ulsan Area (울산 지역 일부 초등학교 3학년생 영양 교육 프로그램 개발 및 효과 - 지역 사회 기반 참여 연구를 중심으로 -)

  • This study was evaluate the effects of community based nutrition education program offered to 3rd grade elementary school students. Students enrolled in intervention programs 4 times per class by nutrition teacher & public health center. The subjects were asked to 606 students fill out a questionnaire before and then after completion of community based nutrition education program. After completion of the community based nutrition education program, nutrition knowledge score increased from $5.07{\pm}1.65$ to $6.24{\pm}1.53$ (p<0.001), dietary attitude score increased from $16.79{\pm}2.70$ to $19.52{\pm}2.71$ (p<0.001), dietary behavior score increased from $4.79{\pm}1.23$ to $5.31{\pm}1.14$ (p<0.001). The changes in nutrition knowledge scores were positively correlated with dietary attitude and dietary behavior. Above results showed that community based nutrition education program was effective for the improvement of dietary habits of 3rd grade elementary school students. Therefore this study is suggested effective Community-Based Participatory Research public health program and this program can be used at school and at public health centers.

Nutrient Intake of the Rural Elderly Living in Kyungnam -Focusing on Health and Aging Status, and Life-Satisfaction- (경남 일부지역 농촌노인의 영양섭취조사 -건강과 노화상태 및 생활만족도를 중심으로 -)

  • The purpose of this study was to investigate the difference in nutrient intake according to the level of self-perception of health status, aging status and life satisfaction of the rural elderly. The factors for the study were surveyed by interview method. The subjects were 270 people(71 male, 129 female) aged over 65 years(73.5 $\pm$ 5.6ys) in the Ham-an area. The obtained results as follows : By evaluation of self-perception of health status, 57.5% of subjects answered they are in a bad health condition. The 91.5% of subjects had diseases(rheumatitis & arthritis 31.4%, cardiovascular disease 20.2%, gastric disease 10.2%). The women had more diseases than the men(p < 0.01). The subjects took medical treatment in private hospital(40.5%) and public health centers(35.0%). The men showed better level of aging status(p < 0.001) and life satisfaction index(p < 0.01) than the worsen. Living with spouse influenced the aging status(p < 0.05) and the more pocket money influenced life satisfaction(p < 0.05) and aging status(p < 0.05). The elderly who eat regularly 3 times a day(p < 0.05) and have a good appetite(p < 0.001) appeared to have positive effect on the self perception of health status and aging status. An increasing level of the self-perception of health status and regular exercise worked to improve aging status(p < 0.001). The habits of smoking and alcohol drinking, however had no effect on any index. The self-perception of health status affected the nutrient intake, but only in female elderly. The aging status and the life satisfaction index related overall positively to the intake of nutrients. In conclusion, the study shows that gender did influence nutrient intake in the elderly. The women who live alone rated lowest in social resources and health condition therefore their nutrient intake was also extremely in deficit. For successful aging, a program for rural elderly is needed, i.e. actions to provide minimum economic life, food delivery and psychological/physical health care through regional public health centers.

A Study on Relationships between Health Literacy, Disease-related Knowledge and Compliance to Medical Recommendations in Patients with Hypertension (고혈압 환자의 건강 문해력과 질환관련 지식 및 치료지시 이행에 관한 연구)

  • Purpose: To survey relationships between health literacy, disease-related knowledge and compliance to medical recommendations in patients with hypertension. Methods: The subjects were 315 patients conveniently selected from public health centers in Gangwon province. Data were collected through a questionnaire (from January 10, 2013 to February 10, 2013) and analyzed by descriptive statistics, t-test, ANOVA, Duncan test and Pearson's correlation coefficient using the PASW statistics 18.0 program. Results: There were significant differences in health literacy according to age, marital status, monthly family income and education. In regard to disease-related knowledge, there were significant differences according to gender, marital status and monthly family income. There were positive correlations between health literacy and disease-related knowledge (r = .39, p = <.001), disease-related knowledge and compliance to medical recommendations (r = .28, p = <.001). However, there was no correlation between health literacy and compliance to medical recommendations. Conclusion: Health literacy and disease-related knowledge have a significantly positive correlation. Further studies are needed to confirm the effects of health literacy on chronic diseases.

