Spontaneous Resolution Rate and Predictive Factors of Resolution in Children with Primary Vesicoureteral Reflux (소아에서 일차성 방광요관역류의 자연소실율 및 관련 인자)

  • Purpose : To analyze the clinical characteristics, spontaneous resolution rate and predictive factors of resolution in children with primary vesicoureteral reflux(VUR). Methods : Between October 1991 and July 2003, 149 children diagnosed with primary VUR at Chonbuk National University Hospital were reviewed retrospectively. All of the patients were maintained on low-dose antibiotic prophylaxis and underwent radionuclide cystograms at 1 year intervals over 3 years after the initial diagnosis of VUR by voiding cystourethrogram was made. Results : The median time to resolution of VUR was 24 months and the total 3 year-cumulative resolution rate of VUR was 61.7%. The following variables were associated with resolution of VUR according to univariate analysis-; age<1 year, male gender, mild grade of reflux, unilateral reflux, congenital hydronephrosis as clinical presentation at time of diagnosis of VUR, absence of focal defects in the renal scan at diagnosis, absence of recurrent UTI, renal scars and small kidney during follow-up. After adjustment by Cox regression model, five variables remained as independent predictors of VUR resolution; age<1 yew, relative risk 1.77(P<0.05), VUR grade I+II 2.98(P<0.05), absence of renal scars 2.23(P<0.05), and absence of small kidney 5.20(P<0.01) during follow-up. Conclusion : In this study, spontaneous resolution rate of VUR, even high grade reflux, is high in infants during medical management, and it was related to age, reflux grade at diagnosis, absence of renal scars and small kidney during follow-up. Therefore early surgical intervention should be avoided and reserved for the selected groups.

  • The artificial compressibility (AC) method for the incompressible Navier-Stokes equations in the generalized curvilinear coordinates using the primitive form is implemented. The main advantage of the AC approach is that the resulting system of equations resembles the system of compressible N-S equations and can thus be integrated in time using standard, well-established time-marching methods. The errors, which are the odd-even oscillation, for pressure field in using the artificial compressibility can be eliminated by using the $4^{th}$ order artificial dissipation term which is explicitly included. Even though this paper focuses exclusively on 2D laminar flows to validate and assess the performance of this solver, this numerical method is general enough so that it can be readily extended to carry out 3D URANS simulation of engineering flows. This algorithm yields practically identical velocity profiles and primary vortex and secondary vortices that are in excellent overall agreement with the results of the vorticity-stream function formulation (Ghia et al., 1982). However, the grid resolution have to be required to be large enough to express the various vortices.

  • Recently many controversies originate for varies reasons, ranging from normal market competition to honest disagreements about rights. Disputes also arise from the clash between institutions and individuals. A free society should provide many systems for resolving controversies. We think of the courts as being primary, but, of course, they are not. In Korea, and in most other parts of the world, disagreements are resolved informally, without the need for judicial intervention. Settlements are worked out privately, usually without lawyers and certainly without judges. Most of judges are finding it difficult to cope with the needs and demands of society. Many businessmen who no longer want to get involved in lawsuits, are looking for alternative methods for resolving their disputes. However, there are actually two systems, litigation and arbitration only, to resolve disputes with binding both parties concerned. Litigation emphasizes on the equity and the justice with allowing three time's judgment for the resonable resolution, and arbitration, which is not subject to appeal, stress on the economic settlement rather than justice. Arbitration process results in a final and binding decisions. Although arbitration is a voluntary procedures that is created by the parties themselves, arbitration differs from mediation and conciliation because of its binding power. Arbitration is today coming into fashion as our primary methods for settling disputes. No company wants to have its funds tied up for long periods. Many parties prefer that the decision be final, rather than facing the prospect of extended appellate litigation. Therefore, government must encourage parties to settle their disputes by arbitration instead of litigation.

Errors of MODIS product of Gross Primary Production by using Data Assimilation Office Meteorological Data (MODIS 총일차생산성 산출물의 오차요인 분석: 입력기상자료의 영향)

  • In order to monitor the global terrestrial carbon cycle, NASA (National Aeronautics and Space Administration) provides 8-day GPP images by use of satellite remote-sensing reflectance data from MODIS (Moderate Resolution Imaging Spectroradiometer) at l-km nadir spatial resolution since December, 1999. MODIS GPP algorithm adopts DAO (Data Assimilation Office) meteorological data to calculate daily GPP. By evaluating reliability of DAO data with respect to surface weather station data, we examined the effect of errors from DAO data on MODIS GPP estimation in the Korean Peninsula from 2001 to 2003. Our analyses showed that DAO data underestimated daily average temperature, daily minimum temperature, and daily vapor pressure deficity (VPD), but overestimated daily shortwave radiation during the study period. Each meteorological variable resulted in different spatial patterns of error distribution across the Korean Peninsula. In MODIS GPP estimation, DAO data resulted in overestimation of GPP by $25\%$ for all biome types but up to $40\%$ for forest biomes, the major biome type in the Korean Peninsula. MODIS GPP was more sensitive to errors in solar radiation and VPD than in temperatures. Our results indicate that more reliable gridded meteorological data than DAO data are necessary for satisfactory estimation of MODIS GPP in the Korean Peninsula.

UNVEILING COMPLEX OUTFLOW STRUCTURE OF UY Aur

  • We present [$Fe\;{\small{II}}$] ${\lambda}1.257{\mu}m$ spectra toward the interacting binary UY Aur with 0".14 angular resolution, obtained with the Near infrared Integral Field Spectrograph (NIFS) combined with the adaptive optics system Altair of the GEMINI observatory. In the [$Fe\;{\small{II}}$] emission, UY Aur A (primary) is brighter than UY Aur B (secondary). The blueshifted and redshifted emission between the primary and secondary show a complicated structure. The radial velocities of the [$Fe\;{\small{II}}$] emission features are similar for UY Aur A and B: ${\sim}-100km\;s^{-1}$ and ${\sim}+130km\;s^{-1}$ for the blueshifted and redshifted components, respectively. Considering the morphologies of the [$Fe\;{\small{II}}$] emissions and bipolar outflow context, we concluded that UY Aur A drives fast and widely opening outflows with an opening angle of ${\sim}90^{\circ}$ while UY Aur B has micro collimated jets.

Assessment of New High-resolution Regional Climatology in the East/Japan Sea

  • This study provides comprehensive assessment results for the most recent high-resolution regional climatology in the East/Japan Sea by comparing with the various existing climatologies. This new high-resolution climatology is generated based on the Optimal Interpolation (OI) method with individual profiles from the World Ocean Database and gridded World Ocean Atlas provided by the National Centers for Environmental Information (NCEI). It was generated from the recent previous study which had a primary focus to solve the abnormal horizontal gradient problem appearing in the other high-resolution climatology version of NCEI. This study showed that this new OI field simulates well the meso-scale features including closed-curve temperature spatial distribution associated with eddy formation. Quantitative spatial variability was compared to the other four different climatologies and significant variability at 160 km was presented through a wavelet spectrum analysis. In addition, the general improvement of the new OI field except for warm bias in the coastal area was confirmed from the comparison with serial observation data provided by the National Fisheries Research and Development Institute's Korean Oceanic Data Center.

Oral Vancomycin Therapy in a Child with Primary Sclerosing Cholangitis and Severe Ulcerative Colitis

  • Primary sclerosing cholangitis (PSC), a rare progressive liver disease characterized by cholestasis and bile duct fibrosis, has no accepted, effective therapy known to delay or arrest its progression. We report a 15 year old female patient diagnosed with PSC and moderate chronic active ulcerative colitis (UC) who achieved normalization of her liver enzymes and bile ducts, and resolution of her UC symptoms with colonic mucosal healing, after treatment with a single drug therapy of the antibiotic oral vancomycin. We postulate that the oral vancomycin may be acting both as an antibiotic by altering the intestinal microbiome and as an immunomodulator. Oral vancomycin may be a promising treatment for PSC that needs to be further studied in randomized trials.

Investigation of Enantiomer Separation Using Chiral Crown Ethers as Chiral Selectors

  • A number of chiral selectors have been developed and applied for enantiomer separation of a variety of chiral compounds. Among these chiral selectors are chiral crown ethers, a class of synthetic host polyether molecules that bind protonated chiral primary amines with high selectivity and affinity. In this paper, two important chiral crown ethers as chiral selectors of bis-(1,1'-binaphthyl)-22-crown-6 and (18-crown-6)-2,3,11,12-tetracarboxylic acid (18-C-6-TA) are focused. They have been widely used to resolve the enantiomers of chiral compounds containing a primary amino moiety using chiral stationary phases (CSPs) or chiral selectors by high-performance liquid chromatography (HPLC), capillary electrophoresis (CE) and so on in chirotechnology. Also, it was described that the commercially available covalent type HPLC CSPs derived from (+)- and (-)-18-C-6-TA have been developed and successfully applied for the resolution of various primary amino compounds including amino acids.

A Case of Primary Pancreatic Lymphoma Presenting with Obstructive Jaundice

  • A 55-year-old man was admitted to the hospital for jaundice. Computed tomography (CT) scans showed a diffuse mass in the pancreas and peripancreatic area, with infiltration to of the whole pancreas, and overall reduced enhancement compared to normal pancreas. Esophagogastroduodenoscopy revealed elevated mucosal lesion covered hyperemic mucosa at duodenal bulb and ulcerative lesion at body of stomach. Endoscopic ultrasonography revealed an irregular mass with unclear boundaries was observed within the pancreas. Abrupt narrowing of mid to distal common bile duct was seen and the stricture was caused by compression of pancreatic mass. Plastic stent was inserted and clinical improvement was achieved including resolution of jaundice. The patient is currently being treated with combination of rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone. We report a case of primary pancreatic lymphoma presenting with obstructive jaundice.

  • Recursive DNS is configured as primary or secondary DNS on user PC and performs domain name resolution corresponding user's DNS query. At present, the amount of DNS traffic is occupied high rate in the total internet traffic and the internet traffic would be increased by failure of IPv6 DNS queries and responses as IPv6 transition environment. Also, existing Recursive DNS service mechanisms is unstable on malicious user's attack same as DoS/DDoS Attack and isn't provide to user trust DNS service fail-over. In this paper, we propose IPv6 Recursive DNS service mechanisms for based on anycast for improving stability. It is that fail-over Recursive DNS is configured IPv6 Anycast address for primary Recursive DNS's foil-over. this mechanisms increases reliability and resiliency to DoS/DDoS attacks and reduces query latency and helps minimize DNS traffic as inducing IPv6 address.