A Study on the Improvement of Nurses' Morale (보건간호사의 사기진작(士氣振作)에 관한 연구)

  • The purpose of this study is to examine public health nurses for their morale, test the effect of each factor on the morale, determine the variables affecting the morale, and thereby, provide for some basic data useful to the development of personnel affair administration for nurses. The survey on the nurses for public health centers was conducted for 10 days from April 23 to May 2, 2001; 96 nurses from Pusan, 101 from Kyonggi-do and 108 from Kangwon-do. The results of the study are as follows; 1. For the demographic variables, those aged between 31 and 39 accounted for the majority $(50.2\%)$ of the sample. $90.8\%$ was married, and $67.9\%$ of the sample graduated from junior colleges. $29.6\%$ of them had worked for 10-14 years and $62.5\%$ was of the seventh rank. $62.3\%$ of the nurses was recruited, and $59.4\%$ had chosen the public job for its guaranteed status. Half the nurses were working for the county population of which was more than 260 thousands, and $53\%$ of the nurses was serving the public health centers with 11-15 members. $47.3\%$ of them perceived that their living standard is middle or lower. 2. The degree of the moral was found as mean score. 2.65, which is interpreted as low. It has 11 factors. The rank of mean score of the factors is job satisfaction (3.36), sense of belonging (3.36), sense of recognition (3.30), stress due to supervision (3.13), degree of participation (3.05) and self-achievement (3.04), while those scoring lower points were assurance of status (2.58), pay level (2.53), working conditions (2.35) and promotion opportunity (2.31). 3. The degree of moral depending on the demographic variables was significantly affected by such variables as age. academic background, career, rank, employment method, motive for the job, size of the population served and living standards. 4. The result of the Stepwise multiple regression analysis for the morale factors, it was found that self-achievement accounted for $61\%$ of the total variance of the degree of morale, followed by stable status $(18\%)$, working conditions $(8\%)$, job requirements $(4\%)$ and promotion opportunities $(2\%)$; these five factors explained about $94.6\%$ of total variance of the degree of nurses' morale. On the other hand. age $(8\%)$ and academic background $(2\%)$ accounted for $10\%$ of the total variance of the degree of morale. 5. The factors affecting nurses' job life was economic need $(26.7\%)$, personnel management $(17.5\%)$, sense of recognition $(13.2\%)$, human relationship $(11.0\%)$, stable status and personal development in rank. On the other hand, the factors influencing on their possible resignation were children's bringing-up $(27.8\%)$, poor personnel management and promotion $(13.3\%)$, workload $(11.1\%)$ and little personal development $(8.9\%)$, Since such morale factors as promotion opportunity, working condition. pay level and stable status scored lower, it is required to reconsider those factors to improve nurses' morale.

A Comparative Study on Main Role, Professional Self Concept and Job Satisfaction of Public Health Nurses and Clinical Nurses (보건 간호사와 임상간호사의 주된 역할, 전문직 자아개념과 직무만족정도 비교)

  • This study was aimed at comparing and evaluating one main role, the professional self concept and job satisfaction of public health nurses and clinical nurses. Method: The data was collected from 72 nurses in 8 health centers and 86 nurses in 5 hospitals in Daegu city and Kyungpook province from May 5th to June 5th, 2001. This study was interviewed by trained social workers. The origin of questionnaires used in this study were PSCNI(Professional Self-Concept of Nurses Instrument) to measure the professional self concept of nurses and JDI(Job Descriptive Index) to measure the job satisfaction of nurses. The data was analyzed by SAS(version 6.12) and statistical methods used were Chi-square test, t-test, ANOVA(analysis of variables) and ANCOVA(analysis of covariate). Result: The findings of this study were as follows: 1. The most important domain in the main role of public health nurses were as counsellors, managers, mediators and evaluators. In clinical nurses, their main roles were, directors of nursing, researchers, spokesmans, and informers. 2. Public health nurses showed lower scores in the professional self concept of nurses than clinical nurses, what statistically significant tool was used to suggest this finding. 3. Public health nurses showed higher scores in job satisfaction than clinical nurses, but again, explain the tool used in these findings for give a rationals. 4. For public health nurses, general characteristics which affected professional self concept were age, educational level, and duration of job. In clinical nurses, the general characteristics which affected professional self concept were age, religion, marital status, and duration of job. 5. For public health nurses, there was no general characteristic which affected job satisfaction. In clinical nurses, the general characteristics which affected job satisfaction were age, religion, and duration of job. Conclusion: Nursing profession should give a training to competent nurses who can deliver high quality care to meet the needs of health consumers. The role of nurses has been expended and specialized as health conception has been changed in Korea.
